Human-Centered Design for Health Information Technology

This chapter presents issues that may arise in human-centered research in health care environments. The authors first discuss why human-centered approach is increasingly employed to study and to design health care technology. They then present some practical concerns that may arise when conducting qualitative research in medical settings, from research design, to data collection and data analysis, and to technology design. Many of these concerns were also experienced in their own human-centered field studies conducted in the last few years. The authors conclude the chapter by illustrating some of these issues using their own research case study that investigated nurses’ information flow in a hospital ward.

Nowadays, the development of information technology has given significant impact on various activities. Easy facilities provided by information technology enable the society, especially costumers, to make transactions at their workplace or home. Such demands make producers and companies feel the need to apply e-commerce their business. This research aims to give information about the expertise, independence, and professional skill of the internal auditors in preventing fraud on e-commerce transactions. This research applies Miles and Huberman model with three activities in the data analysis, such as: data reduction, data model, and conclusion withdrawal. This research uses qualitative research with a case study at PT KeretaApi Indonesia (Persero). The result obtained from this research shows that the audit process of e-commerce at PT KeretaApi Indonesia (Persero) is entirely conducted by the Internal Auditors in IT Unit, because IT Unit has its own Team of Auditors outside the SPI Unit (Internal Auditor Unit) of PT KeretaApi Indonesia (Persero).

 Cloud computing is the buzz word in today’s Information Technology. It can be used in various fields like banking, health care and education. Some of its major advantages that is pay-per-use and scaling, can be profitably implemented in development of Enterprise Resource Planning or ERP. There are various challenges in implementing an ERP on the cloud. In this paper, we discuss some of them like ERP software architecture by considering a case study of a manufacturing company.

As qualitative research projects are conceptualized and conducted, they can grow out of alignment as researchers make choices as to their Area of Curiosity, Mission Question, Data Collected, and Data Analysis. This phenomenon of being muddled is a natural, and sometimes necessary, part of the overall process. The important part of being in a muddle is to recognize it and to work to tidy it up. In this paper, a way to keep qualitative research projects plumb is presented. A case study is also shared to show how one project was found to be out of alignment and how it was realigned using the Qualitative Research Plumb Line.

In the highly competitive nowadays markets, many companies actions assume the project form. In special, Information Technology (IT) projects assume great importance, enabling the dynamic actions that organisations need (Porter, 2001; Tapscott, 2001). However, IT applications assume different roles, from operational support to strategic, according to companies’ strategies and operations, besides the peculiarities of the industry in which they compete (McFarlan, 1984; Porter & Millar, 1985). According to this role (appraised by McFarlan’s Strategic Grid), ex-ante evaluation practices for selecting IT projects to be implemented can vary (Jiang & Klein, 1999). The objective of this paper is to analyse practices for selecting IT projects in Brazilian companies classified in different quadrants of the Strategic Grid and to observe any differences in ex-ante evaluation practices among them. The adopted methodological approach was qualitative research, more specifically case study (Claver, Gonzalez & Llopis, 2000; Yin, 1991) performed in four companies.

In the era of health care reforms, the executives and CEO of the SBC Company are faced with many questions. They wonder about the impact of these changes on their market share. They also wanted to determine the impact on prices they can charge for their services. The changes in the ways the health care will be bought were causing a rapid transformation in the behavior of the consumers. The executives determined that they need to focus on specific areas to keep abreast of all the changes. These will have a profound impact on the information technology implementation across the company. For example, the need for analyzing a large amount of data and data in real time was becoming acute among many departments; there was a need for different skillsets in the employees in almost all the departments across the company. These changes across the industry were presenting new and unique challenges to the executive team.

