Abstract: The prevalence of retinal abnormality in Indonesia reached 0.13% and is the fourth leading cause of blindness after cataract, glaucoma. and refractive disorders. Vitrectomy is a type of surgery to treat retinal disorders that removes the vitreous, therefore, the operation can be performed and vision can be corrected. This study aimed to determine the indications to undergo vitrectomy in retinal disorders. This was a descriptive retrospective study using data of medical records of patients with retinal disorders who underwent vitrectomy at Community Eye Health Center (CEHC) of North Sulawesi Province. There were 27 patients in this study consisted of 17 male patients (63%) and 10 female patients (37%). The major indications of vitrectomy were retinal detachment in 16 patients (59%) and diabetic retinopathy in 11 patients (41%). In case of retinal detachment male patients were the most frequent to undergo vitrectomy (12 patients; 44.44%) than female patients (4 patients; 14.81%). In case of diabetic retinopathy there were no difference between males (18.52%) and females (22.22%)Keywords: retinal disorder, vitrectomy, retinal detachment, diabetic retinopathy  Abstrak: Prevalensi kelainan retina di Indonesia mencapai 0,13% dan merupakan penyebab kebutaan keempat setelah katarak, glaucoma, dan kelainan refraksi. Vitrektomi adalah salah satu jenis operasi untuk mengobati kelainan retina melalui pengangkatan vitreus sehingga retina dapat dioperasi dan penglihatan dapat diperbaiki. Penelitian ini bertujuan untuk mengetahui indikasi dilakukannya vitrektomi pada kelainan retina. Jenis penelitian ini deskriptif retrospektif dengan menggunakan data dari rekam medis penderita dengan kelainan retina yang menjalani vitrektomi di Balai Kesehatan Mata Masyarakat (BKMM) Propinsi Sulawesi Utara. Dalam penelitian ini ditemukan 27 pasien dengan kelainan retina yang menjalani vitrektomi terdiri dari 17 pasien laki-laki (63%) dan 10 pasien perempuan (37%). Indikasi untuk vitrektomi terbanyak pada retinal detachment sebanyak 16 pasien (59%) dan retinopati diabetik sebanyak 11 pasien (41%). Pada indikasi retinal detachment didapatkan pasien laki-laki yang paling banyak menjalani vitrektomi yaitu 12 pasien (44,44%) sedangkan perempuan 4 pasien (14,81%). Pada indikasi retinopati diabetik tidak terdapat perbedaan antara laki-laki (18,52%) dan perempuan (22,22%)Kata kunci: kelainan retina, vitrektomi, retinal detachment, retinopati diabetik

Introduction: Eruption of central incisors occurs at a time when the mixed dentition starts to develop and their impaction is mostly a severe esthetic problem.Aim: Our aim was to assess the frequency of impaction of an upper central incisor and to analyse the factors which affect the successful outcome and the protocols for orthodontic treatment in cases of impacted upper central incisors.Materials and methods: In the present study, we used all medical records of 651 patients treated in our private practice over the last 3 years and also the medical records of 18 patients we diagnosed and treated for this problem over the last 8 years.Results: We found the ratio of impacted upper central incisors to be 1.4% as there were more male than female patients affected. There were 2.8% male patients and 0.7% female patients. The most frequent cause of impaction of the upper central incisor was the presence of mesiodentes and supernumerary teeth (55.5%), followed by presence of follicular cysts (44.4%) and odontoma collections (22.2%) or a combination of these.Several approaches have been used in treating the different positions of impacted incisors. It is necessary first to rotate the axle of the incisor and assure its root full bone coverage. We used directly TPA for support. In other patients, we waited for the spontaneous eruption after removal of the etiological factor and in another patient, we used conventional fixed techniques.Conclusions: It is important that dental doctors should monitor the formation of the dentition so that they detect the impacted tooth early in its development.

The demographic and clinical features of 12 male patients with anorexia nervosa were investigated by means of a retrospective analysis of their medical records. The clinical characteristics of the male patients were found to be remarkably similar to those reported for female patients and our findings concur with previously published literature. There appears however to be a greater tendency to exercise excessively. The importance of recognising anorexia nervosa in males is emphasized.

