Gastrointestinal stromal tumours are the most common mesenchymal tumours of the gastrointestinal tract. This case report highlights the necessity of early surgical intervention in such cases to avoid mortality due to bleeding and to raise the awareness of rare causes of upper gastrointestinal bleed and their management. A 19 year old male presented in the surgery department of Shaheed Suhrawardy medical College Hospital with complaints of recurrent episodes of melena with anorexia and weight loss for 6 months. Apart from anaemia no significant findings were noted on physical examination. Investigations including upper GI endoscopy was not conclusive. So he underwent laparotomy which revealed an exophytic growth along the lesser curvature. Lower partial gastrectomy was done and histopathological and immunohistochemistry report showed evidence of benign low risk GIST. As the tumor was benign with no malignant potential imatinib therapy was not advocated. J Shaheed Suhrawardy Med Coll 2020; 12(2): 119-122

. Objectives To document the frequency of esophageal varices and their importance in a tertiary care hospital of SouthernPunjab. Design: Observational. Setting: Endoscopy unit of Sheikh Zayed Medical College/Hospital. Period: From Nov 2005 to May 2007.Materials & Methods The data of 500 patients who underwent upper GI endoscopy was analyzed. Demographic features, reasons for referraland endoscopic diagnoses were noted. Results: Among 500 patients, 57% were referred due to upper GI bleeding, 9% due to dysphagia, 8%due to persistent vomiting and 7% due to dyspeptic symptoms. Common endoscopic diagnoses were esophageal varices (44%). Conclusion:Major Endoscopic finding in our study was esophageal varices. Gastric or duodenal ulcers, congestive gastropathy, obstructive lesions ofesophagus and stomach were the next most common observations.

Though anorectal foreign body cases are rare, they have become increasingly frequent in recent years. Although entrapped foreign bodies are most often related to sexual behavior, they can also result from ingestion or sexual assault. The diagnosis may be made by rectal examination and metallic objects can be confirmed by plain abdominal radiographs. Transanal removal is only possible for very low-lying objects, while patients with high-lying foreign bodies usually require an operative intervention. An early decision of laparotomy should only be made after subjecting the patient to suitable investigations to determine exact location of the object, in order to avoid any inadvertent damage to the adjoining vasculature as well as anal incontinence. . We report the case of a young male who presented at surgery department of Shaheed Suhrawardy medical college hospital with severe rectal pain due to insertion of an apple into rectum by some eunuchs. It was successfully removed transanally under spinal anesthesia. A case of sub-acute intestinal obstruction due to spontaneous complete transmural migration of a retained surgical mop into the small intestinal lumen has been treated surgically in surgery department of Rajshahi Medical College Hospital. The patient presented with colicky abdominal pain, nausea, occasional abdominal distention and vomiting. She had history of abdominal operation twice previously. Upper GI bleeding is a common medical emergency with a significant mortality, outcome of which depends upon the cause, appropriate and early intervention in a specialized center. This study was carried out to see the outcome of patient with Upper GI haemorrhage. Fifty cases with episode of upper gastrointestinal haemorrhage, admitted into medicine units of Faridpur Medical College Hospital from January 2011 to December 2011, were studied. Duodenal ulcer was the commonest cause of haematemesis and melaena followed by oesophageal varices, gastric ulcer and erosive gastritis. The peak incidence was among 35 to 45 years of age. Over all male female ratio was 4.55:1 but in case of duodenal ulcer it was 9:1. During hospital stay recurrent bleeding was noted in 10% of patients and during subsequent follow up it was 10% of the total and 50% in case of variceal bleeding group. Objective: To assess the efficacy of Hydroxypropylmethylcellulose powder over steroid nasal spray in treatment of allergic rhinitis. Methods: A prospective study of 60 patients suffering from allergic rhinitis was conducted in the ENT and Head & Neck surgery Department, Silchar medical College and Hospital from April 2018 to August 2018.The study subjects were divided into two groups A and B. Group A was treated with oral antihistaminic and steroid nasal spray, while group B was treated with oral antihistaminics and HYDROXYPROPYLMETHYLCELLULOSE powder nasal spray for 4 weeks. The treatment outcomes were evaluated after 14 days and 28 days from the beginning of treatment, by assessing relief of symptom on a five point scale. Results: There was almost similar score in both the groups ranging between score 3 and 4(i.e., relief of nasal obstruction and sneezing) after 14 days of treatment (62% vs 64%). But after 28 days there was significantly better improvement score of group B study subjects over group A in score 5 (i.e., complete relief from symptoms (83.3% vs. 66.6% ).In both the groups, mild sedation at the beginning of the treatment was reported in some patients, which is a well known side effect of antihistaminics. Conclusion: The result obtained comparing both the groups indicated high efficacy of hydroxypropylmethylcellulose powder over steroid nasal spray in the treatment of allergic rhinitis.

