Relevance. Chronic diseases have a long duration and a rather slow progression, and people who have such diseases want not only to live longer but also to live better. Therefore, quality of life (QOL) is one of the most important health issues for the treatment of chronic diseases. Objective: to determine the features of QOL in patients with chronic kidney disease (CKD) stage II-IV. Materials and methods. In 171 patients with CKD of II-IV centuries, in addition to general clinical and laboratory studies, QOL was studied using a questionnaire to assess the quality of life of SF-36. The questionnaire contains 36 questions of the main module, supplemented by multi-point scales aimed specifically at patients with CKD. The answers were evaluated in points - from 0 to 100. The higher the score, the better the patient's QOL. The total components were also calculated: physical total component, mental total component, total points. The obtained research data were subjected to statistical processing, which included parametric (t-test for samples with unrelated variants) and non-parametric (Mann-Whitney method) methods, correlation analysis was used. Results. Most QOL indicators worsen significantly with the progression of CKD. The indicators of total QOL, as well as the indicator "the impact of the disease on everyday life" are most significantly reduced. Age correlates as much as possible with most indicators of QOL in patients with CKD. With age, the QOL of patients decreases, but the manifestations of the disease increase and the mental and physical condition of patients deteriorate. QOL parameters are probably directly and moderately correlated with hemoglobin levels. Most QOL parameters are significantly moderately correlated with systolic blood pressure and diastolic blood pressure. It is assumed that the correction of anemia and blood pressure control, in addition to a positive effect on disease progression and the occurrence and development of complications, will also improve QOL. There were no significant differences in the assessment of QOL in men and women. Conclusions. QOL indicators decrease with the progression of CKD. Age, hemoglobin level, blood pressure affect QOL.

The chronic kidney disease (CKD) co-exist with hypertension in approximately 80 to 85 per cent of patients. The CKD stages can be defined by glomerular filtration rate (GFR), and the deterioration of kidney function or reduction in GFR has observed in those with uncontrolled blood pressure (BP). We had conducted a prospective study to analyse the impact of the angiotensin system-related agents on the quality of life of CKD patients with hypertension. The SF-36 questionnaire, direct patients interview and medical records were the sources for retrieval of information. We observed that male patients were more prone to CKD than female. Hypertension was the primary (77.8%) aetiology behind the incidence of CKD. The angiotensin-converting enzyme inhibitors (ACEI) was responsible for very low (58%) and low (44%) health disabilities to the patients. In contrast, the angiotensin receptor blockers (ARB) even though it has a limited adverse effect, the patients complained of medium (9%) and high disabilities than the ACEIs. The discontinuation of the antihypertensive drugs by the CKD patients was almost negligible (3.4%). The study concludes that a balanced diet and reasonable blood pressure control is essential to prevent the progression of CKD and to improve the quality of life.

