The ESSTI (European Surveillance of Sexually Transmitted Infections) network, funded since December 2001 by the European Commission’s Directorate-General for Health and Consumer Protection (DG SANCO, http://europa.eu.int/comm/dgs/health_consumer/index_en.htm), held its first collaborative group meeting at the Health Protection Agency Communicable Disease Surveillance Centre (CDSC) in the United Kingdom, on 18-19 March 2003. The meeting was preceded by a one day European conference on syphilis outbreaks (advances in surveillance, diagnosis and management), also at CDSC, at which several European collaborators gave presentations on recent national syphilis outbreaks.

Foodborne outbreaks of Salmonella enterica serovar Enteritidis phage type 4 (PT4) infection (n=497), reported to the Health Protection Agency Communicable Disease Surveillance Centre between 1992 and 2002, were compared with other pathogens (n=1148) to determine factors (season, setting, food vehicles, food safety faults) associated with this pathogen. Logistic regression was applied to control for potential confounding. Foodborne general outbreaks of S. Enteritidis PT4 infection were more likely to occur in the spring and summer, and were more often linked to schools, private residences and residential institutions. Eggs, egg products and the use of raw shell egg were strongly associated with this pathogen. Most outbreaks were linked to cross-contamination and inadequate heat treatment. This paper describes the decline in the S. Enteritidis PT4 epidemic, providing evidence that control measures introduced, e.g. improved biosecurity and vaccination, have worked. Continued surveillance of human and veterinary salmonellosis is essential to detect future problems.

During an ongoing investigation into a nosocomial outbreak of S. Enteritidis PT 6a (Nx, CpL) in London, where raw shell eggs were being used, the London Food, Water and Environmental Microbiology Laboratory (LFWE) tested 402 raw shell eggs obtained from the premises. Two hundred and forty of these were labelled as imported, and four of the 40 samples of six pooled whole eggs were positive. The Laboratory of Enteric Pathogens (LEP) at the Public Health Laboratory Service Communicable Disease Surveillance Centre (PHLS CDSC) confirmed the presence of S. Enteritidis PT 6 (one sample), S. Enteritidis PT 13a (one sample) and S. Enteritidis PT 14b (one sample). Of the 27 samples from the unlabelled eggs (162 eggs) one sample was positive for S. Enteritidis PT 6. Results are awaited on the final isolate. Since the raw eggs have been withdrawn from use no further cases have occurred. Hospitals in the United Kingdom are reminded that advice issued by the Chief Medical Officer in 1988 that raw shell eggs should be replaced with pasteurised eggs in recipes in institutions with high risk groups (1) is extant and must be adhered to if vulnerable patients are not to be put at risk.

Diagnoses of Shigella flexneri in the United Kingdom (UK) are usually travel-related. However, since 2009, there has been an overall increase in UK-acquired cases. The Health Protection Agency has been investigating a national outbreak of S. flexneri detected in 2011 and which is still ongoing. Cases occurred mostly in men who have sex with men and were of serotype 3a. The investigation aimed at obtaining epidemiological data to inform targeted outbreak management and control.

Five clinical cases of wound botulism have been reported to the Public Health Laboratory Service (PHLS) Communicable Disease Surveillance Centre and the Scottish Centre for Infection and Environmental Health since the beginning of February 2002 (1,2).
