Objective. To assess a text messaging intervention to promote tuberculosis (TB) treatment adherence.Methods. A mixed-methods pilot study was conducted within a public pulmonary-specialized hospital in Argentina. Patients newly diagnosed with TB who were 18 or older, and had mobile phone access were recruited and randomized to usual care plus either medication calendar (n=19) or text messaging intervention (n=18) for the first two months of treatment. Primary outcomes were feasibility and acceptability; secondary outcomes explored initial efficacy.Results. Feasibility was evidenced by high access to mobile phones, familiarity with texting, most phones limited to basic features, a low rate of participant refusal, and many describing suboptimal TB understanding. Acceptability was evidenced by participants indicating feeling cared for, supported, responsible for their treatment, and many self-reporting adherence without a reminder. Participants in the texting group self-reported adherence on average 77% of the days whereas only 53% in calendar group returned diaries. Exploring initial efficacy, microscopy testing was low and treatment outcomes were similar in both groups.Conclusion. The texting intervention was well accepted and feasible with greater reporting of adherence using text messaging than the diary. Further evaluation of the texting intervention is warranted.

AbstractMobile technologies represent potentially novel and scalable intervention delivery platforms for adolescents living with HIV (ALWH) in low- and middle-income countries. We conducted a prospective, mixed methods pilot study to evaluate the acceptability and feasibility of the WhatsApp® platform to deliver individual counseling services and facilitate peer support for ALWH in western Kenya. Thirty ALWH (17 female, mean age 15.4) on ART, engaged in HIV care and aware of their status, were enrolled. After 6 months, participants described their experiences with the intervention. Treatment adherence, stigma, and mental and behavioral health were assessed prospectively. Participants reported overall positive experiences and indicated that the platform encouraged peer network development. They endorsed potential benefits for treatment adherence, stigma reduction, and mental and behavioral health. All participants supported intervention expansion. In western Kenya, WhatsApp® was an acceptable and feasible platform for mobile counseling and peer support for ALWH.

Introduction: Advances in mobile health have led to numerous relevant studies in diagnosis, treatment, and controlling of various diseases. One of the criteria indicating the quality of the previously published studies is the number of citations. Therefore, investigating the features of highly cited articles and identifying the most frequently used mobile technological interventions can affect future research ideas. This study aimed at identifying 100 highly cited interventional studies on mobile health, type of used mobile technologies, and effect of these technologies in various diseases in top-cited articles.Methods: The database employed in this study was the Web of Science, which without limitations was analysed in April 2020 to identify 100 highly cited interventional studies in the field of mobile health. The identified studies were classified based on the number of citations, year of publication, country of the first author, type of disease, and use of mobile technology.Results: A great majority of the studies in the field of interventional mobile health focused on obesity (n=18), addiction (n=15), diabetes (n=13) and mental health disorders (n=12), respectively. Many studies employed mobile technologies to promote lifestyle (weight loss and increased physical activity) (n=20), disease controls (n=20), and treatment adherence (n=18). The mean number of citations per study was 146±97. The most cited study was in the category of viral disease treatment adherence (n=703), and the most cited articles were published in 2012.Conclusions: Among the reviewed 100 studies, many of the interventional studies regarding mobile health focused on obesity, addiction, diabetes and mental health disorders. Promotion of lifestyle, disease controls, and treatment adherence were effects of mobile technologies in top-cited articles. Text messaging service was used as intervention in most of the studies. Thus, future studies may focus on the use of various mobile applications on different diseases’ prevention, control, and treatment.

There is limited high-quality evidence available to inform the use of text messaging to improve latent tuberculosis infection (LTBI) treatment adherence.We performed a parallel, randomised controlled trial at two sites to assess the effect of a two-way short message service on LTBI adherence. We enrolled adults initiating LTBI therapy from June 2012 to September 2015 in British Columbia, Canada. Participants were randomised in a 1:1 ratio to standard LTBI treatment (control) or standard LTBI treatment plus two-way weekly text messaging (intervention). The primary outcome was treatment completion, defined as taking ≥80% prescribed doses within 12 months (isoniazid) or 6 months (rifampin) of enrolment. The trial was unblinded except for the data analyst.A total of 358 participants were assigned to the intervention (n=170) and control (n=188) arms. In intention-to-treat analysis, the proportion of participants completing LTBI therapy in the intervention and control arms was 79.4% and 81.9%, respectively (RR 0.97, 95% CI 0.88–1.07; p=0.550). Results were similar for pre-specified secondary end-points, including time-to-completion of LTBI therapy, completion of >90% of prescribed LTBI doses and health-related quality of life.Weekly two-way text messaging did not improve LTBI completion rates compared to standard LTBI care; however, completion rates were high in both treatment arms.
