Background: Heart rate variability (HRV) and pulse rate variability (PRV) measures are two kinds of physiological indices that can be used to evaluate the autonomic nervous function of healthy subjects and patients with various kinds of illness.Purpose: In this study, we compared the agreement and linear relationship between electrocardiographic signals (ECG)-derived HRV and photoplethysmographic signals (PPG)-derived right hand PRV (R-PRV) and left hand PRV (L-PRV) measures in 14 patients over 1 year after coronary artery bypass graft (CABG) surgery.Method: The ECG and PPG signals of the patient were recorded simultaneously for 10 min in a supine position. The last 512 stationary RR intervals (RRI) and peak-to peak intervals (PPI) of pulse wave were derived for data analysis. Bland-Altman plot was used to assess the agreement among HRV and both hand PRV measures, while linear regression analysis was used to examine the relationship among corresponding measures of HRV, R-PRV, and L-PRV.Result: The results revealed significant differences in total power (TP), very low-frequency power (VLF), low-frequency power (LF), high-frequency power (HF), and normalized VLF (VLFnorm) among HRV, R-PRV, and L-PRV. Bland-Altman plot analysis showed good agreements in almost all measures between R-PRV and L-PRV, except insufficient agreement was found in LF/HF. Insufficient agreements were found in root mean square successive difference (RMSSD), normalized HF (HFnorm), and LF/HF indices between HRV and L-PRV, and in VLFnorm, HFnorm, and LF/HF indices between HRV and R-PRV. Linear regression analysis showed that the HRV, R-PRV, and L-PRV measures were all highly correlated with one another (r = 0.94 ~ 1; p < 0.001).Conclusion: Though PRV measures of either hand are not surrogates of HRV measures, they might still be used to evaluate the autonomic nervous functions of CABG patients due to the moderate to good agreements in most time-domain and frequency-domain HRV measures and the strong and positive correlations among HRV and both hands PRV measures in CABG patients.

Main funding source(s): The Independent Research Fund Denmark Background Early systolic lengthening (ESL), a paradoxical stretch of myocardial fibers, has been linked to myocardial viability and contractile dysfunction. We assessed the long-term prognostic potential of ESL in coronary artery bypass graft (CABG) patients. Methods We retrospectively included patients (n = 709; mean age 68 years; 85% men) who underwent speckle tracking echocardiography (median 15 days) prior to CABG. Endpoints were cardiovascular death (CVD) and all-cause mortality. We assessed amplitude of ESL (%), defined as peak positive strain, and duration of ESL (ms), determined as time from Q-wave on the ECG to peak positive strain. We applied Cox proportional hazards models adjusted for the clinical risk tool, EuroSCORE II. Results During median follow-up of 3.8 years [IQR 2.7 to 4.9 years], 45 (6%) experienced CVD and 80 (11%) died. In survival analyses adjusted for EuroSCORE II, amplitude of ESL was associated with CVD (HR 1.37 [95%CI 1.13 to 1.66], P = 0.001) and all-cause mortality (HR 1.31 [95%CI 1.13 to 1.54], P = 0.001). Similar findings applied to duration of ESL and CVD (HR 1.17 [95%CI 1.08 to 1.26], P < 0.001) and all-cause mortality (HR 1.14 [95%CI 1.07 to 1.21], P < 0.001). The prognostic value of ESL amplitude was modified by sex (P interaction < 0.05), such that it was greater in women for both endpoints (Figure 1A-B). When adding ESL duration to EuroSCORE II, the net reclassification index improved significantly for both CVD and all-cause mortality. Conclusions Assessment of ESL provides independent and incremental prognostic information in addition to the EuroSCORE II for CVD and all-cause mortality in CABG patients. ABSTRAK Penyakit jantung koroner merupakan penyebab kematian terbesar di dunia. Penatalaksanaannya diantaranya dengan pembedahan seperti Coronarry Artery Bypass Graft (CABG). Tindakan CABG dapat menimbulkan kecemasan disetiap tahapannya. Kecemasan dapat menjadi hambatan penyesuaian psikologis pada pasien jantung dan akan menghambat pemulihan fisik yang berdampak pada kualitas hidup. Tujuan penelitian ini untuk mengetahui hubungan kecemasan dengan kualitas hidup pasien post CABG. Penelitian ini dilakukan di Ruang Rehabilitasi Jantung Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita Jakarta. Desain penelitian Desain penelitian ini yaitu cross sectional dengan jumlah sampel 25 pasien. Teknik sampling menggunakan consecutive sampling. Hasil penelitian ini ada hubungan yang bermakna antara kecemasan dan kualitas hidup pasien post CABG di Ruang Rehabilitasi Jantung RSJPD Harapan Kita Jakarta (p value 0.001).   Kata kunci: CABG, kecemasan, kualitas hidup     ABSTRACT Coronary heart diseases is the biggest cause of death in the world. One of its management is by surgery such as Coronarry Artery Bypass Graft (CABG). CABG prosedure can cause anxiety at every stage. Anxiety can be a barrier to psychological adjustment in heart patients and will inhibit physical recovery that impact on quality of life. The purpose of this study to determine the relationship of anxiety with the quality of life of post-CABG patients. The study was conducted in the heart hehabilitation room of RSJPD Harapan Kita. Research design of this study is cross sectional with a sample of 25 patients. The sampling technique uses consecutive sampling. The results of this study there is a significant relationship between anxiety and quality of life of post-CABG patients in the heart rehabilitation room RSJPD Harapan Kita Jakarta (p value 0.001).   Keywords: CABG, anxiety, quality of life

Abstract Background: In India, with increasing trends of Coronary Artery bypass graft surgery, there has been little consideration in importance of psychological evaluation in those undergoing surgery. Our study aims to evaluate the efficacy of Buteyko breathing technique as a psychological intervention along with Phase 1 Cardiac Rehabilitation on Anxiety, Depression and Self-efficacy in CABG patients.Method: In this study, 44 CABG Patients will be enrol. Conventional group will receive 2 weeks Phase I Cardiac Rehabilitation and Experimental group will receive Buteyko Breathing exercises along with 2 weeks Phase I Cardiac Rehabilitation. The evaluations for Anxiety, Depression and Self-efficacy will be performed. Discussion:Collaborating secondary prevention for Psychological consequences in the management strategy after revascularization will help in decrease in-hospital stay, speedy return to work and can further ameliorate symptoms. This will open a window of opportunity for integrating early rehabilitation for psychosocial consequences to provide holistic management for CABG patients.The clinical trial registry-India(CTRI) registration number for this trial is CTRI/2021/05/033632.
