Objectives. The amount of patients with diabetes is increasing, and such patients experience several long-term complications. Therefore, finding a method to treat the disease and its complications is an urgent issue worldwide. In Vietnam, Phyllanthus amarus Schum. & Thonn. (PA) and Gymnema sylvestre R. Br. (GS) are common herbs used in traditional therapy including diabetes treatment. This study aimed to combine PA and GS to extend their bioactivities in antidiabetes, antioxidant, and anti-inflammatory treatments.Methods. Here, PA and GS powders were mixed at different ratios for extraction. Ethanolic extract was used to detect bioactive compounds, bioactivities, and appropriate ratios of the mixtures.Results. The optimal ratio for the PA and GS combination was 2 : 1 (g/g). The ethanolic extraction of the 2 : 1 sample at 50°C over two hours with a solid/liquid ratio of 1 : 10 achieved a high yield of 14.37%. This sample exhibited good α-glucosidase inhibition activity with a half-maximal inhibitory concentration (IC50) of 9.74 µg/mL, antioxidant activity with an IC50 of 29.87 µg/mL, and anti-inflammatory activity with an IC15 of 400 µg/mL.Conclusions. The study confirmed that combining PA and GS can have high α-glucosidase inhibition as well as antioxidant and anti-inflammatory effects.

As the available therapies for diabetes and obesity are not effective enough, diabetologists and educators search for new methods to collaborate with patients in order to support their health behaviors. The aim of this review is to discuss perspectives for the development of new empowerment-type therapies in the treatment of diabetes/obesity. Empowerment is a process whereby patients gain the necessary knowledge to influence their own behavior to improve the quality of their lives. It is carried out in five stages: (1) identify the problem, (2) explain the feelings and meanings, (3) build a plan, (4) act, and (5) experience and assess the execution. Although many years have passed since the advent and popularization of the concept of empowerment, the area remains controversial, mainly with regard to the methodology of therapy. Some previous studies have confirmed the positive effect of empowerment on body weight, metabolic control, and quality of life of patients with type 2 diabetes; however, few studies have been conducted in patients with type 1 diabetes. There is still a need to confirm the effectiveness of empowerment in accordance with Evidence Based Medicine by performing long-term observational studies in a large group of patients. In future, empowerment may become part of the standard of care for patients with diabetes and/or obesity.

Background: Medicinal plants have been used since centuries to cure various diseases. There is a huge potential to investigate the medicinal impacts of different parts of plants. Roots, stem, leaves and fruits of Calotropis procera are known for their biological activities. Calotropis procera plant shows multiple pharmacological activities like anti-cancer, anti-microbial, antioxidant, anti-malarial, hepatoprotective and anti-diabetic activities. Objectives: The objective of the current research was ethanolic extraction of Calotropis procera leaves and to study the phytochemistry and anti-inflammatory activity. Methodology: In this effort, we used the extract of Calotropis procera leaves for detection of phytochemicals and anti-inflammatory activity in vitro by hypotonicity induced hemolysis on 2% HRBC suspension, using UV-Vis spectrophotometer. Results: Phytochemicals like alkaloids, terpenoids, and flavonoids were present in large amount while tannins, saponins, steroids and cardiac glycosides were in small amount, whereas phlobatannins and anthraquinone were not detected. The potential of the ethanolic extract of Calotropis procera leaves was compared with Diclofenac sodium (100μl/ml, 200μl/ml). The leaves extract of Calotropis procera (100, 200, 300, 400, 500μl/ml each) showed significant anti-inflammatory activity by hypotonicity induced hemolysis on 2% HRBC suspension. Conclusion: The Calotropis procera leaves have potential to cure inflammatory diseases and can be used as anti-inflammatory medicine and analgesic.

According to the WHO, diabetes is one of the five most dangerous chronic non-communicable diseases in the world. The annual increase in the number of patients with diabetes in Ukraine is about 5-10%. In 2020, the WHO published new guidelines for the diagnosis and treatment of diabetes and updated its classification. Along with the already familiar type 1 and 2 diabetes and gestational diabetes, a new category of disease has emerged – hybrid forms of diabetes. In the section «Pharmacological management of diabetes» the WHO gives priority to the level of glycemia, the control of which avoids the development of microvascular complications of type 2 diabetes. Initial treatment, according to the WHO 2020, should begin with the appointment of metformin - a drug of the first line of treatment of patients with this disease. It is important that metformin does not cause weight gain, does not provoke hypoglycemia and is recommended for the treatment of people who do not reach the desired level of glycemia through diet and physical activity. Secondgeneration sulfonylureas can be used as an initial first-line treatment when metformin is contraindicated or intolerant. However, this is quite rare: in most cases, metformin is well tolerated by patients if administered gradually. That is, metformin, and, if necessary, sulfonylureas - is an effective, safe and affordable choice compared to expensive novelties, the effectiveness and safety of which have not been proven in long-term studies. In addition, in type 2 diabetes, metformin drugs will form additional effects: cardioprotection, oncoprotection, hepatoprotection, nephroprotection, additional hypoglycemic effect and will fight insulin resistance.

Improved removal of particles during the treatment of natural aquatic suspensions has been achieved by pre-ozonation and the addition of small quantities of iron salts (βFe ≤ 0.1 mg.L-1; “Fe(III)-assisted filtration”) followed by rapid filtration. As shown by investigations on a large-scale installation at Lake Constance Water Supply, this procedure reliably reduces suspended solids by at least 2-3 powers of ten in long-term use. However, the high efficacy of Fe(III)-assisted filtration cannot be explained on the basis of known coagulation mechanisms (like adsorption-charge neutralization, co-precipitation). Instead, the essential step was found to be the conditioning of the filter medium by coating it with colloids containing Fe(OH)3, and this “Fe coating” process occurs only in the presence of alkaline earths (especially Ca2+). According to further experiments, the enhanced solid-liquid separation was ultimately traced to chemical interactions such as the formation of calcium-organic association structures between the iron hydroxides and other solids. For design of Fe(III)-assisted filtration steps, finally, a βCa/DOC ratio above 40 mg.mg-1 and pre-oxidation with ozone dosages not exceeding 2 mg O3/mg DOC was recommended.
