Background and objective: Urinary tract infection (UTI) is one of the frequently seen infections both in the world and in our country as well. Escherichia coli (E. coli) aremost frequently isolated in complicated or uncomplicated, nosocomial or community acquired urinary tract infections. To guide the empirical therapy, the resistance pattern of E. coli responsible was evaluated throughout the period in this study.Material and Method: Urine samples from outpatient / inpatient department of Ibn Sina Medical College Hospital between 1st January, 2015 and 31st December, 2015 were retrospectively analyzed. Presence of ?105 colony forming units/ml in urine culture was considered as significant for UTI. Isolated bacteria were identified by standard laboratory techniques and antibiotic susceptibility testing was performed by Kirby-Bauer disk diffusion method using Clinical Laboratory Standard Institute (CLSI) criteria.Result: A total of 271(13.4%) uropathogens were isolated. Overall E. coli accounted for 180 (66.42%) of all isolates. Resistance rates of E. coli to antimicrobial agents was demonstrated to be as follows: cefuroxime 82%, nalidaxic acid 74%, azithromycin 56%, cefotaxime52%, ceftazidime50%, cefixime 47%, cotrimoxazole 43%, ceftriaxone 41%, ciprofloxacin38%, amoxicillin- clavulanic acid 31%, cefepime30%, and low resistance which ranges from 9 to 1% included gentamycin 9%, meropenem 3%, imipenem2%, nitrofurantoin2% and amikacin 1%.Conclusion: As resistance rates show regional differences, it is necessary to regularly monitor regional resistance pattern to determine the appropriate empiric antibiotic treatment. INTRODUCTION: Urinary tract infection (UTI) is considered as the most common bacterial infectious disease seen among the pediatric patients. Most commonly, members of Enterobacteriaceae, particularly uropathogenic strains of E. coli and Enterobacter spp. are the primary causative pathogens of UTI in the different part of the world. Emergence of antimicrobial resistance rates among pathogens recovered from urinary tract infections is an increasing problem in the specific region. MATERIALS AND METHODS: Prevalence and anti microbial susceptibility pattern of the bacterial uropathogens isolated from the children attending Gandaki Medical College Teaching Hospital and Reserch Center (GMC) Nepal. A total of 155 children aged upto 15 years were included in this study. Urine cultures were carried out and the isolates were identified by Gram staining and conventional biochemical methods. Antimicrobial susceptibility testing was performed by disk diffusion method according to the Clinical and Laboratory Standards Institute (CLSI). RESULTS: In the present study 21.3 % of the sample size, showed significant bacterial growth. E. coli was the most frequently occurring pathogen (39.40%), followed by Proteus spp. 21.2%, Citrobacter spp. and Streptococcus faecalis (12.1%) Klebsiella spp. (9.1%), and Staphylococcus aureus and Enterobacter (3.0%). Susceptibility rate of E. coli were 69.2% to Gentamycin and Amikacin, 53.8% to Norfloxacin, 38.4% to Nalidixic acid and Norfloxacin. CONCLUSIONS: Pediatric urine culture isolates were becoming increasingly resistant to commonly used antibiotics. Finally, we suggest that empirical antibiotic selection should be based on knowledge of the local prevalence of bacterial organisms and antibiotic sensitivities rather than on universal guidelines. Background Urinary Tract Infection (UTI) is one of the most common infectious diseases which affect almost all ages groups of population. Production of â-lactamases is responsible for antibacterial resistance which is frequently observed in Enterobacteriaceae isolates, particularly by E. coli and Klebsiella pneumoniae. This investigation has been carried out to determine the current status of prevalence and susceptibility of uropathogens isolated among the patients at tertiary care hospital in eastern Nepal.Material and Methods This study was done at the department of Microbiology, Nobel Medical College Teaching Hospital, Biratnagar, Nepal during May 1st 2015 to October 31st 2015. Midstream cleancatch urine was sampled from 1730 suspected urinary tract infection patients of different age and sex groups. Uropathogens were recognized in term of standard and specific microbiological techniques and antimicrobial susceptibility pattern was determined by Kirby Bauer Disc diffusion method