Abstract For a group of normal medical students, we examined the effect of posture on the concentration of a number of constituents in plasma. On standing, there is a significant increase in plasma total protein, albumin, calcium, alkaline phosphatase, bilirubin, and cholesterol--all proteins or substances bound to protein. Although it is possible to make an allowance for postural variations in plasma calcium, no correction is possible for changes in protein concentrations and care is needed whenever the precise values are important, as in the follow-up of patients with the nephrotic syndrome.

Nephrotic syndrome (NS) is a complex kidney disease associated with numerous complications which can subsequently lead to cardiovascular disease among others. This study was aimed at assessing the lipid profile, serum and urinary proteins of patients with Nephrotic syndrome (NS) in Kano metropolis. A total of 50 NS patients and 25  apparently healthy volunteers(controls) were recruited for the study, made up of 32 males and 18 females with the age range of 4-70 years. Blood and urine sample were collected from the participants. Serum urea and creatinine were determined using urease  berthelot’s reaction and Alkaline picrate methods. Serum total protein and albumin were assayed using Biuret and bromocresol green binding method through the manual colorimetrictechnique. Serum lipid profile were measured by an enzymatic spectrophotomeric method and the precipitation enzymatic method was specifically used for evaluating the levels of high-density lipoprotein cholesterol (HDL-C). Urine protein was determined using sulphur salicylic acid test. SPSS software package version 21 was used for the analysis of data. High frequency of NS of 40(80%) was observed in patients of <18 years while patient of >46 years had a lower frequency of 4(8%). Males recorded higher frequency of 32(64%) and the frequency of NS among females was 18(36%), thus the male to female ratio for NS was 1.78:1. The mean values of serum creatinine, urea, urinary protein (UP), total cholesterol (T.C), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), T.C/HDL-C, TG/HDL-C, LDL/HDL-C ratio were significantly higher (p<0.05) in patients with NS than the healthy volunteers. Total protein (TP), albumin (Alb), globulin, HDL-C ratio were significantly lower (p<0.05) in NS when compared to healthy volunteers. There was positive correlation between UP and TC, TG and LDL, however, negative correlation was observed between UP and HDL-C with no statistical significance. Dyslipidemia, decreased serum protein and increase serum creatinine, Urea as well as UP were associated with NS. In conclusion, lipid profile and UP analysis may be a useful tool for diagnosis of NS and early diagnosis can reduce the disease morbidity.

Background: Quantification of proteinuria is usually predicted upon 24-hour urine collection. Multiple factors influence urine collection and the rate of protein and creatinine excretion. A spot urine protein-creatinine (P-C) ratio has been shown over the years to be a reliable alternative to the 24-hour collection for detection and follow up of proteinuria. The objective of the study was to evaluate the accuracy of urine protein creatinine ratio (UP/UC) in a spot sample for quantitative measurement of proteinuria in comparison with 24 hours urinary protein excretion in children of nephrotic syndrome having normal Glomerular Filtration Rate (GFR). Methodology: This was a prospective study conducted in the department of paediatrics, Sir Salimullah Medical College & Mitford Hospital Dhaka over a period of six months from January 2003. Fifty cases of Nephrotic syndrome were included who were on initial attack and relapse cases noted down into the proforma with respect to history, examination and investigation. All the patients were advised regarding 24 hours urine collection. They were asked to give a 24 hours urine sample starting at 9.00 am for total protein excretion rate. A spot urine sample was obtained and urine protein/creatinine ratio was calculated. The data was analyzed by linear regression and by calculating the correlation coefficient between urinary protein/ creatinine ratio and 24-hour urinary protein. Results: Sample size was fifty. Urine total protein in a timed 24-hour sample of nephrotic syndrome patients was in the range of 300-3150mg/m2/hour with the mean value of 1725 mg/m2/hour. While as U(Pr/Cr) ratio ranged from 3.1-27.5 with the mean value of 15.2. A significant correlation was found between timed 24-hour urinary protein and UP/UC ratio (r=0.622, p=<.001.) Conclusions: Spot urine protein-creatinine ratio is highly reliable and rapid test for quantification of proteinuria in children with nephrotic syndrome. A 22-month-old female child with maple syrup urine disease (MSUD) presented with generalised oedema. Diagnostic evaluation revealed nephrotic range proteinuria, hypoalbuminaemia and dyslipidaemia supporting the diagnosis of nephrotic syndrome (NS). Diet, being at the core of the management plan for both MSUD and NS, necessitated regular monitoring and evaluation via dried blood spot collection of leucine. The opposing requirement for total protein for both disorders (that is protein restriction in MSUD and protein supplementation in NS) prompted a careful balancing act of the dietary management. The monitoring, which revealed normal leucine levels on multiple determinations, allowed an eventual increase in dietary protein and daily administration of albumin to address the NS. Dietary protein increase, both in total protein (3.5 g/kg/day) and natural protein (1 g/kg/day) levels, was instituted. It was observed that NS does not trigger leucinosis and allowed easing of protein restriction in MSUD.

