As dementia progresses, the elderly with dementia often have difficulties finishing their meals and weight loss is a common feature at this stage of their dementia journey. Eating disorders cause tremendous caregiver stress and burden. In considering provision of best care for this group of elderly, the hospital is probably not the best place and yet, they are frequently brought in to the hospital to seek help for fever, pneumonia, dislodged feeding tube, medical complications arising from poor feeding and dehydration. It is important to understand the aetiology of poor feeding among this group of patients and advise the patients and their family on the appropriate management strategies to improve the intake of food and to maximize their quality of life. At this stage of dementia, focusing on means to ensure nutritional needs are met may end up causing further harm and distress.

This article reports on a few ethical considerations associated with prescribing for the elderly. A large number of elderly people are on several medications for the treatment of various chronic conditions that develop with age. Most of these treatments involve symptom management and prevention of symptoms likely to occur from untreated diseases. Serious consideration should be given when prescribing for the elderly. These include: allowing the elderly and their families to be involved in their treatment; social support; and cost–benefit considerations. A lack of clear therapeutic guidelines exists for prescribing in this group of people that is confounded by factors such as quality of life and life expectancy.

The demographics of the renal population have evolved, such that the elderly are the fastest growing group. Consequently, the multidisciplinary renal team often encounters clinical issues associated with advancing age. In this chapter, frailty, falls, and cognitive impairment are discussed as underrecognized geriatric syndromes in patients with chronic kidney disease. A case history illustrates how these syndromes tend to co-exist and affect quality of life and survival. The risk factors, management strategies, and future research priorities are discussed, recognizing the importance of a multidisciplinary approach to the management of elderly patients with kidney disease.

The prevalence of malnutrition in people over 60 is 10–40%. Inadequate nutrition worsens the quality of life and the functional status of patients in elderly and senile age. The development of eating disorders may be due to various factors. Diagnosis of malnutrition is aimed at identifying it and finding the causes. Treatment should be comprehensive, including adequate nutritional support and correction of factors leading to the development of malnutrition. It must be remembered that some eating disorders can cause the development or progression of various geriatric syndromes (falls, urinary incontinence, cognitive disorders, depression, etc.)
