Muscle strength assessment is fundamental to track the progress of performance and prescribe correct exercise intensity. In field settings, simple tests are preferred. This study develops equations to estimate maximal muscle strength in upper- and lower-extremity muscles based on submaximal elastic resistance tests. Healthy adults (n = 26) performed a maximal test (1 RM) to validate the ability of the subsequent submaximal tests to determine maximal muscle strength, with elastic bands. Using a within-group repeated measures design, three submaximal tests of 40%, 60%, and 80% during (1) shoulder abduction, (2) shoulder external rotation, (3) hip adduction, and (4) prone knee flexion were performed. The association between number of repetitions and relative intensity was modeled with both 1st and 2nd order polynomials to determine the best predictive validity. For both upper-extremity tests, a strong linear association between repetitions and relative intensity was found (R2 = 0.97–1.00). By contrast, for the lower-extremity tests, the associations were fitted better with a 2nd order polynomial (R2 = 1.00). The results from the present study provide formulas for predicting maximal muscles strength based on submaximal resistance in four different muscles groups and show a muscle-group-specific association between repetitions and intensity.

Context: The infraspinatus muscle plays a particularly important role in producing primary external rotation (ER) torque and dynamic stability of the shoulder joint. Previous studies have reported that prone external rotation with horizontal abduction (PER), side-lying wiper exercise (SWE), and standing external rotation (STER) were effective exercises for strengthening the infraspinatus. However, we do not have enough knowledge about changes in muscle strength and dynamic muscle activity in each exercise under dynamic conditions. Objective: To compare the ER muscle strength, muscle activity among exercise methods and between muscle contraction types during 3 different exercises. Design: Repeated measures design in which ER muscle strength and muscle activities data were collected from subjects under 3 exercise conditions. Setting: Outpatient clinic. Participants: A total of 15 healthy men with no shoulder, neck, or upper-extremity pain were participated. Intervention: The subjects performed 3 different exercises randomly in concentric and eccentric using a Biodex dynamometer at an angular velocity of 60°/s. Main Outcome Measures: The ER peak torque (PT) data was collected and surface electromyography was used to measure the activity of the infraspinatus and posterior deltoid muscles and infraspinatus to posterior deltoid muscle activity ratio. Results: There was significant main effect for muscle contraction type in ER PT (P < .05). The concentric PT was greater during PER (P < .05) and SWE (P < .05) compared with eccentric. The main effect for exercise was found in ER PT and muscle activity (P < .05). The ER PT and infraspinatus muscle activity were the largest increased during PER whereas the lowest during STER in both concentric and eccentric (P < .05). Conclusion: The results of this study suggest that PER is an exercise for strengthening the infraspinatus muscle effectively.

Context:Soft-tissue injuries are commonly treated with ice or menthol gels. Few studies have compared the effects of these treatments on blood flow and muscle strength.Objective:To compare blood flow and muscle strength in the forearm after an application of ice or menthol gel or no treatment.Design:Repeated-measures design in which blood-flow and muscle-strength data were collected from subjects under 3 treatment conditions.Setting:Exercise physiology laboratory.Participants:17 healthy adults with no impediment to the blood flow or strength in their right arm, recruited through word of mouth.Intervention:Three separate treatment conditions were randomly applied topically to the right forearm: no treatment, 0.5 kg of ice, or 3.5 mL of 3.5% menthol gel. To avoid injury ice was only applied for 20 min.Main Outcome Measures:At each data-collection session blood flow (mL/min) of the right radial artery was determined at baseline before any treatment and then at 5, 10, 15, and 20 min after treatment using Doppler ultrasound. Muscle strength was assessed as maximum isokinetic flexion and extension of the wrist at 30°/s 20, 25, and 30 min after treatment.Results:The menthol gel reduced (−42%, P < .05) blood flow in the radial artery 5 min after application but not at 10, 15, or 20 min after application. Ice reduced (−48%, P < .05) blood flow in the radial artery only after 20 min of application. After 15 min of the control condition blood flow increased (83%, P < .05) from baseline measures. After the removal of ice, wrist-extension strength did not increase per repeated strength assessment as it did during the control condition (9−11%, P < .05) and menthol-gel intervention (8%, P < .05).Conclusions:Menthol has a fast-acting, short-lived effect of reducing blood flow. Ice reduces blood flow after a prolonged duration. Muscle strength appears to be inhibited after ice application.

