Supply Chain Management Perspective on Shortages in Drugs Sourcing

Drug supply shortages globally make hospitals vulnerable. It effects service delivery and patient satisfaction apart from endangering lives. Further, drug sourcing to ensure supply to satisfy patients' needs is a strategic priority for any health care system in any country. Katutura hospital and Namibia is no exception. However, frequent drug shortages have created difficulty in ensuring patients get prescribed medication. Adopting a descriptive research design, pharmacists, and nurses were surveyed and 22 questionnaires were distributed out of which 21 (95%) responded in full. The main objective of this study was to identify the causes of the drug supply shortage from a supply chain perspective and suggest strategies to deal with the shortage in an optimum manner. The findings reveal supply chain disruption, management inefficiencies, are inadequate inventory management are some of the main causes for drug supply shortage resulting in difficulties and inefficiencies for proper drug delivery and supply.

BackgroundRegardless of the significant efforts used and put in place to enhance supply chain performance in Rwanda, there is still poor inventory management and big loss due to expiry of medicines at Medical Procurement and Production Division (MPPD).ObjectiveTo assess the factors that contributes to expiry of medicines at MPPD and formulate recommendations for the improvement of the current situation.MethodsThis is a descriptive cross-sectional study used for 25 respondents. A questionnaire containing closed-ended questions was used focusing on factors contributing to expiries of medicines for each product category managed by MPPD from 2014-2018. Additionally, the researcher reviewed all inventory reports, including expiries for the same period under study. The study focused on targeting respondents working at MPPD as pharmacists both technical and administrative, also other non-pharmacist working in the warehouse were considered as respondents. The hard copy of questionnaire was handed over to 26 respondents and requested to provide filled questionnaire within seven days. Twenty-five respondents were able to return the filled questionnaire on time, giving 96% response rate.ResultsThe study found that in MPPD, medicines are still expiring. During five years (from 2014 up to 2018) the total expired products were valued at RWF 6,046,778,655 for all program categories: HIV commodities had the largest share 53.3%, Essential Medicines 22.5%, Malaria 13%, Maternal Child Health commodities 5.7%, Products used for Community health workers 4.5%, TB products 1% and 0.1% for Family Planning products. The study found that major contributing factors for expiry of health commodities at MPPD, are ranked as follows: Supply chain management 90%, other factors 73%, Poor storage management 68% and Excessive drug supply 67%. ConclusionConsidering the study results, it looks like no significant effect of excessive drug supply and inventory management on the expiration of medicines at MPPD. However, the Supply Chain Management components are the most vulnerable to contribute to the expiration of medicines at MPPD. There are many changes to be done inside the institution to remedy the issue of expiration like working according to the standard operating procedures, improving the Skills of personnel in supply chain management, empowering the procurement unit with skilled personnel and improving the communication with stakeholders to facilitate the smooth and quick replenishment and distribution of the stock. With the development of supply chain businesses and natural disasters, the probability of risk of supply chain disruption is increasing and this kind of risk can bring huge losses to the enterprises. In this paper, disruption management strategy is an effective and necessary part of the supply chain disruption risk in supply chain management research, which is a new bright spot in the field. The new management ideas and sophisticated quantitative analysis results continue to emerge, but they are overall relatively fragmented, lack of system. So supply chain disruption research related to risk management is still in its infancy. Through the theories and methods of management science of quantitative supply chain integration and management, optimal configuration of the entire resources and benefit maximization of the whole supply chain can be achieved. The paper reviewed foreign research on quantitative integration of supply chain management, and proposed a comparative analysis, finally provided some possible future research directions.

Importance and impact of the systems related to Agribusiness and Agri-food, are increasing around the world and demand a paramount attention. Collaboration in the inventory management is an integral part of the supply chain management, related to proactive integration among the chain actors facilitating production and supply, in especial in the agroindustrial sector of the Departamento de Antioquia, Colombia. This research establishes the main relationships between latent variables as collaboration, technology, models, optimization and inventory management, based on a literature review and applying a Structural Equation Model to a survey data of a sample of agribusiness companies. The results show that Available Technologies associated with Big Data, generates improvement of Collaboration Strategies, improving also Forecasting and Optimization; besides, Inventory Planning and Collaboration are related to Available Technologies associated with Big Data. A Poisson regression model and a Structural Equation Model estimations detect that the increasing strategies of technologies and Big Data are favorable to apply collaboration in the supply chain management, increasing possibilities to the enterprise competitiveness.

Purpose The study aims to examine the role of health-care supply chain management during the COVID-19 pandemic in a cross-section of 42 selected sub-Saharan African (SSA) countries. Design/methodology/approach The study used cross-sectional robust least square regression for parameter estimates. Findings The results confirmed the N-shaped relationship between the health-care logistics performance index (HLPI) and COVID-19 cases. It implies that initially HLPI increases along with an increase in COVID-19 cases. Later down, it decreases COVID-19 cases by providing continued access to medical devices and personal protective equipment. Again, it increases due to resuming economic activities across countries. Practical implications The continuing health-care supply chain is crucial to minimize COVID-19 cases. The international support from the developed world in providing health-care equipment, debt resettlement and resolving regional conflicts is deemed desirable to escape the SSA countries from the COVID-19 pandemic. Originality/value The importance of the health-care supply chain during the COVID-19 pandemic is evident in the forecasting estimates, which shows that from August 2021 to April 2022, increasing the health-care supply chain at their third-degree level would reduce coronavirus registered cases. The results conclude that SSA countries required more efforts to contain coronavirus cases by thrice increasing their health-care logistics supply chain.

Purpose This paper aims to explore avenue where suppliers and manufacturers are aligned with health-care providers to improve supply chain visibility. Supply chain finance is explored to link suppliers/manufacturers with health-care providers. Design/methodology/approach Existing literature on supply chain visibility in health care forms a basis to achieve the study purpose. Alignment calls also for financial health where supply chain partners’ working capital is readily available to execute joint supply chain plan. Findings There is a disjoint in supply chain alliance between suppliers/manufacturers and providers where providers are unable to trace the origin of supplies. Quality care suffers and cost of care rises as providers search for supplies on an emergency basis. This paper provides a framework where solution can be formulated. Research limitations/implications Suppliers/manufactures form a direct strategic alliance with providers where product visibility enables health-care providers with a better patient management with lower cost of supplies. Inventory management and logistics cost will be lowered as better planning/forecasting is in place. This paper does not call for testing any hypothesis. Perhaps, next move along this line will be to investigate financial health of supply chain partners based on supplier relationship management practices. Originality/value This paper proposes health-care supply chain as an alternative solution to achieve the following twin purposes: controlling the cost while improving quality of care through supply chain finance. As far as we know, this study is the first attempt to achieve the goals.
