Abstract Background The Instrumented Stand and Walk (ISAW) test, which includes 30 seconds of stance, step initiation, gait, and turning, results in many objective balance and gait metrics from body-worn inertial sensors. However, it is not clear which metrics provide independent information about mobility. Objective It was hypothesized that balance and gait represent several independent domains of mobility and that not all domains would be abnormal in individuals with Parkinson disease (PD) or would change with levodopa therapy. Design This was a cross-sectional study. Methods A factor analysis approach was used to identify independent measures of mobility extracted from the ISAW in 100 participants with PD and 21 control participants. First, a covariance analysis showed that postural sway measures were independent of gait measures. Then, the factor analysis revealed 6 independent factors (mobility domains: sway area, sway frequency, arm swing asymmetry, trunk motion during gait, gait speed, and cadence) that accounted for 87% of the variance of performance across participants. Results Sway area, gait speed, and trunk motion differed between the PD group in the off-levodopa state and the control group, but sway frequency (but not sway area) differed between the PD group in the off-levodopa state and the control group. Four of the 6 factors changed significantly with levodopa (off to on): sway area, sway frequency, trunk motion during gait, and cadence. When participants were on levodopa, the sway area increased compared with off levodopa, becoming more abnormal, whereas the other 3 significant metrics moved toward, but did not reach, the healthy control values. Limitations Exploratory factor analysis was limited to the PD population. Conclusions The different sensitivity various balance and gait domains to PD and to levodopa also support neural control of at least 6 independent mobility domains, each of which warrants clinical assessment for impairments in mobility.

AbstractClassrooms can be noisy and are challenging listening environments for children with auditory processing disorder (APD). This research was undertaken to determine if the Listening Inventory for Education-UK version (LIFE-UK) can differentiate children with listening difficulties and APD from their typically developing peers.To investigate reliability and validity of the student and teacher versions LIFE-UK questionnaire for assessing classroom listening difficulties.Cross-sectional quantitative study comparing children with listening difficulties with typically developing children.In total, 143 children (7–12 yr) participated; 45 were diagnosed with APD. Fifteen participants with reported listening difficulties who passed the APD test battery were assigned to a “listening difficulty” (LiD) group. Eighty three children from nine classrooms formed a Control group.Children and teachers completed the LIFE-UK questionnaire student and teacher versions. Factor analysis was undertaken, and item reliability was assessed using Cronbach’s alpha. Teacher and student ratings were compared using Spearman correlations. Correlations between LIFE-UK ratings and APD test results were also investigated.Factor analysis revealed three factors accounting for 60% of the variance in the Control group LIFE-UK ratings. After removing six items with low factor loadings, a shortened seven-item version with three factors accounted for 71.8% of the variance for the student questionnaire; Cronbach’s alpha indicated good internal reliability for this seven-item version of the student questionnaire. Factors were also derived for the teacher questionnaire. Teacher and student ratings were correlated when participant groups were combined. LIFE-UK ratings correlated weakly with some APD measures, providing some support for the questionnaire validity.The results support the use of either the 13- or 7-item student and the teacher versions of the LIFE-UK to evaluate classroom listening and functional consequences of APD. Factor analysis resulted in groupings of items reflecting differences in listening demands in quiet versus noise for the student questionnaire and attentional versus class participation demands for the teacher questionnaire. Further research is needed to confirm the robustness of these factors in other populations.

BACKGROUND: Posturography power spectra (PS) implementation has been proven to discriminate between sensory inputs detriment of vestibular and proprioceptive origin. OBJECTIVE: To deepen the role of posturography testing in the diagnostic route of dizzy conditions, by comparing two groups of patients –93 affected by cervicogenic dizziness (CGD) and 72 by unilateral vestibular hypofunction (UVH) –with a group of 98 age- and gender-matched healthy subjects, serving as control group (CON). METHODS: All participants underwent otoneurological testing including video head impulse test (vHIT) and posturography testing with PS analysis. They also filled in Dizziness Handicap Inventory (DHI), Tampa Scale for Kinesiophobia and Hospital Anxiety and Depression Scale questionnaires. RESULTS: UVH and CGD patients were found to have significant increase in vestibular- and proprioceptive-related PS values when compared with CON. Receiver operating characteristic curves found PS values to reliably discriminate both groups from CON. Positive and negative correlations were respectively found between vestibular-/proprioceptive-related PS domain and DHI in both groups and between PS and vHIT scores in UVH patients. CONCLUSIONS: PS analysis demonstrated to be useful in differentiating CGD and UVH patients each other and when compared to CON, to objectively represent perceived symptoms filled along the DHI scale and to corroborate the rate of vestibular deficit in UVH patients.