following Clinical and Laboratory Standards Institute (CLSI) guidelines.Results Out of 1730 suspected specimens Culture resulted a total of 761 (43.98 %) positive and 969 (56.02%) negative among that significant growths of uropathogens including 700 (91.98 %) unimicrobial and 60 (7.88 %) polymicrobial growths. In term of Gender distribution 443 (25.60 %) were male and 1287 (74.40 %) were female hence the ratio is 0.34:1, respectively. E. coli was the leading isolate (66 %), followed by Klebsiella spp. (12 %), Enterococcus spp. (8 %), Pseudomonas spp. (6 %), Acinetobacter anitratus (5 %), Proteus spp. (3 %).Conclusion The high frequency of multidrug resistance in bacterial uropathogens was seen. Principally, resistance patterns were seen higher for amoxycillin, co-trimoxazole, flouroquinolones and third-generation cephalosporins, Existing uropathogens highlights the highest rate of vulnerability to nitrofurantoin, amikacin and gentamicin which provide much better antibiotic coverage and can be adapted for practical treatment of urinary tract infections.  Journal of Nobel Medical College Vol.5(2) 2016; 51-55

Background: Since antibiotic resistance of uropathogens has gradually been rising, knowledge of antimicrobial resistance pattern of Escherichia coli (Esch. coli), the predominant  pathogen of urinary tract infection (UTI), is important in selecting empirical antimicrobial  therapy.Objectives: To find out the common organisms causing UTI with their antimicrobial  susceptibility pattern in Enam Medical College Hospital.Materials and Methods: This study  was carried out in the department of Microbiology, Enam Medical College Hospital, Savar,  Dhaka between January 2010 to June 2011 to see the antimicrobial resistance pattern of Esch.  coli in urine sample. A total of 512 urine samples were studied from clinically suspected UTI  cases irrespective of age and sex. Esch. coli were isolated and antibiogram of the isolates were  done.Results: The results of antibiogram revealed the resistance pattern of the isolated Esch. coli to ampicillin (93.94%), cefradine (70.71%), trimethoprim-sulfamethoxazole (48.49%),  azithromycin (41.42%), ciprofloxacin (39.40%), ceftazidime (35.56%), nitrofurantoin  (29.30%), ceftriaxone (10.11%), gentamicin (7.08%) and imipenem (3.04%).Conclusion: It  can be inferred that our findings will render useful information to clinicians in determining the  appropriately directed antimicrobial regimen when given empirically.  Objectives: To determine the frequency of Urinary tract infection (UTI) and the antibiotic sensitivity patterns of isolated uropathogens in pregnant females among middle socioeconomic settings. Study Design: Retrospective Cross Sectional study. Setting: Jinnah Medical College Hospital Karachi. Period: From Jan 2017 to Dec 2017. Material & Methods: This retrospective study was carried out in Jinnah Medical College Hospital (JMCH) Karachi after taking ethical review board approval. All the pregnant women who were registered in antenatal clinics from January 2017 to December 2017 was included. Data was taken from medical record department and information about age, parity, gravida, miscarriages, microorganisms of the urine samples and their antibiotic susceptibility pattern was noted. Results: Five hundred and sixty-four pregnant women of age 17-44 years were registered in the hospital in antenatal clinics during the year 2017. Out of 564 females, 48 (8.50%) presented with urinary tract infection some time during their pregnancy period. Majority of patients (54.2%) were infected with Escherichia coli (E coli), followed by Klebsiella pneumonia (16.77%), Pseudomonas spp. (12.5%), Enterococcus spp. (8.3%) and Staphylococus aureus (8.3%). These etiologic agents vary in their sensitivity pattern to antibiotics. Piperacillin-tazobactam had the highest overall sensitivity of (89.6%). This was followed by Meropenem (87.5%), Nitrofurantoin (87.5%) and Fosfomycin (81.3%). Cefixime, Cefotaxime and Amoxicillin/ clavulanic acid had overall sensitivities above 50%. Conclusion: E coli was the common organisms followed by others gram negative and positive organisms. Enteroococus spp. was also a significant bacterial isolates in this settings. Meropenem, Piperacillin-tazobactam, Nitrofurantoin, Fosfomycin were the highly effective antibiotics against isolated uropathogens. Amoxicillin/clavulanic acid and Cephalosporin also had a good results.

