Any piece of information is a selection from a set of possibilities. In this paper, this set is called a “domain”. Digital information consists of number sequences, which are selections from a domain. At present, these number sequences are defined contextually in a very variable way, which impairs their comparability. Therefore, global uniformly defined “domain vectors” (DVs), with a structure containing a “Uniform Locator” (“UL”), referred to as “UL plus number sequence”, are proposed. The “UL” is an efficient global pointer to the uniform online definition of the subsequent number sequence. DVs are globally defined, identified, comparable, and searchable by criteria which users can define online. In medicine, for example, patients, doctors, and medical specialists can define DVs online and can, therefore, form global criteria which are important for certain diagnoses. This allows for the immediate generation of precise diagnostic specific statistics of “similar medical cases”, in order to discern the best therapy. The introduction of a compact DV data structure may substantially improve the digital representation of medical information.

The usability of digital information on the internet strongly depends on its searchability. There are search engines for global language based text search, but language based information representation has only limited value, e.g. concerning reproducibility, comparability, precision. Searchable digital information can be much more. Any piece of digital information is a number sequence. We can define the binary format and value set resp. domain of every number online. Then universal information transport is possible by the online defined "Domain Vector" (DV) data structure: "UL plus number sequence". At this "UL" is an efficient link to the machine readable online definition of the number sequence. The UL also is global identifier of a certain kind of data. The online definition includes additional information, e.g. for similarity comparison of the number sequence. With this a universal numeric search engine can provide precise user defined worldwide search of DVs like in a globalized database. All users can participate. Together we can optimize online definitions and provide defined data as DVs. Initially it is necessary to build a first attractive online presence where users can provide online definitions. We should stay in contact to optimize together the common standard for machine readable online definitions and DVs.

Digital information consists of sequences of numbers that are selections. So far, these are defined by context. We can globalize this by using an efficient global pointer (UL) as “context”. The article explains new globally identified and defined “Domain Vectors” (DVs) for transporting digital information. They have the structure “UL plus sequence of numbers”, where UL is an efficient identifier and global pointer (link) to the unified online definition of the sequence of numbers. Thus, the format of the number sequence and its meaning is defined online. This opens up far-reaching new possibilities for the efficient exchange, comparison and search of information. It can form the basis for a new global framework that improves the reproducibility, search, and exchange of data across systems, borders, and languages.

Since April 1961, all manned space flights have been accompanied by medical control ensuring flight safety. Medical control in space flight has a technological and medical methodology that allows, at a distance from medical specialists, to make an adequate medical decision for the current situation. This work would be impossible if there were no measures taken to unify and standardize equipment and techniques. Telemedicine technologies developed on the basis of flight medical control. The origin of the word telemedicine comes from the expression «telemetric medical information», adopted in space technology. The issues of mutual development and mutual enrichment, standardization of methods and equipment of two areas of medicine, medical control in extreme situations and general telemedicine are considered in this article.

Healthcare systems have evolved to become more patient-centric. Many efforts have been made to transform paper-based patient data to automated medical information by developing electronic healthcare records (EHRs). Several international EHRs standards have been enabling healthcare interoperability and communication among a wide variety of medical centres. It is a dual-model methodology which comprises a reference information model and an archetype model. The archetype is responsible for the definition of clinical concepts which has limitations in terms of supporting complex reasoning and knowledge discovery requirements. The objective of this article is to propose a semantic-mediation architecture to support semantic interoperability among healthcare organizations. It provides an intermediate semantic layer to exploit clinical information based on richer ontological representations to create a “model of meaning” for enabling semantic mediation. The proposed model also provides secure mechanisms to allow interoperable sharing of patient data between healthcare organizations.

Abstract Rapid Prototyping (RP) processes reduce the time consumed in the manufacture of a prototype by producing parts directly from a CAD representation, without tooling. The StereoLithography Apparatus (SLA), and most other recent RP processes build a 3-D object from 2.5-D layers. Slicing is the process of defining layers to be built by the system. In this paper a framework is proposed for the development of algorithms for the representation and definition of layers for use in the SLA, with a view to determine if the slicing algorithms will affect surface finish in any significant manner. Currently, it is not possible to automatically vary slice thicknesses within the same object, using the existent algorithm. Also, it would be useful to use a dense grid for hatching or skin filling any given layer, or to change the hatch-pattern if desired. In addition, simulation of the layered building process would be helpful, so that the user can prespecify parameters that need to be varied during the process. The proposed framework incorporates these and other features. Two approaches for determining contours on each slice are suggested and their implementation is discussed. In the first, the layers are defined by the intersections of a plane with the surfaces defining the object. The plane is moved up from the base of the object as it is being built in increments. All intersections found are stored in a data structure, and sorted in head to tail fashion to define a contour for all closed areas on a layer. The second approach uses a scanline-type search to look for an intersection that will trigger a contour-tracing procedure. The contour-tracer is invoked whenever an unused edge is found in the search. This saves storage and sorting times, because the contour is determined as a chain of edges, in cyclic order. It is envisaged that results of this work on the SLA can be applied to other RP processes entailing layered building.

