European E-Health Framework

European health systems are under mounting pressure to respond to the challenges of population ageing, citizens’ rising expectations, migration, and mobility of patients and health professionals. New technologies have the potential to revolutionize healthcare and health systems and to contribute to their future sustainability. However the organizational and regulatory environment of e-health has not progressed as rapidly as technology both in national and European level. The key issue in the European sphere is whether and to what extent it interferes with public health policy and should be treated separately from the more “traditional” healthcare. The proposed chapter will define e-health from a European perspective, present the different steps of the European policy in the field, insist on the organizational and regulatory issues that arise and discuss drives and barriers towards achieving pan European “patient-friendly” healthcare services.

Population ageing is an unprecedented challenge for human societies, which recently is globally tackled by new technologies. In this chapter technologies tailored for use by the elderly people termed ambient assisted living and e-health are discussed. Focus is only placed on those technologies that can be adapted for home use. Emphasis is drawn both on the technical front as well as on the application front based on recent literature. The scope is to make sure the audience reaches a sufficiently broad understanding of what technology is available for home use by elderly people. Applications and research efforts spent but also funded at the European level with a clear focus on those supported by elderly trials are provided. The chapter is enriched with case studies from various projects.

Abstract Background An increase in chronic illness and multimorbidity together with ‘ageing societies’ and demand for long-term care have created growing need for integrated care and improved coordination of services. However, health systems largely failed to respond to changing needs. Education and training of health professionals are still organised ‘in silos’; connections between health and social care and other groups providing relevant services are usually absent; and fragmentation of care according to specialisation remains the dominant organising principle of healthcare services. In this situation there is an urgent need for change to build skills and competencies, use health human resources more effectively and rearrange care according to population needs and preventative approaches. Objectives This workshop brings together research from different national and European research projects positioned at the interface of healthcare services and workforce development. The major aim is to foster the building of bridges across professions, organisations and sectors. The workshop seeks to explore the benefits and novel insights gained from comprehensive efforts to strengthen coordination and integrated models of care and how this may foster agency of the health professions as well as the implementation of new models of care that strengthen health promotion. The findings illustrate that capacity for innovating health systems can be improved through coordination and the crucial role of health professions in these transformations. Health professionals may act as facilitators of integrated work and care arrangements. Strengthening professional agency and coordination across sectors, organisations and professional groups embodies new opportunities for innovating health service provision and health workforce development. These transformations, in turn, may foster people-centred healthcare services. Key messages Building bridges between sectors, organisations and professions is a key to people-centred care. Coordination improves health workforce agency and service capacity to respond to population needs.

Health professionals’ aim is to provide patients with the best possible care. Unfortunately, in doing this, they face financial and economic difficulties: the demand for healthcare—because of past successes and the emergence of effective new technologies—has often exceeded the available financial and human resources. As a result, healthcare interventions have been evaluated not only for their quality, safety, and effectiveness, but also for their (opportunity) costs, with the aim of investigating their efficiency. Health economic evaluations are becoming more and more useful for planning, assessing, and managing healthcare services and therapies.

AbstractEurope is facing a trend of demographic and social change that is likely to have a significant impact on our health systems in the coming decades. The primary concerns are related to the double challenge of population ageing and population decline. These changes will result in a shrinking workforce to handle the steadily increasing healthcare needs of the very old, who will constitute the fastest growing segment of the population. A number of strategies to meet this challenge are suggested and implemented, not least at the EU level. Adaptation of European health systems to the new situation also makes it necessary to revisit familiar medico-legal issues, in order to clarify and consolidate a number of already established patients' rights.

