Back ground: Pre pubertal labial adhesion is an important pediatric gynecological problem. Parents panic about this condition as absent vagina, which is rather very easy to handle in a very simple, rapid, surgical management.Objective: the purpose of this study was to evaluate the efficacy of surgical treatment on prepubertal girls with labial adhesions.Methods: A period of 3 years from May 2011 to May 2014 was taken to conduct a study about this condition on prepubertal girls. The place of study was Nepalgunj Medical College, Kohalpur, Banke, Nepal. All patients underwent either outpatient surgical treatment or in operation theatre under short anesthesia, those who were either frightened or non cooperative. They were placed in gynecological (dorsal) position. A thin stainless steel lubricated probe or 1-2 mm. size, Hegar dilator was used to separate the labia minora by firm pressure.Result: All patients were successfully treated and at present they are being followed for recurrence.Conclusion: This benign disorder can be treated by any gynecologist as an office procedure.JNGMC Vol. 12 No. 2 December 2014

Labial fusion is sealing of labia minora in midline, also known as labial adhesion or labial agglutination or synechia vulvae. This condition is common in pre-pubertal females usually asymptomatic when oestrogen levels are low and commonly resolves spontaneously post-puberty if unresolved medical treatment includes use of estrogen cream or betamethasone cream application, very rarely surgical treatment required, if not responding to medical treatment due to dense adhesions. This case report is unusual as it has presented in a post-pubertal female requiring surgical management.

Labial adhesion usually occurs in the infancy period and in prepubertal girls. It is a rare entity in reproductive age without any hypoestrogenism condition. Voiding dysfunction is a rare manifestation of these conditions. Here, we report a 19-year-old girl with complete labial adhesion presented with urinary retention. A 19-year-old virgin girl was referred to the obstetrics and gynecology department of Ghaem Hospital. She complained of urinary retention. Physical examination was normal except that was moderate hypogastric tenderness and a huge vesical globe. Genital examination revealed complete fusion of the labia minora in the midline, extended from the posterior fourchette to the region of the clitoris covering the entire vaginal introitus, urethral meatus, and clitoris. Despite the use of topical estrogen cream and surgical labial separation, re-adhesion occurred for the third time. Vulvar biopsy confirmed severe inflammation. Finally, topical anti-inflammatory medication improved the symptoms. Although labial adhesion is very rare in the post-pubertal period, it can successfully be managed by medical and surgical treatment.

INTRODUCTION One of the important responsibilities of an Anaesthesiologist is to maintain a patent airway during any surgical procedure. Since the early days of Anaesthesia, various efforts have been made to dispel the problem of airway maintenance. AIMS AND OBJECTIVES The study entitled “LMAProSeal: An alternative to endotracheal intubation in open appendicectomy operation” was conducted with the aims to compare the efcacy of LMA ProSeal and Endotracheal Tube in patients undergoing Open Appendectomy under General Anaesthesia. MATERIALAND METHODS Study Area: This study was conducted in Medical College, Kolkata (West Bengal), under the department of Anaesthesiology in General Surgery Operation Theatre (C. B. Top OT/ Green OTComplex), after clearance from the Hospital ethical committee, during the period from 1st may 2013 to 31st January 2014. Awritten informed consent was taken from all patients included in the study. StudyPopulation:Patients postedforopenAppendicectomyoperationwithBMIbetween18.50–24.99kg/m2andbodyweightbetween30—60kg. Sample Size: 100 RESULTS AND OBSERVATIONS The effects were observed by monitoring heart rate, blood pressure and SPO2 preoperatively (as baseline), after intubation or placement of LMAProSeal at 1 min, 3 mins, 5mins and every 5 mins thereafter till the reading at removal of the device. For both the groups baseline ETCO2 was taken from connection of ETCO2 cable following placement of airway devices. SUMMARY AND CONCLUSION The study revealed that both the airway devices (ET tube and LMA ProSeal) were successful in operative procedure (open appendicectomy) in all the patients without any signicant complication. In experienced hands and following a strict protocol of insertion, the LMA ProSeal can prove to be an efcient and safe alternative to endotracheal tube for airway management of elective patients undergoing laparotomy procedure like open appendicectomy.

Background: Ectopic pregnancy is a common clinical condition. Diagnosis and managementof this condition is necessary in due to day practice. Methods: A cross sectional study on ectopic pregnancy was conducted in Sir SalimullahMedical College and Mitford Hospital (SSMC & MH) , Dhaka, with the objective to analyzethe risk factors and assess the results of management with respect to maternal morbidityand mortality of ectopic pregnancy during the last two years. Result: Fifty patients were admitted with ectopic pregnancy from 1st January 2008 to 31stDecember 2010.Frequency of ectopic pregnancy were 1.5% of total 3252 pregnancies. Riskfactors were found in 45% of cases. Surgical treatment were performed in total 43 patients.There was one heterotrophic pregnancy and one case of abdominal pregnancy. Four patientswere given intramuscular Methotrexate and two un-ruptured ectopics resolved spontaneouslyafter by expectant management. Conclusion: Conservative management was an option but surgical treatment was donemore often because of late referrals. Screening of high risk cases, early diagnosis and earlyintervention would reduce the morbidity in ectopic pregnancies. Bangladesh J Obstet Gynaecol, 2018; Vol. 33(1) : 49-53

Background: Post-operative wound infection may occur after routine abdominal surgery. Objective: The purpose of the present study was to see the distribution and determinants of post-operative wound infection among the patients underwent routine abdominal surgery. Methodology: This non-randomized clinical trial was conducted in the different surgical units of the Department of Surgery at Sir Sallimullah Medical College & Mitford Hospital, Dhaka, Bangladesh during January 2001 to December 2002 for a period of two (02) years. In the operation theatre, after anaesthesia skin was cleaned with Povidone iodine USP 5% w/w or Spirit (70% methylated spirit in water) or Chlorhexidine. During post-operative period dressing were left undisturbed unless it was felt necessary. Unusual pain in and around the wound was considered to be an indication of infection. A swab was taken from any discharge and was sent for bacteriological examination. Result: In this study, 50 patients were admitted as routine cases and undergone routine abdominal operations in general operation theatre. Out of 50 patients undergone routine abdominal surgery, 5 developed wound infection post operatively. Overall infection rate was 10.0%. In routine abdominal operations, infection was 9.09% in upper midline or extended midline incision, 33.33% in lower midline, 6.25% right subcostal/Kocher's. In routine abdominal operations, the rate of infection in clean contaminated wound was 11.11%, contaminated wound was 33.33%. Wound infection rate was 20.0% cases in patients with malnutrition, 14.28% cases in obesity and 16.66% cases in diabetes mellitus. Idiopathic scrotal calcinosis is an uncommon benign disorder of the scrotal skin which is characterized by multiple calcified intradermal nodules. We report a 33-year old with asymptomatic multiple calcified scrotal skin nodules. He had wide excision of the lesions and direct closure of the scrotum. We review the pathogenesis and surgical treatment options for this rare disease of the scrotum.

Duodenal ulcer patients with or without complications admitted in the surgical departments of Rajshahi Medical College Hospital (RMCH) during the period of 13 years from 1980–1992 were studied retrospectively. Duodenal ulcer accounted for 10/1000 admission and five operations per week. Of the 1623 patients with uncomplicated ulcer, only 623 needed surgical treatment. However, all of the 1599 who presented with pyloric stenosis, 1047 with peritonitis due to perforation, and 13 with hematemesis were operated upon. This study confirms previously published reports of high incidence of duodenal ulcer admissions in the hospital but shows a diminishing number in the uncomplicated cases requiring surgery.
