Characteristics of Muscle Recovery under Experimental Post-Traumatic Contracture

The frequency of traumatic injuries of the maxillofacial area and the degree of their severity, have been increasing in recent years. The introduction of modern methods of treatment made it possible to improve the results of treatment and shorten its duration. Myogenic contractures develop as a complication after facial injuries, soft tissue injuries, in particular, masticatory muscles. The gastrocnemius muscle of a rat has the same striated structure as the pterygoideus medialis muscle of a person. The purpose of the work is to study the regeneration of striatal muscles with post-traumatic contractures on the basis of combined treatment of laboratory rats. Materials and methods. The study was carried out on 45 sexually mature 12 months old outbred white male rats, which weighed 180-200 g. In animals, reflex post-traumatic muscle contractures (traumatic myositis) resulting from the slaughter of calf muscles (without violating the integrity of muscle tissue) were simulated on the posterior left leg. The rats were divided into two groups. Group 1 included 21 animals that did not receive treatment and whose wounds healed by secondary intention. Group 2 included 24 rats that received combined treatment: on the 2nd day after surgery – general (oral) administration of the drug with the active substance cyclobenzaprine hydrochloride – 0.15 mg, once a day with daily procedures of magnetic laser therapy on the affected hind limb. Results and discussion. Depending on the signs of pain behavior (changes in behavior, restriction of movement, increased sensitivity, pain), the intensity of myogenic pain syndrome was assessed (low, moderate, high, very high). It was found that the use of combined treatment (magnetic laser therapy and the use of cyclobenzaprine hydrochloride) led to the normalization of behavioral and motor reactions by the 28th day: 56% of rats didn’t feel any pain and 44% had a low level of pain. The animals with the same trauma, which weren’t treated, showed a high and moderate level of pain. On the 28th day these animals could lean on their hind limbs while they were moving. However, they could not fully move due to a sharp restriction of their left hind limb function. The obtained results are the basis for the development of new approaches to improving the methods of prevention and rehabilitation treatment of contractures using magnetic laser therapy and central muscle relaxants. Conclusion. It was found that the use of magnetic laser therapy and cyclobenzaprine hydrochloride inhibited the development of an acute inflammatory reaction in the damaged muscle tissue of the hind limb of animals, accelerated and optimized reparative processes, which prevented excessive, functionally significant growth of connective tissue

Among the current medical and social problems, injuries and blood loss occupy a prominent place, causing stress on the antioxidant defenses. Hypoxia, which underlies the pathogenesis of the post-traumatic period of both diseases, leads to a significant imbalance in the work of internal organs. Scientists are increasingly attracted by the need to use a tourniquet or intraoperative ligatures, as reperfusion local and systemic damage develops. Antioxidants are considered a promising means of correction.The aim of the study was to investigate the features of metabolic disorders in the liver in the early post-traumatic period on the background of the use of a tourniquet and the effectiveness of thiocetam correction.Materials and methods. The experiment was perfomed on 130 white male rats (200-250 g), which were divided into 4 groups: control – the CG, the EG-1 – combination of limb ischemia-reperfusion (IR) with blood loss, the EG-2 – combination of limb IR with blood loss and mechanical trauma of the thigh; the EG-3 combination of limb IR, blood loss, mechanical injury and thiocetam administration. The Malonic dialdehide level catalase activity were estimated in the liver.Results. The use of thiocetam, which is able to struggle against of ischemia and lipid peroxidation by reactivating antiradical enzymes: superoxide dismutase, catalase and glutathione peroxidase, had a positive effect on the state of antioxidant and prooxidant units in the organ, located far from the place of primary ischemia-reperfusion. If in the group of untrated animals (the EG-2, where massive blood loss was combined with a thigh fracture and the use of hemostatic tourniquet) in the early period, the MDA level exceeded the CG data in 5,4 times, and on the 7th and 14th days remained high – being higher on 2,1 times and on 2,7 times, then in the EG-3 (group of treated animals) on the 1st day the level of MDA exceeded the CG data in 4,3 times, but on the 7th and 14th days was higher by 90,5 % and 64 % respectively. The supportive effect of thiocetam on the activity of catalase in the liver was also noted. Thus, in EG-2 the level of antioxidant enzyme on the 1st day decreased by 71,7 %, and remained almost at this level throughout the all post-experimental period. As for the group of treated animals, the level of activity on the 1st day after the intervention decreased by 44,7%, and was so for almost the entire period. On the 14th day, it remained reduced compared to the CG by 35,1 %, while in EG-2 this index was lower compared to the CG by 70,5 %.Conclusion. Having the positive effect of the introduction of thiocetam in the ischemic area, we can eventually add new complex, given the world experience, which would affect the development of the inflammatory response and the rheological properties of blood.

