The need for a multisectoral approach to tackle stunting has gained attention in recent years. Baduta project aims to address undernutrition among children during their first 1000 days of life using integrated nutrition-specific and nutrition-sensitive interventions. We undertook this cohort study to evaluate the Baduta project’s effectiveness on growth among children under 2 years of age in two districts (Sidoarjo and Malang Districts) in East Java. Six subdistricts were randomly selected, in which three were from the intervention areas, and three were from the control areas. We recruited 340 pregnant women per treatment group during the third trimester of pregnancy and followed up until 18 months postpartum. The assessment of breastfeeding and complementary feeding practices used standard infant and young child feeding (IYCF) indicators in a tablet-based application. We measured weight and length at birth and every three-months after that. The enumerators met precision and accuracy criteria following an anthropometry standardization procedure. Among the breastfed children, the percentage of children who achieved the minimum dietary diversity score (DDS) and minimum acceptable diet (MAD) was higher for the intervention group than the comparison group across all age groups. The odd ratios were 3.49 (95% CI: 2.2–5.5) and 2.79 (95% CI: 1.7–4.4) for DDS and 3.49 (95% CI: 2.2–5.5) and 2.74 (95% CI: 1.8–5.2) for MAD in the 9–11 month and 16–18-month age groups, respectively. However, there was no significant improvement in growth or reduction in the prevalence of anemia. The intervention was effective in improving the feeding practices of children although it failed to show significant improvement in linear growth of children at 18 months of age.

Background: Optimal infant and young child feeding practices (IYCF) are essential to address the increasing burden of malnutrition and for the overall development of the children. The present study was conducted to estimate the proportion of optimal infant and young child feeding practices among rural children aged 0 to 23 months and study the associated socio demographic factors.Methods: A community based cross-sectional study was conducted among 360 children in the age group of 0 to 23 months in a rural field practice area of a medical college in Puducherry. Data on IYCF practices were collected using a standardized tool developed by WHO. Core and optional IYCF indicators were calculated. Chi-square test and Fishers’ exact test were used as tests of significance.Results: Almost 88.0% of infants were initiated early on breastfeeding and 90.8% were exclusively breastfed for six months. Children who were continuously breastfed at one year and two years were 77.4% and 22.4% respectively. About 75.0% were introduced with solid or semisolid foods at 6 to 8 months of age. Among children aged 6 to 23 months, 77.3% had the recommended minimum dietary diversity, 81.3% had the minimum meal frequencies, while 57.7% received the minimum acceptable diet and only 39.4% consumed iron rich foods. Gender was significantly associated with the practice of continued breastfeeding at one year, adequate minimum dietary diversity and minimum meal frequency.Conclusions: The core and optional IYCF indicators were acceptably good in the initial six months of life but thereafter showed suboptimal levels, which should be emphasized among the mothers or primary care givers.

Despite improvements in food production and healthcare services, the burden of malnutrition in Uganda has for the last 30yearsremained unacceptably high with rates of stunting (chronic undernutrition) and anemia (proxy for micronutrient deficiency) currently estimated at 29% and 53%,respectively among young children aged 6-59 months. Considering that both undernutrition and over nutrition are greatly attributed to monotonous diets characterized by limited dietary diversity and overdependence on starchy refined grains or roots as staples,there is need to improve the population’s awareness of appropriate dietary practices. To improve nutrition education, the Infant and Young Child-feeding national counseling cards for community volunteers (IYCF cards)that were developed by United Nations Children’s Fund (UNICEF), are currently the standard package used in Uganda’s health sector to educate caregivers on appropriate child-feeding practices. In this study, the effectiveness of a three-group food guide was evaluated against IYCF cards.A randomized, controlled intervention trial engaged three randomly selected distant groups of child-caregiver pairs (n=40) concurrently in one of three treatments namely: (i) nutrition education using a three-foodgroup guide (FG), (ii) nutrition education using age-appropriate IYCF cards, and (iii) negative control group that engaged in hair-plaiting sessions. At baseline, all groups had randomly selected caregivers of children aged 6-14 months and were met once a week for five consecutive weeks during the intervention. Caregivers were interviewed at baseline and 2 months after the interventions to determine changes in child-feeding practices while their children were concurrently measured to determine changes in their nutritional status.At baseline, caregivers in the three treatment arms exhibited inappropriate child-feeding practices indicated by low child-feeding index (CFI) scores,which were also related to poor nutritional status of their children. After the interventions, children in FG group were given more varied animal-source foods than those in IYCF cards group (p =0.02). Compared to controls, caregivers in FG group gave their children significantly more snacks (p = 0.01), their child-feeding practices indicated by CFI scores significantly improved (p = 0.001) and their children exhibited better growth patterns indicated by weight-for-age (p = 0.02) and MUAC-for-age (p = 0.03) Z-scores.These findings,therefore,indicate that the three-group food guide is more likely to improve child-feeding practices and growth patterns than IYCF cards.Hence, there is need to integrate the food guide into IYCF materials to foster child-feeding practices and growth.

