Dermatillomania or skin picking disorder (SPD) is a chronic, recurrent, and treatment resistant neuropsychiatric disorder with an underestimated prevalence that has a concerning negative impact on an individual’s health and quality of life. The current treatment strategies focus on behavioral and pharmacological therapies that are not very effective. Thus, the primary objective of this review is to provide an introduction to SPD and discuss its current treatment strategies as well as to propose biomaterial-based physical barrier strategies as a supporting or alternative treatment. To this end, searches were conducted within the PubMed database and Google Scholar, and the results obtained were organized and presented as per the following categories: prevalence, etiology, consequences, diagnostic criteria, and treatment strategies. Furthermore, special attention was provided to alternative treatment strategies and biomaterial-based physical treatment strategies. A total of six products with the potential to be applied as physical barrier strategies in supporting SPD treatment were shortlisted and discussed. The results indicated that SPD is a complex, underestimated, and underemphasized neuropsychiatric disorder that needs heightened attention, especially with regard to its treatment and care. Moreover, the high synergistic potential of biomaterials and nanosystems in this area remains to be explored. Certain strategies that are already being utilized for wound healing can also be further exploited, particularly as far as the prevention of infections is concerned.

The ability to scrutinize our surroundings remains heavily dependent on the sense of smell. From the ability to detect dangerous situations such as fires to the recollection of a fond memory triggered by an odor, the advantages of an intact olfactory system cannot be overstated. Outcomes studies have highlighted the profound negative impact of anosmia and parosmia on the overall quality of life. The National Institute on Deafness and Other Communication Disorders estimates that ∼1.4% of the United States population experiences chronic olfactory dysfunction and smell loss. Efforts have focused on improving both the diagnosis of olfactory dysfunction through olfactory testing and improved reporting of treatment outcomes of olfactory training. The purpose of this article was to review the differential diagnosis, workup, and current treatment strategies of anosmia and smell disorders.

Patients suffering from schizophrenia are usually treated with antipsychotic agents posing a threat both to their cognitive and physical skills. Anticholinergic and antihistaminic side effects are well known to have a negative impact on mental as well as physical health and pose a major hazard regarding progress in reintegration.Strategies changing the fate of patients have moved away from just dampening dopaminergic noise in the limbic system. Nowadays, the new focus is on cognitive remediation. The centre of these efforts is to enhance strategic and executive skills and thereby alter patients' motivation to take their life into their own hands again.The present report aims at presenting evidence from both physical treatment strategies with the power to enhance hippocampal circuit capacity as well as cognitive remediation tools with the the aim of augmenting executive skills by rewiring cognitive circuitry in the brain.Preliminary results demonstrate not only improved processing of mental load found with fMRI measured circuitry connectivity, but they also point to the improvement of physical parameters taken in a continuous clinical context.The reasons for these favourable findings are not straight forward and trivial, but they seem to be the result of complex interaction between various factors. The latter are to be pursued in a large cohort and presented at a later point in time.

Abstract Background : Pneumonia remains the leading cause of death in children under five worldwide, accounting for about 1.6 million deaths a year. Streptococcus pneumonia , has proved to be the most competent bacterial pathogen causing pneumonia infections among the infants and the elderly, leading to great morbidity and mortality. Currently there are over 94 different serotypes of Streptococcus pneumonia , and quite a number have emerged as drug-resistant strains which introduce a twist to the current treatment strategies. Detailed information of current antibiotic susceptibilities, resistance and serotype distribution will be important in designing new strategies for treatment and management of pneumococcal infections. Methods: A total of 309 S. pneumoniae isolates were collected from patients attending the Kisii Teaching and Referral Hospital between 2017 and 2018. Nasopharyngeal samples collected and serotyped using PCR. The samples were cultured and their sensitivity to drugs tested using various concentrations of; Oxacilin, Erythromycin, Tetracycline, Ofloxacin, Trimethoprim/sulfamethoxazole and Chloramphenicol. Results: Out of the 309 samples collected, 235 were positive for S. pneumonia, although 10 of the samples had incomplete data. A total of 41 S. pneumonia serotypes isolated from the 309 samples with the most prevalent serotypes being; Non-typable (11%), 15B (7.6%), 19F (7.0%), 19A (6.7%), 23B (5.8%), 23F (5.3%), 6A (4.4%). All the serotypes were highly resistant to Oxacilin, and trimethoprim/sulfamethoxazole, while showing high sensitivity to Erythromycin, Ofloxacin and chloramphenicol. The rate of sensitivity to Tetracycline was 75% with the other 25% showing intermediate sensitivity. Conclusions: The data collected showed a high rate of S. pneumonia (76%) consisting of various serotypes for various ages. The various serotypes isolated showed a marked resistance routinely used antibiotics for patient samples of various ages. The isolated serotypes also consisted of serotypes 19A, 6A, and 5B that are lacking in the pneumococcal vaccine (PCV 10). This data will provide relevant information for a better approach to both clinical treatment, management and prevention of infections with S. pneumonia.

