Ceramides are an integral component of the epidermal barrier system. Alterations in ceramides levels are associated with various inflammatory dermatoses such as psoriasis, atopic dermatitis, ichthyosis, Gaucher’s disease, acne vulgaris, and skin aging. Various formulations containing ceramides have been developed so that exogenous ceramides can repair the barrier function. Herein, the authors have provided an overview of the basic anatomy, structure, functioning, and importance of ceramides and their role in healthy skin and various skin disorders. In addition, the authors review conventional and newer technologies for delivery of ceramides in various skin diseases.

The cutaneous immune response is important for the regulation of skin aging well as for the development of immune-mediated skin diseases. Aging of the human skin undergoes immunosenescence with immunological alterations and can be affected by environmental stressors and internal factors, thus leading to various epidermal barrier abnormalities. The dysfunctional epidermal barrier, immune dysregulation, and skin dysbiosis in the advanced age, together with the genetic factors, facilitate the late onset of atopic dermatitis (AD) in the elderly, whose cases have recently been on the rise. Controversial to the healthy aged skin, where overproduction of many cytokines is found, the levels of Th2/Th22 related cytokines inversely correlated with age in the skin of older AD patients. As opposed to an endogenously aged skin, the expression of the terminal differentiation markers significantly increases with age in AD. Despite the atenuated barrier disturbances in older AD patients, the aged skin carries an impairment associated with the aging process, which reflects the persistence of AD. The chronicity of AD in older patients might not directly affect skin aging but does not allow spontaneous remission. Thus, adult- and elderly subtypes of AD are considered as a lifelong disease.

Objective - To increase awareness of the psychosocial impact of Kushtha, Visarpa, Mukhadushika, Sheetpitta, Udarda and Kotha in Ayurveda and Acne vulgaris, Urticaria, Various types of fungal infections, Atopic dermatitis, Psoriasis etc. in modern point of view. Quality Of Evidence - A literature review was based on a MEDLINE search (1966 to 2000). Selected articles from the dermatologic and psychiatric literature, as well as other relevant medical journals, were reviewed and used as the basis for discussion of how skin disease affects patients’ lives and of appropriate management. Message - Dermatologic problems hurt patients’ quality of life. skin disease can produce stress, anxiety, anger, depression, low self-esteem, embarrassment, and other psychological, personal, professional and social life problems that affect patients’ lives in ways comparable to arthritis or other disabling illnesses, as well as showing a bidirectional relationship between skin disease and psychological distress. This review focuses on the effects of five common skin diseases seen by family physicians- Acne, Urticaria, Various types of fungal infections, Atopic dermatitis and Psoriasis. Conclusion - How skin disease affects psychosocial well-being is un- derappreciated. Increased understanding of the psychiatric comorbidity associated with skin disease and a biopsy- chosocial approach to management will ultimately improve patients’ lives. The skin is directly exposed to the polluted atmospheric environment, and skin diseases, such as atopic dermatitis and acne vulgaris, can be induced or exacerbated by airborne particulate matter (PM). PM can also promote premature skin aging with its accompanying functional and morphological changes. PM-induced skin diseases and premature skin aging are largely mediated by reactive oxygen species (ROS), and the harmful effects of PM may be ameliorated by safe and effective natural antioxidants. Experimental studies have shown that the extracts and phenolic compounds derived from many plants, such as cocoa, green tea, grape, pomegranate, and some marine algae, have antioxidant and anti-inflammatory effects on PM-exposed cells. The phenolic compounds can decrease the levels of ROS in cells and/or enhance cellular antioxidant capacity and, thereby, can attenuate PM-induced oxidative damage to nucleic acids, proteins, and lipids. They also lower the levels of cytokines, chemokines, cell adhesion molecules, prostaglandins, and matrix metalloproteinases implicated in cellular inflammatory responses to PM. Although there is still much research to be done, current studies in this field suggest that plant-derived phenolic compounds may have a protective effect on skin exposed to high levels of air pollution.