Urinary tract infection is a common complication of Nephrotic syndrome, many cases remain asymptomatic. This study was aimed to determine the possible underlying causes of urinary tract infection in Nephrotic syndrome. The study population in this descriptive study included children aged six months to fourteen years old with Nephrotic syndrome referred to Paediatric ward of Faridpur Medical College Hospital from June 2015 to March 2016. The mean age was 6.12±3.25 years in UTI group and 7.26±3.39 years in without UTI group. Male were predominant in both groups, 15(60.0%) in UTI group and 24(64.9%) in without UTI group. Serum albumin, total protein, urinary protein excretion, the number of white blood cells, hemoglobin, erythrocyte sedimentation rate, blood creatinine, nitrogen, blood urea, serum triglycerides were not statistically significant between the groups. The most common microorganisms involved in urinary tract infection were: Escherichia coli (13%) & Klebsiella (13%). Majority 29(46.8%) patients had fever, 12(19.4%) had diarrhea, 11(17.7%) had bad smell of urine, 10(16.1%) had vomiting associated with diarrhea and 10(16.1%) had respiratory symptoms. Children with nephrotic syndrome are frequently predisposed to urinary tract infection and in most cases it is asymptomatic, often undiagnosed. Background : Urinary tract infection (UTI) is one of the most important causes of morbidity and mortality in the developing countries like Bangladesh. Antimicrobial agents are the frequently used drug for its treatment. Periodic evaluation of antimicrobial activity of different antibiotics is essential as the pattern of antibiotic sensitivity may vary over periods. Increasing antibiotic resistance among urinary pathogens to commonly prescribed drugs has become a global reality today.Objective : To determine the prevalence and to find out the causative agents of UTI and their antibiotic sensitivity pattern among suspected UTI patients attending Northern International Medical College Hospital (NIMCH), Dhaka.Methods : A retrospective study was conducted at Northern International Medical College Hospital, Dhaka, Bangladesh by analyzing the records of urine samples collected for culture and sensitivity tests over a period of six months (January 2014 to June 2014). Of the total 878 clean catch mid-stream urine samples collected from suspected cases of UTI patients of all ages and both sexes. Urine specimens were cultured for isolation of microbial agents of urinary tract infection. The isolated bacteria were identified using biochemical test. The diffusion susceptibility test was used to determine susceptibility of bacterial agents to antibiotics. Computerized data was collected from Microbiology department of NIMCH and analyzed by Microsoft Excel Version 2013.Results : In this study, 182 (20.73%) out of 878 urine sample were positive for pathogenic organisms. Of the various pathogenic organisms isolated, Escherichia coli constituted for 85.16% followed by Pseudomonas sp, Acinatobactersp, Group D Streptococcus, Staphylococcus aureus, Klebsiellasp, Enterobactersp and others. E.coli was found to be most sensitive to Imipenem, Amikacin and Meropenem and resistant to most commonly used oral drugs like Azithromycin, Cefexime, cotrimoxazole and Ciprofloxacin and Levofloxacin.Conclusion : Mainly Gram negative bacilli is responsible for UTI and most frequent isolated bacteria was E-coli. The most effective antibiotics were Imipenem, Amikacin, Meropenem,all of them are parentral. Majority E-coli were resistant to commonly used oral drugs like Azithromycin, Cefexime, cotrimoxazole and Ciprofloxacin. Background: Hospital acquired urinary tract infection among the hospitalized catheterized patient due to different indications are a common morbility. The objective of study to see the prevalence hospital acquired urinary tract