How can Telemedicine Benefit from Broadband Technologies?

The present chapter focuses on some aspects of the state of the art of telemedicine systems and their use over broadband. It starts with a brief summary of the most popular telecommunication technologies to give the reader an overview of today’s broadband technology and methods. Some important deployment data are included showing the global growth and use in many countries. Results of very significant pilot projects using videocommunication implemented in Italy and in Europe are taken into consideration, demonstrating the benefits of the patient’s psychological status in conjunction with health care assistance. The chapter proposition is to show a business model, based on an Italian reality exploiting the Marche region population development and healthcare statistics. The simulation example is the adaptation of telemedicine solution for early hospital discharge applied to a public healthcare structure, typically a hospital. The model takes into account the possible adaptation of an “early discharge solution” implemented with portable telemonitoring light videocommunication terminals, which, thanks to broadband availability, may be installed at the patient’s home for a predefined period (typically 7 to 10 days). The simulation aims to highlight and dimension cost reduction or, in a more appropriate view, give the percentage of resource that can be relocated to provide a better service and how a national healthcare service may take advantage of these scenarios.

Aims: Both survey and healthcare register data struggle as data sources to capture the phenomenon of alcohol problems. We study a large group of people for whom survey data and two types of register data are available, and examine the overlaps of similar or related measures in the different data sources to learn about potential weaknesses in each. We also examine how register-based data on the prevalence of alcohol problems change depending on which register data are used. Design: We use data from the Regional Health and Wellbeing Study (ATH) of the adult Finnish population collected in 2013 and 2014 ( n = 69,441), individually linked with data on two national healthcare registers (Care Register for Health Care; Register of Primary Health Care visits) for the survey year and previous year. Results: The prevalence of substance-abuse-related healthcare was almost two-fold if data on outpatient primary care visits were included in addition to hospitalisations. Forty-six per cent of the survey respondents self-reporting substance-abuse-related healthcare service use were identified in the registers, and 22% of all respondents with such service use according to registers reported this in the survey. Records of substance-abuse-related healthcare service use, controlled for self-reported alcohol use and self-reported substance-abuse-related service use, were found more often for men, the middle-aged, people with basic education only, and the non-employed. Conclusions: The results are suggestive of underreporting in both data sources. There is an evident need to develop recording practices in the healthcare registers regarding substance use disorders.

For many decades, the Egyptian people have suffered from deplorable public healthcare service featured by indolence, malfunction and traditionalism. Although, healthcare is a distinctive service industry concerning various complicated responsibilities; but the consecutive government of Egypt had badly handled such issue. Then, the apathetic performance of the public healthcare service becomes a dilemma for the people of Egypt; especially the poor among them. Therefore, the foundation of an adequate public healthcare service system, that respect the dignity of the people and respond to their arising health care needs; was frequently on the agenda priorities of the Egyptian governments after the 2011 and 2013 uprisings. Hence recently, the government -after reaching political and economic stability- seeks to build an ambitious newly public health care system to meet the expectation of the people to acquire high standard inexpensive and hasty public healthcare services. Consequently, in order to realize such aim; the Egyptian government had established the public agency for accreditation and quality control according to law no.2 of comprehensive healthcare insurance system issued at 2018. Then, it urges a national campaign to reform the public healthcare sector and to develop the efficacy and quality of its services. Hence, this paper aims to propose how the public healthcare organizations in Egypt can tackle various challenges and enhance adequately its capabilities; in order to be able to adopt the proposed Lean Six Sigma (LSS) methodology effectively; which can then provide an adequate framework for creating organized improvement exertions in healthcare; necessary to bestow guidelines on how to manage a quality service system to patient satisfaction by decreasing waste, variation and work disparity in the service processes.

Background : Healthcare quality should be improved to give patients satisfaction, that will lead them to use the service again and recommend the health center to the other people. The quality also affects the patient revisit rate. Good service and quality have to fulfill five dimensions of service quality, namely responsiveness, assurance, tangible, empathy, and reliability. This research is conducted to acknowledge the relation between health care service quality and out-patient revisit rate in Mijen Community Health Center, Semarang.Methods : This research is analytical quantitative research. The method used in this research is cross-sectional design. The sample was selected using purposive sampling technique, with a total sample of 74 respondents and using  informed consent for the filled in by respondents. The data was taken by using a questionnaire with the rank spearman trial analysis method.Result : 58.1 respondents state that the healthcare service quality is good. 77.0 respondents stated that they are willing to revisit Mijen Community Health Center, Semarang. The result of rank spearman statistic trial shows that there is a correlation between health care service quality and out-patient revisit rate in Mijen Community Health Center, Semarang (p=0,000) which is lower than ά = 0,05.Conclusion : The better healthcare service quality given to the patient, the higher patient revisit rate in Mijen Community Health Center, Semarang.