Abstract: Diabetic retinopathy is one of the vascular disorders that can occur in type 1 and 2 diabetes mellitus cases after 10-15 years. Treatment of diabetic retinopathy is based on its severity; one of the treatment is laser photocoagulation.This study aimed to find out the indication of laser photocoagulation procedure among diabetic retinopathy cases at BKMM North Sulawesi Province. This was a descriptive retrospective study using the medical records of diabetic retinopathy cases who underwent laser photocoagulation. There were 35 cases as samples who underwent laser photocoagulation during January– December 2012. The results showed that most cases were proliferative diabetic retinopathy (86,6%).  The most frequent age group was 41-60 years (68.6%),  predominantly females (60%). Moreover, there were 80% cases who underwent one time of laser photocoagulation meanwhile the others 20% underwent more than one photocoagulation during  January– December 2012. Conclusion: The most frequent indication of laser photocoagulation among diabetic retinophaty cases at BKMM North Sulawesi Province was proliferative diabetic retinopathy. Penatalaksanaan retinopati diabetik dibuat berdasarkan pada tingkat kelainan penyakitnya; salah satu cara ialah dengan menggunakan terapi fotokoagulasi laser. Penelitian ini bertujuan untuk mengetahui indikasi fotokoagulasi laser pada kasus retinopati diabetik di Balai Kesehatan Mata Masyarakat (BKMM) Propinsi Sulawesi Utara. Metode penelitian ini deskriptif retrospektif dengan menggunakan catatan rekam medik kasus retinopati diabetik yang telah dilakukan fotokoagulasi laser. Sampel berjumlah 35 kasus retinopati diabetik yang menjalani fotokoagulasi laser pada periode Januari – Desember 2012 dengan persentase terbanyak retinopati diabetik proliferatif (88,6%). Kelompok usia tersering 41-60 tahun (68,6%), sedangkan jenis kelamin  didominasi oleh perempuan (60%). Kasus yang terbanyak (80%) ialah yang menjalani hanya satu kali fotokoagulasi laser sedangkan sisanya (20%) menjalani tindakan tersebut lebih dari satu kali. Simpulan: Pada kasus retinopati diabetik di BKMM Propinsi Sulawesi Utara indikasi tersering untuk dilakukan fotokoagulasi laser ialah retinopati diabetik proliferatif. Introduction: We report a novel phenotype of mandibular hypoplasia, deafness, and progeroid features with lipodystrophy (MDPL) syndrome with POLD1 mutation in a Chinese girl. Case description: Diabetic retinopathy was detected as the primary manifestation in a Chinese girl with MDPL syndrome carrying a known POLD1 mutation (c.1812_1814delCTC, p.Ser605del). Typical characteristics of the syndrome including mandibular hypoplasia, deafness, progeroid features, and diabetes were detected after comprehensive examinations. The patient suffered from blurred vision and eye pain due to the neovascularization of the retina (vitreous hemorrhage and retinal detachment) and iris (neovascular glaucoma). The literature review revealed that the prevalence of hepatomegaly and abnormal triglyceride levels were significantly higher in female than in male with MDPL syndrome carrying POLD1 mutations. Conclusion: These results expand our knowledge regarding the clinical phenotypes of MDPL syndrome with POLD1 mutations. Diabetic retinopathy is a non-negligible complication of MDPL syndrome. The phenotype varies among female and male patients with the syndrome. Hepatomegaly and abnormal triglyceride levels are more common in female patients with MDPL syndrome.

Abstract Background: The sociodemographic characteristics and clinical features of dementia patients in psychiatric hospitals have not been well discussed in China. This study was to explore gender differences in outpatients with dementia from a psychiatric hospital in China.Methods: This retrospective study examined outpatients with dementia from January 2013 to August 2019 using data in the OMOP common data model (OMOP-CDM) in Beijing Anding Hospital. Age, sex, number of visits, use of drugs and comorbid conditions were recorded by medical records.Results: 9,004 patients were recruited, and the mean number of visits weas 6.92. There were 3,433 (38.13%) male patients and 5,571 (61.87%) female patients. The most common comorbidities were generalized anxiety disorder, nonorganic insomnia, delusional disorder and depressive disorder. The proportion of use of drugs was highest in antidementia (68.30%), followed by benzodiazepines (48.83%), antipsychotics (45.43%), antidepressants (22.24%) and nonbenzodiazepines (19.96%). Patients with dementia showed a significant gender difference in average age (t=6.36, P<0.0001). Compared to male patients, female patients had a higher number of visits (7.40±12.90 vs 6.15±10.50, t=4.81, P<0.0001). There were significant differences in comorbidity composition between male and female patients (t=23.09, P<0.0001).Conclusions: Our present findings suggested significant gender differences in the proportion of age, number of visits and comorbidity composition in outpatients with dementia.