Abstract Objectives In the context of large-scale interventions to improve infant and young child feeding (IYCF) practices, we examined differences in characteristics of those exposed and not exposed to interventions; exposure to interpersonal communication (IPC), including overlap with other interventions and timing and number of contacts; and relationship between frequency of IPC contacts and IYCF practices. Methods We used endline survey data from Bangladesh (2000 mothers with children <2y, 1000 per program group), Ethiopia (1720 mother with children aged 6–23.9 mo, 1360 per group), and Vietnam (2005 mothers with children <2y, 1000 per group). Regression models were used to test differences in exposure to specific or combined interventions between two arms and relationships between the frequency of IPC contacts or exposure to interventions and IYCF practices, adjusted for child age and sex and geographic clustering. Results Those exposed to interventions had higher socioeconomic status, food security, and maternal dietary diversity. There was large overlap in exposure to IPC with other interventions in intensive-intervention areas in all 3 countries. On average, mothers received 8 visits in the last 6 mo, 2 visits in the last 3 mo, and 1 visit in the last 6 mo in Bangladesh, Ethiopia, and Vietnam respectively. In Bangladesh, there was a positive, nonsignificant association between intervention exposure and IYCF practices in intensive areas. In Ethiopia, exposure to IPC with other interventions was associated with higher odds of minimum meal frequency (OR: 1.6), minimum dietary diversity (OR: 1.8), and consumption of iron-rich foods (OR: 4.7). In Vietnam, exposure to IPC alone or with mass media was associated with 3.7 and 2.8 higher odds respectively of exclusive breastfeeding. At least monthly visits was associated with higher odds of IYCF practices in Bangladesh and Ethiopia. In Vietnam, any number of IPC visits was associated with 2 times higher odds of exclusive breastfeeding. Conclusions Effects of combined behavior-change interventions and frequency of IPC contacts may be context-specific. ABSTRACT Background Suboptimal infant and young child feeding (IYCF) practices contribute to child undernutrition. Sierra Leone Demographic and Health Survey data show that IYCF practices remain poor despite modest improvements. Recent studies have identified the role of grandmothers as critical to child nutrition; however, in Sierra Leone to date, the potential for grandmothers to influence IYCF practices has not been investigated. Objectives We examined how an innovative grandmother-inclusive approach (GMIA) can be used to address suboptimal IYCF practices. Methods Using a quasi-experimental design, we compared IYCF beliefs and practices between GMIA intervention communities (receiving monthly dialogue sessions on nutrition, quarterly community praise sessions, and intergenerational forums) and comparison communities (receiving standard nutrition education) in Bum chiefdom from 2013 and 2016. The quantitative endline survey targeted 101 pregnant women, 291 women with children aged &lt;2 y, and 219 grandmothers. Statistical analyses utilized t tests and χ2 tests to examine differences between intervention and comparison communities at endline. Multivariate regression was used to determine the intervention's effect on IYCF outcomes of interest. Results Awareness of and participation in the GMIA was high among mothers and grandmothers in intervention communities. The percentage of infants and young children aged 0–23 mo (n = 291) exclusively breastfed during the first week of life was significantly higher in the intervention group (90.2% compared with 79.4%, P = 0.01). Among infants aged 6–23 mo (n = 219), the percentage achieving minimum dietary diversity and minimum acceptable diet was significantly higher in the intervention group (77.2% compared with 51.8%, P &lt; 0.001; and 53.8% compared with 22.6%, P &lt; 0.001, respectively). Differences in percentages achieving minimum meal frequency (MMF) were only significant for infants aged 9–23 mo, with the intervention group achieving a higher MMF (54.6% compared with 36.9%, P = 0.02). Conclusions Results suggest that a GMIA that recognizes grandmothers’ roles and strengthens their knowledge can contribute to improved IYCF practices.