Background Despite the increasing use of remote measurement technologies (RMT) such as wearables or biosensors in health care programs, challenges associated with selecting and implementing these technologies persist. Many health care programs that use RMT rely on commercially available, “off-the-shelf” devices to collect patient data. However, validation of these devices is sparse, the technology landscape is constantly changing, relative benefits between device options are often unclear, and research on patient and health care provider preferences is often lacking. Objective To address these common challenges, we propose a novel device selection framework extrapolated from human-centered design principles, which are commonly used in de novo digital health product design. We then present a case study in which we used the framework to identify, test, select, and implement off-the-shelf devices for the Remote Assessment of Disease and Relapse-Central Nervous System (RADAR-CNS) consortium, a research program using RMT to study central nervous system disease progression. Methods The RADAR-CNS device selection framework describes a human-centered approach to device selection for mobile health programs. The framework guides study designers through stakeholder engagement, technology landscaping, rapid proof of concept testing, and creative problem solving to develop device selection criteria and a robust implementation strategy. It also describes a method for considering compromises when tensions between stakeholder needs occur. Results The framework successfully guided device selection for the RADAR-CNS study on relapse in multiple sclerosis. In the initial stage, we engaged a multidisciplinary team of patients, health care professionals, researchers, and technologists to identify our primary device-related goals. We desired regular home-based measurements of gait, balance, fatigue, heart rate, and sleep over the course of the study. However, devices and measurement methods had to be user friendly, secure, and able to produce high quality data. In the second stage, we iteratively refined our strategy and selected devices based on technological and regulatory constraints, user feedback, and research goals. At several points, we used this method to devise compromises that addressed conflicting stakeholder needs. We then implemented a feedback mechanism into the study to gather lessons about devices to improve future versions of the RADAR-CNS program. Conclusions The RADAR device selection framework provides a structured yet flexible approach to device selection for health care programs and can be used to systematically approach complex decisions that require teams to consider patient experiences alongside scientific priorities and logistical, technical, or regulatory constraints.

This research is qualitative research with case study method. The focus of this research is to explore the value of ethnomathematics in the culture of the Bali Mula society in the Kintamani sub-district, Bali Province. The research starts from the exploration phase of ethnomathematics, mapping ethnomathematics with school mathematics, explores aspects of pedagogy in the preservation of ethnomathematics and concludes on how ethnomathematics is applied to mathematics learning in schools. Data collection was carried out through observation of Mejejahitan activities carried out by Bali Mula women. Data from observations are then linked to the results of documentation analysis and strengthened by the results of interviews. The triangulation method is carried out to check the validity of the research data. Based on the results of data analysis there is a relationship between the activities of Mejejahitan with school mathematics. The mathematical concepts contained in the Majejahitan activity that is reflecting (reflecting) and predicting or predicting patterns. Because there is a relationship between Majejahitan activities with school mathematics, it can be said that there are ethnomathematics values in the culture of the Bali Mula society in Kintamani District, Bali Province.

BACKGROUND Health information technology (HIT) and associated data analytics offer significant opportunities for tackling some of the more complex challenges currently facing the health care sector. However, to deliver robust health care service improvements, it is essential that HIT solutions be designed by parallelly considering the 3 core pillars of health care quality: clinical effectiveness, patient safety, and patient experience. This requires multidisciplinary teams to design interventions that both adhere to medical protocols and achieve the tripartite goals of effectiveness, safety, and experience. OBJECTIVE In this paper, we present a design tool called <i>Integrated Patient Journey Mapping</i> (IPJM) that was developed to assist multidisciplinary teams in designing effective HIT solutions to address the 3 core pillars of health care quality. IPJM is intended to support the analysis of requirements as well as to promote empathy and the emergence of shared commitment and understanding among multidisciplinary teams. METHODS A 6-month, in-depth case study was conducted to derive findings on the use of IPJM during <i>Learning to Evaluate Blood Pressure at Home</i> (LEANBH), a connected health project that developed an HIT solution for the perinatal health context. Data were collected from over 700 hours of participant observations and 10 semistructured interviews. RESULTS The findings indicate that IPJM offered a constructive tool for multidisciplinary teams to work together in designing an HIT solution, through mapping the physical and emotional journey of patients for both the current service and the proposed connected health service. This allowed team members to consider the goals, tasks, constraints, and actors involved in the delivery of this journey and to capture requirements for the digital touchpoints of the connected health service. CONCLUSIONS Overall, IPJM facilitates the design and implementation of complex HITs that require multidisciplinary participation. CLINICALTRIAL
