A 38 years male, admitted in the department of Internal Medicine with a history of snake bite in his left lower leg during agriculture work. He was treated with anti snake venom after admission. He had history of dimness of vision in both eyes. On examination, swelling and subcutaneous hemorrhage was noted in his left leg. His best corrected visual acuity (BCVA) was 6/60 in the both eyes. Fundus examination with 90 D lens and Indirect Ophthalmoscopy of both eyes showed retinal hemorrhage with cotton wool spots. Blood tests revealed increased titers of D-dimer and fibrin degradation products. The patient was followed up regularly at 2 weeks interval and BCVA and Fundus examination was carried out. At the end of 10 weeks, retinal hemorrhages had significantly cleared in both eyes with improvement of vision in both eyes (BCVA of right eye at last follow-up 6/6 and left eye 6/9).Bangladesh Journal of Medical Science Vol.14(3) 2015 p.297-298

We report a case of 55-year-old women, with a medical history of hypertension and diabetes, who complained of a sudden loss of vision of her left eye. Her VA was less than 20/200; slit lamp and gonioscopy were unremarkable. Dilated fundus examination had shown an Aspect of HRVO with a Thunder of flame shaped retinal hemorrhages covering the inferior half of the retina, tortuous veins and cotton-wool spots.

Purtscher-like retinopathy is associated with retinal hemorrhages and ischaemia probably due to the complement-mediated leukoembolization. It is a rare and severe angiopathy found in conditions such as acute pancreatitis.Case. We present a case of a 53-year-old man who presented with a Purtscher-like retinopathy associated with the development of acute pancreatitis in the context of a Klatskin tumour (a hilar cholangiocarcinoma). The ophthalmologic evaluation revealed the best corrected visual acuity (BCVA) of 20/32 in the right eye (RE) and of 20/40 in the left eye (LE); biomicroscopy of anterior segment showed scleral icterus and fundoscopy revealed peripapillary cotton-wool spots, optic disc edema, and RPE hypo- and hyperpigmentation in the middle peripheral retina in both eyes with an intraretinal hemorrhage in the LE. 15 months after the initial presentation, without ophthalmological treatment, there was an improvement of BCVA to 20/20 in both eyes and optical coherence tomography (OCT) revealed areas of reduction of retinal nerve fiber layer thickness corresponding to the previous cotton-wool spots.Conclusion. Purtscher-like retinopathy should not be neglected in complex clinical contexts. Its unclear pathophysiology determines an uncertain treatment strategy, but a meticulous follow-up is compulsory in order to avoid its severe complications.

It is known that no single method for the determination of fibrinogen/fibrin degradation products (FDP) indicates their nature which is important to establish the state of activation of the fibrinolytic system. Therefore we applied two simple and reproducible laboratory methods to study eleven preoperative patients with pelvic masses: 1) the latex fibrin polymerization inhibition test (Finkelstein test)and 2) the staphylococcal cumpling test. The normal values obtained were 0-3 polymerization inhibition units (PIU) and 2.5-4 μg fibrinogen equivalents per milliliter, respectively. Seven clinically healthy females of the same age range served as controls.Out of seven cases with confirmed postoperative diagnosis of ovarian carcinoma, five (71%) had positive FDP by both methods with average values of 12.2 ± 5.9 PIU and 10.4 ± 4.6 μg/ml. The ratio between PlU/ml and μg/ml was determined and the range found was 0.53-1.75 which is indicative of the presence of FDP of the “late” type. The values are in agreement with previous reports of the incidence of late FDP in other malignancies. All the negative FDP, except two, corresponded to benign ovarian cyst, although there was a case of haemorrhagic cyst with positive FDP.The presence of “late” FDP indicates an activated fibrinolytic process and could constitute a cautionary sign for the postoperative care of the patient. We consider that the application of two methods for FDP permits to detect an increased amount and also the type of the circulating FDP. The data presented here clearly warrant detailed investigation of the potential benefits of knowing the nature of FDP in monitoring the postoperative follow up of patients with pelvic masses.

In the past few months, medical practice has changed faster than in the past few decades. This was due to the coronavirus disease (COVID-19) pandemic. Ophthalmic problems are most significant in patients with severe pneumonia. The exact incidence of conjunctivitis in COVID-19 patients ranges from 0.8 to 31.6 %. Optical coherence tomography demonstrated hyperreflective lesions at the level of ganglion cells and inner plexiform layers, more pronounced in the papillomacular bundle of both eyes. Retinal hemorrhages were found in at least one eye in five patients (9.25 %), cotton wool spots were detected in four people (7.4 %), and drusen was observed in six patients (11.1 %). On examination of the fundus, dilated veins were revealed in 15 patients (27.7 %), tortuous vessels — in 7 individuals (12.9 %). The literature reports describe patients who were diagnosed with COVID-19 after the detection of diplopia and ophthalmoparesis. The incidence of ocular complications in patients of the intensive care units in different studies ranges from 3 to 60 %. The most common manifestations are ocular surface diseases, increased intraocular pressure, and disorders of the anterior and posterior segments.