Background and Objectives: Chronic Kidney Disease (CKD) is a worldwide public health problem with increasing incidence and prevalence. The causes of CKD may be diabetes mellitus, hypertension and polycystic kidney disease. The main objective of this study is to study quality of life of chronic kidney disease stage 5 patients on hemodialysis. Material and Methods: This is a prospective cross sectional study conducted at National Academy of Medical Sciences, B & B Hospital and Blue Cross Hospital, Kathmandu, Nepal on 50 CKD stage 5 patients on hemodialysis. Quality of life among hemodialysis patients was studied using Short Form-36. The result was obtained by comparing the patient’s physical, social and mental status at the beginning and conclusion of a 2 month period. The data was analyzed using the software Statistical Package for Social Sciences (SPSS for Windows version 16). Results: Out of the total 50 patients on hemodialysis, 32 were male (64%) and 18 were female (36%) with mean and median age of patients of 47.14 ± 16.65 and 48.50 years respectively. Out of eight domains studied, energy level, feeling of happiness with life and thought of full energy on self and worning out of life and tiredness perception was found to be equal on pre and post stage. Physical functioning was found to be decreased. Patients on hemodialysis reported improvements in nearly all aspects of general functioning and psychological well-being. Conclusion: This result demonstrates that there were several changes in Quality of life. Hemodylasis improves the Quality of life however, there was significant decrease in physical functioning, role limitation due to physical ill health, role limitation due to decreased emotional wellbeing, and reduced general health. Inappropriate polypharmacy is likely in older adults with chronic kidney disease (CKD) owing to the considerable burden of comorbidities. We aimed to describe the impact of pharmacist-led geriatric medication management service (MMS) on the quality of medication use. This retrospective descriptive study included 95 patients who received geriatric MMS in an ambulatory care clinic in a single tertiary-care teaching hospital from May 2019 to December 2019. The average age of the patients was 74.9 ± 7.3 years; 40% of them had CKD Stage 4 or 5. Medication use quality was assessed in 87 patients. After providing MMS, the total number of medications and potentially inappropriate medications (PIMs) decreased from 13.5 ± 4.3 to 10.9 ± 3.8 and 1.6 ± 1.4 to 1.0 ± 1.2 (both p < 0.001), respectively. Furthermore, the number of patients who received three or more central nervous system-active drugs and strong anticholinergic drugs decreased. Among the 354 drug-related problems identified, “missing patient documentation” was the most common, followed by “adverse effect” and “drug not indicated.” The most frequent intervention was “therapy stopped”. In conclusion, polypharmacy and PIMs were prevalent in older adults with CKD; pharmacist-led geriatric MMS improved the quality of medication use in this population.

ABSTRAKLatar Belakang. Hemodialisis aman dan bermanfaat untuk pasien, namun bukan berarti tanpa efek samping. Berbagai komplikasi dapat terjadi pada saat pasien menjalani hemodialisis. Komplikasi ini dapat mengakibatkan timbulnya masalah baru yang lebih kompleks, yaitu dapat mempengaruhi kualitas hidup bahkan menimbulkan kematianTujuan. Untuk mengetahui hubungan komplikasi intra hemodialisis dengan kualitas hidup  pada pasien Chronic Kidney Disease (CKD) Stage V yang menjalani hemodialisis.Metode. Jenis penelitian adalah analitik korelasional dengan pendekatan cross-sectional. Sampel pada penelitian ini berjumlah 166 responden diambil dengan teknik non probability sampling yaitu total sampling. Pengumpulan data menggunakan kuesioner dan lembar observasi,dianalisa dengan uji non parametric (Chi Square).Hasil. Rata – rata jumlah komplikasi intra hemodialisis adalah kurang dari dua komplikasi (sedikit komplikasi). Hipertensi intra hemodialisis adalah komplikasi terbanyak yang ditemukan (52,3%). Kejang dan penurunan kesadaran merupakan komplikasi yang tidak pernah dialami oleh responden selama penelitian (0%).Kualitas hidup pasien CKD stage V yang menjalani HD di BRSU Tabanan masuk dalam kategori kualitas baik. Hasil uji statistik diperoleh nilai p < 0.001 bahwa ada hubungan yang signifikan antara komplikasi intra hemodialisis dengan  kualitas hidup pada pasien yang menjalani HD. Dari hasil analisis diperoleh pula nilai OR = 0,005, artinya responden yang memiliki banyak komplikasi mempunyai peluang 0,005 kali memiliki kualitas hidup buruk dibanding responden yang memiliki sedikit komplikasi.Kesimpulan. However, there are various complications may occur when the patients undergo hemodialysis. These complications can lead to the emergence of new problems which are more complex and affect to the quality of life and even cause death.Aim: To determine the relationship of intra hemodialysis complications with quality of life in patients with Chronic Stage Kidney Disease (CKD) Stage V who undergo hemodialysis.Method:This study employed correlational analytic design with cross-sectional approach. To conduct this study, there were 166 respondents recruited as the sample by using probability sampling with total sampling technique. The data were collected by using questionnaire and observation sheet. Further, the data were analyzed by non-parametric test (Chi Square).Finding: The findings indicated that the average number of intra hemodialysis complications is less than two complications (few complications). There were found that 52.3% patients who had complication of hypertension intra hemodialysis. On the other hand, there was 0%of the respondents experienced seizures and decreased awareness complications during the study. The quality of life of CKD stage V patients underwenthemodialysis at BRSU Tabanan is categorized as good quality. The statistical test obtained p <0.001, it meant that there was a significant relationship between intra hemodialysis complications with quality of life in patients underwenthemodialysis. It also found that the analysis results of OR = 0.005, it meant that respondents who had many complications have a chance of 0.005 times experienced poor quality of life rather than the patients who had few complications.Conclusion: There is a significant relationship between intra hemodialysis complications and quality of life in patients undergo hemodialysis. Keywords: Complications of Intra Hemodialysis, Quality of Life, Hemodialysis

