Immunosuppressive drugs are the main stay of treatment for autoimmune dermatoses. The main disadvantage of these drugs is the increased susceptibility to life-threatening infections. Hence, in recent years, there has been an enthusiastic search for newer groups of drugs that can reduce this risk. Immune enhancing agents are considered as the key players of future. Immune enhancers function by activating various elements of the immune system and thereby amplifying the immune responses. They can be specific or non-specific in action. The main autoimmune dermatoses where the benefits of these drugs have so far been utilized include alopecia areata, vitiligo, psoriasis, lichen planus, and discoid lupus erythematosus. Immunostimulants are available in both topical and systemic forms. Topical immune- enhancing agents include contact sensitizers (diphenylcyclopropenone, dinitrochlorobenzene, and squaric acid dibutyl ester), anthralin, topical zinc, and interferons. Systemic agents include levamisole, zinc, probiotics, and so on. The exact mechanism of action of some of these drugs and other autoimmune conditions where they can be benefited is not completely understood. Another therapeutic agent that may come up in the future is individualized vaccines. Let us look forward to the days when individualized vaccines work wonders in the management of autoimmune diseases.

<p class="abstract">Presentation of two papulosquamous disorders in a same individual is rare condition till date. Independently, psoriasis and Lichen planus (LP) are common inflammatory skin conditions affecting around 2-3% and 1% of HIV (Human immune deficiency) positive population respectively. As reviewed in the literature, psoriasis may be independently associated with other autoimmune conditions like vitiligo, alopecia areata, lichen planus, and discoid lupus erythematosus. In this article, we presented a case report of a HIV seropositive patient who suffered from psoriasis and lichen planus. The coexistence of psoriasis and lichen planus in one individual is rare and underreported in literature. Psoriasis or lichen planus may be the presenting feature of HIV infection and tends to be more severe, to have atypical presentations. Psoriasis and lichen planus can be coexistent or successionally appear one after other in one individual though rare presentation. High index of suspicion is always required while dealing with papulosquamous lesions in PLHIV.</p><p> </p>

Dermatoscopy is a valuable auxiliary non−invasive method used in the diagnosis of inflammatory, parasitic and viral skin diseases. Treatment of dermatoses is based on the results of analysis of melanin, follicular−horny and vascular components. Diagnosis begins with polarized dermatoscopy and then progresses to non−polarized using immersion fluid. At dermatoscopic inspection of a psoriatic plaque the point vessels evenly distributed along all the surface (a symptom of "scattered red pepper") are noted. Eczema is characterized by focal accumulation of blood vessels in the form of dots, peeling, yellowish crusts. Examination of discoid lupus erythematosus foci often reveals individual linear or branched vessels, their location is random. Red herpes zoster is dermatoscopically characterized by vascular structures in the form of large granular horny plugs of whitish color with a pearly sheen. The most informative is dermatoscopy in the differential diagnosis of erythematous form of rosacea and seborrheic dermatitis. On the erythematous background, dilated vessels around the sebaceous hair follicles, large vascular polygons formed from vessels thicker than in healthy skin and seborrheic dermatitis are found. At inspection of the fresh centers of a sclero−atrophic lichen diffuse unstructured zones of white color with a peripheral erythematous corolla and with numerous light comedic structures on a surface are visualized. At dermatoscopy of the Little − Lassueur syndrome in follicular papules on skin gray, violet points located in the form of a circle are noted. Dermatoscopy is increasingly used in dermatology, especially in the differential diagnosis of dermatoses of inflammatory and parasitic nature.

Vitiligo vulgaris is a disease with multiple etiological factors including genetic, autoimmunity and several environmental factors. It has a huge psychological burden in the patient. Discoid lupus erythematosus (DLE) in the other hand can be quite disfiguring as well. Thyroid disease along with these chronic skin diseases if occurs in a same patient is really disappointing to him and his family. We report a case of 55years old man with such a rare coexistence in a patient.

Dermoscopy is a helpful non invasive bedside technique in clinical dermatology practice which allows us to make a quick and accurate diagnosis of many complicated and atypical skin diseases. Hence its acquaintance is imperative for all the dermatologists. The aim of this article is to make the science of dermoscopy lucid and easy. This article is intended to highlight the common signs in dermoscopy which will foster the usage of dermatoscope in regular cutaneous examination. The conditions discussed in this review article are psoriasis vulgaris, lichen planus, lichen planus pigmentosus, vitiligo, pityriasis versicolor, eczema, pityriasis rosea, melasma seborrheic keratosis, discoid lupus erythematosus ,prurigo nodularis, lupus vulgaris, alopecia areata , nevus sebaceous, dermatofibroma, pigmented purpuric dermatosis and cutaneous small vessel vasculitis.

