Erysipelas is a common skin infection of the upper dermis. Its most common complications are local; these include abscess formation, skin necrosis, etc. In the present article, we introduce a case of a 75-year-old patient with erysipelas of the face complicated with acute exudative pericarditis. The patient came to Kaunas Clinical Hospital complaining of extreme fatigue and fever, oedema of the left side of the face, and erythema typical for erysipelas. The patient also felt sternum and epigastric pain, especially during breathing, and dyspnoea. Heart work was rhythmic 100 bpm; blood pressure was 142/70 mmHg. Pericardial friction rub was heard over the left sternal border. There were no alterations in other systems. In the electrocardiogram, concave ST segment elevation in leads II, III, and aVF was identified. In addition, during hospitalisation, the patient experienced atrial fibrillation paroxysm, which was treated with amiodarone intravenously. The blood test showed C-reactive protein: 286 mg/L; white blood cells: 20 × 109/L; troponin I was within the normal range. During echocardiography, pericardial fluid in pericardial cavity was identified. As no changes in troponin I were observed, according to the ST segment elevation, the woman was diagnosed with erysipelas of the left side of the face complicated with acute exudative pericarditis. Antibacterial treatment of cephalosporins was administered. After the treatment, C-reactive protein decreased to 27.8 mg/L; whereas, in the electrocardiogram, the return of the ST segment to the isoline was observed, and pericardial fluid resorbed from the pericardial cavity. To the best of the authors’ knowledge, this case is a rare combination of erysipelas complicated with acute exudative pericarditis.

Aim. To identify and characterize the associations of the presence and severity of atherosclerosis of various localization with the blood level of biochemical parameters, as well as to assess the potential of their use as markers of metabolic disorders with increased atherogenic potential.Material and methods. The study included 216 patients (men, 53%) aged 24-87 years (mean age, 61,5±10,73 years). All patients underwent coronary angiography, carotid (CA) and femoral arterial (FA) duplex ultrasound to assess the presence and severity of atherosclerosis. In blood serum/plasma, biochemical parameters were analyzed using standard methods.Results. Based on the analysis of circulating biomarker profile, diagnostic complexes have been established that allow assessing atherosclerosis of different localization. According to the data obtained, the determinants of coronary and CA atherosclerosis are endothelial dysfunction (concentration of nitric oxide metabolites <36,0 μmol/L) and an increased level of creatinine (≥73,0 μmol/L). The specific markers associated with severe atherosclerosis of coronary and FAs (but not CA) were low high-density lipoprotein cholesterol (≤1,0/1,2 μmol/L for male/ female, respectively) and an increased C-reactive protein level (≥1,0 mg/l). Severe peripheral atherosclerosis (CA and FA involvement) was associated with hyperglycemia (glucose ≥6,1 μmol/L), while severe FA atherosclerosis — with hyperinsulinemia (insulin ≥14,0 μU/ml).Conclusion. The analysis of associations of circulating biochemical parameters with atherosclerosis localization and severity revealed a number of metabolic markers associated with the increased atherogenic potential. It is possible to distinguish both universal parameters that are associated with atherosclerosis, regardless of its localization and/or severity, and specific biomarkers that characterize either the localization or the severity of atherosclerosis, or both.

Objective: To determine the diagnostic accuracy of the Lactate Dehydrogenase, C-Reactive Protein and Ferritin in suspected patients of COVID-19. Study Design: Cross-sectional validation study. Place and Duration of Study: Pathology department of Combined Military Hospital Lahore in the month of May 2020. Methodology: We included 101 adult (>18 years) symptomatic suspected COVID-19 patients of both genders. Children, pregnant women and asymptomatic patients were excluded from study. Age, gender and results of Reverse Transcriptase Polymerase Chain Reaction, Lactate Dehydrogenase, C-Reactive Protein, ferritin were recorded. Results: Lactate Dehydrodenase had highest sensitivity (75%) with positive predictive value of 71.6% and diagnostic accuracy of 65.3% among three biochemical parameters studied. Receiver Operator Characteristic curve was studied. Area under curve of Lactate Dehydrogenase (AUC=0.65) and Ferritin (AUC=0.59) reflected their ability to prognosticate the presence of COVID19 disease. However, C-Reactive Protein (AUC=0.42) appeared to be a poor predictor of the disease. Conclusion: Raised serum Lactate Dehydrogenase (>490 U/L) and Ferritin (>152 ng/L) levels can be used to predict the Reverse Transcriptase Polymerase Chain Reaction positivity for COVID-19 in the population of suspected patients of COVID19. However, C-Reactive Protein is a poor predictor of COVID-19.