ABSTRACT Background Social and behavior change communication interventions are integral to improving dietary and care practices, but evidence on the impact of the combination and intensity of these interventions in different contexts is scarce. Objectives We examined the extent of and factors associated with intervention exposure: interpersonal communication (IPC) alone or with other interventions (i.e., mass media, community mobilization, or nutrition-sensitive agricultural activities), number of and factors associated with IPC contacts, and combinations of intervention components and number of contacts associated with infant and young child feeding (IYCF) practices. Methods We used endline survey data from impact evaluations in Bangladesh, Ethiopia, and Vietnam (n = 1001, 1720, and 1001 mothers with children aged &lt;2 y, respectively). Multivariable regression models were used for analyses. Results Exposure to the interventions varied in all 3 countries. On average, mothers received 8 visits in the last 6 mo in Bangladesh, 2 visits in the last 3 mo in Ethiopia, and 1 visit in the last 6 mo in Vietnam. Across countries, the factors associated with intervention exposure and number of IPC contacts differed. In Ethiopia, exposure to IPC with other interventions was associated with higher odds of achieving minimum meal frequency (OR: 1.6), minimum dietary diversity (OR: 1.8), and consumption of iron-rich foods (OR: 4.7). In Vietnam, exposure to IPC alone or with mass media was associated with higher odds of exclusive breastfeeding (EBF; OR: 2.8–3.7). Near-monthly visits were associated with 2–3 times higher odds of IYCF practices in Bangladesh and Ethiopia. In Vietnam, even 1 IPC visit was associated with 2 times higher odds of EBF. Conclusions Exposure matters for impact, but the combination of behavior change interventions and number of IPC contacts required to support IYCF behavior change are context specific. Abstract Background Interaction within mothers’ social networks can theoretically diffuse messages from interventions and campaigns into norms and practices for infant and young child feeding (IYCF). Objectives We hypothesized that mothers’ social networks, diffusion of information, and social norms differed in intensive [intensive interpersonal counseling (IPC), community mobilization (CM), and mass media (MM)] compared with nonintensive (standard IPC and less-intensive CM and MM) intervention areas, were associated with IYCF practices, and partly explained practice improvement. Methods We conducted household surveys at endline in 2014 and follow-up in 2016 (n = ∼2000 each round). We used multiple regression to test differences and changes in networks, diffusion, and norms within intervention areas. We analyzed paths from intervention exposure to IYCF practices through networks, diffusion, and norms. Results Mothers’ networks were larger in intensive than in nonintensive areas in 2014 and increased in both areas over time [25–38 percentage points (pp)]. The prevalence of receipt of IYCF information was high, with no changes over time in intensive areas but an increase in nonintensive areas (8–16 pp). In both areas, more family members and health workers provided IYCF information over time. Sharing of information increased 17–23 pp in intensive and 11–41 pp in nonintensive areas over time. Perceived descriptive norms improved 8–16 pp in intensive and 17–28 pp in nonintensive areas. Perceived injunctive norms were high in both areas. Breastfeeding practices were associated with networks, diffusion, and norms (OR: 1.6–4.4 times larger comparing highest with lowest quartile). Minimum dietary diversity was associated with larger networks and diffusion (OR: 1.5–2.2) but not with social norms. Indirect paths from intervention exposure to practices explained 34–78% of total effects. Conclusions Diffusion of IYCF information through social networks, reinforced by positive social norms for messages promoted over time, will contribute to positive changes in IYCF practices that may be achieved and sustained through large-scale social and behavior change interventions. Background: Age appropriate feeding practices is an essential determinant of physical growth as well as mental health of under-five children. WHO and UNICEF have formulated the infant and young child feeding practices guidelines to enhance appropriate feeding practices in infants and young children.Methods: A descriptive, observational, cross-sectional study was conducted in the immunization clinic of KPC medical college and hospital from October to December 2018 among children in 0-23 months age group. The mothers were interviewed regarding their sociodemographic details as well as the IYCF practices as per WHO.Results: It was found that appropriate practices in terms of prelacteal feeding, colostrum feeding; early initiation of breast feeding and exclusive breast feeding was present in 78.4%, 80%, 77.5% and 50% children respectively. Timely initiation of complementary feeding was found in 81%, breast feeding was continued upto 1 year in 68.8% and consumption of iron rich or iron fortified food was found in 54.5% children. Appropriate practices in terms of minimum meal frequency, minimum dietary diversity and minimum appropriate diet were found in 49.7%, 27% and 32.3% children respectively. Sex wise distributions have found significant association with minimum dietary diversity and minimum acceptable diet. Age wise distribution revealed significant association with minimum dietary frequency, minimum dietary diversity and minimum acceptable diet (p<0.005).Conclusions: Thus infant and young child feeding practices were not satisfactory. Mothers should be made aware about the appropriate feeding practices and health education should be given regarding correct child feeding practices.

