Use of 3-D Printed Models for Implant Placement - A Case Series

Long-term stability of a dental implant depends on various factors, one among them is proper position within the alveolar bone. Pre-operative thorough examination of the surgical site is preliminary step while planning dental implants. Conventional techniques such as Intra-Oral Periapical Radiograph and Ortho Pantamogram sometimes not sufficient and clinical scenario always surprises the surgeon. Later advent of Cone-Beam Computed Tomography alleviated this problem to some extent but there are some clinical situations such as resorbed ridge where better visualisation is needed. This led to the innovation of rapid prototyping techniques for the fabrication of three-dimensional complex structures. This technology has numerous applications from the development of an instrument, 3-D printed anatomical models to the fabrication of 3-D printed tissues for regeneration procedures. A 3-D printed model is much more useful than plaster models in many aspects such as demonstrating anatomical structures, better treatment planning, and better understanding for the patient. Hence, this case series focuses on the use of 3-D printed models in the placement of dental implants.

Preservation of tooth structure has always been the prime aim of dental practice. Teeth with advanced periodontal disease in multirooted teeth often pose a significant challenge to the clinician, complicating the treatment plan and compromising long term prognosis. The outcome of the treatment is determined by proper case selection and surgical, endodontic and prosthetic parameters. Resective and regenerative osseous procedures are often employed to help preserve the tooth structure, alveolar bone involving the retained root of multirooted teeth and are cost effective over the other treatment options like ridge preservation/augmentation followed by implant placement. This case series describes application of resective and regenerative techniques for molars with extensive combined periodontal-endodontic disease.

Subject. Since the mid-20th century, there has been an increase of interest in implantation for the treatment of partial and complete loss of teeth. P. I. Branemark was one of the first pioneers who developed an implant that forms osseointegration. Anatomical restrictions and restorative requirements prompt the implantologist to achieve the accuracy of planning and surgical positioning of dental implants. The ideal placement of the implant facilitates the creation of favorable forces on implants and prosthetic structures, and also provides an aesthetic result. Therefore, it is advisable to establish a logical continuity between the planned restoration and the surgical stages, it is important to use the transmitting device, which undoubtedly increases the effectiveness of the implantation. In 1987, M. J. Edge et al. recommended the use of surgical guides before the implantation operation. Currently, surgical templates have become increasingly used. The article presents data on the features of planning of implant placement and the technology of applying a surgical template for positioning the dental implant unit. Goal. To show the value of using surgical templates for the rational installation of dental implants and preventing possible errors and complications. Methodology. A search for literature using search engines such as Google, Yahoo and scientific bibliographic databases such as PubMed, Medline and textbooks was conducted until September 2017 using medical thematic headings like «Dental Implants», «Surgical Templates». More than 300 sources of literature were identified, after studying which they reduced to 28 sources of articles and literary reviews. Conclusions. The study of available to us literature, revealed the advantages and disadvantages of using surgical templates. The main advantages: precise placement of implants, preservation of anatomical structures; three-dimensional technology allows to accurately estimate anatomical points, such as the size of the maxillary sinus in the upper jaw and the location of the alveolar nerve in the lower jaw, high observable accuracy of 0.1 mm, reduction of the time of surgical intervention. Disadvantages: lack of visibility and tactile control during the surgical procedure, insufficient opening of the mouth jeopardizes the result of complex treatment, there is a risk of damage to vital anatomical structures.

Purpose: Recurrent contracted sockets are complex situations where previous surgeries have failed, disabling the wear of an ocular prosthesis. A combined method of surgery and long-term fixation using custom-made, three-dimensional (3D) printed conformers is evaluated. Methods: Retrospective case series of nine patients with recurrent excessive socket contraction and inability to wear a prosthesis, caused by chemical burns ( n = 3), fireworks ( n = 3), trauma ( n = 2) and enucleation and radiotherapy at childhood due to optic nerve glioma ( n = 1) with three average previous socket surgeries (range 2–6). Treatment consisted of a buccal mucosal graft and personalized 3D-printed conformer designed to be fixated to the periosteum and tarsal plates for minimal 2 months. Primary outcome was the retention of an ocular prosthesis. Secondary outcome was the need for additional surgeries. Results: Outcomes were measured at final follow-up between 7 and 36 months postoperatively (mean 20 months). Eight cases were able to wear an ocular prosthesis after 2 months. Three cases initially treated for only the upper or only the lower fornix needed subsequent surgery for the opposite fornix for functional reasons. Two cases had later surgery for cosmetic improvement of upper eyelid position. Despite pre-existing lid abnormalities (scar, entropion, lash deficiency), cosmetic outcome was judged highly acceptable in six cases because of symmetric contour and volume, and reasonably acceptable in the remaining two. Conclusions: Buccal mucosal transplant fixated with a personalized 3D-designed conformer enables retention of a well-fitted ocular prosthesis in previously failed socket surgeries. Initial treatment of both upper and lower fornices is recommended to avoid subsequent surgeries for functional reasons.

