 Objectives: To determine the frequency of Urinary tract infection (UTI) and the antibiotic sensitivity patterns of isolated uropathogens in pregnant females among middle socioeconomic settings. Study Design: Retrospective Cross Sectional study. Setting: Jinnah Medical College Hospital Karachi. Period: From Jan 2017 to Dec 2017. Material & Methods: This retrospective study was carried out in Jinnah Medical College Hospital (JMCH) Karachi after taking ethical review board approval. All the pregnant women who were registered in antenatal clinics from January 2017 to December 2017 was included. Data was taken from medical record department and information about age, parity, gravida, miscarriages, microorganisms of the urine samples and their antibiotic susceptibility pattern was noted. Results: Five hundred and sixty-four pregnant women of age 17-44 years were registered in the hospital in antenatal clinics during the year 2017. Out of 564 females, 48 (8.50%) presented with urinary tract infection some time during their pregnancy period. Majority of patients (54.2%) were infected with Escherichia coli (E coli), followed by Klebsiella pneumonia (16.77%), Pseudomonas spp. (12.5%), Enterococcus spp. (8.3%) and Staphylococus aureus (8.3%). These etiologic agents vary in their sensitivity pattern to antibiotics. Piperacillin-tazobactam had the highest overall sensitivity of (89.6%). This was followed by Meropenem (87.5%), Nitrofurantoin (87.5%) and Fosfomycin (81.3%). Cefixime, Cefotaxime and Amoxicillin/ clavulanic acid had overall sensitivities above 50%. Conclusion: E coli was the common organisms followed by others gram negative and positive organisms. Enteroococus spp. was also a significant bacterial isolates in this settings. Meropenem, Piperacillin-tazobactam, Nitrofurantoin, Fosfomycin were the highly effective antibiotics against isolated uropathogens. Amoxicillin/clavulanic acid and Cephalosporin also had a good results.

Background and objective: Urinary tract infection (UTI) is one of the frequently seen infections both in the world and in our country as well. Escherichia coli (E. coli) aremost frequently isolated in complicated or uncomplicated, nosocomial or community acquired urinary tract infections. To guide the empirical therapy, the resistance pattern of E. coli responsible was evaluated throughout the period in this study.Material and Method: Urine samples from outpatient / inpatient department of Ibn Sina Medical College Hospital between 1st January, 2015 and 31st December, 2015 were retrospectively analyzed. Presence of ?105 colony forming units/ml in urine culture was considered as significant for UTI. Isolated bacteria were identified by standard laboratory techniques and antibiotic susceptibility testing was performed by Kirby-Bauer disk diffusion method using Clinical Laboratory Standard Institute (CLSI) criteria.Result: A total of 271(13.4%) uropathogens were isolated. Overall E. coli accounted for 180 (66.42%) of all isolates. Resistance rates of E. coli to antimicrobial agents was demonstrated to be as follows: cefuroxime 82%, nalidaxic acid 74%, azithromycin 56%, cefotaxime52%, ceftazidime50%, cefixime 47%, cotrimoxazole 43%, ceftriaxone 41%, ciprofloxacin38%, amoxicillin- clavulanic acid 31%, cefepime30%, and low resistance which ranges from 9 to 1% included gentamycin 9%, meropenem 3%, imipenem2%, nitrofurantoin2% and amikacin 1%.Conclusion: As resistance rates show regional differences, it is necessary to regularly monitor regional resistance pattern to determine the appropriate empiric antibiotic treatment. Diabetic patients have more outcomes of urinary tract infection than non diabetes, mortality of urinary tract infection (UTI) is 5 times higher in patients with diabetes old age, the incidence of urinary tract infection in pregnant women is slightly higher than non pregnant women, urinary tract infection in pregnant women either as a symptomatic bacteriiuria or symptomatic infection, which is more complication during pregnancy this present study was done in March 2016 to July 2016, the patients were females, total samples of 80 patient of pregnant and non pregnant with diabetes mellitus, selected from medical and general surgical wards, the samples were sending to teaching laboratories from the same hospital, the present study show that the incidence of UTI at non pregnant diabetic women occur at age (9-11) years (45%), the most common causative agents of UTI in diabetic pregnant females is Esherichia coli 20(50%), the results of microscopical examination of urine of pregnant with diabetes mellitus of urine of pregnant with diabetes mellitus revealed that (43), pyuria and (28) had crystals, and the result of microscopical examination of urine of non pregnant females with diabetes mellitus revealed that the common causative agent is Esherichia coli 3(30%), (9) had pyuria and (4) had crystals.

