Health Grids offer new solutions and alternatives to existing models for the delivery of Healthcare Services to diverse Populations across dissimilar Geographical and Political Regions. Incorporating new medical science, legal systems, systems and networks, financing, technologies, processes, procedures, business and government participation, competitive, cost-effective integration with existing, experimental and competing delivery models is a basic requirement. Integration is likely to be performed locally and may be required to avoid the disruption of existing models, e.g., Patient, Practitioner and Payer choice. This chapter addresses a selection of issues that have been encountered in other high-technology integrations. Less-complex and more limited-in-scope than Health Grids they indicate a need for adaptability and multiple solutions. Choice and options will be important. The ability for Users to personalize, and re-structure as needed or desired, a Health Grid will be paramount.

Whether in our professional or private lives, we are all aware of the system wide efforts to provide quality healthcare services while containing the costs. Telemedicine as a method of service delivery has expanded as a result of changes in reimbursement and service delivery models. The growth and sustainability of telehealth within speech-language pathology and audiology, like any other service, depends on the ability to be reimbursed for services provided. Currently, reimbursement for services delivered via telehealth is variable and depends on numerous factors. An understanding of these factors and a willingness to advocate for increased reimbursement can bolster the success of practitioners interested in the telehealth as a service delivery method.

The major risk associated with low platelet count in pregnancy is the increased risk of bleeding during the childbirth or post that. There is an increased blood supply to the uterus during pregnancy and the surgical procedure requires cutting of major blood vessels. Women with thrombocytopenia are at increased risk of losing excessive blood. The risk is more in case of caesarean delivery as compared to vaginal delivery. Hence based on above findings the present study was planned for Assessment of the Platelet Count in the Pregnant Women in IGIMS, Patna, Bihar. The present study was planned in Department of Pathology, Indira Gandhi Institute of Medical Science, Patna, Bihar, India. The present study was planned from duration of January 2019 to June 2019. In the present study 200 pregnant females samples received for the platelet estimation were enrolled in the present study. Clinically platelet indices can be a useful screening test for early identification of preeclampsia and eclampsia. Also platelet indices can assess the prognosis of this disease in pregnant women and can be used as an effective prognostic marker because it correlates with severity of the disease. Platelet count is a simple, low cost, and rapid routine screening test. Hence the data generated from the present study concludes that platelet count can be used as a simple and cost effective tool to monitor the progression of preeclampsia, thereby preventing complications to develop during the gestational period. On the basis of a cost-benefit analysis it was decided to expand the Arnsberg WWTP by a multistage biological process which allows for cost-effective integration of the existing facilities. Carbon removal will then be accomplished in a high-loaded activated sludge stage for which the existing primary clarifier is to be reconstructed. The existing trickling filters will be used for nitrification during a midterm period and will be replaced later on either by a moving bed system or by new trickling filters. Line 3 of the existing secondary clarifiers will be reconstructed and used for post denitrification in a moving bed system. The carbon needed for denitrification will be provided by means of sludge hydrolysis and the use of an external carbon source.

This article describes how the exponential growth of mobile applications has changed the way healthcare services function, and mobile healthcare using the Cloud is the most promising technology for healthcare industry. The mobile healthcare industry is in a continuous transition phase that requires continual innovation. There has been identified some of the challenges in the area of security protocols for mobile health systems which still need to be addressed in the future to enable cost-effective, secure and robust mobile health systems. This article addresses these challenges by proposing a secure robust and privacy-enhanced mobile healthcare framework (SRPF) by adopting a Community Cloud (CC), WPKI cryptosystems, Universal Integrated Circuit Cards (UICCs) and a Trusted Platform Module (TPM). All the security properties are provided within this framework. SRPF overcomes replay attacks, Man in the Middle (MITM) Attacks, Impersonation attacks and Multi-Protocol attacks as SRPF was successfully verified using a scyther tool and by BAN logic.

The NHS is facing an unprecedented backlog in both patient care and building maintenance, with severe implications for service delivery, finance and population wellbeing. This article is the first in a series discussing modular healthcare facilities as a potential solution to these issues, providing flexible and cost-effective spaces to allow services to increase capacity without sacrificing care quality. The first of three instalments, this paper will outline the problems facing the NHS estate, many of which have been exacerbated to critical levels by the COVID-19 pandemic, and what this means for service delivery. It will then make the case for modular infrastructure, outlining the potential benefits for healthcare services, staff and patients alike. Using modern methods of construction, this approach to creating physical space in healthcare can provide greater flexibility and a reduced impact on the environment. The next two articles in this series will go on to provide detailed case studies of successful modular implementation in NHS trusts, an analysis of the cost implications and guidance on the commissioning process and building a business case.

Abstract Objective The aim is to compare the cost-effectiveness of two long-lasting insecticidal nets (LLINs) delivery models (standard vs. new) in universal coverage (UC) campaigns in rural Mozambique. Results The total financial cost of delivering LLINs was US$ 231,237.30 and US$ 174,790.14 in the intervention (302,648 LLINs were delivered) and control districts (219,613 LLINs were delivered), respectively. The average cost-effectiveness ratio (ACER) per LLIN delivered and ACER per household (HH) achieving UC was lower in the intervention districts. The incremental cost-effectiveness ratio (ICER) per LLIN and ICER per HH reaching UC were US$ 0.68 and US$ 2.24, respectively. Both incremental net benefit (for delivered LLIN and for HHs reaching UC) were positive (intervention deemed cost-effective). Overall, the newer delivery model was the more cost-effective intervention. However, the long-term sustainability of either delivery models is far from guaranteed in Mozambique’s current economic context.

Although Information and Communication Technologies (ICTs) in the healthcare sector are extensively deployed globally, they are not used effectively in developing countries. Many resource poor countries face numerous challenges in implementing the ICT interventions. For instance, many health applications that have been deployed are not user-centric. As a result, such ICT interventions do not benefit many health consumers. The lack of an ICT framework to support patient-centric healthcare services in Malawi renders the e-health and mhealth interventions less sustainable and less cost effective. The aim of the study was therefore to develop an ICT Framework that could support patient-centric healthcare services in the public health sector in Malawi. The comprehensive literature review and semi-structured interviews highlighted many challenges underlying ICT development in Malawi. An ICT framework for patient-centric healthcare services is therefore proposed to ensure that eHealth and mobile health interventions are more sustainable and cost effective. The framework was validated by five experts selected from different areas of expertise including mhealth application developers, ICT policy makers and public health practitioners. Results show that the framework is relevant, useful and applicable within the setting of Malawi. The framework can also be implemented in various countries with similar settings.

Tinnitus can be a debilitating hearing-related symptom. Access to evidence-based tinnitus interventions remain limited. Tele-audiology can assist by providing a clinically and cost-effective tinnitus management route. This chapter highlights how this is made possible by focusing on one form of tele-audiology, namely an internet-based intervention. Guidelines are provided for the development of such interventions. A framework outlining the various processes involved in evaluating newly developed interventions is also provided. The chapter closes by discussing factors that may facilitate or hamper the dissemination of new interventions into existing service delivery models. This well-defined outline for intervention development and evaluation can be applied and used to guide innovative intervention models by stakeholders.
