Pharmaceutical care models and practices differ in various countries. Reimbursement for cognitive services, for example, varies across countries in Europe, Asia, and the Americas. Practice-based research has blossomed in many countries, with different emphases and challenges. This international series will describe the organization of community pharmacy within the healthcare system and report the status of practice-based research. Each paper will focus on one country. The series will conclude with a summary by the series editors describing the key themes across the papers, outlining milestones yet to be achieved, and proposing a research agenda for community pharmacy practice.

Introduction: The scope of pharmacy practice has evolved over the last few decades to focus on the optimization of medication therapy. Despite this positive impact, the lack of reimbursement remains a significant barrier to the implementation of innovative pharmacist practice models. Summary: We describe the successful development, implementation and outcomes of three types of pharmacist collaborative care models: (1) a pharmacist with physician oversight, (2) pharmacist–interprofessional teams and (3) physician–pharmacist teams. The outcome measurement of these pharmacist care models varied from the design phase to patient volume measurement and to comprehensive quality dashboards. All of these practice models have been successfully funded by affiliated health systems or grants. Conclusions: The expansion of pharmacist services delivered by clinical faculty has several benefits to affiliated health systems: (1) significant improvements in patient care quality, (2) access to experts in specialty areas, and (3) the dissemination of outcomes with national and international recognition, increasing the visibility of the health system.

Zusammenfassung. Die chronisch obstruktive Lungenerkrankung (COPD) ist vor allem unter Rauchern eine weit verbreitete Krankheit in der Schweiz. Der Verlauf ist oft progredient und die chronisch kranken und oft polymorbiden Patienten sind auf funktionierende Behandlungsnetzwerke angewiesen. Gesundheitspolitisch und auch medizinisch ist mehr Netzwerkdenken gefragt und notwendig und die Erarbeitung von eines chronic care models (CCM) äusserst sinnvoll. Die Basis dafür ist das evidenz-basierte Vorgehen. In diesem Artikel werden die Guidelines zur Diagnostik und Behandlung der COPD und deren Implementierung beleuchtet. Vorwegzunehmen ist, dass bereits ein breites Behandlungsangebot besteht und viele innovative Projekte lanciert wurden, um die Behandlungsqualität der COPD zu optimieren.

The general administration of pharmaceutical care started potential pharmacy practice program. The program is part of accreditation professional’s process of national and international regulations. The adult’s parenteral nutrition was one of the critical programs. The most healthcare professionals are not familiar with the new system. The new initiatives system adult’s standardized concentration formulation of total parental nutrition as complementary to the previous one. The new formulation consisted of all parental nutrition requirements based on national and international standards. The new system can be converted as computerized physician orders. The new initiatives may implement as project management model over one year or less than that’s. The new system prevents nutrition-related problems, and medication errors, and improve clinical outcomes of the adults’ population in the Kingdom of Saudi Arabia.