Objectives: The aim of this study was to investigate the effects of limb dominance and gender on isometric, eccentric and concentric strength of the shoulder internal and external rotator muscles in adolescent volleyball players. Methods: Forty adolescent volleyball players [Male: 23( Age: 15.5±1.4 yrs, Body weight: 72±10.2 kg, Height: 184.4±7.6 cm, BMI: 20.9±2.6 kg/m2), Female: ( Age: 16.7±0.9 yrs, Body weight: 60.7±8.2 kg, Height: 172.5±5.3 cm, BMI: 20.4±2.3 kg/m2) participated in this study. Isomed 2000 isokinetic dynamometer was used to measure muscle strength testing. Isometric strength testing of shoulder internal and external rotator muscles were performed at 90 ° shoulder abduction and external rotation position. In the same position, concentric and eccentric muscle testing was performed at 90°/s angular velocity. Strength outcomes were recorded as Nm/kg. 2-way repeated measures of ANOVA was used for statistical analysis. Results: Dominance by gender interaction was not found significant for internal rotator (IR) and external rotator (ER) muscles' strength (IR: F(1,72)=2.87, p=0.06, ER: F(1,72)=1.98, p=0.15). There was a significant strength by dominance interaction for internal rotator muscles (F(2,72)=18.52, p<0.001). Isometric strength was greater in dominant limb (p<0.001)while concentric strength was found greater in non-dominant limb(p=0.006). Eccentric strength was found similar for limbs (p=0.18). IR muscles showed greater strength during eccentric (1.03±0.05), concentric (0.76±0.03) and isometric test (0.69±0.03), respectively. On the other hand, external rotators showed greater strength during eccentric (0.49±0.4), isometric (0.40±0.3) and concentric test (0.36±0.3), respectively. There was no significant gender effect on the strength (IR: F(2,72)=0.31, p=0.73, ER: F(2,72)=0.42, p=0.66). Conclusion: The strength of shoulder internal and external rotator muscles do not differ according to gender in adolescent volleyball players. Limb dominance has an effect on the strength of internal rotator muscles while it has no effect on the strength of external rotators. Both muscle groups show greater strength during eccentric testing.

The current study aimed to compare the shoulder kinematics (3D scapular orientation, scapular angular displacement and scapulohumeral rhythm) of asymptomatic participants under unloaded and loaded conditions during unilateral shoulder elevation in the scapular plane. We used a repeated-measures design with a convenience sample. Eleven male participants with an age range of 21–28 years with no recent history of shoulder injury participated in the study. The participants performed isometric shoulder elevation from a neutral position to approximately 150 degrees of elevation in the scapular plane in intervals of approximately 30 degrees during unloaded and loaded conditions. Shoulder kinematic data were obtained with videogrammetry. During shoulder elevation, the scapula rotated upwardly and externally, and tilted posteriorly. The addition of an external load did not affect 3D scapular orientation, scapular angular displacement, or scapulohumeral rhythm throughout shoulder elevation (P> .05). In clinical practice, clinicians should expect to observe upward and external rotation and posterior tilt of the scapula during their assessments of shoulder elevation. Such behavior was not influenced by an external load normalized to 5% of body weight when performed in an asymptomatic population.