Climate change is a global problem, which affects the various geographical regions at different levels. It is also associated with a wide range of human health problems, which pose a burden to health systems, especially in regions such as Africa. Indeed, across the African continent public health systems are under severe pressure, partly due to their fragile socioeconomic conditions. This paper reports on a cross-sectional study in six African countries (Ghana, Nigeria, South Africa, Namibia, Ethiopia, and Kenya) aimed at assessing their vulnerabilities to climate change, focusing on its impacts on human health. The study evaluated the levels of information, knowledge, and perceptions of public health professionals. It also examined the health systems’ preparedness to cope with these health hazards, the available resources, and those needed to build resilience to the country’s vulnerable population, as perceived by health professionals. The results revealed that 63.1% of the total respondents reported that climate change had been extensively experienced in the past years, while 32% claimed that the sampled countries had experienced them to some extent. Nigerian respondents recorded the highest levels (67.7%), followed by Kenya with 66.6%. South Africa had the lowest level of impact as perceived by the respondents (50.0%) when compared with the other sampled countries. All respondents from Ghana and Namibia reported that health problems caused by climate change are common in the two countries. As perceived by the health professionals, the inadequate resources reiterate the need for infrastructural resources, medical equipment, emergency response resources, and technical support. The study’s recommendations include the need to improve current policies at all levels (i.e., national, regional, and local) on climate change and public health and to strengthen health professionals’ skills. Improving the basic knowledge of health institutions to better respond to a changing climate is also recommended. The study provides valuable insights which may be helpful to other nations in Sub-Saharan Africa.

Introduction Delivery of healthcare services makes up a complex system and it requires providers to be competent and to be able to integrate each of the institute of medicine’s (IOM) 5 core competencies into practice. However, healthcare providers are challenged with the task to be able to understand and apply the IOM core competencies into practice. Objective The purpose of the study was to examine the factors that influence health professional’s likelihood of accomplishing the IOM core competencies. Methods A cross-sectional study design was used to administer a validated online survey to health providers. This survey was distributed to physicians, nursing professionals, specialists, and allied healthcare professionals. The final sample included 3,940 participants who completed the survey. Results The study findings show that younger health professionals more consistently practice daily competencies than their older counterparts, especially in the use of evidence-based practice, informatics, and working in interdisciplinary teams. Less experienced health professionals more consistently applied quality improvement methods but less consistently used evidence-based practice compared to their more experienced counterparts. Conclusion There is a need to understand how health professionals’ age and experience impact their engagement with IOM’s core competencies. This study highlights the need for educational resources on the competencies to be tailored to health providers’ age and experience.

Civil wars produce immense humanitarian crises, causing millions of individuals to seek refuge in other countries. The rate of disease prevalence has inclined among the refugees, increasing the cost of healthcare. Complex medical conditions and high numbers of patients at healthcare centers overwhelm the healthcare system and delay diagnosis and treatment. Point-of-care (PoC) testing can provide efficient solutions to high equipment cost, late diagnosis, and low accessibility of healthcare services. However, the development of PoC devices in developing countries is challenged by several barriers. Such PoC devices may not be adopted due to prejudices about new technologies and the need for special training to use some of these devices. Here, we investigated the concerns of end users regarding PoC devices by surveying healthcare workers and doctors. The tendency to adopt PoC device changes is based on demographic factors such as work sector, education, and technology experience. The most apparent concern about PoC devices was issues regarding low accuracy, according to the surveyed clinicians.

This article describes how the exponential growth of mobile applications has changed the way healthcare services function, and mobile healthcare using the Cloud is the most promising technology for healthcare industry. The mobile healthcare industry is in a continuous transition phase that requires continual innovation. There has been identified some of the challenges in the area of security protocols for mobile health systems which still need to be addressed in the future to enable cost-effective, secure and robust mobile health systems. This article addresses these challenges by proposing a secure robust and privacy-enhanced mobile healthcare framework (SRPF) by adopting a Community Cloud (CC), WPKI cryptosystems, Universal Integrated Circuit Cards (UICCs) and a Trusted Platform Module (TPM). All the security properties are provided within this framework. SRPF overcomes replay attacks, Man in the Middle (MITM) Attacks, Impersonation attacks and Multi-Protocol attacks as SRPF was successfully verified using a scyther tool and by BAN logic.
