Strict quarantine measures and the unpreparedness of the medical industry have exacerbated public health problems on all continents. The number of patients with advanced stages of cancer has increased at the end of the year. Several approaches could mitigate the negative effects of Covid-19 on screening programs. International oncology organization «European Society of Medical Oncology» (ESMO) has launched a section COVID 19 on its website. We believe that some aspects of the ESMO recommendations should be widely presented. The feasibility of vaccinating cancer patients against COVID-19 is undeniable

Background: Intestinal parasitic infections are the one of the most common health problems in developing countries. Objective: A number of patients die annually due to complications caused by these parasites.Therefore, the aim of this study was to investigate the rate and type of parasitic infections, determine the factors affecting them in Ilam city and also provide strategies to prevent them.In this descriptive cross-sectional study conducted in one of the Ilam labs in 2016, 417 stool specimens were randomly collected. All specimens were examined using direct and ethanol formaldehyde.Suspect specimens were examined using Trichrom staining. Demographic information was also recorded in a questionnaire, and finally the results were analyzed using statistical software SPSS 20.The data were then compared with Chi-square test. Results: Out of the 417 patients examined, 59 (14.1%) were infected with intestinal parasites. The type of parasitic infection in 9.4% was Blastocystis hominis, 3.6% Entamoeba coli, 0.5% Entamoeba histolytica, 0.5% Giardia and 0.2% Trichomonas hominis. Conclusion: Despite the improvement of public health, parasitic infections are still considered as one of the health problems in the city of Ilam. Therefore, proper planning, public health education, raising the level of health in the area and the provision of safe drinking water are some of the ways to reduce parasitic infections in the region.

AbstractBackgroundThe absence of a chronic kidney disease (CKD) registry in Ecuador makes it difficult to assess the burden of disease, but there is an anticipated increase in the incidence of end-stage kidney disease along with increasing diabetes, hypertension and population age. From 2008, augmented funding for renal replacement treatment expanded dialysis clinics and patient coverage.MethodsWe conducted 73 in-depth interviews with healthcare providers in eight provinces. Findings were analyzed using qualitative methods and triangulated with quantitative data on patients with CKD diagnoses from six national-level databases between 2015 and 2018. We also reviewed grey and scientific literature on CKD and health systems in Ecuador.ResultsDatasets show a total of 17 484 dialysis patients in 2018, or 567 patients per million population (pmp), with an annual cost exceeding 11% of Ecuador’s public health budget. Each year, there were 139—162 pmp new dialysis patients, while doctors report waiting lists. The number of patients on peritoneal dialysis was stable; those on hemodialysis increased over time. Only 13 of 24 provinces have dialysis services, and nephrologists are in major cities, which limits access, delays medical attention, and adds a travel burden on patients. Prevention and screening programs are scarce, while hospitalization is an important reality of CKD patients.ConclusionCKD is an emerging public health crisis that has increased dramatically over the last decade in Ecuador and is expected to continue, making coverage for all patients impossible and the current structure, unsustainable. A patient registry would permit to estimate the demand and progression of patients with consideration for comorbidities, requirements and costs, and mortality, and identify where to focus prevention efforts. Health policy should clearly state CKD definitions and required patient data, including cause, disease stage and follow-up statistics. Organized monitoring of patients would benefit from improvements in patient referral.

Colorectal cancer (CRC) is a malignant disease with an incidence of over 1.8 million new cases per year worldwide. CRC outcome is closely related to the respective stage of CRC and is more favorable at less advanced stages. Detection of early colorectal adenomas is the key to survival. In spite of implemented screening programs showing efficiency in the detection of early precancerous lesions and CRC in asymptomatic patients, a significant number of patients are still diagnosed in advanced stages. Research on CRC accomplished during the last decade has improved our understanding of the etiology and development of colorectal adenomas and revealed weaknesses in the general approach to their detection and elimination. Recent studies seek to find a reliable non-invasive biomarker detectable even in the blood. New candidate biomarkers could be selected on the basis of so-called liquid biopsy, such as long non-coding RNA, microRNA, circulating cell-free DNA, circulating tumor cells, and inflammatory factors released from the adenoma into circulation. In this work, we focused on both genetic and epigenetic changes associated with the development of colorectal adenomas into colorectal carcinoma and we also discuss new possible biomarkers that are detectable even in adenomas prior to cancer development.

Diagnosis, tratament and follow-up of patients with melanoma during COVID-19 pandemic is quite challenging. These patients are often immunocompromised, but, on the other hand, management of this malignant skin cancer should not be delayed. It is necessary to diagnose and stage the melanoma as soon as possible, in an attempt to provide a better prognosis. There are few data regarding the treatament of melanoma during COVID-19 pandemia. However, the general recommandations suggest testing all cancer patients prior administration of the therapy. The European Society for Medical Oncology (ESMO) provided guidelines regarding therapy of this skin cancer during COVID-19 pandemic. Every patient is different, and it is always important to evaluate the risks and benefi ts.

Abstract An increasing number of patients seek advice at community pharmacies in terms of prevention, detection and initial management of certain conditions and diseases. Some countries have introduced pharmacy-based screening programs and counseling. The objective of this work was to analyze the situation on providing screening and counseling at community pharmacies in Armenia. The staff of randomly selected community pharmacies located in various regions of Armenia was asked to complete pre-tested questionnaire. The data were analysed with SPSS statistical software, version 22.0. 62.9% of 353 responded professionals reported availability of conditions for monitoring blood pressure, 14.4% - for monitoring glucose level, 11.3% - for doing pregnancy test, 9.6% - for testing cholesterol, 2.0% - for monitoring asthma using peak flow meter. 86.7% indicated accessibility of conditions for private discussion with patients. Small number of pharmacy professionals reported that during the last year they were engaged in diabetes management (10.2%) and HIV/AIDS/TB management (7.9%). 7.4% of respondents were involved in family planning activity. 97.5% of pharmacy professionals considers that being active in providing general health information to patients is important and 89.2% reported about being active. The number of professionals who are active in providing general health information to patients is significantly higher among those who attended continuing education courses than among those who did not (p = 0.003). Only part of community pharmacies is able to provide point-of-care testing. Professionals are rarely involved in disease management. They are active in providing general health information. Continuing education improves situation with engagement of pharmacy professionals in public health activity. Key messages There is need to improve the situation with providing point-of-care testing at community pharmacies. Training increases involvement of community pharmacists in public health.
