The census enumerators' books (CEBs) have provided fertile ground for studies of workhouse populations in recent years, though it has been acknowledged that work remains to be done on different regions and periods to develop our understanding of these institutions and the paupers who resided therein. This article will examine the indoor pauper populations of the Preston union, in Lancashire, over three census years from 1841. The region, which is notable for a protracted campaign of resistance to the New Poor Law and its associated workhouse system, has been previously neglected in studies of workhouse populations focusing on the decades immediately after the Poor Law Amendment Act of 1834. It will be shown that the profile of the union's workhouse populations broadly mirrors those found elsewhere at the aggregate level, but that important variations reflected local and central policy. A high concentration of able-bodied paupers—in particular—seems to indicate ideas governing local policy which were not carried out elsewhere.

The article considers the concept of startups and ecosystems of startup development. It investigates the innovativeness ratings of countries and regional ecosystems of startups and the factors influencing the successful development of startups. The article considers the ranking of the best regional startup ecosystems in the world. It covers the experience of the leading regions, whose effectiveness is driven by attention to financing, networking, expanding access to markets, attracting and nurturing talents, accumulating experience and scientific development. Particular attention is given to the impact of the specialization and effectiveness of the regional focus strategy. Based on the methodology and results of the Global Startup Ecosystem Ranking 2019, authors have empirically investigated the startup ecosystem of the Lviv city. Authors described and systemized the startups operating in the city, the elements, and dynamics of the urban startup infrastructure. The preconditions for successful development of startups - financing, talents, experience, connectedness, access to markets - have been identified. The effectiveness of the activity of city authorities, the local policy of promoting the startup environment development - documents, measures and results – have been investigated. The main factors of Lviv’s regional leadership in the development of digital startups in Ukraine are: the dynamic development of the IT industry, the development of digital competencies and the concentration of talent in leading universities, the development of corporate universities and innovative infrastructure, the development of infrastructure and comfort in the city. For the development of the Lviv startup ecosystem, it is recommended to focus the efforts on further accumulation and transfer of experience, retention of talents, improvement of technology transfer systems, focusing on areas of exclusive competence while expanding access to finance and the global market.

A three-tier division ofthe local self-government creates opportunities for engagement of community in the rational planning model and local resource management, as well as usage of production factors; for socio-economic growth and improvement in the quality of life of the residents. The aim of the article is to show the functional structure and role of local government units in formation of regional and local policy toincrease economic growth, while maintaining the environmental protection requirements.

Abstract Background Improving children and young people’s (CYP) health and addressing health inequalities are international priorities. Reducing inequalities is particularly pertinent in light of the Covid-19 outbreak which has exacerbated already widening inequalities in health. This study aimed to explore understandings of inequality, the anticipated pathways for reducing inequalities among CYP and key factors affecting the development and implementation of policy to reduce inequalities among CYP at a local level. Methods We carried out a qualitative case study of one local government region in the North of England (UK), comprising semi structured interviews (n = 16) with service providers with a responsibility for child health, non-participant observations of key meetings (n = 6 with 43 participants) where decisions around child health are made, and a local policy documentation review (n = 11). We employed a novel theoretical framework, drawing together different approaches to understanding policy, to guide our design and analysis. Results Participants in our study understood inequalities in CYP health almost exclusively as socioeconomically patterned inequalities in health practices and outcomes. Strategies which participants perceived to reduce inequalities included: preventive support and early intervention, an early years/whole family focus, targeted working in local areas of high deprivation, organisational integration and whole system/place-based approaches. Despite demonstrating a commitment to a social determinants of health approach, efforts to reduce inequalities were described as thwarted by the prevalence of poverty and budget cuts which hindered the ability of local organisations to work together. Participants critiqued national policy which aimed to reduce inequalities in CYP health for failing to recognise local economic disparities and the interrelated nature of the determinants of health. Conclusions Despite increased calls for a ‘whole systems’ approach to reducing inequalities in health, significant barriers to implementation remain. National governments need to work towards more joined up policy making, which takes into consideration regional disparities, allows for flexibility in interpretation and addresses the different and interrelated social determinants of health. Our findings have particular significance in light of Covid-19 and indicate the need for systems level policy responses and a health in all policies approach.