Abstract Background Quality-of-life is an essential outcome for clinical care. Both chronic kidney disease (CKD) and diabetes have been associated with poorer quality-of-life. The combined impact of having both diseases is less well understood. As diabetes is the most common cause of CKD, it is imperative that we deepen our understanding of their joint impact. Methods This was a prospective, longitudinal cohort study of community-based Australians aged ≥25 years who participated in the Australian Diabetes, Obesity and Lifestyle study. Quality-of-life was measured by physical component summary (PCS) and mental component summary sub-scores of the Short Form (36) Health Survey. Univariate and multivariate linear mixed effect regressions were performed. Results Of the 11 081 participants with quality-of-life measurements at baseline, 1112 had CKD, 1001 had diabetes and of these 271 had both. Of the 1112 with CKD 421 had Stage 1, 314 had Stage 2, 346 had Stage 3 and 31 had Stages 4/5. Adjusted linear mixed effect models showed baseline PCS was lower for those with both CKD and diabetes compared with either disease alone (P &lt; 0.001). Longitudinal analysis demonstrated a more rapid decline in PCS in those with both diseases. Conclusions The combination of CKD and diabetes has a powerful adverse impact on quality-of-life, and participants with both diseases had significantly poorer quality-of-life than those with one condition.

We report a child with Hermansky-Pudlak Syndrome (HPS) and chronic kidney disease (stage II) with histological diagnosis of focal segmental glomerulosclerosis (FSGS). A 15-year-old male of Puerto Rico ancestry with history of HPS, hypertension (HTN), asthma, obesity, and chronic kidney disease (CKD) stage II presented with new-onset proteinuria without edema. His blood pressure had been controlled, serum creatinine had been 0.9–1.4 mg/dL, and first morning urine protein/creatinine ratio (UPC) ranged from 0.2 to 0.38. Due to persistent nonorthostatic proteinuria with CKD, renal biopsy was performed and FSGS (not otherwise specified) with chronic diffuse tubulopathy (tubular cytoplasmic droplets) and acute tubular injury was reported. Ceroid-like material is known to infiltrate tissues (i.e., lungs, colon, and kidney) in HPS, but the reason for the renal insufficiency is unknown. Nonspecific kidney disease and in one adult case IgA nephropathy with ANCA-positive glomerulonephritis have previously been reported in patients with Hermansky-Pudlak syndrome. To our knowledge, we report the first pediatric renal pathology case of HPS associated with CKD. This paper discusses presentation and management of renal disease in HPS.

