Relevance. The issue of quality endodontic treatment of teeth with complicated caries is still known as requiring thoughtful research. A significant percentage of periodontitis occurs in the area of furcations of multi-rooted teeth, sometimes alone, but more often as combined with apical periodontitis. Among the number of unsolved tasks, there is question about furcation areas and in particularly their antiseptic treatment and obturation in the complex treatment of chronic apical periodontitis of multi-rooted teeth. Aim: to investigate the histo-morphological features of the furcation area of the first, second molars of the upper and lower jaws and, according to the results of the experimental study, to develop a scheme of complex treatment of chronic apical periodontitis of multi-rooted teeth. Materials and methods. Ten first and second molars of both jaws not affected by fluorosis and removed according to orthodontic or surgical indications in patients aged from 25 to 50 years, were used for this study. In the first stage, transverse and longitudinal sections of the molars were made using diamond-coated separation disks. Using an orthopaedic tip, the first half of the molars were cut longitudinally in the mesio-distal direction with separation disks, under water cooling. The other half of the molars were cut transversely along the neck of the tooth and histochemically stained. In the second stage of the study, we used the experimental application of photodynamic therapy for antiseptic treatment of furcation channels and sealing them with citrate buffer. For this purpose, the next two first and two second mandibular molars were selected, respectively dissected in two planes. Samples were fixed in a vertical position; cotton rolls soaked with 2% methylene blue were placed onto the bottom of each half for 30 s., 1 min., 5 min., and 24 h. In this study we used light binocular stereoscopic microscope MBS-9; histochemical dyes: CHIC-thionine, ammonia silver solution "Argentat", reducing agent (hydroquinone), methylene blue, device "UFL - 122" company "Lux Dent", citrate buffer solution and white clay. The results were recorded with a digital camera mounted on a microscope tube. Results and discussion. The results of histochemical examination of ten molars showed the presence of furcation channels in four teeth. Two samples were stained with CHIC-thionine, the others were stained with two-component "Argenate". The selected samples were dissected in the sagittal plane through the area of localization of the furcation channels and studied under the microscope (magnification: 8x32). In teeth dissected in the transverse plane, the crown and mouth pulp were instrumentally removed without dilation of the root canals and stained accordingly. Thus, when modelling the stage of photodynamic treatment using a 2% solution of methylene blue, there is a significant filling of furcation channels and root canals along their entire length. The photosensitizer 2% methylene blue at the exposure of 30s demonstrates the highest efficiency at the minimum staining of surrounding fabrics. The microscopy was performed to confirm the ability of the photosensitizer to penetrate into the furcation channels at х8 – х32 magnification. After the experimental filling of furcation channels and root canals, we can see clear white stripes along their entire length on the dentin surface of the longitudinal section of the root; these stripes are obliquely directed towards the canal surface and consist of numerous dentinal tubules impregnated with citrate buffer. Numerous thin white lines are clearly visible on the surface of the section that run in parallel, tangentially through almost the entire thickness of the root wall and end up their course in the area around the peripulpal dentin. This indicates the formation of calcium citrate in the root canal that tightly covers its entire surface impregnating the dentinal tubules and completely fills them along their entire length. Therefore, the experimental model of using 2% methylene blue photosensitizer at a 30 s. exposure for photodynamic therapy and citrate buffer for obturating furcation and root canals can be applied in the treatment of patients with chronic apical periodontitis and lesions of the furcation area. Conclusions. Thus, morphological studies of the first and second molars of both jaws have shown the presence of furcation channels, which anatomical structure is quite complex and demanding in terms of their antiseptic treatment and filling. Therefore, we developed new effective for disinfection and obturation techniques including applying photodynamic therapy for disinfection of furcation channels and citrate buffer for their obturation.

Background. The success of endodontic treatment is provided by a thorough instrumental and antiseptic treatment of infected root canals, and it depends on the composition of filling material, the degree of adhesion to dentin, hermetic obturation of apical foramen, solubility of sealer.Objective. The study was aimed to evaluate the effectiveness of root canal obturation with BioRootTM RCS sealer in the treatment of patients with chronic apical periodontitis.Materials and methods. Endodontic treatment of 23 teeth in 20 patients with chronic apical periodontitis by method of lateral compaction of gutta-percha was carried out. In the main group root canals were obturated with BioRootTM RCS, in the control group the canals were filled with Apexit Plus. The percentage of efficient or non-efficient cases was evaluated on the basis of radiographic comparison of treated chronic apical periodontitis immediately after obturation, in three, six months and one year. Radiographic conditions were defined as existing state, improvement and worsening.Results. In a year of dynamic evaluation the final results were: in the main group – 54.55% of the patients had complete bone healing, in 27.27% of cases the focus of bone destruction was decreased by ½ or more of the initial sizes, 18.18% – resorption lesion was decreased by less than ½; in the control group – 33.33% of improvement, 25.0% of existing state and 41.66% of worsening.Conclusions. BioRootTM RCS using for root canals obturation in the treatment of chronic apical periodontitis we proved the high effectiveness of the treatment undertaken: complete healing or improvement of radiographic conditions of periapical bone destruction with X-ray signs of bone regeneration.

