Differential attainment in career progression in the NHS is a complex issue with many interplaying factors apart from individual protected characteristics. In this paper, we examine the attainment gap, causes for these disparities and some recommendations to reduce the gap. Our review shows that there is significant DA between groups of doctors on the basis of gender, ethnicity, race and country of primary medical qualification. The likely causes are bias, lack of opportunity, poor supervision, mentorship, sponsorship, dichotomous treatment of doctors based on training or non-training status and cultural exclusion. Data is not monitored or reported and there is little organisational accountability. Solutions are likely to include transparent data on recruitment as well as progression for benchmarking, training support for all doctors, initiatives which are sensitive to gender, parental responsibility, cultural heritage, language and robust supervision including mentorship and sponsorship. This scoping review forms part of the Alliance for Equality in Healthcare Professions project on Differential Attainment chaired by the British Association of Physicians of Indian Origin (BAPIO) and will be integrated into the Bridging the Gap project undertaken by BAPIO Institute for Health Research (BIHR). This work is part of six domains of doctors' careers in the NHS.

Aim – to review the evidence on differential attainment in leadership positions in the NHS and develop an expert consensus In this review we will explore in-depth, the current data surrounding differential attainment in leadership roles in the UK NHS, possible reasons for these disparities, and what interventions may address this inequality. This scoping review forms part of the Alliance for Equality in Healthcare Professions project on DA chaired by the British Association of Physicians of Indian Origin (BAPIO) and will be integrated into the Bridging the Gap project undertaken by BAPIO Institute for Health Research (BIHR).

The COVID-19 pandemic has exposed the devastating impact of inequalities that have plagued societies for generations. The timing of the events spiraling from the unlawful killing of an apparently innocent black man in the United States of America led to an uprising of sorts across many countries. It touched the psyche of the people in the UK and came at a time when the British Association of Physicians of Indian origin reaches its 25th anniversary (1996-2021). One of the fundamental visions and values that the organisation was created on, was to promote excellence through equality and diversity. Therefore in its silver jubilee year, BAPIO launches an over-arching Alliance for Equality in Healthcare Professions. The Alliance is tasked to conduct a review of the evidence of differential attainment across the spectrum of healthcare careers, engage with stakeholders from the grassroots to the responsible organisations and finally generate an expert consensus on recommendations for the changes necessary to tackle such inequalities.

Abstract Objectives Mindfulness-based cognitive therapy (MBCT) is a well-evidenced relapse-prevention intervention for depression with a growing evidence-base for use in other clinical populations. The UK initiatives have outlined plans for increasing access to MBCT in clinical settings, although evidence suggests that access remains limited. Given the increased popularity and access to MBCT, there may be deviations from the evidence-base and potential risks of harm. We aimed to understand what clinicians believe should be best clinical practice regarding access to, delivery of, and adaptations to MBCT. Methods We employed a two-stage Delphi methodology. First, to develop statements around best practices, we consulted five mindfulness-based experts and reviewed the literature. Second, a total of 59 statements were taken forward into three survey rating rounds. Results Twenty-nine clinicians completed round one, with 25 subsequently completing both rounds two and three. Forty-four statements reached consensus; 15 statements did not. Clinicians agreed with statements regarding sufficient preparation for accessing MBCT, adherence to the evidence-base and good practice guidelines, consideration of risks, sufficient access to training, support, and resources within services, and carefully considered adaptations. The consensus was not reached on statements which reflected a lack of evidence-base for specific clinical populations or the complex decision-making processes involved in delivering and making adaptations to MBCT. Conclusions Our findings highlight the delicate balance of maintaining a client-centred and transparent approach whilst adhering to the evidence-base in clinical decisions around access to, delivery of, and adaptations in MBCT and have important wide-reaching implications.

This article draws on the 2018 Writers Guild of Great Britain report ‘Gender Inequality and Screenwriters’, and original interviews with female screenwriters, to assess how the experience of genre plays out in the UK television industry. The report focuses on the experience of women, as a single category, but we aim to reveal a more intersectional understanding of their experiences. Our aim is to better understand the ways in which women are, according to the report, consistently ‘pigeonholed by genre and are unable to move from continuing drama or children’s programming to prime-time drama, comedy or light-entertainment’. Considering the cultural value of genre in relation to screenwriting labour and career progression, we analyse how genre shapes career trajectory, arguing that social mobility for female screenwriters is inherently different and unequal to that of their male counterparts.