Abstract Background: Gender refers to the socially constructed roles, behaviors, expressions, and identities of girls, women, boys, men, and gender diverse people. It influences self-perception, individual’s actions and interactions, as well as the distribution of power and resources in society. Gender-related factors are seldom assessed as determinants of health outcomes, despite their powerful contribution.Methods: Investigators of the GOING-FWD project developed a standard methodology applicable for observational studies to retrospectively identify gender-related factors to assess their relationship to outcomes and applied this method to selected cohorts of non-communicable chronic diseases from Austria, Canada, Spain, Sweden.Results: The following multistep process was applied. Step 1 (Identification of Gender-related Variables): Based on the gender framework of the Women Health Research Network (i.e. gender identity, role, relations, and institutionalized gender), and available literature for a certain disease, an optimal “wish-list” of gender-related variables/factors was created and discussed by experts. Step 2 (Definition of Outcomes): each of the cohort data dictionaries were screened for clinical and patient relevant outcomes, using the ICHOM framework. Step 3 (Building of Feasible Final List): A cross-validation between gender-related and outcome variables available per database and the “wish-list” was performed. Step 4 (Retrospective Data Harmonization): The harmonization potential of variables was evaluated. Step 5 (Definition of Data Structure and Analysis): Depending on the database data structure, the following analytic strategies were identified: (1) local analysis of data not transferable followed by a meta-analysis combining study-level estimates; (2) centrally performed federated analysis of anonymized data, with the individual-level participant data remaining on local servers; (3) synthesizing the data locally and performing a pooled analysis on the synthetic data; and (4) central analysis of pooled transferable data.Conclusion: The application of the GOING-FWD systematic multistep approach can help guide investigators to analyze gender and its impact on outcomes in previously collected data.

BACKGROUND Despite increasing interest in digital public health, there appears to be a lack of clarity and consensus on its definition. OBJECTIVE In this scoping review, we assessed formal and informal definitions of digital public health in the literature. METHODS We conducted a scoping literature search in OVID Medline, Embase, Google Scholar and on 14 government and intergovernmental agency websites encompassing six geographical regions. Among a total of 409 full articles identified, we reviewed 12 publications that formally defined digital public health and/or informally described the integration of digital technologies in public health in relation to digitization, digitalization, and digital transformation, and conducted a thematic analysis of the identified definitions. RESULTS We identified two explicit definitions of digital public health with divergent meanings. While one definition is predicated on digitalization, and involves the integration of digital technologies to achieve existing public health goals, the other implies transformation of public health services and goals, leveraging digital technology to improve health outcomes. Further, we found that when described in relation to public health, digitization refers to the conversion of analog records to digital information, digitalization involves the integration of digital technologies into public health operations, and digital transformation involves a cultural shift that pervasively integrates digital technologies and reorganizes service based on the health needs of the public. CONCLUSIONS The definition of digital public health remains contested in the literature. Public health researchers and practitioners need to clarify these conceptual definitions to harness opportunities to integrate digital technologies in public health in a way that maximizes their potential to improve public health outcomes. The Medical Information System (MedISys) is a fully automatic 24/7 public health surveillance system monitoring human and animal infectious diseases and chemical, biological, radiological and nuclear (CBRN) threats in open-source media. In this article, we explain the technology behind MedISys, describing the processing chain from the definition of news sources, scraping and grabbing articles from the internet, text mining, event extraction with the Pattern-based Understanding and Learning System (PULS, developed by the University of Helsinki), news clustering and alerting, to the display of results. The web interface and service applications are shown from a user’s perspective. Users can display world maps in which event locations are highlighted as well as statistics on the reporting about diseases, countries and combinations thereof and can apply filters for language, disease or location or filters with orthogonal categories, e.g. outbreaks, via their browser. Specific entities such as persons, organizations and locations are identified automatically.
