Effect of a resin-based and a glass-ionomer sealant on the treatment of noncavitated occlusal caries lesions in teenagers

ABSTRACTObjective: Considering the minimally invasive approach to dentistry, the scientific community has focused on non-invasive treatments for caries lesions. The aim of this study was to evaluate the efficacy of a nonsurgical approach to arrest occlusal non-cavitated dentin lesions through glass ionomer sealing.Methods: In this controlled clinical trial, 51 teeth with clinically non-cavitated occlusal caries radiographically located beneath the enamel-dentine junction (radiolucent area) were selected among patients presenting a moderate to high risk of caries. The teeth were randomly divided into two groups: an experimental group receiving an application of Vidrion-R (SS White) glass ionomer and a control group not submitted to any clinical intervention. Caries progression was monitored by clinical and radiographic examination at 4-monthly intervals over a period of one year. In addition, marginal integrity of the sealant was evaluated in the experimental group.Results: Clinical examination showed no statistical difference between the groups (P=.13). On the other hand, sealed teeth presented lower caries progression when analyzed by radiographic examination (P=.004).Conclusion: A glass ionomer sealant over non-cavitated occlusal caries lesions in dentin may not be sufficiently effective in arresting their progression. Introduction: The purpose of this study was to compare the effect of the bioactive glass, the glass ionomer, and the Erbium YAG laser as liners on the remineralization of the affected dentin. Methods: The present study was conducted on 64 healthy extracted human molars divided into 4 groups, 1 control group and 3 experimental groups. After artificially inducing dentinal caries lesions, each of the experimental groups was applied to the cavity floor and then restored with a composite. The samples were stored after thermocycling in an incubator for two months. Finally, the hardness of the cavity floor was measured at 3 depths of 20, 50 and 100 μm by the Vickers microhardness tester. The dentin conditions underneath the liners were also evaluated with FESEM. Statistical analysis was performed by two-way ANOVA and the post-hoc Games-Howell test (P<0.05). Results: Among the groups, the lowest microhardness value was in the control group (P<0.05) except at a depth of 100 μm; therefore, there was no significant difference between the control group and the bioactive glass (P>0.05). The laser group had the highest microhardness value, which was significantly different from the control group (P<0.05). There was a significant difference between the laser and bioactive glass (P<0.05), except at a depth of 20 μm. The laser and glass ionomer had only a significant difference at a depth of 100 μm (P<0.05). The microhardness value induced by glass ionomer was higher than bioactive glass, which in no depth was significant (P>0.05). Partial dentinal tubule occlusion was observed with FESEM in each of the experimental groups as compared to the control group. Conclusion: The microhardness values were higher in all groups than in the control group. The laser might be more successful in remineralization than the other ones.

Objective: This study aimed to investigate the association of two fluorescent dyes and Laser Fluorescence (LF) device in detecting smooth and occlusal natural caries in primary teeth in vitro.Methods: Measurements were performed with the LF and with LF associated with tetrakis (N-methylpyridyl)porphyrin (LF TMPyP) and protoporphyrin IX (LF PPIX) in 72 smooth (63 primary molars) and 134 occlusal sites (81 primary molars). For validation, surfaces were sectioned and sections obtained were evaluated under stereomicroscope. Smooth surfaces were also evaluated using polarized light microscopy and Knoop microhardness. For both smooth and occlusal surfaces, ROC analyses were performed, and sensitivities, specificities and accuracies were assessed. In smooth surfaces, Pearson’s correlation coefficients between LF values and lesions hardness or lesions depth were calculated.Results: LF TMPyP presented higher correlation with hardness and lesion depth than other methods in smooth surfaces. No differences were observed in other parameters among the methods, in both smooth and occlusal surfaces.Relevance: The LF TMPyP might improve performance in quantifying smooth-surface caries lesions in primary teeth. However, the sensitivity is improved at D2 (caries extending into inner half of the enamel but not to amelodentinal junction) threshold when using PPIX in smooth caries lesions. The association of LF with fluorescent dyes does not improve the performance on occlusal caries.

ABSTRACT Objective: To evaluate the sealing of cavities of dentinal occlusal caries lesions, reproduced in vitro, with flow resin compared to cavity restorations presenting healthy dentin using microhybrid composite resin. Methods: The sample consisted of 27 healthy deciduous molars where cavities of approximately 2 mm in the fossa region were performed and occlusal cleft of each tooth were sealed, impermeabilization was performed and the sample was randomly divided into 2 groups: group I underwent cariogenic challenge and occlusal sealing with resin flow. The teeth of group II were restored with microhybrid composite resin. The teeth were immersed in 5% methylene blue for 8 hours at 37° C and washed until all the dye was removed from the surface. The teeth were sectioned in the mesio-distal direction. The penetration of the dye was evaluated: 0- no penetration; 1- dye penetration up to 1/3 of the restoration; 2- dye penetration up to 2/3 of the restoration depth; 3 - penetration of dye into the pulp wall. The results were analyzed by the Biostat 4.0 program. Descriptive analysis and the mode among the examiners submitted to the Mann-Whitney test. Results: There was no significant difference in microleakage between restoration performed in healthy dentin with microhybrid composite resin or maintenance of infected dentin in primary teeth sealed with resin flow (p = 0.6035). Conclusion: It was concluded that the marginal infiltration of primary molars sealed with microhybrid composite resin and resin flow was not influenced by the removal -or not -of the carious tissue or the material used.

The dental records of 435 dogs seen in a dental referral practice were reviewed. Twenty-three dogs (5.3%) had one or more caries lesions. Of the 47 caries lesions, 19 (40%) were pit and fissure caries, 17 (36%) were smooth surface caries, and 11 (23%) were root caries. Twelve dogs had symmetrical lesions. The teeth most commonly involved were the fourth premolar and first and second molar teeth. Twenty affected teeth were extracted and 17 were treated by cavity preparation and restoration with composite or glass ionomer materials. Ten restorations in four dogs were examined one year or more following treatment; all of the restorations were intact and there was no progression of the caries.

The goal of this study was to evaluate the influence of a PVC Seal Wrap as barrier protection and the influence of autoclaving on the in vitro performance of a laser fluorescence device in detecting occlusal caries lesions in primary teeth. The clinical significance of the experiment is to create a protocol to avoid the possibility of underestimating caries in clinical setting.