Background: Indicators of infant and young child and feeding (IYCF) practices have been devised by WHO in order to help objectively assess the prevalence of these practices in the population. IYCF practices play a vital role in the control of infant and child deaths. This study was undertaken to assess the IYCF practices among children aged less than 2 years among the tribal population. Objective of this study to measure the indicators of IYCF practices in children in the 0-23 months age group.Methods: It is a community based, cross sectional study of infant and young child feeding practices among tribal mothers having children younger than 24 months, in Sambalpur district of Odisha from October 2017 to December 2019. The study participants were tribal mothers having children less than 24 months of age. The sample size was 384.Results: Among the study participants, 93.5% belonged to the 21-30 years age bracket, 37.5% had middle school certificate education and 75.3% belonged to the upper lower socioeconomic class. The indicators were continued breastfeeding at 1 year (100%), continued breastfeeding at 2 years (100%) and children ever breastfed (100%), exclusive breastfeeding (98.71%) and bottle feeding (16.36%). A significant association was found between delivery by lower segment caesarean section and delayed initiation of breastfeeding.Conclusions: The indicators related to breastfeeding and complementary feeding were adequate except for minimum dietary diversity and minimum adequate diet.

Abstract Objectives Poor infant and young child feeding practices (IYCF) can undermine the nutrition and health status of individuals in vulnerable households. Bauchi state, Nigeria, has the fourth highest prevalence of child malnutrition in the country, yet over 80% of the population is involved in agriculture. This study evaluated the predictors of IYCF practices of rural farming households in Bauchi state, Nigeria Methods This study was conducted in six Local Government Areas of Bauchi state. A total of 720 mothers and fathers were selected from households with at least one child under 5 years. Household socio-demographic features and information on infant and young child feeding (IYCF questionnaire of the World Health Organization), were collected using Open Data Kit Collect, a mobile app. Descriptive statistics were computed, and relationships between variables were evaluated using Spearman's rank correlation and multivariate analyses. Results The mean age (27.2 ± 7.2 years vs. 41.7 ± 10.6 years) and monthly income (∼$4.85 vs. ∼$39.88) of the mothers and fathers respectively, were different (P < 0.05). Twenty-six (3.9%) mothers and 165 (23%) fathers had completed secondary or post-secondary education. The rates of exclusive (20.7%) and timely initiation (29.0%) of breastfeeding were poor. Only 26.0% of children 6–23 mo. received the minimum acceptable diet (minimum dietary diversity and meal frequency). Mothers’ education was positively correlated with the practice of exclusive breastfeeding, while continued breastfeeding at 15 months of age was positively associated with fathers’ income (P < 0.05). Both mothers’ and fathers’ income and education were positively associated with dietary diversity (P < 0.05). Low income and poor education of mothers were negative predictors of timely initiation of breastfeeding, while mothers’ occupation, household size, and the number of children per household were negative predictors of dietary diversity (P < 0.05). Conclusions Infant and young child feeding practices in the rural farming area were influenced by parents’ income and education, and were below optimum levels needed to prevent malnutrition. Abstract Background Appropriate complementary feeding is critical for optimal nutrition in infant and young children as it ensures their growth, health and development to attain their full potential. However, evidence shows children from developing countries do not meet the core indicators for appropriate complementary feeding. We evaluated mothers’ competency in infant and young child feeding recommendations and their association to complementary feeding practices among children aged 6–23 months.Methods This cross-sectional study included 200 children aged 6–23 months and their mothers/care givers recruited during the child welfare clinics of two health facilities in Ghana. Data was collected using a structured questionnaire. Multivariate logistic regression was used to assess determinants of appropriate diet.Results Sixty-eight percent of the mothers knew the recommended duration of continued breastfeeding, 56.5% how to ensure dietary diversity and enrich their children’s diets and 94% (n = 188) had positive attitude towards recommended infant and young child feeding practices. Majority of the mothers (92%, n = 183) practiced continued breastfeeding, 10.5% of the children met minimum diet diversity score, 39.5% minimum meal frequency and 8.5% received minimum acceptable diet. Determinants of receipt of minimum adequate diet were: having high knowledge in child feeding recommendations (100% vs. 0.0; p < 0.001) and child’s father reportedly earning enough financially (AOR = 12.1 (1.32–109.72; p = 0.027).Conclusion Poor complementary feeding practices were common and mother’s knowledge levels regarding infant and young child feeding recommendations as well as the income levels of the child’s father were important determinants of appropriate diet. Nutrition education should emphasis on improving mothers’ nutrition knowledge regarding infant and young child feeding and supporting mothers to overcome barriers to feed their children with appropriate complementary foods.