Abstract This article uses the “communicative repertoire” conceptual framework to investigate the evolving linguistic practices in the Sri Lankan Tamil (SLT) diaspora, looking specifically at how changing mobility patterns have had an influence on heritage language use. Drawing on fieldwork undertaken with 42 participants of diverse migration trajectories in London, the study finds that onward migration has important implications for Tamil language maintenance and use in the UK, and for the introduction of European languages into the community. It argues that Tamil practices can only be fully understood if we consider them within the context of participants' communicative repertoires. Further, the definition of Tamil needs to be expanded to include different varieties, registers and styles that have been shaped by onward migration. As the trend of multiple migrations is becoming increasingly common in globalization processes, studying the recent change in SLT migratory patterns is also crucial to gaining insight into the diversities and transnational links that exist within and across diaspora communities respectively.

The British Horseracing Industry has been experiencing a labour shortage since the 1970’s and despite recent improvements reported in staff retainment, the industry still reports high staff turnover. We investigated staffing challenges experienced by UK horseracing employers and employees, to formulate strategic targets to address any issues identified. Thirty UK trainers and staff (12 staff, 9 senior staff, 9 trainers) participated in semi-structured inductive focus groups lasting on average 39±9 min. Questions explored staff background in racing, perceptions of the current staff challenges, retention and recommendations for improvement. Thematic analysis revealed four higher order themes; (1) lack of skills, (2) differences in perceived work ethic, (3) job conditions, and (4) media portrayal. This study concluded that there are dedicated staff working in the industry, but significant skills gaps, high workloads, poor work-life balance and a lack of training and career progression negatively influence retention in the horseracing industry.

The chapter examines the relationship between orthography, phonology, and morphology in Turkish and what this means for Turkish-English bilingual language processing. Turkish offers a unique language medium in pitching theoretical perspectives both in linguistics and psycholinguistics against each other because of its properties. Empirical and theoretical considerations are employed from both domains in order to shed light on some of the current challenges. In line with contemporary thought, this chapter is written with the view that bilingual speakers engage a singular language or lexical system characterized by fluid and dynamic processes. Particular focus will be given to English-Turkish speaking bilinguals in the UK, which includes heritage (HL) and non-heritage language speakers. Evidence from monolingual developmental research as well as neuropsychology will be examined to confirm findings of previous studies in other European contexts, and also to raise attention to various challenges which need to be addressed across all contexts.

ObjectivesLocal reviews of the care of women who die in pregnancy and post-birth should be undertaken. We investigated the quantity and quality of hospital reviews.DesignAnonymised case notes review.ParticipantsAll 233 women in the UK and Ireland who died during or up to 6 weeks after pregnancy from any cause related to or aggravated by pregnancy or its management in 2012–2014.Main outcome measuresThe number of local reviews undertaken. Quality was assessed by the composition of the review panel, whether root causes were systematically assessed and actions detailed.ResultsThe care of 177/233 (76%) women who died was reviewed locally. The care of women who died in early pregnancy and after 28 days post-birth was less likely to be reviewed as was the care of women who died outside maternity services and who died from mental health-related causes. 140 local reviews were available for assessment. Multidisciplinary review was undertaken for 65% (91/140). External involvement in review occurred in 12% (17/140) and of the family in 14% (19/140). The root causes of deaths were systematically assessed according to national guidance in 13% (18/140). In 88% (123/140) actions were recommended to improve future care, with a timeline and person responsible identified in 55% (77/140). Audit to monitor implementation of changes was recommended in 14% (19/140).ConclusionsThis systematic assessment of local reviews of care demonstrated that not all hospitals undertake a review of care of women who die during or after pregnancy and in the majority quality is lacking. The care of these women should be reviewed using a standardised robust process including root cause analysis to maximise learning and undertaken by an appropriate multidisciplinary team who are given training, support and adequate time.