Macular peeling refers to the surgical technique for the removal of preretinal tissue or the internal limiting membrane (ILM) in the macula for several retinal disorders, ranging from epiretinal membranes (primary or secondary to diabetic retinopathy, retinal detachment…) to full-thickness macular holes, macular edema, foveal retinoschisis, and others. The technique has evolved in the last two decades, and the different instrumentations and adjuncts have progressively advanced turning into a safer, easier, and more useful tool for the vitreoretinal surgeon. Here, we describe the main milestones of macular peeling, drawing attention to its associated complications.

Abstract: Eye infections are caused by viruses, bacteria, fungi, or parasites. Infections can be marked with red eyes, painful, watery, and light sensitive. This study was aimed to obtain the profile of eye infections in Rumah Sakit Mata Provinsi Sulawesi Utara (eye hospital) from July 2017 to June 2019. This was a retrospective and descriptive study using data of medical records at Rumah Sakit Mata Provinsi Sulawesi Utara. The results showed that there were 546 eye infection patients, and the most frequent eye infection was conjunctivitis (231 patients ~ 42.31%). Female patients were predominant than males (53.85% vs 46.15%). Based on occupation, housewifery was the most frequent occupation related to eye infections (110 patients ~ 20.15%). Based on age, eye infections mostly occured in the category of age 36-45 years (98 patients ~ 17.95%). Most patients’ complaint was red eye (295 patients ~ 54.03%). Based on location, eye infections mostly occured unilaterally (364 patients ~ 66.67%). Purpose. To evaluate the effect of preoperative intravitreal conbercept (IVC) injection on patients with proliferative diabetic retinopathy (PDR). Methods. Medical records of patients who underwent vitrectomy due to complications of PDR were retrospectively reviewed. Patients were grouped as the IVC group and non-IVC group according to preoperative IVC. Preoperative, intraoperative, and postoperative data of both eyes were collected. The main outcome measures were best-corrected visual acuity (BCVA), intraocular pressure (IOP), central retinal thickness (CRT), and incidence of tractional retinal detachment (TRD). Results. A total of 37 cases were included, 16 in the IVC group and 21 in the non-IVC group. Preoperative IVC significantly reduced surgical duration (IVC vs. non-IVC, 88.9 ± 11.9 min vs. 97.8 ± 12.8 min, p < 0.05). In the vitrectomized eye, no significant difference existed between the IVC group and non-IVC group regarding postoperative BCVA (logMAR, 1.20 ± 0.53 vs. 1.27 ± 0.54, p = 0.68), IOP (16.5 ± 2.9 mmHg vs. 15.6 ± 3.7 mmHg, p = 0.44), and CRT (330.1 ± 35.2 μm vs. 319.2 ± 32.5 μm, p=0.34). In the fellow eye, 6 cases in the IVC group were diagnosed with TRD during postoperative follow-up, while only 2 cases were diagnosed in the non-IVC group (p < 0.05). Conclusion. Preoperative intravitreal injection of conbercept has effectively facilitated vitrectomy in PDR patients, but it potentially promotes tractional retinal detachment in the fellow eye following preoperative injection.

Congenital Heart Defects (CHD) is the most common congenital disease that affects 8 out of every 1000 births. CHD can be classified as cyanotic and acyanotic. Acyanotic is the most frequently diagnosed CHD. The most common complication in acyanotic CHD is pulmonary hypertension. This descriptive study was based on medical records of acyanotic CHD patients with pulmonary hypertension in RSUD Abdul Wahab Sjahranie Samarinda from 2015 to 2016 who were diagnosed by a cardiologist and confirmed with echocardiography. There were 62 patients diagnosed with acyanotic CHD and pulmonary hypertension, which 58% of patients were diagnosed with Atrial Septal Defect (ASD), Ventricular Septal Defect (VSD) (21%), and Patent Ductus Arteriosus (PDA) (21%). Defect sizes distribution were mostly large (66%), then medium (29%), and small (5%). There were 56% of patients diagnosed before 18 years old and 44% diagnosed after they reached 18 years old. There were 68% of female patients and 32% male patients. Pulmonary hypertension severity distribution was mild (52%), moderate (26%), and severe (22%). The most diagnosed acyanotic CHD was ASD. The defect size in acyanotic CHD was mostly large, and mostly found in female patients. The severity of pulmonary hypertension was mostly mild.
