From impact assessment to effective management plans: learning from the Great Brak Estuary in South Africa

Background: Healthcare practitioners should provide patients with information regarding their clinical conditions. Patients should also feel free to seek clarity on information provided. However, not all patients seek this clarity.Objectives: To explore the reasons inpatients gave for not seeking clarity on information that was received but not understood.Methods: This was a qualitative arm of a larger study, titled ‘Are inpatients aware of the admission reasons and management plans of their clinical conditions? A survey at a tertiary hospital in South Africa’, conducted in 2010. Of the 264 inpatients who participated in the larger study, we extracted the unstructured responses from those participants (n = 152) who had indicated in the questionnaire that there was information they had not understood during their encounter with healthcare practitioners, but that they had nonetheless not sought clarity.Data were analysed thematically.Results: Themes that emerged were that inpatients did not ask for clarity as they perceived healthcare practitioners to be ‘too busy’, aloof, non-communicators and sometimes uncertain about patients’ conditions. Some inpatients had unquestioning trust in healthcare practitioners,whilst others had experiences of bad treatment. Inpatients had poor self-esteem, incapacitating clinical conditions, fear of bad news and prior knowledge of their clinical conditions. Some inpatients stated that they had no reason for not seeking clarity.Conclusion: The reasons for not seeking clarity were based on patients’ experiences with the healthcare practitioners and their perceptions of the latter and of themselves. A programme should be developed in order to educate inpatients on effective communication with their healthcare practitioners.

South Africa is faced with a huge challenge of addressing the high burden of tuberculosis-human immune virus (TBHIV) co-infection, and this challenge is more pronounced in the province of KwaZulu-Natal which has one of the highest burdens of TB-HIV co-infection in the world. The study explored the experiences of doctors and nurses with regard to the management of tuberculosis and HIV coinfection in a TB-HIV high burden community in northern KwaZulu-Natal, South Africa. The particular focus was to provide insight and to inform policy and programme development for effective management of TB-HIV co-infection in the TB-HIV high burden community of northern KwaZulu- Natal. An interpretivist exploratory qualitative approach was employed through individual semi-structured interviews of 16 participants comprising eight doctors and eight nurses, with a total interview time of 8.95 hours. Purposive sampling was used to select the doctors and nurses from the public and private sector of the TB-HIV high burden community of northern KwaZulu- Natal. Thematic analysis was used to analyse the data. Five key themes emerged from this study and these themes were discussed together with the sub-themes based on the various participant responses. The five key themes were practical experience about the management of TB-HIV co-infection; access to information and training on the management of TB-HIV co-infection; challenges and concerns about the management of TB-HIV co-infection; perception about local beliefs; and knowledge of policies and guidelines. Overall, this study highlights barriers that hamper the effective management of TB-HIV co-infection in northern KwaZulu-Natal. Recommendations of this study point towards an urgent need to scale up the management of TB-HIV co-infection through effective policies, improved capacity and infrastructure, stronger partnerships of all stakeholders, and further research.

Protected areas are the most frequently used tool for the mitigation of threats to biodiversity. However, without effective management, the creation of new protected areas may be ineffective. In Brazil, protected areas must have both a governing body (consultative or deliberative council) and an official management plan. Here, we analyzed general trends and patterns in the approval of the management plans for Brazilian federal protected areas. We considered all federal protected areas, and compiled data on (i) the year the area was created, (ii) the type of protected area (integral protection vs. sustainable use), (iii) year its management plan was approved, (iv) year in which the management plan was revised after its approval, (v) total area (in hectares), and (vi) the biome in which the area is located. We stablished three groups of protected area: 1) Group A: protected areas created prior to 1979, 2) Group B: protected areas created between 1979 and 1999, and 3) Group C: protected areas created between 2000 to the present time. Finally, we tested whether time for the approval of the management plan suffered a simultaneous effect of the type of biome and type of categories of protected area (strictly protected vs. sustainable use areas). We found 211 (63.17% of the 334) protected areas with management plan. On average, the time taken for the creation and approval of a management plan far exceeds the deadlines (5 yrs.) defined under current Brazilian law. All Brazilian biomes are poorly covered by protected areas with effective management plans, with the highest and lowest value observed in the Pantanal (100%) and Caatinga (46.42%), respectively. Our results suggest that the effectiveness of many federal protected areas in Brazil can be reduced considerably by the lack of a management plan, with deleterious consequences for the country’s principal conservation strategies.