ABSTRACTObjective: to ascertain the perception regarding humanizing healthcare among the National Healthcare Service (NHS) patients in the area of Évora - Portugal. Method: this is a descriptive study, exploratory, with a quantitative approach using a random sampling starting from the guiding question: What is the NHS patients’ perception on humanizing the care provided by the healthcare services? Data were collected with an informed consent questionnaire, administered between September and October 2010 to 62 individuals who were patients in the NHS, according to the approval of the Ethic Committee on Research Involving Human Beings, with the 203/2008 legal opinion. Results: concerning humanizing health care, the respondents of this study outlined as particularly important aspects the following: being attended by gracious professionals who facilitate the dialogue and provide quality care in a short period of time. The value placed on being able to select one’s doctor and nurse is explained by aspects that patients find important such as availability, confidentiality, competence as well as the continuity of care and trust for the health professionals. Conclusion: the results highlight the following aspects: communication; attitudes and competence of the health professionals; accessibility and exercise of autonomy. A colheita de dados foi realizada através de um questionário com consentimento livre e esclarecido, aplicado entre setembro e outubro de 2010 a 62 sujeitos usuários do SNS, conforme aprovação do Comité de Ética em Pesquisa Envolvendo Seres  Humanos. Resultados: acerca da humanização dos cuidados de saúde os informantes deste estudo evidenciaram como aspectos particularmente importantes, um atendimento por profissionais simpáticos, que facilitem o diálogo e prestem cuidados de qualidade num curto espaço de tempo. A importância da escolha do médico e do enfermeiro de familia, pelos usuários do SNS, é justificada pela disponibilidade, a confidencialidade, a competência, assim como a continuidade de cuidados e a confiança nos profissionais de saúde. Conclusão: os resultados remetem-nos para um acentuado destaque dos seguintes aspectos: a comunicação; atitudes e competência dos profisionais de saúde;  acessibilidade e exercício da autonomia. La cosecha de datos fue hecha utilizando un cuestionario con consentimiento libre y clarificado, aplicado entre septiembre y octubre de 2010 a 62 usuarios del SNS, con aprobación del Comité de Ética en la Investigación en Seres Humanos, con el dictamen legal 203/2008. Resultados: referente a la humanizácion de los cuidados de salud, los informadores de este estudio habían evidenciado particularmente como aspectos importantes, la atención de profesionales agradables que facilitan el diálogo y dan cuidados de calidad en poco tiempo. La seleccion del doctor y de la enfermera es justificada por la importancia que asume para los usuarios la disponibilidad, la confidencialidad, la competencia, así como la continuidad de cuidados y la confianza en los profesionales de salud. Conclusión: los resultados nos apuntan para la importancia de los aspectos siguientes: la comunicación; actitudes y capacidad de los profisionales de la salud;  accesibilidad y ejercicio de la autonomía. Background: South Africa is one of the countries in Africa adversely affected by rabies, a notifiable disease which can be fatal. Fatalities can be prevented if health care is sought timeously and people are educated about the disease. The Province of Kwa- Zulu-Natal, in particular, has had rabies outbreaks in the past which have led to loss of many lives and devastation of entire families. Objective: The aim of the study was to explore the experiences of people affected by rabies in the eThekwini district of Kwa- Zulu-Natal, South Africa. Methods: The study was guided by a qualitative, exploratory, descriptive design. The sample was purposively selected, and a semi-structured interview was used to collect data from people affected by rabies in the eThekwini district. Data saturation was reached after 12 participants were interviewed. Data was analysed by using Tesch’s eight steps of thematic analysis. Results: The themes included family stability and support structures, exposure to risk factors and risky practices, factors that hindered participants from seeking health care assistance, limited knowledge about rabies and the effects of rabies. Conclusion: It was evident that participants experienced many challenges during their rabies exposure. Individuals, who were directly affected by rabies through contact with rabid animals, were expected to take responsibility for their own lives.
