Features of responding to stress of elderly patients with chronic brain ischemia

The aim of the study. To study the peculiarities of responding to stress, the stress resistance and adaptation of older men and women with chronic brain ischemia, as well as the stressboard effect of Mexidol.Material and methods. 124 patients aged 60–74 years old are surveyed: 72 men and 52 women (average age, respectively, 65.3 ± 0.4 and 64.7 ± 0.7 years) with Chronic Brain Ischemia I–II stage against the background of arterial hypertension and its combinations with atherosclerosis of cerebral vessels. The level of psychosocial stress was determined on the Holmes-Ray scale. Features of the response of patients to stress was studied using the methodology Scale of Psychological Stress PSM-25 and S. Rogenzweig. Stress resistance was investigated using S. Kuhlen's stress resistance self-resistant test and Villianson. The level of anxiety was determined using a scale of Ch.D. Spilberger and Yu.L. Khanin, depressed – backup questionnaire. The type of adaptation reactions was studied in the leukocyte blood formula on the percentage ratio of lymphocytes and segmented neutrophils, taking into account the representation of other formed elements.Results. The level of stress in older women with Chronic Brain Ischemia was higher than in men. The predominance of the intrinsic orientation of the reaction to stress and resolving the type of response to men, and the extrapunitive or self-defense type among women, which may indicate the largerness of the latter. The level of stress resistance was lower in women than in men, which correlated with higher indicators of situational anxiety. Adverse adaptation reactions were more often registered in women than in men. The course of treatment with Mexidol of elderly patients with chemical leads to a decrease in the severity of subjective and objective symptoms, alarming disorders, increases the stress resistance and adaptive capabilities of the body, which is confrmed by an increase in the number of persons with favorable adaptation reactions. The high effciency and safety of sequential therapy with Mexidol (injections, then the tableted form of Mexidol Forte 250) is shown.

Cerebrovascular diseases (CVD) are one of the main causes of mortality and permanent disability in patients. Chronic brain ischemia (CBI) is a slowly progressive dysfunction of the brain with gradually increasing defects in its functioning, which is accompanied by energy deficiency. Early use of energy correctors, one of the representatives of which is ethylmethylhydroxypyridine succinate (EMHPS), is recommended to preserve the viability of nervous tissue in patients with CBI. A number of experimental, clinical, and randomized studies have shown that medicinal preparation (MP) containing EMHPS improve brain metabolism and blood supply, improve microcirculation, and reduce platelet aggregation. The main mechanisms of action are: antioxidant and membranotropic effects, the ability to reduce glutamate excitotoxicity, modulate the functioning of receptors and membrane-bound enzymes, restore neurotransmitter balance, and increase the energy status of the cell. The liberal MP EMHPS included in the standards of medical care for patients with stroke, angina pectoris, acute myocardial infarction. The presented data from the results of numerous studies and our own observation indicate that it is possible to correct cognitive, motor, coordination, and adaptive capabilities while taking EMGPS. Used for today the scheme of appointment of EMHPS in patients with CBI: starting from 500 mg once a day intravenously (i/v) in drip for 14 days, followed by a transition to oral reception (o/r) at a dose of 250 mg 3 times a day, a course of 60 days.

Difficulties in treating discirculatory encephalopathy (DE) in the elderly are caused by a combination of degenerative and vascular processes, which mutually burden each other. The development of the disease against the background of natural aging of the body makes it necessary to take into account age-related changes in the reactivity of the body, a decrease in its adaptive capabilities, and a violation of the mechanisms of autoregulation [6]. This whole complex of pathological changes requires a special therapeutic approach that considers the body as a single functional system. It can be assumed that drugs of a non-specific plan will be effective, mobilizing the internal reserves of an aging organism.