Context:Functional knee braces (FKB) are used prophylactically and in rehabilitation to aide in the functional stability of the knee.Objective:To determine if alterations in select lower extremity moments persist throughout a one hour period in healthy individuals.Design:2X5 repeated measures design.Setting:Biomechanics Laboratory.Subjects:Twenty subjects (14 male and 6 female, mean age 26.5±7 yrs; height 172.4±13 cm; weight 78.6±9 kg), separated into braced (B) and no brace (NB) groups.Intervention:A one-hour exercise program divided into three 20 minute increments.Main Outcome Measures:Synchronized three-dimensional kinematic and kinetic data were collected at 20-minute increments to assess the effect of the FKB on select lower extremity moments and vertical ground reaction forces.Results:Increase in hip moment and a decrease in knee moment were noted immediately after brace application and appeared to persist throughout a one hour bout of exercise.Conclusions:The FKB and the exercise intervention caused decreases in knee joint moments and increases in hip joint moments.

The occurrence of shoulder impingement syndrome (SIS) is associated with the frequent handling and lifting of heavy loads and excessive repetitive work above the shoulder level. Thus, assembly workers have a high prevalence of shoulder injuries, including SIS. The purpose of this study was to investigate differences in shoulder ROM, muscle strength, asymmetry ratio, function, productivity, and depression between workers with and without SIS.Sixty-seven assembly line male workers (35 workers with SIS and 32 workers without SIS) participated in this study. The four shoulder ROMs and the five muscle strengths were measured using a Smart KEMA system. The asymmetry ratios were calculated using the asymmetry ratio formula; shoulder functions were measured using the shoulder pain and disability index (SPADI), disabilities of the arm, shoulder, and hand (DASH), and visual analogue scale (VAS); and Endicott work productivity scale (EWPS). Severity of depression was measured using the Beck depression inventory (BDI). Independent t-tests were performed for statistical analysis.The SPADI, DASH, and VAS values of workers with SIS were significantly higher than those of workers without SIS. Also, workers with SIS had significantly smaller shoulder internal rotation and shoulder abduction compared to workers without SIS. In addition, workers with SIS exhibited significantly lower SIR muscle strength than workers without SIS. Workers with SIS had significantly higher asymmetry ratios in shoulder internal rotation, shoulder external rotation, and elbow flexion muscle strength than workers without SIS.The SPADI and DASH scores, which indicate shoulder function, were higher, and the intensity of self-aware pain was higher in workers with SIS. Also, workers with SIS exhibited reduced SIR and SAB ROMs; decreased SIR muscle strength. Particularly, the asymmetry ratios of SIR, SER, EF muscle strength are good comparable factors for workers with and without SIS. In addition, the asymmetry ratios of shoulder muscle strengths could provide an important baseline comparison for the workers with SIS.

BackgroundHigh-intensity resistance training plays an essential role in the prevention and rehabilitation of musculoskeletal injuries and disorders. Although resistance exercises with heavy weights yield high levels of muscle activation, the efficacy of more user-friendly forms of exercise needs to be examined.ObjectiveThe aim of this study was to investigate muscle activation and perceived loading during upper-extremity resistance exercises with dumbbells compared with elastic tubing.DesignA single-group, repeated-measures study design was used.SettingExercise evaluation was conducted in a laboratory setting.ParticipantsSixteen female workers (aged 26–55 years) without serious musculoskeletal diseases and with a mean neck and shoulder pain intensity of 7.8 on a 100-mm visual analog scale participated in the study.MeasurementsElectromyographic (EMG) activity was measured in 5 selected muscles during the exercises of lateral raise, wrist extension, and shoulder external rotation during graded loadings with dumbbells (2–7.5 kg) and elastic tubing (Thera-Band, red to silver resistance). The order of exercises and loadings was randomized for each individual. Electromyographic amplitude was normalized to the absolute maximum EMG amplitude obtained during maximal voluntary isometric contraction and exercise testing. Immediately after each set of exercise, the Borg CR10 scale was used to rate perceived loading during the exercise.ResultsResistance exercise with dumbbells as well as elastic tubing showed increasing EMG amplitude and perceived loading with increasing resistance. At the individually maximal level of resistance for each exercise—defined as the 3 repetitions maximum—normalized EMG activity of the prime muscles was not significantly different between dumbbells (59%–87%) and elastic tubing (64%–86%). Perceived loading was moderately to very strongly related to normalized EMG activity (r=.59–.92).LimitationsThe results of this study apply only for exercises performed in a controlled manner (ie, without sudden jerks or high acceleration).ConclusionsComparably high levels of muscle activation were obtained during resistance exercises with dumbbells and elastic tubing, indicating that therapists can choose either type in clinical practice. The Borg CR10 can be a useful aid in estimating intensity of individual rehabilitation protocols.