Escherichia coli (E. coli) is the most common type of pathogen that causes Urinary tract infection disease. It can be presented as a pathogenic or non-pathogenic strain and found not only in the animal but also in the human intestine. This bacterium can cause opportunistic infection when the human host comprised of thalassemia patients or changes the healthy hemostatic flora. This study aimed to analyze the presence of bacteria in thalassemia patients with urinary tract infection. A total of 303 samples were collected during the period from August 2019 to January 2020 from thalassemia patients who suffered from urinary tract infection. The results showed that there were 6.9% of patients infected with E. coli, 2.6% of patients were infected with S. aureus, 0.7% with both Proteus and Klebsiella, while 89.1% of patients had a negative sample for bacteria. Also, the incidence of urinary tract infections in females is higher than in males. Besides, its occurrence in rural areas is higher than in city residents. Moreover, among 16 antibiotics tested to sensitize bacteria to antibiotics, Imipenem showed 100% efficacy on all isolated bacteria. In contrast, Netilmicin showed 80.1% efficacy, Gentamycin 80.1%, and Amikacin 76.2%. Ampicillin, Aztreonam, Amoxicillin-Clavulanic Acid, Tetracycline, and Ticarcillin-Clavulanic Acid, did not show any effectiveness toward the bacteria while other antibiotics showed different activities. Furthermore, the isolated microbes from thalassemia patients were the highest resistance to antibiotics in comparison with other studies, and this antibiotic-resistant may be due to the weakening of the patient's immune status and frequent blood taking and the antibodies it contains.

Urinary tract infection (UTI) is a major bacterial infection causing serious health problem in pregnant women. The physiological and anatomical changes in pregnancy facilitate urinary tract infection (UTI) during pregnancy. Asymptomatic bacteriuria in pregnancy is associated with pyelonephritis, preterm labour and low birth weight infants. The study was designed to characterise phenotypically and genetically the major organism associated with UTI among pregnant women in Osun State. A cross-sectional study design was used to collect mid-stream urine samples between March 2018 to September 2018 from 150 pregnant and 50 non-pregnant women which serve as control. Samples were inoculated into Cysteine Lactose Electrolyte Deficient (CLED) medium, subcultured onto MacConkey and Blood agar plates. A standard agar disc diffusion method was used to determine antimicrobial susceptibility pattern of the isolates and the molecular detection of the antibiotic resistant genes were done. Data were subjected to descriptive statistics. The ages of women enrolled in this study ranges from 22 to 43 years (mean ± standard deviation = 25 ± 4.7 years). The predominant bacteria identified were E. coli (34.5%), S. aureus (10.3%), coagulase negative Staphylococci [CoNS] (17.2%), Klebsiella species (6.9%) and Enterobacter species (31.0%). Majority of Gram-negative bacteria isolates were resistant to ampicillin (70%), cefotaxime (62%), while 75–100% of the Gram positive isolates were resistant to ampicillin. Multiple drug resistance was observed, all the E. coli isolates were resistant to Cefotaxime, meropenem and ampicillin. Of E. coli isolates, 4, 3 and 6 were positive for the VIM, ctx-M and TEM genes respectively.Similarly, the risk of UTI was higher in those had previous UTI history (OR = 2.29, 95% CI = 1.15–4.56, P = 0.019) as compared to those who had no previous history of UTI.