Abstract Atopic dermatitis (AD) is the main focus of this review which mostly starts with children in early life. Beside the etiological factors like environmental, dietary or medical exposures, Gut-skin axis microbiome studies have an impact to investigate and to understand the relation between the gut microbiome and changes to the skin microbiom and some skin diseases resulting like AD. Infants start forming their microbiome in early life and some researches suggest that this time have a crucial role in development of AD. Balanced bacterial composition is important to maintain healthy skin as the gut microbiome dysbiosis may result in dramatic shifting in the skin microbiome that gives better chance for some bacteria such as Staphylococcus aureus to over prevail which has been reported to contribute in AD development. BACKGROUND: The literature has shown that the presence of emotional disturbances in caregivers of children with skin diseases affects the course and treatment of the disease. Anxiety and depression are among the most frequently reported psychiatric diagnoses related to this fact. OBJECTIVE: To evaluate the presence of anxiety and depression in caregivers of pediatric patients with chronic skin disorders, exemplified by atopic dermatitis, psoriasis and vitiligo, and correlate them to the quality of life of the patients. METHODS: The sample consisted of 118 patients with atopic dermatitis, vitiligo and psoriasis, monitored by their main caregiver. The levels of anxiety and depression in the caregivers were assessed using the Hamilton Anxiety Scale and the Beck Depression Inventory, respectively. The Children's Dermatology Life Quality Index was applied. RESULTS: Anxiety was observed in 36% of the caregivers of the patients with atopic dermatitis, in 36% of those of children affected by psoriasis, and in 42% of those responsible for pediatric patients with vitiligo. Depression occurred in 36% of the caregivers of patients with atopic dermatitis, in 36% of those of children affected by psoriasis and in 26% of those responsible for pediatric patients with vitiligo. There was a significant correlation between poor quality of life scores in patients with vitiligo and the presence of depression and anxiety in their caregivers. CONCLUSION: Emotional disorders tend to be present among close family members of children with the chronic skin diseases studied and their prevention can help in controlling and treating these diseases.

Virechan is one of the Shodhan Chikitsa described in ayurvedic texts. It is mainly advised in the pitta and rakta Pradhan diseases. All skin diseases in ayurveda have been considered under kustha. At present time antibiotics and antifungal medicines are being resistant, so it is becoming difficult day by day to cure such skin diseases, but in Ayurveda, Virechan karma is a very powerful therapy to combat these diseases. Skin is the index of our body. Healthy skin represents a healthy body. Improper usage of foods and drinks along with unhealthy activities mani- fest diseases. Virechan karma is the second therapy in the sequence of panchakarmas, the most widely used pu- rificatory procedure especially for paitika and raktas diseases. It is also useful in the disorders in which pitta is associated with vata or kapha. Ayurveda states that sluggish liver (dysfunction of the liver) associated with doshic imbalance causes skin diseases. There are so many skin diseases that are not curable by other procedures. Accord- ing to Ayurveda Virechan karma is very much beneficial in curing the different obstinate skin diseases as psoria- sis, eczema, acne vulgaris, vitiligo, leukoderma etc. Ayurveda always looks to the root cause so the patient can restore their body to a healthy state and eliminate the symptoms of skin diseases. AbstractBackgroundAtopic dermatitis (AD) and psoriasis, two chronic inflammatory skin diseases, affect a large number of individuals worldwide, and are associated with various comorbidities. Circular RNA (circRNA) constitute a major class of non-coding RNAs that have been implicated in many human diseases, although their potential involvement in inflammatory skin diseases remains elusive.ObjectivesTo compare and contrast the circRNA expression landscapes in paired lesional and non-lesional skin from psoriasis and AD patients relative to skin from unaffected individuals. Moreover, to correlate circRNA expression to disease severity.MethodsWe analyzed high-depth RNA-seq data from paired lesional and non-lesional skin samples from 27 AD patients, 28 psoriasis patients, and 38 healthy controls. CircRNAs and their cognate linear transcripts were quantified using the circRNA detection algorithm, CIRI2.ResultsWe identified 39,286 unique circRNAs in total and found that psoriasis and AD lesional skin could be distinguished from non-lesional and healthy skin based on circRNA expression landscapes. In general, circRNAs were less abundant in lesional relative to non-lesional and healthy skin. Differential expression analyses revealed many significantly downregulated circRNAs, mainly in psoriasis lesional skin, and a strong correlation between psoriasis and AD-related circRNA expression changes was observed. A subset of circRNAs, including ciRS-7, was specifically dysregulated in psoriasis and show promise as biomarkers for discriminating AD from psoriasis.ConclusionPsoriasis and circRNA transcriptomes share expression features, including a global downregulation, but only psoriasis is characterized by several circRNAs being dysregulated independently of their cognate linear transcripts.