Patients with chronic venous disease (CVD) have elevated levels of leucocyte elastase (LE) released from the activation of leucocytes. In acute deep venous thrombosis (DVT), LE can degrade fibrin from the thrombus resulting in cross-linked fibrin degradation products (E-XDP) being released into the bloodstream. In patients with CVD the levels and significance of circulating E-XDP are unknown. We aimed to investigate the association between plasma E-XDP concentration and severity of CVD. Levels of E-XDP were quantified with a specific enzyme-linked immunosorbent assay (ELISA) in plasma from 142 consecutively recruited CVD patients (mean age 64 years, (range 23–89), 81 were females and 61 males). Patients were also divided into three groups based on CVD severity using the C-class of the Clinical-Etiological-Anatomical-Pathophysiological (CEAP) classification, with C 0–1 class as the reference group, C 2–3 as the second group and C 4–6 as the third group with the most severely affected patients. We found significantly elevated levels of E-XDP in patients with C 4–6 compared with patients with C 0–1 (p = 0.007) and increased with increasing disease severity across the groups (p = 0.02). Significant independent association was observed between levels of E-XDP and the classes C 4–6 after adjustment for age and sex (p < 0.05), but the association was no longer significant after further adjustment for use of statins, use of anticoagulants and history of DVT (p = 0.247). This exploratory study shows that E-XDP levels are elevated in patients with CVD, encouraging further studies on the role of E-XDP in CVD.

This report describes a patient with bilateral endogenous candida chorioretinitis. The patient had a 2-day history of bilateral blurred vision. Fundus photography revealed multiple chorioretinal infiltrations in both eyes and a parafoveal hemorrhage in the left eye. After 2 days, fundus examination showed an increased number of infiltrations and hemorrhages in both eyes and worsening vitreous inflammation. A large infiltrative intraretinal lesion and a retinal hemorrhage of the left eye were discovered on optical coherence tomography. Candida albicans was diagnosed from blood culture. The bilateral candida chorioretinitis had not responded to systemic or topical antifungal medication. The chorioretinitis was refractory to intravitreal amphotericin B as well. Intravitreal voriconazole injection in both eyes and intravitreal bevacizumab injection in the left eye were performed thereafter. The chorioretinal infiltrations and hemorrhages decreased in both eyes. Intravitreal voriconazole injection was effective in the treatment of intractable candida chorioretinitis.

Background: This is a Prospective observational study conducted in Department of Obstetrics and Gynaecology, BSMMU, Dhaka.Objective: The purpose of this study was to evaluate maternal and perinatal outcome of pregnancy with history of previous caesarean section.Method: Data were collected as per questionnaire by researchers herself by interviewing the patients and by observing the operations, investigation records and post-operative follow-up. A total number of 150 patients admitted with pregnancy with history of one or more previous caesarean section.Outcome measure: To find out antepartum complications, per-operative and post-operative complications specially related to previous caesarean section and to find out perinatal mortality and morbidity.Results: Among 150 patients who were delivered after one or more previous caesarean section, 88 (52%) patients had antepartum complications, the majority (88%) pregnancies were term pregnancy. The per-operative problem was difficulty to reach lower uterine segment due to adhesion with bladder was 16%. Overall post-operative complications were 20% cases. The common complications were wound infection (86%). Perinatal complications were 20 (30%) cases.Conclusion: The wide spread improvement in anaesthesia, surgical technique, antibiotics and blood transfusions have decreased the morbidity and mortality from caesarean section, but it is not without hazard.Bangladesh Journal of Medical Science Vol.15(3) 2016 p.392-398

Choroidal folds present as parallel bright and dark lines, which may be detected with funduscopy. Optical coherence tomography, red free fundus photography, autofluorescence and fluorescein angiography may be also helpful to establish the diagnosis. The authors present the case of a 70-year-old male who was evaluated because of a 1-month history of blurred vision on his right eye. Dilated fundus examination revealed choroidal folds in both eyes, which failed to affect visual acuity. No neurogical pathologies were found. There was no change in the patient condition durind a follow-up period of 6 months. The authors note that choroidal folds are often not recognized because they are usually asymptomatic. There are several possible causes which should be considered. The diagnosis of idiopathic choroidal folds is based on the exclusion of other pathologies. Observation of the patient’s visual acuity and monitoring for fundus changes are needed. Orv. Hetil., 2014, 155(27), 1083–1086.