Carcinoma stomach is a condition that occurs due to various causes. In order to diagnose a case ofcarcinoma stomach, a high index of suspicion is required. A detailed history and a thorough clinicalexamination must be done. Patients may have various complaints such as vomiting, abdominal painand anemia. Diet has also been shown to play a role in the occurrence of carcinoma stomach. Thepatient’s socio-economic status also plays a very important role in the incidence of carcinomastomach. Various investigations are available today in order to diagnose a case of carcinomastomach. Investigations such as upper GI endoscopy are very useful to visualize a growth in thestomach and to take tissue for biopsy. A contrast enhanced CT (CECT) scan of the abdomen is alsovery useful to diagnose the stomach neoplasia as well as to find out if any metastatic lesions arepresent. On histopathology, adenocarcinoma is the most common type of carcinoma that may befound. Other tumors such as GI stromal tumors and lymphomas may also be found. Treatment ofcarcinoma involves surgery as well as chemotherapy. Our study was carried out from March 2015to February 2019. The study was carried out at SRM Medical College Hospital and Research Center,Kattankulathur, Tamil Nadu, India. The total number of patients studied was 75. The results obtainedwere tabulated and compared with other studies.

A 60 years old postmenopausal lady was admitted in the surgery department of Islami Bank Medical College Hospital with complaints of a lump in anal region and occasional per rectal bleeding for six months. Examination revealed a small well defined globular mass in anal canal which extended from the anal verge up to lower rectum internally. Internal part was ulcerated from where foul smelling discharge was coming out. She had no history of exposure to any radiation or any surgery over the anal canal. On radiogram of the chest a cannon ball shadow was found. Incisional biopsy of the lesion revealed malignant melanoma. Abdomino-perineal resection of rectum and anal canal was done for the lesion with an uneventful postoperative period. Adjuvant therapy could not be instituted because the patient could not afford it. Fingertip includes the portion of the digit which is distal to the insertion of flexor digitorum profundus (FDP) tendon and the extensor tendon. It is the most vulnerable part of the hand to be injured. So, reconstruction of this part requires a good outcome in sensation and function. To meet the goal of the reconstruction principle, "Like with Like" local flap is the most preferable. Such type of flap is a Volar oblique triangular flap based on volar perforator vessels from the Digital artery proper. The aim of the study is to identify its reconstruction outcome in viability, sensation, range of movement of DIPJ, and patients' satisfaction regarding contour.  This was a prospective observational study; ten patients were included who were admitted in the Burn and Plastic surgery Department of Rajshahi Medical College Hospital, Rajshahi, from September 2019 to December 2020.  Thirteen flaps were harvested to cover for the fingertip defect of ten patients; among them, three were female. The age range of our operated patient was 2- 38 years. None of the flaps was lost except marginal necrosis or epidermal loss. The range of movement of DIPJ and 2SPD were within the normal limit. Patients were satisfied with their contour.  So, the modified volar oblique, triangular flap is one of the good options for fingertip defects. TAJ 2021; 34: No-1: 63-69

Inadequate postoperative pain control causes significant morbidity and mortality. ht the presets mum in the paediatric surgery department the effective pain control is am mandardkert For this reason this study was undertaken to find out the degree of effective postoperative pain control in children by using Visual Analogue Scale iVA.St It was a crospsemional study for two years. Sample sine was 90 with a mean age of 8.7 years. Patients were selected from the Department of Paediatric Surgery of Bangabandhu Sheikh Mujib Medical University (BSMMU). Dhaka; Bangladesh Institute of Child Health & Dhaka Shishu Hospital HliCH &DSH). Dhaka; and Dhaka Medical College hospital (DMC111. Dhaka. Children aged five to 12 years receiving postoperative analgesics were included in this study. All of them were mentally healthy. well responsive and cmoperative. Patients were selected in such a way that neither thee hod any malignant disease nor were they stoffernsg front any chronic pain or postoperative pain due to magical wound sepsis. Postoperative pain was measured by VAS from the first postoperative day to the seventh postoperative day and was graded by she Sriwatanakars method. It vans Plod that difference in pain scams were statistically nowsigo0ficant among the three institutes front the first postoperative day to the seventh postoperative day. Patients experienced moderate pain up to the third postoperative day, mild pain up to the fifth postoperative day and. ome pain up to the sowsh postoperative day. Postoperative pain was mu effectively controlled by currently used analgesics in paediatric surgery departments in these hospitals. So so find at the degree of effrctice postoperative pain control in children fierther study should be carried 0111 with large number of patients in different mares of Bangladesh.

Introduction. Circumcision is a common procedure carried out around the world. Due to religious reasons, it is routinely done in Bangladesh, by both traditional as well as medically trained circumcisers. Complications include excessive bleeding, loss of foreskin, infection, and injury to the glans penis. Myiasis complicating male circumcision appears to be very rare.Case Presentation. In 2010, a 10-year-old boy presented to the OPD of Dhaka Medical College Hospital with severe pain in his penile region following circumcision 7-days after. The procedure was carried out by a traditional circumciser using unsterilized instruments and dressing material. After examination, unhealthy granulation tissue was seen and maggots started coming out from the site of infestation, indicating presence of more maggots underneath the skin. An emergency operation was carried out to remove the maggots and reconstruction was carried out at the plastic surgery department.Conclusion. There is scarcity of literature regarding complications following circumcision in developing countries. Most dangerous complications are a result of procedure carried out by traditional circumcisers who are inadequately trained. Incidence of such complications can be prevented by establishing a link between the formal and informal sections of healthcare to improve the safety of the procedure.
