Study of Nasal Fractional Exhaled Nitric Oxide (FENO) in Children with Allergic Rhinitis

(1) Background: Exhaled nitric oxide (NO) has been considered as a biomarker of airway inflammation. The measurement of fractional exhaled NO (FENO) is a valuable test for assessing local inflammation in subjects with allergic rhinitis (AR). (2) Objective: To evaluate (a) the correlation between nasal FENO with anthropometric characteristics, symptoms of AR and nasal peak flows in children without and with AR; and (b) the cut-off of nasal FENO for diagnosis of AR in symptomatic children. (3) Methods: The study was a descriptive and cross-sectional study in subjects with and without AR < 18 years old. All clinical and functional characteristics of the study subjects were recorded for analysis. They were divided into healthy subjects for the control group and subjects with AR who met all inclusion criteria. (4) Results: 100 subjects (14 ± 3 years) were included, including 32 control subjects and 68 patients with AR. Nasal FENO in AR patients was significantly higher than in control subjects: 985 ± 232 ppb vs. 229 ± 65 ppb (p < 0.001). In control subjects, nasal FENO was not correlated with anthropometric characteristics and nasal inspiratory or expiratory peak flows (IPF or EPF) (p > 0.05). There was a correlation between nasal FENO and AR symptoms in AR patients and nasal IPF and EPF (p = 0.001 and 0.0001, respectively). The cut-off of nasal FENO for positive AR diagnosis with the highest specificity and sensitivity was ≥794 ppb (96.7% and 92.6%, respectively). (5) Conclusion: The use of nasal FENO as a biomarker of AR provides a useful tool and additional armamentarium in the management of allergic rhinitis.

ABSTRACT Objective: To investigate whether there is an association between suicidal ideation (SI) and allergic diseases in adults. Methods: This was a comparative cross-sectional study involving individuals ranging from 20 to 50 years of age recruited from a university hospital in the city of Guadalajara, Mexico. We included patients with a confirmed diagnosis of allergic asthma, those with a confirmed diagnosis of allergic rhinitis, and healthy controls. All subjects completed the Beck Depression Inventory-II (BDI-II), which includes an item that evaluates the presence of suicidal thoughts or desires within the last two weeks, in order to identify SI. Results: The sample comprised 115 patients with allergic asthma, 111 patients with allergic rhinitis, and 96 healthy controls. The number of individuals identified with SI in the three groups were, respectively, 17 (14.8%), 13 (11.7%), and 8 (8.3%). Regarding the presence of SI, no statistically significant association was found in the allergic asthma group (OR = 1.98; 95% CI: 0.78-4.64; p = 0.154) or in the allergic rhinitis group (OR = 1.46; 95% CI: 0.58-3.68; p = 0.424) when they were compared with the control group. However, the presence of depression was associated with SI in the three groups: allergic asthma (OR = 12.36; 95% CI: 2.67-57.15; p = 0.001); allergic rhinitis (OR = 6.20; 95% CI: 1.66-23.14; p = 0.006); and control (OR = 21.0; 95% CI: 3.75-117.36; p < 0,001). Conclusions: In comparison with the control group, no association was found between SI and the groups with allergic diseases. In contrast, there was association between SI and depression in the three groups.

Purpose:  To evaluate the accuracy of the Corvis Biomechanical Index (CBI) in screening of patients with Keratoconus. Study Design:  Cross sectional study. Place and Duration of Study:  Amanat Eye Hospital Peshawar from July 2018 to June 2019. Material and Methods:  One thousand eyes were included in this study. Patients who came for keratorefractive laser procedure or collagen cross linkage were included in the study by convenient sampling technique. Six hundred and eleven patients had early Keratoconus and 389 were normal controls. Control group included those individuals who had visual acuity of 6/6 with no clinical feature of Keratoconus and normal TBI. All individuals included in the study underwent a thorough ocular examination, CBI and TBI tests. SPSS version 23 was used for statistical analysis of collected data. Results: The mean CBI value was 0.3186 ± 0.407 standard deviation (SD), the standard error (SE) of the mean was 0.0129 with a 95% confidence interval (CI) of 0.293 to 0.343. The minimum CBI value was 0.00 and the maximum value was 1.00. The mean Tomographic Biomechanical Index (TBI) value was 0.465 ± 0.392 SD, SE was 0.124 with a 95% CI of 0.222 to 0.708. The range of TBI values was 0.00 to 1.00. For CBI, sensitivity was 58.2%, specificity was 100%, positive predictive value was 100%, and negative predictive value was 61.2%. Conclusion:  Although CBI is accurate in terms of specificity and sensitivity, the result should be interpreted in combination with clinical data and other topographic and tomographic parameters.
