Background: Metabolic and physicochemical evaluation is recommended to manage the condition of patients with nephrolithiasis. The estimation of the saturation state (β values) is often included in the diagnostic work-up, and it is preferably performed through calculations. The free concentrations of constituent ions are estimated by considering the main ionic soluble complexes. It is contended that this approach is liable to an overestimation of β values because some complexes may be overlooked. A recent report found that β values could be significantly lowered upon the addition of new and so far neglected complexes, [Ca(PO4)Cit]4− and [Ca2H2(PO4)2]. The aim of this work was to assess whether these complexes can be relevant to explaining the chemistry of urine. Methods: The Ca–phosphate–citrate aqueous system was investigated by potentiometric titrations. The stability constants of the parent binary complexes [Cacit]− and [CaPO4]−, and the coordination tendency of PO43− toward [Ca(cit)]− to form the ternary complex, were estimated. βCaOx and βCaHPO4 were then calculated on 5 natural urines by chemical models, including or not including the [CaPO4]− and [Ca(PO4)cit]4− species. Results: Species distribution diagrams show that the [Ca(PO4)cit]4− species was only noticeable at pH >8.5 and below 10% of the total calcium. β values estimated on natural urine were slightly lowered by the formation of [CaPO4]− species, whereas [Ca(PO4)cit]4− results were irrelevant. Conclusions: While [CaPO4]− species have an impact on saturation levels at higher pHs, the existence of ternary complex and of the dimer is rejected.

An understanding of the principles of complex equilibria and species distribution in different solutions is important in expounding and correlating the interaction of different ligands with different metal ions in complex formation. Therefore, acid-base equilibria involved in the formation of binary and ternary complexes of Co (II), Cu (II) and Pb (II) with methionine (Met) and uracil (Urc) have been determined by potentiometric titration technique. The stability constants of the complexes were evaluated at 35 ± 0.1°C and 0.02 M ionic strength (kept constant with NaNO3) in aqueous and organic-aqueous media. The species distribution in solutions as a function of pH was determined using the Hyss program. The stability of the ternary complexes relative to the corresponding binary complexes of the secondary ligand is measured in terms ΔlogK and % RS values. The ternary complexes are observed to be more stable than binary complexes in the media except for [CuMetUrc] ternary complex in organic-aqueous medium where the ternary complex is less stable than the binary complex of the uracil. The overall stability of the ternary complexes was higher in organic-aqueous system than aqueous system. The stability of the complexes was found to be correlated with the covalent index of the metal ions and Jahn Teller distortion. pH-studies of these systems revealed an increase in the concentrations of the ternary complexes with increase in pH. The formation of binary complexes was shown to be favoured in physiological pH range (3-7) while that of the ternary complexes is observed to be favoured in the pH range 5-10.

The stability constants of the 1:1 binary complexes of Ni(II) and Co(II) with 3-amino-1,2,4-triazole (AT), leucine (Leu) and glutamic acid (Glu), and the 1:1:1 ternary complex of them and the protonation constants of the ligands were determined potentiometrically at a constant ionic strength of I = 0.10 mol L-1 (NaClO4) in aqueous solutions at 15.0 and 25.0 ?C. The thermodynamic parameters ?Gf0, ?Hf0 and ?Sf0 are reported for the formation reactions of the complexes. The enthalpy changes of all the complexations were found to be negative but the entropy changes positive. While the driving force for the formation of the Ni(II), Co(II) ? AT complexes is the enthalpy decrease, the driving force for the ternary complexes of AT is the entropy increase.