Abstract Background and purpose The aim of this clinical study was to investigate the clinical long-term and patient-reported outcome of dental implants in patients with oral cancer. In addition, analysis of the influence of radiation therapy, timing of implant insertion, and augmentation procedures on implant survival was performed. Material and methods This retrospective study investigated the clinical outcome of 711 dental implants in 164 oral cancer patients, inserted by experienced surgeons of the Department of Oral and Maxillofacial Surgery, University Medical Center Mainz, Germany. Oral health-related quality of life (OHRQoL) was evaluated. Results Cumulative 5-year and 10-year implant survival rates for all included implants were 87.3% and 80.0%. Implants placed straight after ablative surgery (primary implant placement) and implants placed after completing the oncologic treatment (secondary implant placement) showed a comparable implant survival (92.5% vs. 89.5%; p = 0.635). Irradiation therapy had no significant influence on implant survival of secondary placed implants (p = 0.929). However, regarding implant site (native bone vs. augmented bone) and radiation therapy (non-irradiated bone vs. irradiated bone), implants inserted in irradiated bone that received augmentation procedures showed a statistically significant lower implant survival (p < 0.001). Patients reported a distinct improvement in OHRQoL. Conclusions Promising long-term survival rates of dental implants in patients after treatment of oral cancer were seen. In addition, patients benefit in form of an improved OHRQoL. However, bone augmentation procedures in irradiated bone may result in an impaired implants’ prognosis.

Abstract Cellular membranes provide ideal archetypes for molecule or ion separations with sub-angstrom scale precision, which are featured with both extremely high permeability and selectivity due to the well-defined membrane protein channels. However, the development of bioinspired membranes with artificial channels for sub-angstrom scale ethylene/ethane (0.416 nm / 0.443 nm) separation remains an uncharted territory and a significant challenge. Herein, a bioinspired nano-ordered liquid membrane is constructed by a facile ion/molecule self-assembly strategy for highly efficient ethylene/ethane separation, which mimics the structure of cellular membrane elegantly and possesses plenty of three-dimensional (3D) nanochannels. The elaborate regulation of non-covalent interactions by optimizing the ion/molecule compositions within membrane confers the nano-ordered liquid structure with interpenetrating and bi-continuous apolar domains and polar domains, which results in the formation of regular carrier wires and enormous 3D interconnected ethylene transport nanochannels. By virtue of these 3D nanochannels, the bioinspired nano-ordered liquid membrane manifests simultaneously super-high selectivity, excellent permeance and long-term stability, which exceeds previously reported ethylene/ethane separation membranes. This methodology in this work for construction of bioinspired membrane with tunable 3D nanochannels through ion/molecule self-assembly will enlighten the design and development of high-performance separation membranes for angstrom/sub-angstrom scale ion or molecule separations.

It is important to obtain sufficient bone mass before implant placement on alveolar bone, and synthetic bone such as biphasic calcium phosphate (BCP) has been studied to secure this. This study used a BCP block bone with a specific structure of the three-dimensional (3D) hexahedron channel and coating with recombinant human bone morphogenetic protein-2 (rhBMP-2) impregnated carboxymethyl cellulose (CMC) was used to examine the enhancement of bone regeneration of this biomaterial in rat calvarial defect. After the preparation of critical-size calvarial defects in fifteen rats, defects were divided into three groups and were implanted with the assigned specimen (n = 5): Boneplant (untreated 3D hexahedron channeled BCP block), Boneplant/CMC (3D hexahedron channeled BCP block coated with CMC), and Boneplant/CMC/BMP (3D hexahedron channeled BCP block coated with CMC containing rhBMP-2). After 4 weeks, the volumetric, histologic, and histometric analyses were conducted to measure the newly formed bone. Histologically, defects in the Boneplant/CMC/BMP group were almost completely filled with new bone compared to the Boneplant and Boneplant/CMC groups. The new bone volume (P < 0.05) and area (P < 0.001) in the Boneplant/CMC/BMP group (20.12% ± 2.17, 33.79% ± 3.66) were much greater than those in the Boneplant (10.77% ± 4.8, 16.48% ± 9.11) and Boneplant/CMC (10.72% ± 3.29, 16.57% ± 8.94) groups, respectively. In conclusion, the 3D hexahedron channeled BCP block adapted rhBMP-2 with carrier CMC showed high possibility as an effective bone graft material.