The aquatic snail genus Dianella (Gastropoda: Hydrobiidae) has only two representatives in Greece: Dianellaschlickumi Schütt, 1962 and Dianellathiesseana (Kobelt, 1878). D.schlickumi, a narrow endemic species to Lake Amvrakia (in Aitoloakarnania, western-central Greece), is considered as Critically Endangered (Possibly Extinct, sensu IUCN 2017). Our study confirmed its presence in Lake Amvrakia, where it had not been detected for more than 30 years. We document the unknown anatomical characters based on the D.schlickumi specimens. Moreover, the presence of D.thiesseana in the nearby lakes Trichonis and Lysimachia was also confirmed, while morphometric analyses enabled the discrimination between the two species. Redundancy Analysis revealed conductivity, dissolved oxygen and pH as the main environmental variables related to the above species’ distribution, shaping their community structure. Both Dianella species require urgent conservation measures to be enforced, due to their habitat degradation from human activities, which are limiting and fragmenting their range. For that purpose, effective management plans have to be elaborated and implemented at the mentioned lakes, focusing on the reduction of human pressures and on the improvement of their habitats.

AbstractThe seasonal activity of adults of eight forensically important blow fly species [Calliphora croceipalpis (Diptera: Calliphoridae), Jaennicke, Lucilia sericata (Meigen) (Diptera: Calliphoridae), L. cuprina (Wiedemann) (Diptera: Calliphoridae), Chrysomya chloropyga (Wiedemann) (Diptera: Calliphoridae), Ch. albiceps (Wiedemann) (Diptera: Calliphoridae), Ch. marginalis (Wiedemann) (Diptera: Calliphoridae), Ch. putoria (Wiedemann) (Diptera: Calliphoridae), Ch. megacephala (Fabricius) (Diptera: Calliphoridae)] was quantified from collections in Makhanda, South Africa. Flies were collected in traps baited with chicken liver and placed in the field at eight locations for four consecutive days each fortnight for 14 mo. The seasonal distribution of each species compared well to published seasonal distributions of these blow flies elsewhere in South Africa, with evidence of year-to-year variation within seasons that might be explained by weather. This information is important for determining when and where certain species are likely to occur and will be of use in forensic investigations and myiasis management plans.

Biofouling is a major problem shared among all maritime sectors employing submerged structures where it leads to substantially increased costs and lowered operational lifespans if poorly addressed. Insight into the ongoing processes at the relevant marine locations is key to effective management of biofouling. Of specific concern for the marine renewable energy (MRE) sector is the fact that information on biofouling composition and magnitude across geographies is dispersed throughout published papers and consulting reports. To enable rapid access to relevant key biofouling events the present work describes a European biofouling database to support the MRE sector and other maritime industries. The database compiles in one document qualitative and quantitative data for challenging biofouling groups, including non-native species associated with MRE and related marine equipment, in different European Ecoregions. It provides information on the occurrence of fouling species and data on key biofouling parameters, such as biofouling thickness and weight. The database aims to aid the MRE sector and offshore industries in understanding which biofouling communities their devices are more susceptible to at a given site, to facilitate informed decisions. In addition, the biofouling mapping is useful for the development of biosecurity risk management plans as well as academic research.

This paper describes the types of controls available to Australian land use planners, along with a brief survey of current issues in the use and management of the arid zone. The authors discuss possible roles for each of the four main types of available controls (zoning, site and management plans, performance standards and project impact assessment) in the arid zone. They conclude that performance standards and project impact assessment procedures are the controls most likely to prove useful in the management of the arid zone.