Aim: The aim of the study was to assess the effectiveness of magnetic-laser therapy in complex treatment of patients with zygomatic fractures accompanied by the infraorbital nerve damage. materials and methods: The study included 32 patients with single fractures of the zygomatic bone accompanied by infraorbital nerve injury. Patients were divided into two groups: the main group (n=17) and the control one (n=15). In both groups the complex treatment was used, which included: the fragment reposition operation, broad-spectrum antibiotic, nonsteroidal anti-inflammatory drug, anti-edematous therapy, vitamin B complex. In the main group of patients in the postoperative period the magnetic-laser therapy was applied. Assessment of qualitative and quantitative indices of pain was carried out on the 1st, 7th and 14th days of treatment. Results: In the main group of patients the magnetic-laser therapy significantly reduced the intensity of nociceptive pain, led to moderate regression of all types of neuropathies associated with infraorbital nerve damage. When comparing 2 groups the best pain relief had patients of the main group. At the end of treatment a significant positive dynamics of improving the electrical sensory of infraorbital nerve has been noted, as indicated by recovery of sensory threshold, pain threshold and pain tolerance almost to the level of intact contralateral side. Conclusions: The use of magnetic-laser therapy in the combined treatment of infraorbital nerve damage allows to improve the recovery of infraorbital nerve sensory changes following zygomatic fractures, reduce the pain, increase the effectiveness of treatment and provide rapid postoperative rehabilitation of patients.

The aim of the work. To find out the dynamics of indicators of biligenic function of the liver in response to cranial skeletal trauma associated with haemorrhage. Materials and Methods. Experiments were performed on 54 outbred white male rats, weighing 180–200 g. Animals were divided into three groups: a control one and two experimental. Closed cranio-skeletal injury according to the established technique in our own modification was mo­delled under thiopental-sodium anaesthesia. A single hit on each hip that caused a closed fracture of femoral bones, was performed by means of an individually designed device. The striking force was 0.375 J, which corresponded to a moderate injury. Results and Discussion. It has been first established that the complication of cranio-skeletal trauma with additional blood loss exa­cerbates and accelerates liver dysfunction. The content in bile of total bile acids, cholesterol, the degree of conjugation of bilirubin, the rate of biliary excretion and excretion of the main components of bile, as well as absorption-excretory and glycogen synthesizing functions are impaired already on the first day of the post-traumatic period and in all times of the follow-up period they significantly differ from the similar indices of the injured animals with no blood loss. Conclusions. In the presence of experimental cranio-skeletal trauma, the rates of liver biligenic function change up to a maximum declination in 7 days of post-traumatic period. Additional haemorrhage intensifies the revealed abnormalities: the content of total bile acids and cholesterol in bile, as well as the amount of direct bilirubin and the degree of its conjugation in post-traumatic period are significantly lower during the whole period of observation.

Summary. Relevance. Diabetic joint damage is common in the clinic. The blood vessels of the masticatory muscles that affect the morphogenesis of diabetic arthropathy of the temporomandibular joint remain poorly studied in this pathology. Objective: to study the features of remodeling of blood vessels of the masticatory muscles of the temporomandibular joint in experimental hyperglycemia by quantitative morphological methods. Materials and Methods. The study material was 45 laboratory mature white male rats, which were divided into 3 groups. The 1st group consisted of 15 intact apparently healthy animals, the 2nd included 15 rats with one-month diabetic angiopathy, and the 3rd had 15 rats with two-month specified pathology. Diabetes mellitus was simulated by a single intraperitoneal injection of streptozotocin from Sigma at a dose of 50 mg/kg. Euthanasia of animals was performed by bloodletting under thiopental anesthesia one and two months after the start of the experiment. The blood glucose concentration was determined. Histological micropreparations were made from the temporomandibular joint and the temporal and external pterygoid muscles. Morphometric measurements of small caliber arteries (outer diameter 26-50 μm), veins and microvessels of the hemomicrocirculatory tract were performed on muscle micropreparations; relative volumes of damaged synoviocytes and chondrocytes were determined on micropreparations of joints. Quantitative indicators were processed statistically. Results and Conclusions. It was revealed that the gleam of arteries considerably decreases, their wall thickens, and endotheliocytes are damaged in the studied muscles with the modeled pathology. Venous vessels are dilated, full-blooded with a reduced wall thickness. The most pronounced changes in these experimental conditions are the structure of microvessels, which is characterized by narrowing of arterioles, precapillary arterioles and hemocapillaries, pronounced expansion of postcapillary venules and venules. Plethora, stasis, thrombosis, irregularity of the lumen, sacculation, and varicose expansions were noted in venous microvessels. Venous fullness led to hypoxia, disorders of trophism and metabolism, dystrophy, necrobiosis of muscle fibers and stromal structures, foci of infiltration and sclerosis. It was revealed that morphological changes in the vascular bed of the masticatory muscles affect the morphogenesis of diabetic arthropathy of the temporomandibular joint.