Tinea incognito is a variant of dermatophyte infection of the skin modified by an erroneously applied topical or systemic steroid. Aim of the study: to describe the various clinic-epidemiological aspects of tinea incognito found among our patients. A prospective study was carried out in the Department of Dermatology, Basra Teaching Hospital, Basra, Iraq. The clinical diagnosis was confirmed by mycological tests. Clinical data were described in more detail and the lesions were classified according to the site, shape, and extent. Ninety cases of tinea incognito were seen, median age was 34years. The initial inaccurate diagnoses were eczema in 60 patients (67%), intertrigo in 16 (18 %) and psoriasis in 14 (15%) patients. The type of topical drugs applied was topical corticosteroid (potent and fluorinated) in most cases (54.4%), fixed drug combination creams in 36.6% and a topical calcineurin inhibitor in 5.5%. Commonly presented as acute eczema-like, on hands and trunk, discoid lupus erythematosus-like lesions on the face and psoriasiform lesions on the scalp. Due to the wide range of clinical manifestations, tinea incognito imitates many skin diseases and should, therefore, be considered in any chronic, erythematous, scaly skin lesions not responding to topical treatment.

ABSTRACT Chronic granulomatous disease is genetic disorder characterized by the inability of phagocytes to produce sufficient oxidative burst needed to kill intracellular organisms. Patients have recurrent, life-threatening infections involving multiple systems including the lungs, skin, lymph nodes, and liver. The majority of patients with chronic granulomatous disease are diagnosed in childhood although some may present in adulthood due to a milder phenotype. Unfortunately, these patients may also present with concomitant autoimmune diseases. We describe a 48-year-old woman with a history of immune thrombocytopenia and systemic lupus erythematosus on immunosuppressive therapy. She developed subsequent bacterial and fungal infections initially attributed to immunosuppressive drugs. Further evaluation revealed the diagnosis of chronic granulomatous disease. We review the diagnosis and treatment of chronic granulomatous disease in hopes to increase awareness of this disease in adulthood in order to initiate potential life-saving prophylactic antibiotics.

Background: Discoid lupus erythematosus (DLE) is a common canine autoimmune skin disease, in which systemic manifestations are absent. Skin Lesions are usually present on the nasal planum, and characterised by erythema, depigmentation, erosion, ulceration, and crusting. The diagnosis is based on histopathological results, which should demonstrate lymphoplasmacytic lichenoid-interface dermatitis. Human intravenous immunoglobulin (hIVIg) has been used in veterinary medicine to treat cutaneous diseases including erythema multiforme, PF, and severe adverse cutaneous drug reactions. In human medicine, it has been effective to treat DLE. This report firstly describes the clinical response to hIVIg in a dog with DLE resistant to common immunosuppressive drugs.Case: A 5-year-old, intact female Shih Tzu presented with a 1-month history of slowly progressive black crusting on the nasal planum, chin, and claw. Based on the results of a dermatologic examination, superficial pyoderma was diagnosed. The skin lesions did not improve during and after anti-infective treatment. After removing the crusts, a skin biopsy was obtained from the muzzle. Histopathology of lesional skin biopsy specimens revealed lymphoplasmacytic interface dermatitis at the dermoepidermal junction. Microscopic examination also revealed vacuolar changes and pigmentary incontinence of the basal layer as a lichenoid tissue reaction. No mites or fungi were detected on the skin section. The absence of acantholytic cells excluded pemphigus foliaceus, which is also characterised by the lesions of the nasal planum. Based on the distribution of the lesions, histopathology and exclusion of other dermatoses, the dog was diagnosed with DLE. The skin lesions temporarily improved after treatment with prednisolone (2 mg/kg PO q12h). However, after tapering the dose of prednisolone, new black crusts developed on the nasal planum and claw. Although the dog was successively treated with other immunosuppressive drugs, including azathioprine, cyclosporin with dexamethasone, and mycophenolate mofetil, black crusts still remained. Due to the low efficacy of these immunosuppressive drugs, hIVIg was administered at 0.5 g/kg once daily for 4 days, for a total dose of 2 g/kg. During hIVIg administration, the crusted lesions gradually improved. After the hIVIg administration, the dog was treated with prednisolone (1 mg/kg PO q12h). The lesions were almost in complete remission at 21 days after an additional application of prednisolone. The skin lesions did not recur, and the treatment was eventually discontinued after 6 weeks of additional prednisolone application.Discussion: The standard treatment of canine DLE includes glucocorticoids, and second-line immunosuppressive drugs, such as azathioprine and cyclosporine, are usually added in cases resistant to steroids. This case suggests that hIVIg may be beneficial as an adjunctive treatment option for canine DLE, especially when the application of standard immunosuppressive drugs is limited due to adverse effects or low efficacy. There is evidence from several studies that the steroid-sparing effect of hIVIg is significant in human patients. In the current case, the effective dose of prednisolone was reduced to 2 mg/kg/day after hIVIg administration, and prednisolone therapy was finally discontinued completely. The hIVIg appears to lower the daily steroid dose requirement in this dog. Keywords: autoimmune skin disease, discoid lupus erythematosus, canine dermatology, immunosuppressive drug, human intravenous immunoglobulin.

<p class="abstract">Discoid lupus erythematosus (DLE) is an autoimmune disease characterized by atrophic and discoid plaques over sun-exposed areas of skin and is the most common form of chronic cutaneous lupus erythematosus. Vitiligo is also an autoimmune disease known to be associated with other autoimmune conditions. However coexistence of DLE and vitiligo has been reported uncommonly. We report a case of a 60-year-old lady who developed DLE lesions over pre-existing vitiligo lesions.</p>