Abstract Introduction Cauda equina syndrome (CES) is a spinal emergency that cannot be reliably detected through clinical examination alone and as a result requires prompt MR imaging to provide a diagnosis. This audit examined compliance to standard of care following service improvements in line with the updated SBNS/BASS national guidelines for CES. Method A retrospective analysis of 200 patients referred to neurosurgery for suspected CES: 100 pre- and 100 post-service improvement SBNS guideline implementation. The online neurosurgical database was reviewed, cases assessed for completeness of referral information (including appropriate exam and pre-referral MRI) with patient demographics, referring hospital and outcome also recorded. Results Prior to the SBNS guidelines only 19 patients received MRI prior to referral, 70% of all referrals were incomplete or contained erroneous clinical information. Post-service improvements there was a 68% increase of pre-referral MRI (32 cases), and an improvement in quality of clinical information with only 19% of referrals providing insufficient or unreliable information. Conclusions Through relatively simple changes to local policy, patient care flow and education of emergency department clinicians we have significantly improved pre-referral MRI rates as well as overall referral quality across the whole DGH network.

Abstract Despite increasing research into populist parties in power, their impact on subnational institutions has been neglected. Taking a novel multilevel perspective, this article inquires into the policy consequences of populist radical right parties (specifically, the FPÖ and Lega) in local government, and the effect of their simultaneous participation in national government. The article shows the expansion of exclusionary policy that follows their concurrent presence in national and local government. The process that leads from national government entry to local policy influence is traced using interview and newspaper data. The article argues that the influence of central parties over these ‘showcase’ localities is rooted in different multilevel governance configurations. These vary cross-nationally according to two factors: the strength of mayors’ linkages with higher government levels in the different institutional settings and, due to the different extent of party nationalization, the strategic value of the municipality to the central party.

Abstract Background Global guidelines recommend exclusive breastfeeding (EBF) for the first 6 months of life. South African EBF rates have steadily increased but still only average 32% for infants below 6 months of age. Malnutrition and developmental delays continue to contribute substantially to the morbidity and mortality of South African children. MomConnect, a national mHealth messaging system used to send infant and maternal health messages during and after pregnancy, has a specific focus on improving rates of breastfeeding and has achieved high rates of population coverage. Methods For this qualitative study, we interviewed women who were registered to MomConnect to investigate their breastfeeding and other infant feeding practices, decision-making pre- and post-delivery, and the role of the health system, family members and the wider community in supporting or detracting from breastfeeding intentions. Data were collected from February–March 2018 in South Africa’s KwaZulu-Natal, Free State and Gauteng provinces. Framework analysis was conducted to identify common themes. Results Most women interviewed had breastfed, including HIV-positive women. Even when women had delivered by caesarean section, they had usually been able to initiate breastfeeding a few hours after birth. Understandings of EBF varied in thoroughness and there was some confusion about the best way to cease breastfeeding. Most women felt well-equipped to make infant feeding decisions and to stick to their intentions, but returning to work or school sometimes prevented 6 months of EBF. Advice from the health system (both via clinics and MomConnect) was considered helpful and supportive in encouraging EBF to 6 months, although family influences could thwart these intentions, especially for younger women. Mothers reported a range of breastfeeding information sources that influenced their choices, including social media. Conclusions Efforts to improve EBF rates must include consideration of the social and economic environment surrounding women. Interventions that focus only on improving women’s knowledge are valuable but insufficient on their own. Attention should also be paid to infant behaviors, and how these affect women’s breastfeeding choices. Finally, although there is strong local policy support for EBF, more rigorous implementation of these and other broader changes to create a more enabling structural environment ought to be prioritized.