Introduction.Coccygodynia is a multidisciplinary disease which is diffi cult to treat. It seriously limits the ability to work and signifi cantly affects the quality of life of patients. The study of somatic dysfunctions in patients with coccygodynia and the analysis of the results of osteopathic treatment of such patients makes it possible to justify the necessity of osteopathic correction of coccygodynia.Goal of the study— to determine the structure of the leading somatic dysfunctions in patients with coccygodynia and to study the effectiveness of osteopathic treatment of this pathology.Materials and methods.The study involved 44 patients from 25 to 65 years old, randomly divided into two groups. The main group of 24 people (20 women and 4 men) received osteopathic treatment, in accordance with the identifi ed leading somatic dysfunctions. Patients of the control group (16 women and 4 men) were treated locally with soft manual techniques (the treatment area was limited by the pelvic region). In order to assess the results of the treatment, we examined the intensity of the pain syndrome and the psycho-emotional state of patients. The severity of the pain syndrome was assessed in accordance with the visual analogue scale (VAS). The psycho-emotional state (with physical and mental components) was assessed with the help of the SF-36 quality of life questionnaire.Results.Somatic dysfunctions typical for patients with coccygodynia have been identifi ed. Osteopathic treatment has proven to be more effective in comparison with local manual therapy of coccygodynia both in early periods and in 3 months after the end of the treatment course.Conclusion.Osteopathic treatment of post-traumatic coccygodynia is effective, and can be recommended for treatment of such patients.

The aim of the paper is to study thymus variability in white rats, which were exposed to formaldehyde, and to compare these data with the indicators in control animals. Materials and Methods. The trial enrolled 72 white male rats, initial body weight 40–50 g. The animals were divided into 2 groups (36 rats in each). The first group consisted of control rats. Animals of the second group were exposed to formaldehyde inhalation, 2.766 mg/m3. To characterize the variability of the organ size, centroids were determined. The superposition of landmark configurations was performed using the generalized Procrustes analysis method, MorphoJ 1.06d program. The principal component analysis and canonical analysis of the obtained data were carried out. Results. One-Way ANOVA revealed a high level of intergroup differences in Procrust distance (F=1.34; p<0.0001). The significant effect of the duration of formaldehyde exposure on centroid size was established. The Kruskal-Wallis criterion was 19.778 (p=0.0014). The analysis of the principal components indicated that each of the first 10 components stands for more than 1 % of Procrustes coordinate variance. In this case, the first 7 components compatibly explain 91.398 % of thymus variability. The proportion of the first main component to the total variance of the Procrustes coordinates is 40.236 %. PC1 (-) shows changes in the thymus shape, mostly affecting the tops of its lobes, the middle part of the right boundary and the entire left thymus boundary. The scattering ellipses of the thymus ordinates in rats exposed to formaldehyde, in the first two canonical variables are located higher than those in the control animals. Conclusion. Formaldehyde inhalation leads to thymus changes in white rat. The most significant differences with control data are determined along the second canonical variable. Изучение изменчивости формы тимуса белых крыс, находившихся в условиях влияния формальдегида, и сравнение этих данных с показателями, полученными у контрольных животных. Материалы и методы. Работа выполнена на 72 белых крысах-самцах с начальной массой тела 40–50 г. Животные были разделены на 2 серии (по 36 крыс). Первую серию составляли контрольные крысы. Животные второй серии подвергались ингаляционному воздействию формальдегида (ФА) в концентрации 2,766 мг/м3. Для характеристики изменчивости размеров органов определяли размер их центроидов. Процедуру суперимпозиции конфигураций ландмарок выполняли методом генерализованного прокрустова анализа с использованием программы MorphoJ 1.06d. Проводили анализ главных компонент и канонический анализ полученных данных. Результаты. Однофакторный дисперсионный анализ выявил высокий уровень межгрупповых различий по показателю прокрустовых расстояний (F=1,34; р<0,0001). Установлено значительное влияние продолжительности нахождения животных в условиях воздействия ФА на размер центроида. Критерий Краскела–Уоллиса составил 19,778 (р=0,0014). Анализ главных компонент указывал на то, что каждая из первых 10 компонент объясняет более 1 % дисперсии прокрустовых координат. При этом первые 7 компонент совместно объясняют 91,398 % изменчивости формы тимуса. Вклад первой главной компоненты в общую дисперсию прокрустовых координат составляет 40,236 %. РС1 (–) показывает изменения формы тимуса, в большей степени затрагивающие верхушки его долей, среднюю часть правого контура и весь левый контур тимуса. Эллипсы рассеивания ординат тимусов, принадлежащих крысам, подвергавшимся влиянию ФА, в пространстве первых двух канонических переменных расположены выше по отношению к таковым контрольных животных. Заключение. Ингаляционное воздействие формальдегида приводит к изменению формы тимуса белых крыс. Наибольшие различия с контрольными данными определяются вдоль второй канонической переменной. Despite nearly a century of research on diseases affecting the temporomandibular joint (TMJ), masticatory muscles and associated structures, this problem still remains open. Pain syndrome caused by TMJ diseases ranks first in terms of prevalence among non-odontogenic pain syn-dromes in the maxillofacial region and leads to a signifi-cant deterioration in the quality of life of patients. The high prevalence of these diseases in female patients and the presence of a number of factors of a somatic, psycho-logical and social nature, contributing to such a high prevalence, indicate the need for more close attention of researchers to these aspects, which can become a way to develop new effective methods for the diagnosis and treatment of TMJ diseases in this category of patients. A review of scientific literature was carried out in order to identify topical scientific issues related to the study, di-agnosis and treatment of TMJ diseases.