Conservation of deciduous teeth with pulp alterations caused by caries and trauma is a major therapeutic challenge in pediatric dentistry as a result of the internal anatomy and life cycle characteristic. It is essential that the root canal procedures sanitizers have a performance in eliminating bacterial. In this context, antimicrobial photodynamic therapy (PAT) is promising and emerging as adjuvant therapy in an attempt to eliminate the microorganisms persistent to chemi-mechanical preparation. Since there is presence of oxygen in cells, photosensitizer activated by light can react with molecules in its vicinity by electrons’ or hydrogen’s transfer, leading to microorganism death. This paper reports the case of 4-year-old patient, female, with early childhood caries. The proposed endodontic treatment incuded chemomechanical treatment allied to PAT in the decontamination of root canals using methylene blue dye 50 μg/mL during 3–5 minutes and 40 J/cm2as energy density, taking into account the need for tissue penetration and effectiveness of PAT inside the dentinal tubules.

Microbiological reports of apical periodontitis have revealed that yeasts can be isolated from approximately 5-20% of infected root canals. They occur either in pure cultures or together with bacteria. Almost all isolated yeasts belong to the genus Candida, and the predominant species is C. albicans. Pheno- and genotypic profiles of C. albicans isolates show heterogeneity comparable with those of isolates from other oral sites. C. albicans expresses several virulence factors that are capable of infecting the dentin-pulp complex, including dentinal tubules. This causes, consequentially, an inflammatory response around the root apex, which suggests a pathogenic role for this organism in apical periodontitis. Yeasts are particularly associated with persistent root canal infections that do not respond favorably to conservative root canal therapy. This may be due to the resistance of all oral Candida species against a commonly used topical medicament, calcium hydroxide. However, other antimicrobial agents may offer alternative therapeutic approaches and improve the treatment of these persistent cases of apical periodontitis.

Introduction: Photodynamic therapy (PDT) has emerged as a powerful tool for conventional endodontic treatment, capable of eradicating microorganisms present in endodontic infections. Despite this, the use of PDT in the surgical endodontic treatment is not well-known. Case Report: This paper describes the case of a patient with asymptomatic apical periodontitis in a mandibular incisor submitted to surgical endodontic treatment associated with PDT. After conventional procedures, photosensitizer methylene blue (0.01%) was applied to the retrograde cavity for 3 minutes, followed by irradiation with diode laser (100 mW and 660 nm) for 3 minutes. Six months after the procedure, the patient was asymptomatic, and the radiographic examination showed healthy periradicular tissues. Conclusion: The association of PDT with surgical endodontic treatment was effective, suggesting that this therapy may provide additional benefits to patients when compared to the conventional surgical technique.

This review highlights the possibilities of photodynamic therapy (PDT) using drugs based on chlorin e6, aluminum phthalocyanine, methylene blue as photosensitizers for bacterial and fungal pathologies. This method was developed initially to treat tumor diseases, where it had shown its high efficiency and safety. Now photodynamic therapy is actively used in the treatment of cancers of the skin, bronchi, stomach, cervix, larynx, or other regions. However, numerous studies have been carried out for the entire existence of the method, demonstrating new possibilities of its application. This review highlights a number of studies in which the efficacy and safety of antimicrobial and antimycotic PDT were studied in vivo and in vitro. It has been proven to have a positive effect on the reparative processes in the wound. An experimental study was carried out to study the effectiveness of photodynamic therapy in the treatment of peritonitis in mice. Demonstrated anti-inflammatory potential in the treatment of autoimmune diseases.

Abstract Background To evaluate the antibacterial effect of diode laser and photodynamic therapy using methylene blue dye when used alone or in combination with Moringa oleifera extract on root canal microbes. A total of 50 single root canals with single straight canals were prepared with ProTaper files, sterilized in an autoclave device, contaminated with root canal microbes (Enterococcus faecalis and Candidia albicans) and placed in incubator for 21 days. They were then randomly divided into 6 groups. Laser group: (10 root canals) were subjected to laser only. Moringa group: (10 root canals) were irrigated using plant extract only (Moringa). PDT group: (10 root canals) were irrigated using Moringa and Methylene blue then subjected to laser. Methylene blue and laser group: (10 root canals) were irrigated using methylene blue then subjected to laser. Negative control group: (10 root canals) weren't irrigated. After treatment microbial assessment (colony count) was done. Data was analyzed using ANOVAs test. Results Moringa/laser group and PDT group showed statistically least bacterial count for both E. faecalis and Candida albicans. Moringa only group showed statistically less bacterial count than control group. Conclusion The combined use of PDT together with Moringa oleifera has the best antimicrobial effect.