Automobile spray painters are often prone to chemical toxicity due to their routine work. This study was aimed at the occupational effect of spray painting fumes on some biochemical parameters in automobile spray painters. Five milliliters of blood specimen were collected into lithium heparin anti-coagulated bottles from twenty five automobile spray painters with ≤ 10 years working experience (experimental group one), ≥ 11 years working experience (experimental group two) and non-automobile spray painters which were monitored as control group respectively. After this, plasma alanine aminotransferase, aspartate aminotransferase, C-reactive protein, urea and creatinine were measured quantitatively using a spectrophotometer. The mean values of all the measured biochemical parameters showed no statistically significant differences (p>0.05) in the automobile spray painters with ≤ 10 years working experience as compared with that of the control group with the exception of plasma C-reactive protein, while those with ≥ 11 years working experience showed statistically significant differences (p<0.05) in plasma alanine aminotransferase, aspartate aminotransferase and C-reactive protein however, the plasma urea and creatinine showed no statistically significant differences (p>0.05). In conclusion, the concentration of plasma alanine aminotransferase, aspartate aminotransferase and C-reactive protein may be altered in automobile spray painters with ≥ 11 years working experience, thus these biochemical parameters should be monitored regularly in spray painters within this category of working experience

Abstract Short-term intensive exercise may be associated with many short-lasting metabolic changes. These changes depend on the duration and intensity of the exercise. The aim of our study was to determine potential changes in C-reactive protein (CRP) and selected haematological and biochemical parameters in clinically healthy German shepherd dogs before short duration high-intensity exercise and 6 hours after the exercise. During the study, the dogs were subjected to the following defensive training: detaining a figurant running away from a dog (2×) and detaining with a counterattack (2×). The running distance was 200 m at a mean speed of 28 km.h-1. The investigation of haematological parameters revealed a significant decrease in the mean values of platelet haematocrit (PCT) 6 hours after the training. Significant differences in the levels determined before and after exercise were observed also for phosphorus (P) and for iron (Fe). At the same time, we observed a significant increase in the mean calcium (Ca) level and a significant decrease in sodium (Na) and chlorides (Cl). The activity of aspartate aminotransferase (AST) significantly increased after exercise. Concentrations of acute phase C-reactive proteins were increased 6 hours after exercise in comparison to those before exercise but the difference was insignificant. Although the concentration of CRP was increased insignificantly after the exercise, when interpreting concentrations of this nonspecific indicator of inflammation, one should keep in mind that intensive exercise may also affect its concentrations.

As a marker of systemic inflammation, raised (C-reactive protein (CRP)) concentrations which are still within the normal range have been associated with an increased inflammation of chronic renal diseases (CRD). The current study aimed to establish potential determinats of raised CRP concentrations in patients who treated in Heamodialysis room,then study the relationship between CRP& some biochemical parameters related CRD We used a CRP latex reagents Kit which is based on an immunological reaction between CRP antisera bounded to the biologically inert latex particles or with CRP in the test specimens of 19 patients with (CRD) mean age 48 years ,range = 30?65 & in 21 healthy subjects as control group their age range = 30 ?45 years. The results are classified according to visible agglutination to: 1-A positive result / is indicated by the obvious agglutination pattern of the latex, in a clear solution. 2-A negative result / is indicated by no change in the latex suspension on the test slide. Then, we correlated the results of the precipitin test with the quantitative data on C-reactive protein. This study has found that: CRP concentrations in patients with CRD were increased very clearly than normal subjects , and established that CRP concentration in male was more than in female for patients specimens. Biochemical studies have shown raising (CRP) concentration is a marker of systemic inflammation. The relation between CRP & S.creatinine ,hemoglobine ,blood sugar is negative ,while positive with blood urea . Other biochemical parameter related to CRD'' blood urea, blood creatinine & hemoglobine and blood sugar'' were assayed for both subjects in order to asses the disease by compared the results.
