Introduction Endogenous endophthalmitis is a rare but devastating ocular infection, resulting from microbial seeding of the eye during bacteremia or fungemia. Among other sources of septicemia, intravenous drug abuse is a significant risk factor, and endogenous endophthalmitis should always be thought of in any patient with a history of injectable drug use, presenting with a painful vision loss and a red inflamed eye. Case Description We report the case of a 47-year-old male, with a history of injectable drug consumption, who presented to our emergency department complaining of a painful red eye and recent deterioration of visual acuity. After the first clinical hypothesis of endogenous endophthalmitis being considered, the final diagnosis of Human Leukocyte Antigen (HLA) B27-associated acute anterior uveitis was established. Conclusion Ocular inflammation is one of the most challenging fields in all ophthalmology. Several inflammatory disorders, ranging from a wide variety of infectious and non-infectious conditions, can present themselves with the same clinical signs and symptoms, making their diagnosis extremely defiant.

One of the downstream areas of the Komering River with potential for capture fisheries is Sungai Dua Village. One of the types of fish caught by fishermen in Sungai Dua is the three spot gourami (Trichogaster trichopterus). The three spot gourami fish is a type of freshwater consumption fish that has high economic value. But the fishes only rely on catches from nature. If the fishing is carried out continuously, without domestication and cultivation, it will cause a decline in the fish population. The research about the biological aspects of red-eye septic fish (Trichogaster trichopterus) which include food habits, relationship length and weight, condition factors and stomach fullness index. This research was conducted from April to June 2017. Sampling was carried out in the downstream waters of the Komering River in Sungai Dua Village, Banyuasin Regency, South Sumatra. Fish samples were analyzed at the Animal Taxonomy Laboratory, Department of Biology, Faculty of Mathematics and Science, Sriwijaya University. The determination of stations in sampling was carried out using multiple sampling methods, namely purposive sampling and quota sampling. The result of this research showed that (1) the Cyanophyta is the main food for the three spot gourami, Diatomae and Desmidiacae as a complementary food, while the additional foods are Rhizopoda, Chlorophyta, Ostracoda, and Entomostraca, (2) The growth pattern of red-eyed fish is negative allometric with a b value of -2 (b <3), the increase in length is faster than the weight gain, (3) The highest condition factor value is found in the size range 6.01-7.00 cm of 23.25 gr / cm and the lowest condition factor value is in the size range 5.01-6.00 cm of 0.62 gr / cm, and (4) The highest hull fullness index value of the red eye sepat fish hull is in the size range 7.01-8.00 cm at 32.86% and the lowest hull fullness index value at the size range 8.01-9.00 cm is 2.05% .

Background: The Red Reflex is described as the red to orange reflection of light from the fundus of the eye, observed while using a retinoscope or an ophthalmoscope. Red Eye Reflex Test is determined by the optical media transparency which includes vitreous humor, aqueous humor, cornea and tear film and reflection of light from the  back of the eye (fundus) through optical media and into the aperture of the ophthalmoscope. Factors that will block or impede the passage of light through this transparent media or affects its reflection back from fundus will produce an abnormal Red Eye Reflex. Red Eye Reflex (RER) testing is important & effective tool for early detection of ocular abnormalities such as retinal abnormalities, cataract, retinoblastoma and glaucoma. Red reflex is cost effective, can be performed very easily, requires minimal setting, can screen important ocular abnormalities and facilitate their early detection with prompt intervention to prevent long term sequelae associated with the disease. Aim: To find out the diagnostic accuracy of Red Reflex test for diagnosing ocular abnormalities in newborns. Methodology: Ours will be a prospective cross-sectional study where RER examination will be performed within one week of birth of a newborn in a darkened and will correlate the examination findings with respect to ocular findings determined by ophthalmologist to determine its efficacy, sensitivity and specificity in detecting neonatal ocular abnormalities. Expected results: After completion of the study we will be able to determine the accuracy of RRT for ocular abnormality detection in the neonates. We will be able to determine the sensitivity, specificity, PPV & NPV of the red reflex test. If the specificity and the sensitivity is good then we can use RRT as a routine screening method for detection of intraocular abnormalities. Conclusion: To find the Red reflex Test efficacy in screening of the ocular abnormalities in the new born.

The red eye is one of the most common cause encountered in ophthalmic practice but a red eye is not always related to eye diseases, instead, it may be a clinical sign of several systemic diseases which may the alarmin signal of sight-threatening or life-threatening condition. Frequently, GPs, pediatricians, immunologists, and rheumatologists are the first landing of patients with a “red eye.” This paper is addressed to non-ophthalmic specialists who may be faced with patients having a red eye. Inspection of the external eye under standard office lighting or with a bright light can be easily and accurately made by a general practitioner. Three major caveats should alert the GPs to promptly refer the patient to the ophthalmologist: the presence of pain; the loss of the natural corneal transparency and specular reflex; and any patient-described reduction of visual acuity. In most cases, a red eye is due to occasional and mild ocular surface reaction as consequence of exposure of the external eye to irritants and naturally occurring environmental agents. In these cases washing the eye with a tear lubricant may help in relief of symptoms. If this treatment fails within a few days, a consultation with an ophthalmologist is suggested. The role of the general physician is crucial in the decision making to judge the severity of the ocular condition. Managing a red eye often requires the support of other specialists. Our goal is not only to preserve vision but to globally cure the patient health.

Five cases (one male, four females aged between 32-65 years) of acute painful red eye following application of steroid eye drops have been presented. Their presentation followed a similar trend viz: sudden painful eyes, redness, tearing, photophobia, tenderness, and blepharospasm following topical steroid eye use. There was immediate relief within an hour of instillation of atropine 1% eye drop and full recovery after two weeks of presentation following further treatment with atropine 1% eye drop, tablet paracetamol, chloramphenicol eye drop and flurbiprofen eye drop. Steroids are important group of drugs that are employed in the management of many medical conditions. When used properly, they bring good results most of the time. Acute red eye was observed in this case series following topical steroid use on the eyes and the patients need to be educated on the proper use of this group of drugs so as to reduce its adverse effects.
