Background: the main goal of the study was to investigate the prevalence of the articular cartilage defects (ACD) in the patellofemoral (PF) region of the knee joint based on the anatomical shapes of patella and its impact on the level of physical activity in the population needing arthroscopic procedures for all types of pathologies in the knee. Methods: The articular cartilage status of the PF region was obtained from 1098 arthroscopic procedures of the knee joint. The ACD were correlated to Wiberg’s shape of the patella and classified according to the degree, size and depth of the ACD in the PF region using the ICRS (International Cartilage Repair Society) system: group I consisting of patients with Wiberg type I shape (W1), group II—patients with Wiberg type II shape (W2) and group III—patients with Wiberg type III shape (W3). The Tegner physical activity scale was used to evaluate the physical activity of the patients. Results: The mean of ACD size (PF region) in the W3 group was 3.10 ± 0.99 cm2, which was a statistically significantly larger area in comparison with the W1 (1.90 ± 0.63 cm2; p < 0.0000) and W2 (1.95 ± 0.71 cm2; p < 0.0000). The patients from the W3 group (mean 3.10 ± 0.99) were less physically active (<4 Tegner) compared to the W2 group (mean of 4.48 ± 0.88; p = 0.004) and W1 group (mean of 4.55 ± 0.72; p = 0.002). Conclusions: The patients with the Wiberg type III patella shape had a higher incidence and larger size of ACD in the PF of the knee compared to the groups of Wiberg type I and II. Wiberg III patients with a lower level of physical activity had a larger size of ACD in the PF joint.

Background. Osteoarthritis is characterized by a decrease in the quality of life, which is currently considered as the main measure of well-being of an individual and a criterion for the effectiveness of treatment and rehabilitation measures. Modern research emphasizes the need for an integrated approach to the treatment of osteoarthritis, which should include pharmacological and non-drug methods of treatment. Aim. The purpose of the study was to evaluate the effectiveness of pharmacopuncture administration of Alloplant biomaterial by patients with osteoarthritis of the knee joint from the standpoint of influencing the immunological mechanisms of the disease. Methods. The randomization methodology was divided into 3 groups, comparable in terms of clinical and functional characteristics. Group I patients (n = 39) have standard basic drug therapy according to the federal clinical guidelines Osteoarthritis approved by the Association of Rheumatologists of Russia (2013), with additions from 2016; in patients of group II (n = 39), in addition to drug therapy, dispersed Alloplant biomaterial was used according to the original method; group III patients (n = 39) against the background of basic drug therapy are additionally prescribed a combination of dispersed Alloplant biomaterial at biologically active points and magnetic therapy with a pulsed magnetic field. Results. Against the background of the treatment of patients with osteoarthritis using pharmacopuncture with Alloplant biomaterial and magnetotherapy, there was a positive dynamics in clinical data with a significant decrease in pain intensity when walking and at rest, stiffness, with the restoration of immune status parameters and an improvement in the quality of life in the field of physical and psychological health . A more pronounced and lasting positive dynamics was observed in patients of group III against the background of the combined effects of pharmacopuncture with Alloplant biomaterial and magnetotherapy. Conclusion. The Alloplant biomaterial has a positive effect in the treatment of patients with stage III osteoarthritis according to the KellgrenLawrens classification. The similarity of the mechanism of action with drugs with a structurally modifying effect on cartilage allows us to recommend pharmacopuncture with Alloplant biomaterial for widespread use in the treatment of osteoarthritis.