infection among the patients of indwelling catheter admitted in the department Medicine of Chittagong Medical College Hospital. Methods: It was a hospital based obsevational study conducted on purposively selected (Non-probability) hospitalized patients who got indwelling catheter had admitted into the Medicine ward, Chittagong Medical College Hospital from 1st January 2013 to 30th June 2013. Total 50 patients were selected. Urine was analyzed for growth of organism and sensitivity was also done in case where growth was positive. Data was analyzed by statistical method with the software SPSS-20. Results: Results showed that 15 (30%) developed bacteriuria or urinary tract infection with catheter. Development of bacteriuria was not affected by sex, age. People who introduce the catheter has some influence with the occurrence of UTI. E.coli, Klebsella, Proteas and Pseudomonas aeroginosa were the most common bacteria isolated. Imepenam was found the most sensitive antibiotic. Conclusion: Catheter play an important risk for occurrence of UTI. So it should be avoided if possible. Chatt Maa Shi Hosp Med Coll J; Vol.17 (1); Jan 2018; Page 42-45

Among hospitalized patients, the most common nosocomial infection is Urinary tract infection (UTI). The knowledge about the type of pathogens responsible for UTI and susceptibility and resistance pattern of the causative agents at a specific area may help the doctors to choose correct treatment regimen. This study was aimed to investigate the antibiotic susceptibility and resistance pattern of isolated urinary pathogens. This study was done at Anwer Khan Modern Medical College Hospital, Dhaka during January- June, 2011. Out of 498 clinical samples of urine collected, 245 (49.19%) showed significant bacterial growth. The most common pathogens isolated were Escherichia coli (142, 58.0%), Streptococcus feacalis (38, 15.5%), Pseudomonus (20, 8.2%), Klebsiella species (20, 8.2%) and Staphylococcus epidermidis (14, 5.7%). Members of the Enterobacteriaceae were 75%-100%sensitive to Amikacin and Nitrofurantoin while they were found variably sensitive to other commonly used antibiotics. Pseudomonas species were found 90% sensitive to Meropenem and 70% to Amikacin. Strep. feacalis were found 94.7% sensitive to Amoxicillin, 84.2% to Amoxiclave and 78.9% to Ciprofloxacin, 65.5% to Cephalexin, 50% to Ceftriaxone. The clinicians should use Meropenem and Amikacin selectively in cases of un-responsiveness to commonly used antibiotics. Urinary tract infection (UTI) is the most common nosocomial infection among hospitalized patients. Area-specific monitoring studies aimed to gain knowledge about the type of pathogens responsible for UTIs and resistance pattern of the causative agents may help clinicians to choose correct treatment regimen. So, the present study was aimed to investigate the antibiotic resistance of urinary pathogens isolated at Chittagong Medical College Hospital (CMCH) during January to June, 2007. Out of 360 clinical samples of urine collected, 175 (48.61%) showed significant bacterial growth. The most common pathogens isolated were Escherichia coli (66, 37.71%), Klebsiella species (60, 34.29%), Proteus species (17, 9.71%) and Pseudomonas species (16, 9.14%). Members of the Enterobacteriaceae were 80%-100% sensitive to Imipenem while they were found variably sensitive to other commonly used antibiotics. Pseudomonas species were 100% sensitive to Imipenem, 75% to Amikacin, and 50% to Ceftazidime. Staphylococcus aureus was found 100% sensitive to Vancomycin, 80% to Gentamicin and Amikacin each, and 55% to Oxacillin. Enterococcus species were 80% sensitive to Imipenem, 60% to Ciprofloxacin, 50% to Vancomycin and Amikacin each. The clinicians should use Imipenem selectively in cases of un-responsiveness to commonly used antibiotics.Bangladesh J Med Microbiol 2008; 02 (02): 17-21