В работе изучено влияние разных режимов ритмических экстремальных холодовых воздействий на состояние вегетативной регуляции сердечного ритма у молодых и старых крыс. По данным спектрального анализа вариабельности сердечного ритма установлено, что у молодых крыс использование температурных режимов ритмических экстремальных холодовых воздействий, РЭХВ (-120 °С; -120 °С; -120 °С) и (-60 °С; -120 °С; -120 °С) значительно повышало адаптационные возможности организма за счет активации собственных гомеостатических регуляторных систем. При этом комбинированный режим РЭХВ (-60 °С; -120 °С; -120 °С) оказался наиболее оптимальным для старых животных, поскольку его применение не сопровождалось чрезмерной активацией симпатико-адреналовой системы на ранних этапах экспериментальных исследований в отличие от режима (-120 °С; -120 °С; -120 °С). Кроме того, использование данного режима охлаждения способствовало статистически значимому увеличению показателей общей мощности спектра нейрогуморальной регуляции не столько за счет повышения активности гуморально-метаболического звена регуляции, преобладание которого характерно для пожилого возраста, сколько благодаря увеличению тонуса вегетативных центров, причем парасимпатические влияния на миокард превалировали над симпатическими. The influence of various rhythmic extreme cold effects on the state of autonomic regulation of heart rate in young and aged rats was studied. According to the spectral analysis of heart rate variability, it has been found that in young rats, using rhythmic extreme cold exposures (RECE) temperature regimens of (-120 °С; -120 °С; -120 °С) and (-60 °С; -120 °С; -120 °С) significantly increased adaptive capabilities of the body due to the activation of its own homeostatic regulatory systems. At the same time, the combined regimen of RECE (-60 °С; -120 °С; -120 °С) occurred to be the most optimal for aged animals, since its use was not accompanied with an excessive activation of sympathoadrenal system at the early stages of experimental studies, in contrast to the regimen (-120 °С; -120 °С; -120 °С). In addition, the use of this cooling mode contributed to a statistically significant increase in the total power of spectrum of neurohumoral regulation not so much due to a rise in activity of humoral-metabolic regulation link, the prevalence of which is characteristic of an old age, but due to an increase in the tone of vegetative centers, herewith the parasympathetic effects on the myocardium prevailed sympathetic ones.

Adaptive potential is an indicator of the level of adaptability of the human body to various and changing environmental factors. The adaptive capabilities of the body are influenced not only by external factors, but also by internal - the state of the body as a whole, by the presence or absence of chronic diseases. Therefore, the adaptive potential was used as an integral indicator of a person's adaptability to production factors. The aim of the study was to assess the adaptive capabilities of the organism of workers in greenhouse farms in the Voronezh region. The study was carried out at the enterprises of the closed ground of the region by the method of questioning, the objects of the study were 125 women aged 27-45 years. In most cases, there are violations of the adaptive capabilities of the organism among the studied workers of greenhouse farms, and even a breakdown in adaptation was revealed in 0.8%. In less than half of the cases, the adaptive potential of the greenhouses was within the normal. The adaptive reserves of the greenhouses workers decrease with age.

The aim of our study was to investigate the prevalence and severity of depression in patients with the consequences of ischemic strokes and in patients with chronic brain ischemia. Material and Methods. We examined 100 patients with consequences of ischemic strokes and 17 patients with chronic cerebral ischemia. The Hamilton Depression Rating Scale was used to assess the presence and degree of depression. Conclusions. Slightly less than half of the patients with chronic cerebral ischemia (47.1%) had no depression, 42.1% had mild depression, and only 11.8% of the patients had moderate and severe depression. A different situation was observed in the group of patients with the consequences of ischemic strokes. Among them, only 22.0% of patients had no depression, 44.0% had mild depression, and 34.0% of patients had moderate, severe, and extremely severe depression (p < 0.05).

Старение организма сопровождается истощением физиологического резерва различных органов и систем, что приводит к развитию гериатрических синдромов в пожилом и старческом возрасте. Однако гериатрический статус пациентов с офтальмологическими заболеваниями изучен недостаточно. Цель исследования проведение гериатрического обследования пациентов с офтальмологическими заболеваниями и выявление особенностей гериатрического статуса больных. Гериатрические синдромы изучены нами у 115 больных старческого возраста с первичной закрытоугольной глаукомой и 118 больных того же возраста с катарактой. Установлено, что гериатрический статус пациентов с первичной закрытоугольной глаукомой ухудшается существеннее, чем при катаракте. Это обусловлено нарушениями ходьбы и устойчивости умеренной степени, синдромом гипомобильности, психологическим состоянием удовлетворительного и плохо качества, наличием нолипрагмазии. Установленные особенности гериатрического статуса рекомендуется использовать при организации специализированной офтальмологической и гериатрической помощи данным пациентам Aging of the body is accompanied by the depletion of the physiological reserve of various organs and systems, which leads to the development of geriatric syndromes in the elderly and senile age. However, the geriatric status of patients with ophthalmic diseases has not been sufficiently studied. The aim of the study was to conduct a geriatric examination of patients with ophthalmological diseases and to identify the features of the geriatric status of patients. Geriatric syndromes were studied in 115 elderly patients with primary angle-closure glaucoma and 118 patients of the same age with cataracts. It was found that the geriatric status of patients with primary angle-closure glaucoma deteriorates significantly more than in cataracts. This is due to moderate walking and stability disorders, hypomobility syndrome, a psychological state of satisfactory and poor quality, and the presence of nolipragmasia. The established features of the geriatric status are recommended for use in the organization of specialized ophthalmological and geriatric care for these patients