Abstract Background Staphylococcus aureus (S. aureus) accessory gene regulator (agr) system plays a critical role in staphylococcal pathogenesis. Our study aimed to investigate the relationship between agr group, the genetic background, virulence genes and disease types distribution of S. aureus isolated from different clinical sources among Chinese children.Methods S. aureus strains were isolated from Beijing Children’s hospital from October 2017 to October 2019. Isolates were typed by multilocus sequence typing (MLST), staphylococcal protein A (spa), agr, and staphylococcal cassette chromosome mec (SCCmec) typing (for methicillin-resistant S. aureus [MRSA] isolates). Furthermore, all isolates were tested for the presence of 19 selected virulence genes.Results A total of 191 non-repetitive S. aureus clinical isolates were collected and the agr type I was the most prevalent (84.8%). S. aureus isolates were divided into 33 sequence types (STs), 20 clonal complexes (CCs) and 59 spa types, ST59 (39.8%) and t437 (37.7%) were the predominant types. CC59, CC25, CC22, CC951, CC8, and CC398 isolates possessed agr group I; CC15 isolates harbored agr group II; CC30 strains were characterized as agr group III, and CC121 harbored agr group IV. Of the 19 virulence genes, the tst gene was more prevalent among agr group III compared to other groups (p = 0.006); eta and etb genes were more prevalent among agr group IV than other groups (p = 0.003 and 0.001, respectively); nearly all strains that harbored the lukS/F-PV gene (98.3%) belonged to agr group I (p = 0.004); the frequencies of bbp and ebpS genes belonged to agr group I were statistically lower than that of other groups (p < 0.001). Among 161 diagnoses, the frequency of strains from cellulitis patient harbored agr group III was higher than that of other groups (p = 0.046), and one strain isolated from staphylococcal scalded skin syndrome (SSSS) patient, which was identified as agr type IV (p = 0.021).Conclusions The results indicated that the S. aureus agr type was linked to the genetic background. Besides, a possible relationship between the agr group, several virulence determinants, and specific disease types was observed.

The present study was performed to examine the effect of intra- articular injection of bone marrow mesenchymal stem cells (BMMSCs) and chondrogenic differentiated mesenchymal stem cells (CD- MSCs) on the repair of articular cartilage defects in rabbits. Twenty-five adult female baladi rabbits were used in this work. 5 rabbits were used for preparation of bone marrow mesenchymal stem cells (BM-MSCs) and their left knees were not subjected for the surgical procedure and used as normal control group. The remaining twenty rabbits were subjected for surgically induced cartilage defects in their left knees through a small medial parapatellar incision using bone curette. In the next day, the rabbits were divided into four groups: group I was not injected intraarticularly, group II injected intra-articularly by a single dose of saline, group III injected intra-articularly by a single dose of BM-MSCs and group IV injected intra-articularly by a single dose of CD-MSCs. After 8 weeks from the time of intra-articular injection. On time the rabbits were sacrificed and the entire knee joints were excised and examined. Groups I and II showed marked degenerative changes in their articular cartilage. The articular surface healed by fibrocartilage in group III, while in group IV the articular surface healed by hyaline cartilage. Treatment by CD-MSCs promotes a better healing effect on the articular cartilage defects of injured knee joints in rabbit’s model and has a remarkable superiority of repair than BM-MSCs. This can prevent the progress of cartilage defect into osteoarthritis which was a harmful disease.

Ligoniai ir metodai 1998–2002 metais KMU Ortopedijos ir traumatologijos klinikoje atliktos 87 autologinės mozaikinės transplantacijos operacijos esant kelio sąnario kremzlės ir kaulo pažeidimams. Pacientų amžiaus vidurkis operacijų metu buvo 24,74 ± 7,20 metų (14–40 metų). Simptomai vidutiniškai truko 21,32 ± 5,57 mėnesio, o pacientai įvertinti praėjus vidutiniškai 24,4 mėnesio (nuo 12 iki 60 mėnesių) po operacijos. Rezultatai apibendrinti naudojant ICRS (International Cartilage Repair Society) anketą, 13 (15%) atvejų – remiantis kartotinių artroskopijų metu atliktu makroskopiniu įvertinimu, 9 (11%) atvejais – histologiniu tyrimu, 57 (67%) atvejais – BMR ir visais atvejais – rentgeno tyrimais. Visus anketinius įvertinimus atliko nepriklausomas gydytojas ortopedas prieš operacijas ir po operacijų praėjus dvylikai, dvidešimt keturiems, trisdešimt šešiems, keturiasdešimt aštuoniems ir šešiasdešimčiai mėnesių. Nepriklausomi nuo tyrimo gydytojai, radiologas ir patologas, atliko visus BMR ir histomorfologinius įvertinimus. Rezultatai Praėjus vidutiniškai 24,4 mėnesio po mozaikinės transplantacijos, ICRS anketos būdu nustatytas klinikinis rezultatų pagerėjimas (p < 0,05). Funkcinis