Abstract Background Polycystic ovary syndrome (PCOS), as the most common endocrine disorder in reproductive-aged women, is characterized by oxidative stress and ovarian tissue inflammation. Green tea extract (GTE) potentially possesses therapeutic effects for PCOS because of the antioxidant and anti-inflammatory compounds. This systematic review evaluates the potential roles of GTE on metabolic variables, hormone levels, and ovarian function in PCOS. Methods A systematic review was conducted of published studies reporting the effects of GTE on PCOS. Several major databases, including PubMed, SCOPUS, and Google Scholar, were searched up from inception to April 2021. Clinical trials and animal studies that assessed the effects of GTE on PCOS were eligible for inclusion. Results Of 314 articles found in the search, four human studies and four animal studies were included. All studies in humans showed the effects of GTE on weight loss. GTE’s effect on decreasing testosterone levels in humans and LH levels in animals were also reported. In addition, increases in FSH and progesterone levels in animal models were observed. Although GTE improved fasting blood sugar and insulin levels, the effect of GTE on inflammatory parameters, such as TNF-alpha and IL-6 and antioxidant status, was limited to animal studies. Conclusion Therefore, this review suggests that GTE could be considered a potential agent to attenuate PCOS complications mainly due to its effect on weight loss and glycemic levels. However, more studies are needed to formulate conclusions about the effects and mechanisms of GTE in PCOS.

Background: Emerging evidence suggests that gut microbiota plays a vital role in the occurrence of multiple endocrine disorders including polycystic ovary syndrome (PCOS). Shaoyao-Gancao Decoction (SGD), a classical Chinese prescription, has been widely used in the treatment of PCOS for decades. In previous studies, we found that SGD treatment could effectively reduce ovarian inflammation in PCOS rats. However, whether the anti-inflammation effect of SGD involves the regulation of the gut microbiota remains elusive.Methods: Letrozole-induced PCOS rat models were established, and the therapeutic effects of SGD were evaluated. Specifically, body weight, serum hormone concentrations, estrus phase and ovary histopathology were assessed. Then the structure of gut microbiota was determined by 16s rRNA sequencing. Additionally, the serum levels of pro-inflammatory cytokines and LPS were measured by ELISA kits. The key gene and protein expressions of TLR4/NF-κB signaling pathway were detected by quantitative real-time PCR and western blot.Results: SGD could effectively reduce body weight, regulate estrous cycles and ameliorate hyperandrogenism in PCOS rats. In addition, SGD treatment decreased releases of pro-inflammatory cytokines, enhanced the expressions of tight junction (occludin and claudin1), and then prevented a translocation of LPS into bloodstream. SGD could significantly reduce the ratio of Firmicutes to Bacteroidetes, decrease the abundance of LPS-producing pathogens Proteobateria and enrich the abundance of Butyricicoccus, Coprococcus, Akkermansia Blautia and Bacteroides in PCOS rats. Furthermore, SGD blunted the key gene and protein expressions of TLR4/NF-κB signaling pathway both in vivo and in LPS-induced RAW264.7 cells.Conclusion: SGD administration could ameliorate the inflammatory response in PCOS rats by remodeling gut microbiome structure, protecting gut barrier, and suppressing TLR4/NF-κB signaling pathway.

Abstract Objective To compare the therapeutic effects of compound Xuanju capsules combined with hormone therapy versus hormone therapy alone on polycystic ovary syndrome (PCOS)-related infertility using a meta-analysis. Methods Electronic databases including PubMed, The Cochrane Library, Web of science, Chinese Biomedical Literature Database (CBM), China National Knowledge Infrastructure (CNKI), Wanfang Data, and VIP database were manually searched. The quality of included studies was evaluated based on the Cochrane systematic review standards, and the valid data were extracted for meta-analysis using Revman 5.3 software. Results A total of 14 randomized controlled trials accounting for 1249 patients were included. Meta-analysis showed that patients in the compound Xuanju capsule + hormone therapy group had higher estradiol levels and overall rates of effective treatment than those in the hormone therapy alone group. Moreover, they also exhibited lower levels of luteinizing hormone and follicle-stimulating hormone as well as lower Kupperman scores than the hormone therapy alone group. Conclusions The combination of compound Xuanju capsules and hormone therapy is more effective than hormone therapy alone in the treatment of PCOS-related infertility. However, the quality of current studies is low, and high-quality clinical trials are warranted.

Low glucagon like peptide1 (GLP-1) level may contribute to the metabolic dysfunction in polycystic ovary syndrome (PCOS).  In this study, prospective therapeutic effects of incretin-based drugs; linagliptin versus liraglutide were investigated on letrozole induced PCOS rats. Animals were divided into five groups (control, PCOS, linagliptin, liraglutide and combined). Letrozole was administered for seven weeks (1mg/kg/day,orally). Linagliptin (3mg/kg/day,orally), liraglutide (1.2mg/kg/day,sc) and combined drugs were given for 4 weeks. Measurements of anthropometric, hemodynamics, blood glucose indices, HOMA-IR, serum lipids, TNF-α, NF-kB, and sex hormones were estimated. Antioxidant activities alongside immunohistochemical (PCNA) studies were assessed. The present results proved that both drugs significantly ameliorated most of anthropometric, glycemic, lipid, hormonal, inflammatory and antioxidant indices. Obvious histological improvement was obtained by linagliptin and combined therapy while being questionable by liraglutide . In conclusion, linagliptin caused evident ovarian histological advance rather than liraglutide. Linagliptin may represent a promise in alleviating metabolic, hormonal and unique beneficial histologic effects of PCOS.