Eprinomectin is a broad spectrum endectocides used against gastrointestinal, pulmonary nematodes and ectoparasites in cattle. The main objective of the present study was to investigate effects of eprinomectin in cows following subcutaneous and pour-on administrations; on serum DNA apoptosis, acute phase response, Total Antioxidant Status (TAS), total protein, creatinine, urea levels, and AST, ALT and GGT activities. Ten Holstein cows were divided into two groups. Group 1 received subcutaneous (0.2 mg/kg) and group 2 received pour-on (0.5 mg/kg) administration of eprinomectin. Blood samples were collected from vena jugularis prior to and following the drug administration at intervals of 36 hrs, 3rd, 6th, 12nd and 20th days, respectively. Samples were centrifuged at 2500 g for 15 minutes and separated sera stored at -20 ºC. Results showed that the eprinomectin application decreases the sera levels of acute phase proteins. Statistically significant increase was observed in antioxidant capasity (P less than 0.05) and total protein (P less than 0.01). However, there was non-significant increase in apoptosis rate. No remarkable alterations were observed in AST, ALT and GGT activities including urea and creatinine levels. Results showed that the eprinomectin application reduces the sera levels of acute phase proteins (ceruloplasmin and sialic acid) in cows. However, statistically significant increase was measured in antioxidant capasity (P less than 0.05) and total protein (P less than 0.01) levels. It could be concluded from the study that eprinomectin do not have adverse effects on liver and kidney.

Medicinal plants are efficient modulators of oxidative stress associated with diabetes mellitus. This study evaluated the cardio-, reno-, and hepato-antioxidant status of hydroethanolic extract of Costus afer on streptozotocin-intoxicated diabetic rats. Experimental animals were daily administered with hydroethanolic extract of C. afer by oral intubation for eight weeks (60 days), after which the levels of catalase (CAT), superoxide dismutase (SOD), glutathione (GSH), and lipid peroxidation marker (MDA) were evaluated in the heart, liver, and kidney homogenates. Plasma biochemical parameters such as aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH), total protein, creatinine, and urea were determined. Meanwhile, parts of the heart, kidneys, and liver were histopathologically examined. Streptozotocin administration induced toxicity in the cardiac, hepatic, and renal tissues by stimulating significant increases (p<0.05) in the levels of CAT and SOD, GSH, and MDA. Similarly, significant increases (P<0.05) in the levels of ALT, AST, urea, and total protein were observed in streptozotocin treated rats, whereas decreases were observed in the levels of ALP, LDH, and creatinine. Following the treatments with C. afer hydroethanolic extract prevented the effect of streptozotocin by maintaining the tissue antioxidant status (CAT, SOD, GSH, and MDA) and the plasma biochemical parameters (AST, ALT, ALP, LDH, creatinine, and urea) towards the normal ranges. The histopathological examination revealed hepatovascular congestion and leucocyte infiltration as well as renovascular congestion, glomerulosclerosis, and tubular clarification in the untreated diabetic control and their absence in the group of animals treated with a high dose of C. afer extract. The findings of the present investigation suggest that C. afer possesses antioxidant activities capable of regulating drug induced tissue damage.

ABSTRACT Background: Healing ability of nonhealing chronic ulcers can be assessed by estimating hydroxyproline, total protein and enzymatic antioxidants such as glutathione peroxidase (GPx), glutathione S-transferase (GST) in the granulation tissue. Materials and Method: A total of 34 patients were analysed from two groups: Limited access dressing (LAD) group (n = 17) and conventional dressing group (n = 17). Results: Patients treated with LAD that exerts combination of intermittent negative pressure and moist wound-healing had shown a significant increase in the hydroxyproline (P = 0.026), total protein (P = 0.004), GPx level (P = 0.030) and GST level (P = 0.045). Conclusion: Patients treated with LAD indicated significantly better anabolic effect on wound-healing compared to that of patients treated with conventional dressing.