Abstract Background: Optimal clinical care of patients with chronic kidney disease (CKD) requires collaboration between primary care physicians (PCP) and nephrologists. We undertook a randomised trial to determine the impact of superimposed nephrologist care compared to guidelines-directed management by PCPs in CKD patients after hospital discharge. Methods: Stage 3b-4 CKD patients were enrolled during a hospitalization and randomised in two arms: Co-management by PCPs and nephrologists (interventional arm) versus management by PCPs with written instructions and consultations by nephrologists on demand (standard care). Our primary outcome was death or rehospitalisation within the 2 years post-randomisation. Secondary outcomes were: urgent renal replacement therapy (RRT), decline of renal function and decrease of quality of life at 2 years. Results: From November 2009 to the end of June 2013, we randomised 242 patients. Mean follow-up was 51 + 20 months. Survival without rehospitalisation, GFR decline and elective dialysis initiation did not differ between the two arms. Quality of life was also similar in both groups . Compared to randomised patients, those who either declined to participate in the study or were previously known by nephrologists had a worse survival. Conclusion: These results do not demonstrate a benefit of a regular renal care compared to guided PCPs care in terms of survival or dialysis initiation in CKD patients. Increased awareness of renal disease management among PCPs may be as effective as a co-management by PCPs and nephrologists in order to improve the prognosis of moderate-to-severe CKD.

ABSTRAK Hipertensi merupakan penyebab gangguan ginjal kronik melalui suatu proses yang mengakibatkan hilangnya sejumlah besar nefron fungsional yang progresif dan irreversible. The Kidney Disease Outcome Quality Initiative (K/DOQI) of the national kidney foundation (NKF) merekomendasikan target tekanan darah pada pasien gangguan ginjal kronik < 140/90 mmHg sehingga dapat mengurangi risiko terjadinya penyakit kardiovaskular. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian obat antihipertensi terhadap kualitas hidup pasien GGK yang menjalani hemodialisis di RSUP DR.Hasan Sadikin Bandung. Penelitian menggunakan desain cross sectional observational konkuren dengan mengkaji data rekam medis dan status harian pasien selama periode Desember 2015-Febuari 2016 dan hasil pengisian kuisioner Kidney Disease and Quality of Life (KDQOLTM-36). Subyek penelitian merupakan pasien GGK yang menjalani hemodialisis dan mendapatkan terapi obat antihipertensi (OAH), laki-laki dan perempuan, usia lebih dari 18 tahun. Terdapat perbedaan bermakna penurunan tekanan darah sistolik(p=0,011) dan diastolik (p=0,023) untuk setiap terapi OAH, kombinasi 2 OAH memberikan efek penurunan tekanan darah sistolik dan diastolik yang paling baik dan berbeda bermakna (p=0,001). Pengobatan tunggal dan kombinasi dua obat antihipertensi memberikan efek penurunan tekanan darah yang paling baik.Setiap jenis terapi obat antihipertensi tidak memberikan perbedaan bermakna terhadap kualitas hidup kecuali domain aspek efek penyakit (p=0,041). Kata kunci : Hipertensi, gagal ginjal kronik, hemodialisis, antihipertensi, KDQOL, kualitas hidup. 40 ABSTRACT Background: Hypertension is the leading cause of chronic kidney disease (CKD) through a process that resulted in loss of a large number of functional nephron on progressive and irreversible. The Kidney Disease Outcome Quality Initiative (K/DOQI) of The National Kidney Foundation (NKF) recommends a target blood pressure values of < 130/80 mmHg in CKD patients, it can reduce the risk of cardiovascular disease. Objective : The purpose of this study is to understand the antihypertension medication effects to the quality of life of hemodialysis patients in RSUP DR.Hasan Sadikin Bandung. Methods : The design of study is cross sectional observational with concurrent medical record with patients daily status analyses during December 2015 - February 2016 with questionnaire by the Kidney Disease and Quality of Life (KDQOL-36. Results : The analysis result shows significant differences in the reduction of systolic (p=0, 011) and diastolic (p=0, 023) blood pressure in each group of antihypertension therapy. Furthermore, 2 combination of antihypertension therapy gives the most significant different in reduction of systolic and diastolic blood pressure (p=0, 001). Conclusions : Second combination of antihypertension therapy can be reduce of blood pressure better than other therapy. Second combinations of antihypertension therapy is best for decrease of blood pressure and creatinin serum. In all domain of quality of lifes, symptom/problems have a highest score, and burden of kidney disease have a lowest score of quality of life.