ir objektyvus įvertinimas pagal ICRS anketą buvo toks: 93% atvejų gauti geri ir labai geri operacinio gydymo rezultatai, kitais atvejais (7%) gydymo rezultatai buvo patenkinami. Kartotinių artroskopijų metu (remiantis ICRS protokolu), po operacijų praėjus vidutiniškai 12,4 mėnesio, 11 (84%) iš 13 atvejų buvo nustatytas geras ir labai geras sąnario kremzlės makroskopinis atsitaisymas. Sąnario kremzlės biopsijos ir histologinis tyrimas buvo atliktas 11% pacientų, ir daugumos jų (remiantis ICRS protokolu) sąnario kremzlės ir pokremzlinio kaulo atitaisymo koeficientas buvo geras. BMR tyrimas parodė, kad 94% pacientų sąnario paviršius atkurtas gerai arba labai gerai. Išvados Mūsų tyrimas parodė, kad po mozaikinės autologinės transplantacijos praėjus vidutiniškai 24,4 mėnesio (nuo 12 iki 60 mėn.) fiziškai aktyvių pacientų, kuriems buvo pažeista kelio sąnario kremzlė, klinikinė ir funkcinė būklė labai pagerėjo. Dauguma pacientų po kremzlės mozaikinės autologinės transplantacijos atgauna prieš pažeidimą buvusį fizinio aktyvumo lygį. Histomorfologinis persodintų transplantatų tyrimas parodė, kad visais tirtais atvejais išsilaikė hialininės kremzlės struktūra ir fibroelastinio audinio intarpai tarp transplantatų. Patients and methods Between 1998 and 2002, a total of 85 patients underwent an osteochondral autologous transplantation (mosaicplasty) procedure for osteochondral or chondral knee joint injury. The patients were evaluated using the ICRS score, arthroscopically, histologically, with MRI and X-ray examinations. The mean duration of symptoms was 21.32 ± 5.57 months and the mean follow-up was 24.4 months (range, 12 to 60 months). The mean age of the patients during the surgery was 24.74 ± 7.20 years (range, 14 to 40 years). An independent observer performed a follow-up examination after six, twelve, twenty-four, thirty-six and forty-eight months. In 13 (15%) of 85 cases 12.4 months postoperatively, arthroscopy with biopsy for histological evaluation was carried out. A radiologist and a pathologist, both blinded to each patient’s treatment, did the radiological and histological evaluations. Results After 24.4 months all the patients showed a significant clinical improvement (p < 0.05). The cartilage Repair Society (ICRS) score, functional and objective assessment revealed 93% to have excellent or good results after mosaicplasty; 7% were fair 24.4 months (range, 12–60 months) after the operations. The ICRS evaluation showed a significant improvement after the mosaicplasty procedure 24.4 months following operations (p = 0.005). No serious complications were reported. The ICRS for macroscopic evaluation during arthroscopy 12.4 months after mosaicplasty demonstrated excellent or good repair in 11 (84%) of 13 cases. Biopsy specimens were obtained from 9 (69%) of 13 patients, and histological evaluation of repair showed good scores (according to ICRS) for most samples after mosaicplasty. A MRI evaluation demonstrated excellent or good repairs in 94% after mosaicplasty. Conclusions On an average after 24.4 months (range, 12 to 60 months) of follow-up, our clinical study has shown a significant improvement of the clinical status of the physically active patients after the mosaic type autologous osteochondral transplantation for the repair of articular cartilage defects in the knee. AbstractPhysical activity brings significant health benefits to middle-aged adults, although the research to date has been focused on late adulthood. This study aims to examine how ageing affects the self-reported and accelerometer-derived measures of physical activity levels in middle-aged adults. We employed the data recorded in the UK Biobank and analysed the physical activity levels of 2,998 participants (1381 men and 1617 women), based on self-completion questionnaire and accelerometry measurement of physical activity. We also assessed the musculoskeletal health of the participants using the dual-energy X-ray absorptiometry (DXA) measurements provided by the UK Biobank. Participants were categorised into three groups according to their age: group I younger middle-aged (40 to 49 years), group II older middle-aged (50 to 59 years), and group III oldest middle-aged (60 to 69 years). Self-reported physical activity level increased with age and was the highest in group III, followed by group II and I (P < 0.05). On the contrary, physical activity measured by accelerometry decreased significantly with age from group I to III (P < 0.05), and the same pertained to the measurements of musculoskeletal health (P < 0.05). It was also shown that middle-aged adults mostly engaged in low and moderate intensity activities. The opposing trends of the self-reported and measured physical activity levels may suggest that middle-aged adults over-report their activity level as they age. They should be aware of the difference between their perceived and actual physical activity levels, and objective measures would be useful to prevent the decline in musculoskeletal health.