Interventional therapies using angioplasty and stenting of symptomatic stenosis of the proximal supraaortic vessels have evolved as safe and effective treatment strategies. The aim of this paper is to summarize the current treatment concepts for stenosis in the subclavian and brachiocephalic artery with regard to clinical indication, interventional technique including selection of the appropriate vascular approach and type of stent, angiographic and clinical short-term and long-term results and follow-up. The role of hybrid interventions for tandem stenoses of the carotid bifurcation and brachiocephalic artery is analysed. A systematic review of data for angioplasty and stenting of symptomatic extracranial vertebral artery stenosis is discussed with a special focus on restenosis rate.

: Fabry disease is an X-linked disorder of glycosphingolipid metabolism that results in progressive accumulation of neutral glycosphingolipids, predominantly globotriaosylsphingosine (Gb3) in lysosomes, as well as other cellular compartments of several tissues, causing multi-organ manifestations (acroparesthesias, hypohidrosis, angiokeratomas, signs and symptoms of cardiac, renal, cerebrovascular involvement). Pathogenic mutations lead to a deficiency of the lysosomal enzyme alpha-galactosidase A (GLA). In the presence of high clinical suspicion, a careful physical examination and specific laboratory tests are required. Finally, the diagnosis of Fabry’s disease is confirmed by the demonstration of the absence of or reduced alpha-galactosidase A enzyme activity in hemizygous men and gene typing in heterozygous females. Measurement of the biomarkers Gb3 and Lyso Gb3 in biological specimens may facilitate diagnosis. The current treatment of Anderson-Fabry disease is represented by enzyme replacement therapy (ERT) and oral pharmacological chaperone. Future treatments are based on new strategic approaches such as stem cell-based therapy, pharmacological approaches chaperones, mRNA therapy, and viral gene therapy. : This review outlines the current therapeutic approaches and emerging treatment strategies for Anderson-Fabry disease.

This study reviews the literature and the practice of accounting for research and development (R&D) costs from the first reference in 1917 to the current treatment. The conceptual treatment of R&D is compared to current financial accounting rules and explanation of the evolution of the current rules is presented. The economic and social consequences of the current rules which require R&D costs to be expressed are examined. The paper explores possible alternative treatment of R&D costs. As a contrast to U.S. practice, the accounting treatment of R&D costs in other countries is discussed. Given the findings of this paper, a strong case can be made for changing the way that R&D costs are accounted for in the United States.

The treatment of meniscus injuries has recently been facing a paradigm shift toward the field of tissue engineering, with the aim of regenerating damaged and diseased menisci as opposed to current treatment techniques. This review focuses on the structure and mechanics associated with the meniscus. The meniscus is defined in terms of its biological structure and composition. Biomechanics of the meniscus are discussed in detail, as an understanding of the mechanics is fundamental for the development of new meniscal treatment strategies. Key meniscal characteristics such as biological function, damage (tears), and disease are critically analyzed. The latest technologies behind meniscal repair and regeneration are assessed.

AbstractSchizophrenia is a chronic mental condition presenting a wide range of symptoms. Although it has a low prevalence compared to other mental conditions, it has a negative impact on social and occupational functions. This study aimed to assess the appropriateness of antipsychotic medications administered to schizophrenic patients and describe current treatment patterns for schizophrenia. A retrospective cohort study was conducted in all patients over the age of 15 with an active diagnosis of schizophrenia and treated with antipsychotics between 2008 and 2013 in the Valencia region. A total of 19,718 patients were eligible for inclusion. The main outcome assessed was inappropriateness of the pharmacotherapeutic management, including polypharmacy use. Altogether, 30.4% of patients received antipsychotic polypharmacy, and 6.8% were prescribed three or more antipsychotics. Overdosage affected 318 individuals (1.6%), and 21.5% used concomitant psychotropics without an associated psychiatric diagnosis. Women and people with a comorbid condition like anxiety or depression were less likely to receive antipsychotic polypharmacy. In contrast, increased polypharmacy was associated with concomitant treatment with other psychoactive drugs, and only in user on maintenance therapy, with more visits to the mental health hospital. Overall, we observed a high level of inappropriateness in antipsychotic prescriptions. Greater adherence to guidelines could maximize the benefits of antipsychotic medications while minimizing risk of adverse effects.