Background: Urinary tract infection (UTI) is a common and frequently encountered serious morbidity that afflicts the tool not only to all segments of human population but also results in increasing antibiotic resistance due to persistence and mismanagement of the ailment. The present study aimed to ascertain the current situation of antimicrobial resistance of urinary tract infection. Objectives: To find out pyuria by direct microscopy, isolation and identification of the organisms by culture and to know the susceptibility pattern of organisms causing urinary tract infection. Materials and Methods: A total of 180 midstream urine samples were subjected to aerobic bacteriologic culture in the department of Microbiology, Cox’s Bazar Medical College and 250-bedded Sadar Hospital, Cox’s Bazar during the period of March to October 2012. Specimens were collected from hospitalized and outdoor patients of different age and sex groups. All specimens were examined by routine microscopy to find out significant pyuria (>5 pus cells/HPF). Strict aseptic precautions were taken all through the culture system. Results: Out of 180 specimens, 101 (56.11%) culture yielded significant growth of single organism and 79 (43.89%) yielded no growth. The isolated organisms were E. coli 74.26%, Klebsiella species 12.87%, Enterococci 4.95%, Staphylococcus saprophyticus 3.96%, Pseudomonas species 1.98% and Proteus species 1.98%. The highest sensitivity was shown by imipenem (100%) followed by ceftriaxone (65%), azithromycin (65%), ciprofloxacin (60%) and less sensitive to amoxycillin, cotrimoxazole, cephradine and nalidixic acid ranging 15–30%. Conclusion: The findings of the present study recommends that UTI should be treated by selective antibiotics obtained from culture and sensitivity test to minimize increasing trend of drug resistance. INTRODUCTION: Urinary tract infection (UTI) is considered as the most common bacterial infectious disease seen among the pediatric patients. Most commonly, members of Enterobacteriaceae, particularly uropathogenic strains of E. coli and Enterobacter spp. are the primary causative pathogens of UTI in the different part of the world. Emergence of antimicrobial resistance rates among pathogens recovered from urinary tract infections is an increasing problem in the specific region. MATERIALS AND METHODS: Prevalence and anti microbial susceptibility pattern of the bacterial uropathogens isolated from the children attending Gandaki Medical College Teaching Hospital and Reserch Center (GMC) Nepal. A total of 155 children aged upto 15 years were included in this study. Urine cultures were carried out and the isolates were identified by Gram staining and conventional biochemical methods. Antimicrobial susceptibility testing was performed by disk diffusion method according to the Clinical and Laboratory Standards Institute (CLSI). RESULTS: In the present study 21.3 % of the sample size, showed significant bacterial growth. E. coli was the most frequently occurring pathogen (39.40%), followed by Proteus spp. 21.2%, Citrobacter spp. and Streptococcus faecalis (12.1%) Klebsiella spp. (9.1%), and Staphylococcus aureus and Enterobacter (3.0%). Susceptibility rate of E. coli were 69.2% to Gentamycin and Amikacin, 53.8% to Norfloxacin, 38.4% to Nalidixic acid and Norfloxacin. CONCLUSIONS: Pediatric urine culture isolates were becoming increasingly resistant to commonly used antibiotics. Finally, we suggest that empirical antibiotic selection should be based on knowledge of the local prevalence of bacterial organisms and antibiotic sensitivities rather than on universal guidelines. Background Urinary Tract Infection (UTI) is one of the most common infectious diseases which affect almost all ages groups of population. Production of â-lactamases is responsible for antibacterial resistance which is frequently observed in Enterobacteriaceae isolates, particularly by E. coli and Klebsiella pneumoniae. This investigation has been carried out to determine the current status of prevalence and susceptibility of uropathogens isolated among the patients at tertiary care hospital in eastern Nepal.Material and Methods This study was done at the department of Microbiology, Nobel Medical College Teaching Hospital, Biratnagar, Nepal during May 1st 2015 to October 31st 2015. Midstream cleancatch urine was sampled from 