The purpose of the study was to describe Chinese women’s postpartum physiological and psychological health and adherence to “doing-the-month” practices. A descriptive repeated measures design was used, with data collected at 3 days and 6 weeks postpartum. The convenience sample consisted of 198 healthy childbearing women with a term birth. Maternal physical health was measured by the Six-Minute Walk (endurance), Chair Stand test (muscle strength), severity of physical symptoms, and physical health subscales of SF36v2. Maternal psychological health was measured by the Edinburgh Postnatal Depression scale. Adherence was measured by the Adherence to Doing-the-Month Practices questionnaire. Aerobic endurance and lower-body muscle strength improved significantly across time ( p < .001) but remained suboptimal for maternal age. Women who delivered by Cesarean section had significantly poorer physical health than those who had a vaginal delivery. Physical functioning significantly increased, but general health and role limitations due to physical health significantly decreased over time. Postpartum physical symptoms decreased in number and severity. Depression increased over time ( p < .001). Adherence to doing the month was negatively correlated with aerobic endurance and positively correlated with depression at 6 weeks ( p < .05). These findings challenge the assumption that practices of doing the month are healthy for Chinese women’s recovery after childbirth. Research-based evidence needs to be integrated into doing-the-month practices. Education of Chinese women and families, whether living at home or abroad, is needed about the adverse health effects of doing the month. Routine screening for postpartum depression is also advised.

Objectives: The aim of this study was to determine whether a static stretch of posterior deltoid could reduce the glenohumeral internal rotation deficit (GIRD) and the total arc of movement deficit (TAMD) in professional swimmers after competition. Methods: Participants: A total of 74 professional swimmers aged from 16-33 years volunteered to participated in the study. Their competition experience were more than 2 years at national level. All the subjects were informed in written and verbal form and signed their informed consent before being assessed. Design and procedures: A randomized repeated measures design was used to assess the glenohumeral rotation in 3 moments: prior to the race, just after finishing their trial and after performing a static passive stretch of posterior deltoid muscles of 90-sec. In randomized order the computer selected 20 subjects as a control group (CG) who didn’t perform the stretching. The experimental group (EG) included 54 swimmers. The glenohumeral internal and external rotation (IR and ER) were recorded by a video camera (IPhone 6 S, version 10.1), in sagittal plane, with the center of the screen at shoulder high. Subjects were laying on supine position over a massage table, with the glenohumeral joint at 90° of abduction, the elbow at 90° of flexion, and the researcher controlling the scapula movements by pushing the shoulder over coracoid apophasis. The App Thechnique (Ubersense ©) was used to measure the glenohumeral rotation degrees between the vertical line (controlled by a plumb) and the forearm segment. Results: The multifactorial ANOVA showed that there were significant differences on GIRD and TAMD between the experimental and the control group performing the stretching F(2,70)=49.150, P=0.000, η2p=0.992. The experimental group reduced the GIRD a16.2% and the TAMD a 6.7%. The dominant IR mean values changed significantly from 66.3±12.5 to 79.2±10.4 degrees for EG ( P=0.00) and non-significantly for CG, from 74.6±12.7 to 77.6±13.9 degrees ( P=0.11). The dominant TAM means changed significantly in EG from 173.2±16.8 to 192.0±17.0 degrees ( P=0.00) and non- significantly for CG 181.5±21.7 to 188.2±23.3 degrees ( P=0.12). Conclusion: To apply a static passive stretching on posterior deltoid during 90-sec reduced GIRD and the TAMD in professional swimmers after competition, which suggest a reduced risk of shoulder injury in these overhead athletes.