Abstract Background Despite the availability of information about the effect of aqua-surfactants media on the stability of binary complexes, data related to mixed ligand complexes of Phen and Orn with essential metals in aqua-surfactant medium is scarce. The aim of this study was, to determine the stabilities of the mixed ligand complexes of essential metal ions (Co2+, Ni2+, Cu2+) with Phen and Orn in aqua-TBAB and PEG-400 surfactants with an eye on the prediction of the bioavailability of the metal complexes. Results Complexes of some essential metal (M) ions (Co2+, Ni2+, Cu2+) of L-ornithine (L) and 1,10-phenanthroline (X) ligands in various concentrations (0.0–2.5% v/v) of aqueous tetrabutylammonium bromide (TBAB) or polyethylene glycol-400 (PEG-400) surfactant were synthesized pH-metrically at 298 K and 0.16 mol L− 1 ionic strength. The relative amounts of L:M:X were 2.5:1.0:2.5; 5.0:1.0:2.5; 2.5:1.0:5.0. The data acquisition of acid-base equilibria and determination of stability constants were performed using MINIQUAD75 algorithm. The distribution patterns of the complexes with varying pH and compositions of surfactants were presented from the plots of SIM run data. Conclusions The best fit chemical models were found to be MLXH, MLX2H in the lower pH, and MLX, ML2X in the higher pH ranges for all the metals. The logged values of stability constants decreased linearly with increasing concentrations of surfactants, indicating the dominance of electrostatic factors. The log of the disproportionation constant and the change in log values of the mixed ligand constant indicated additional stability of the mixed ligand complexes, compared to the parent binary complexes due to interactions outside the coordination sphere. This makes the mixed ligand complexes more amenable to metal ion storage and transport and threw light to have information on the less stable binary complexes easily bioavailability while the mixed ligand complexes could be stored and transported in the bio-fluids. Significant change on the magnitudes of the stability constants, high values of standard deviation and rejection of some of the proposed chemical models were observed due to pessimistic error, indicating the sufficiency of the models to represent the data and accuracy of the method employed.

Rapidly progressive glomerulonephritis is one of the most dramatic and tragic presentations of lupus nephritis (LN) or renal manifestation of systemic lupus erythematosus (SLE). A 35-year-old Bangladeshi gentleman presented with worsening oedema, scanty, high colored, frothy urine and deteriorating renal function. He had puffy face, anaemia, oedema, normal jugular venous pressure (JVP), high blood pressure (150/90 mm Hg), ascites and bilateral pleural effusions. Diagnostic work-up confirmed SLE with class IV LN. His initial response to specific therapy showed improvement Birdem Med J 2020; 10(2): 137-138

The formation constants of the binary and ternary complexes of palladium(II) with diethylenetriamine and amino acids as ligands have been determined potentiometrically at 25 °C in 0.1 M NaNO3 solution. The relative stability of each ternary complex was compared with that of the corresponding binary complexes in terms of ∆logK values. The mode of chelation was ascertained by conductivity measurements.

The artery of Percheron (AOP) is a rare arterial variant of the thalamic blood supply. Due to the densely packed collection of nuclei it supplies, an infarction of the AOP can be devastating. Here we highlight a patient who had an AOP stroke in the community, which was initially managed as cardiac arrest. AOP strokes most often present with vague symptoms such as reduced conscious level, cognitive changes and confusion without obvious focal neurology, and therefore are often missed at the initial clinical assessment. This case highlights the importance of recognising an AOP stroke as a cause of otherwise unexplained altered consciousness level and the use of MRI early in the diagnostic work-up.

Background: etiological investigations are not done for all adult patients with bronchiectasis because of the availability and interpretation of tests. The aim of the study was to elaborate a score to identify patients at high risk of having cystic fibrosis or primary ciliary dyskinesia (CF/PCD), which require appropriate management. Methods: diagnostic work-ups were carried out on a French monocenter cohort, and results were subjected to logistic-regression analyses to identify the independent factors associated with CF/PCD diagnosis and, thereby, elaborate a score to validate in a second cohort. Results: among 188 patients, 158 had no obvious diagnosis and were enrolled in the algorithm-construction group. In multivariate analyses, age at symptom onset (8.69 (2.10–35.99); p = 0.003), chronic ENT symptoms or diagnosed sinusitis (10.53 (1.26–87.57); p = 0.03), digestive symptoms or situs inversus (5.10 (1.23–21.14); p = 0.025), and Pseudomonas. aeruginosa and/or Staphylococcus aureus isolated from sputum (11.13 (1.34–92.21); p = 0.02) are associated with CF or PCD. Receiver operating characteristics curve analysis, using a validation group of 167 patients with bronchiectasis, confirmed the score’s performance with AUC 0.92 (95% CI: 0.84–0.98). Conclusions: a clinical score may help identify adult patients with bronchiectasis at higher risk of having CF or PCD.

SummaryHLH is a life-threatening disease, which is characterized by a dysregulated immune response with uncontrolled T cell and macrophage activation. The often fulminant course of the disease needs a fast diagnostic work-up to initiate as soon as possible the appropriate therapy. We present herein the case of a 71-year-old patient with rapidly progressive hyperinflammatory syndrome, which post mortem resulted in the diagnosis of EBV-associated HLH. With this case report, we intend to highlight the relevance of the HScore in the diagnosis of HLH, to create a greater awareness for EBV as a trigger of HLH, and to demonstrate the importance of treating EBV-associated HLH as early as possible.
