Pediatric inflammatory multisystem syndrome due to severe acute respiratory syndrome coronavirus-2 infection is not very common. It may vary from a simple febrile inflammatory response to a severe multisystem involvement including myocarditis and shock. Here, we report the case of a 10-year-old female child with fever, rash, and mucositis who presented with shock and complete heart block along with encephalopathy and convulsions. The primary investigations revealed raised inflammatory markers along with a high titer of COVID antibodies. She was given full inotrope support along with immunomodulators. She underwent temporary pacing but ultimately needed a dual-chamber pacemaker. The novel coronavirus in children can present with a myriad of manifestations affecting almost every organ. In these pandemic times, one must always keep in mind the possibility of COVID-19 infection in a child that presents with shock, carditis, and arrhythmias. These children need intense immunosuppression and must be managed very aggressively

: We present a case of 75-year-old lady with effort intolerance and baseline ECG showing 2:1 atrio-ventricular block, in whom it was unclear as to requirement of permanent pacing, even after long term ECG monitoring. She underwent a tread mill test during which her QRS became wide and developed complete heart block within 2 minutes of the test. Thus, a simple exercise test helped in confirming level of block to be infra nodal without need for invasive study. In patients with exertional symptoms, even in elderly, and in those where ECG masquerades as a benign entity, exercise testing is useful to differentiate benign cases of atrio-ventricular block from the more serious cases that mandate a pacemaker implantation.

The disaster due to the novel coronavirus disease 2019 (COVID-19) around the world has made investigators enthusiastic about working on different aspects of COVID-19. However, although the pandemic of COVID-19 has not yet ended, it seems that COVID-19 compared to the other coronavirus infections (the Middle East Respiratory Syndrome [MERS] and Severe Acute Respiratory Syndrome [SARS]) is more likely to target the heart. Comparing the previous presentations of the coronavirus family and the recent cardiovascular manifestations of COVID-19 can also help in predicting possible future challenges and taking measures to tackle these issues.

BACKGROUND The expanse of the coronavirus disease 2019 or COVID-19 is huge. The impact is multispectral and affected almost all aspects of human life. SUMMARY Respiratory impact of the COVID-19 is the most felt and widely reported impact. As the novel coronavirus maintained its history of affecting lungs as seen previously in severe acute respiratory syndrome (SARS) outbreak. Ventilators and oxygen support system are required mostly in comorbid patients particularly amongpatientsbearing illnesses like asthma, bronchial impairment and so on. CONCLUSION More study needs to be done in order to assess the impact on the respiratory functioning of the body. Respiratory care must be including proper instruments so that more efficient result can be obtained. Research is needed to promote the invention of specific therapy for targeted action for respiratory functioning improvement.

Abstract This case illustrates acute myocarditis with complete heart block in a 13-year-old teenager as a rare complication of acute dengue illness. He required urgent temporary pacing with inotropic support and antifailure medications. Complete heart block in dengue myocarditis is an acute but reversible condition. A similar presentation in a dengue-endemic country or with a history of travelling to tropical countries warrants a suspicion of dengue infection.

In spite of general complacency about first-degree heart block in acute rheumatic fever, abnormal conduction with dysrhythmias, occasional complete heart block, and, rarely, Stokes-Adams attacks are important early signs of acute rheumatic fever and may precede other signs. Every person with episodic fainting is entitled to an ECG, and frequent ECGs are imperative in any case of rheumatic fever with signs of arrhythmias. Changing atrioventricular block necessitates continuous monitoring for dysrhythmias. A 13-year-old boy who appeared with Stokes-Adams attacks secondary to acute rheumatic fever was successfully treated by temporary pacing.