1730 suspected urinary tract infection patients of different age and sex groups. Uropathogens were recognized in term of standard and specific microbiological techniques and antimicrobial susceptibility pattern was determined by Kirby Bauer Disc diffusion method following Clinical and Laboratory Standards Institute (CLSI) guidelines.Results Out of 1730 suspected specimens Culture resulted a total of 761 (43.98 %) positive and 969 (56.02%) negative among that significant growths of uropathogens including 700 (91.98 %) unimicrobial and 60 (7.88 %) polymicrobial growths. In term of Gender distribution 443 (25.60 %) were male and 1287 (74.40 %) were female hence the ratio is 0.34:1, respectively. E. coli was the leading isolate (66 %), followed by Klebsiella spp. (12 %), Enterococcus spp. (8 %), Pseudomonas spp. (6 %), Acinetobacter anitratus (5 %), Proteus spp. (3 %).Conclusion The high frequency of multidrug resistance in bacterial uropathogens was seen. Principally, resistance patterns were seen higher for amoxycillin, co-trimoxazole, flouroquinolones and third-generation cephalosporins, Existing uropathogens highlights the highest rate of vulnerability to nitrofurantoin, amikacin and gentamicin which provide much better antibiotic coverage and can be adapted for practical treatment of urinary tract infections.  Journal of Nobel Medical College Vol.5(2) 2016; 51-55

Background: The present study is undertaken to estimate the prevalence of urinary tract infection in febrile preschool children (1 month to 5 years of age) in febrile children visiting at tertiary care centre of Jhalawar, Rajasthan Methods: It was a cross-sectional, prospective, observational, non interventional study, carried out to analyse the prevalence of urinary tract infection in febrile preschool children (1 month to 5 years of age) in febrile children visiting medical college, Hospital Jhalawar. The study was conducted in Department of Paediatrics, between Dec. 2019  to  March 2020. The study was approved by Ethics Committee.   Results: During the study period, we have screened total 1379 preschool children (under 5 years of age) with fever. Out of this total 1379 febrile children, 86 children found to be culture positive cases for UTI (CP-UTI). The prevalence of culture positive cases for UTI in this study was 6.23%. In the prospect of age, 24 (27.91%) children were found to be infant and 62(72.09%) children were found to be of age between 1 years to 5 years. Conclusion: UTIs in preschool children are often having vague and variable symptoms, often fever is the only symptoms. An untreated UTI can lead to subsequent damage and impairment of renal structure and function, it is very important to diagnose and treat UTI in preschool children. Background: Urinary tract infection (UTI) is common during pregnancy and especially soin pregnant patients with diabetes mellitus. The emergence of drug resistance and particularlythe extended spectrum Beta-lactamase production by Escherechia Coli and Methicillinresistance in staphylococci, limits the choice of anti-microbials. Method: A cross-sectional study was conducted on 50 pregnant women with sign andsymptoms of UTI in the Department of Obstetrics and Gynaecology, BIRDEM generalHospital-2 from January to June 2017. The objectives of the study were to determine thepresence and type of pathogens found in pregnant diabetic women presenting with featuresof urinary tract infection, and to analyze their antibiotic sensitivity pattern. Clean catch midstreamurine samples were collected using standard procedure and culture and sensitivitywas done following standard bacteriological method. Results: Mean age of the study subjects was 29.84±5.29 yrs; 48% were in third trimester ofpregnancy; 52% had Gestational Diabetes Mellitus (GDM) and 48% had Diabetes Mellitus(DM). Out of fifty pregnant women, thirty eight (76.0%) study subjects had positive growth.Of them, Escherechia coli was found in 57.9% cases, Klebsiellain 21.1%, Staphylococcusaureus in 7.9%, Enterococcus and Pseudomonas aeruginosa each in 2.6% case. Amikacinwas found to be sensitive to 100% cases of E. Coli, Klebsiella& Enterococcus but 100%resistant to Streptococcus. Conclusion: In this study, E. Coli was the most common organisms causing UTI, which isless susceptible to conventional oral antibiotics but more susceptible to amikacin.
