INTRODUCTION: Chronic urticaria (CU) is a common skin disorder affecting 15– 20% of the people in the general population. It is a frequent disease which presents with wheals, angiooedema or both and lasts for over six weeks or more. OBJECTIVES: To determine the etiology of IgE specific allergens in the causative of CU in the Nepalese population. MATERIALS & METHODS: A prospective cross sectional study was carried out between November 2014- October 2016 on all patients diagnosed as having CU. Blood samples were taken from all patients and asemiquantitative determination of IgE antibodies against a panel of individual allergens was done and data analyzed and interpreted in terms of numbers and percentages. RESULTS: Three hundred and fourteen patients were included in the study with 56% (175) being female and 44% (139) being male. The age of the patients ranged from 5years to 84 years with a mean of 37.33± 14.39 years. The duration of the disease also ranged from 0.15 to 20 years in which 116(36.9%) patients presented with disease of less than 6 months duration while 163(51.9%) patients had the disease for more than 6 months. Two hundred and seventy nine patients (88.9%)  showed a positive test to the allergens amongst which most were allergic to plants which included weeds and grass.CONCLUSION: In patients with CU whenever possible it is advisable to send the patient for serum allergy test to determine the causative agent, the avoidance of which can help in the management of urticaria in troublesome patients.Journal of Universal College of Medical Sciences (2017) Vol.05 No.01 Issue 15, Page: 33-36

***Determination of glycemic index and glycemic load of hospital diets served for diabetics***   AIMS: To determine the glycemic index and the glycemic load of diets usually offered by hospitals to patients with diabetes mellitus.   METHODS: A cross-sectional study evaluated menus served to diabetic inpatients of hospitals in the city of Curitiba, Parana, Brazil. Analyzing the menus, we determined the energy content, macronutrients, glycemic index and glycemic load of the meals offered to the patients.    RESULTS: Five general hospitals of the city participated in the study and 10 menus for diabetes were evaluated. The structure of the menus was different mainly in the quantitative supply of fruits and milk. Diets ranged from 1317.6 to 2013.2 kcal, with 18.9 to 27.6% of proteins, 21.9 to 29.4% of lipids, 48.2 to 53.3% of carbohydrates and 24.7 to 33.6 g of fibers. Daily glycemic index ranged from 47 to 57% and daily glycemic load from 81 to 109%.   CONCLUSIONS: All offered diets were hyperproteic, normolipid and normoglicidic. However, inadequate levels of glycemic load were observed in all the evaluated menus, although with adequate levels of glycemic index. It is necessary to review the diet plans elaborated for diabetics, aiming at the best dietary treatment for this population.

This descriptive, cross-sectional study investigated problems experienced by patients after undergoing a thyroidectomy. The study included 60 first-time, post-thyroidectomy patients diagnosed with benign thyroid disease from a university hospital’s general surgery clinic in Ankara, Turkey. The data were collected in two stages: interviews with patients on the first day following surgery and postoperative follow-up telephone interviews in each of the first 4 weeks following surgery. The follow-ups revealed that patients principally experienced varying degrees of pain and difficulties in connection with work and recreation, communication, body image, and movement, for up to 4 weeks after surgery. These results showed that patients were particularly prone to problems on the first day and during the first week of the postoperative period; therefore, patients should be provided with follow-up telephone interviews to facilitate easier recovery and to help them overcome any problems experienced during the postoperative period.

Introduction: Sudden outbreak of COVID-19 pandemic has affected the educational system worldwide, forced the medical colleges to close due to lock down, and disrupted the classroom face-to-face teaching process. As a result, medical colleges shifted to an online mode of teaching. The aim of this study is to find out the perception towards online classes during COVID-19 lockdown period among MBBS and BDS students at a medical college of Nepal. Methods: This was a descriptive cross-sectional study carried out at Universal College of Medical Sciences and Teaching Hospital among first and second year Bachelor in Medicine, Bachelor of Surgery and Bachelor in dental surgery students from 1st June 2020 to 30th August 2020. Ethical approval was taken from Institutional Review Committee of Universal College of Medical Sciences and Teaching Hospital (IRC UCMS, Ref: UCMS/IRC/025/20). Convenient sampling method was used. Semi-structured questionnaire was used. Statistical Package for Social Sciences 22 was used for analysis and frequency and percentage was calculated. Results: One hundred fifty six (73.93%) students were enjoying online learning only to some extent, 135 (63.98%) felt online class not equally effective as face-to-face teaching. The students had disturbance during online classes as internet disturbance 168 (79.60%), and electricity problem 47 (22.3%). Similarly, many students 155 (73.50%) felt external disturbance, headache 26 (12.3%), and eye strain 26 (12.3%). Conclusions: Most of the students suffered from disturbances during online classes probably because of internet and electricity problem. When compulsory to conduct online classes, students felt that not more than three online classes per day should be conducted to avoid eye strain and headache.