Abstract Objective To investigate the effect of a cognitive task on postural control during treadmill walking. We hypothesized that postural sway acceleration would decrease during dual-task compared to single-task gait. Method Ten (5 females, 5 males) physically active, healthy young adults (Age: 22.50 ± 3.01) with no history of cognitive deficits or head injury were recruited from a University campus for this cross-sectional study. Participants reported to a biomechanics laboratory for a single testing session and completed two, 5-minute walking trials on a treadmill at 90% of maximal gait-speed under single and dual-task conditions while instrumented with a single inertial measurement unit (IMU) on the lower-back. During dual-task gait, participants concurrently performed a Stroop Test displayed on a mounted screen in front of the treadmill during the entire trial. Root mean square (RMS) of acceleration was analyzed in the medial-lateral, anterior-posterior, superior-inferior, and resultant planes during the dual-task trial. Paired t-tests and effect sizes (ES) were calculated for each sway plane between single and dual-tasks. Results No differences were identified between single and dual-task RMS acceleration in the medial-lateral (p = 0.98, ES = 0.01), anterior-posterior (p = 0.49, ES = 0.23) or superior-inferior (p = 0.98, ES = 0.01) planes. However, the RMS acceleration in the resultant plane was lower in the dual-task condition (Mean Difference: 0.031 ± 0.037; p = 0.02, ES = 0.85). Conclusions This novel dual-task gait paradigm utilized constant sub-maximal gait speed to investigate the influence of a sustained cognitive dual-task on postural sway using a single IMU. These preliminary results indicate that this dual-task gait paradigm may affect gross postural sway in healthy adults.

Background. Different measures have been used to quantify body balance; some of which use technology to measure postural sway, others are physical performance or self-reported. However, there is little information on the best postural sway measures associated with aging, physical performance, and cognitive function measures. Objective. To evaluate the relationship between postural sway measures and aging, physical performance, and cognitive function measures. Methods. A total of 51 subjects (53% female, mean age 53.2±21 years) participated in this cross-sectional study. The participants completed the Activities-specific Balance Confidence (ABC) Scale questionnaire, the Functional Gait Assessment (FGA), the Montreal Cognitive Assessment (MoCA) test, and gait speed. Afterward, the participants performed 8 balance exercises, and their postural sway was measured using a force plate. Spearman’s rank correlation coefficient was used to examine the relationship between the study variables. Results. Age was negatively associated with cognitive function, gait speed, ABC scores, and FGA scores. In addition, cognitive ability was associated positively with ABC scores (r=0.38, p≤0.01). Age, FGA scores, and gait speed were significantly associated with the postural sway of the AP direction in some exercises and in all exercises in the ML directions (p<0.05). The cognitive function and ABC scores were significantly associated with only postural sway measures in the ML direction (p<0.05). Conclusion. The postural sway measures in the lateral direction had more and stronger associations with age, physical performance, and cognitive function measures compared to those in the AP direction.

Abstract Background The purpose of this study was to investigate the efficacy of dance in the experienced older dancers compared to the inexperienced older adults. We explored the effect of dance on the composition of muscle groups and multi-muscle synergies stabilizing the center of pressure (COP) displacement in preparation to take a step during support surface translation. Methods Eight dance experienced elderly participants were asked to take a step in response to support surface perturbations. Uncontrolled manifold analysis was used to identify muscle modes (M-modes) as factors in the muscle activation space. Variance components in the M-mode space and indices of M-mode synergy stabilizing COP displacement were computed. Results The reciprocal M-modes were observed more frequently in the dance group than in the control group prior to the step initiation. Dance led to higher indices of multi-muscle synergies and earlier anticipatory synergy adjustments during preparation for making a step in response to the support surface translations. Conclusions Dance appeared to be associated with adjustments in both the composition of M-modes and M-mode co-variation patterns resulting in stronger synergies stabilizing COP coordinate in older adults. The results reported here could have clinical relevance when offering a dance approach to balance training for impaired individuals.

Objective: To assess the frequency of somatization and its association with motor, nonmotor symptoms, and quality of life in persons with Parkinson disease (PD). Methods: A cross-sectional case–control study was carried out. Assessments included the List of 90 Symptoms somatic factor (SCL-90-R SOM), Movement Disorder Society Unified Parkinson’s Ratings Scale (MDS-UPDRS), Non-Motor Symptom Scale (NMSS), Montreal Cognitive Assessment (MoCA), and Parkinson Questionnaire-8 (PDQ-8). Results: A total 93 persons with PD and 93 controls were included. Somatization within the PD group was 2 times more frequent compared to the control group (43% vs 21.5%, P = .003). Persons with PD had higher NMSS total scores (48.6 ± 42.6 vs 28.3 ± 30.4, P = .001). Patients with PD with somatization had worst MDS-UPDRS, NMSS, MoCA, and PDQ-8 (all P < .05). Conclusion: Somatization is more frequent in persons with PD compared to healthy controls. Somatization in PD is associated with nonmotor symptoms and worst quality of life.

Background: Thyroid hormones influence nearly all major metabolic pathways. Their most obvious and well-known action is the increase in basal energy expenditure obtained by acting on protein, carbohydrate and lipid metabolism. The lipid metabolism is more influenced by the thyroid hormone. Methods: A cross-sectional study was conducted on 100 patients with suspicion of thyroid disorders were taken as cases. One hundred patients with normal thyroid profile and no history of other chronic diseases were taken as control group. Results: The serum TC, TG and LDL levels in hypothyroid individuals (both overt and subclinical) were significantly higher than euthyroid subjects but the levels were comparable between hyperthyroid and euthyroid group. Conclusion: Dyslipidemias are associated with thyroid disorders, so biochemical screening for thyroid dysfunction in all dyslipidemic patients. Therefore, patients presenting with dyslipidemia are recommended for investigation to explore thyroid dysfunction. Keywords: Thyroid profile, Total cholesterol, Triglycerides and LDL

Background: Leukoderma is an acquired disease of pigmentation which is presented by depigmented areas of different shape and sizes on the skin. In various researches it was reported that these defects in melanocytes occurred due to autoimmune pathway or the complex causation of oxidative stress and genetics. Material & Methods: In the present cross‑sectional prospective study 100 patients who were diagnosed with Leukoderma and controls who were not having depigmentation of skin and without any known clinical disease were enrolled for present study by simple random sampling. Written informed consent was taken from each study participant. Clearance from institutional ethical committee was also taken prior to the study. Results: The prevalence of psychiatric illness among leukoderma group was 22% and among 78% patients we did not found any psychiatric illness. The most common symptom was depression which was present among 22% of patients with Leukoderma and 18% patients with Leukoderma had anxiety symptoms. Among the control group depression was present in 5 % subjects and anxiety symptoms were present in 7% of subjects. The mean WHO-Quality of life scores was low among Leukoderma group in relation to the control group (p value <0.05). On correlation with psychiatric illness, observations were statistically non-significant (p > 0.05) with HAMA-A and HAMA- D. The observations were statistically significant (p < 0.05) with GHQ total and BSA score. Conclusion:  Higher prevalence of psychitric morbidity among patients with leukoderma and it was found associated with duration of disease and BSA scores. The most common symptom was depression which was followed by anxiety symptoms. Key words: Leukoderma, psychiatric illness, anxiety, depression.

Background: Noise pollution or noise is an unwanted sound which is disturbing to human beings.However small or soft the sound, if it is undesirable it is considered as noise. Noise induced hearingloss is a sensorineural hearing loss that is commonly encountered second to presbycusis. Purpose: Toknow the effect of traffic noise exposure on hearing impairment to the employees of the Parking DistrictCompany of the Denpasar city and to improve diagnostic detection on hearing impairment caused bynoise. Method: A cross sectional study was conducted at the Parking District Company office. Thepopulations of this study were the employees of the Parking District Company. Samples of this study were the employees who were exposed to traffic noise and control samples were an employee who was unexposed. Samples were selected by simple random sampling. Results: From 40 parking attendants,27 persons (67.5%) aged above 35 years old. The parking attendants who had been working for ten to fifteen years were 36 persons (90%) and no history using ear protection when working. Seven persons(17.5%) had referred DPOAE upon examination with increase hearing threshold on audiogram result.In this study the parking attendants who had hearing deficit induced by noise were 7 persons (17.5%)and only one person (2.5%) in control group. There was a statistically significant effect of traffic noiseto hearing function deficit (p<0.05). Conclusion: Traffic noise has effect in hearing function deficit onthe parking attendants.ORLI Vol. 40 No. 2 Tahun 2010Key words: NIHL, parking attendant, audiometry, DPOAE.
