Objectives. The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) is a well-established and internationally recognized scale for measuring patient experience with hospital inpatient care. This study aimed to develop a Japanese version of the HCAHPS and to examine its structural validity, criterion-related validity, and internal consistency reliability. Design. Multicenter cross-sectional study. Setting. A total of 48 hospitals in Japan. Participants. Patients aged ≥ 16 years who were discharged from the participating hospitals. Results. We translated the HCAHPS into Japanese according to the guidelines. Psychometric properties were examined using data from 6,522 patients. A confirmatory factor analysis showed excellent goodness of fit of the same factor structure as that of the original HCAHPS, with the following composites: communication with nurses, communication with doctors, responsiveness of hospital staff, hospital environment, communication about medicines, and discharge information. All hospital-level Pearson correlation coefficients between the Japanese HCAHPS composites and overall hospital rating exceeded the criteria. Results of inter-item correlations indicated adequate internal consistency reliability. Conclusions. We developed the Japanese HCAHPS, and evaluated its structural validity, criterion-related validity, and internal consistency reliability. This scale could be used for quality improvement based on the assessment of patient experience with hospital care and for health services research in Japan.

Hospitals are struggling to involve patients and learn from their experience. The risk factor of patient experience is increasingly recognized as a critical component in improving patient experience. Our study explored risk factors of negative patient experience in order to improve the health-service quality of public hospitals. We conducted a cross-sectional study in Hubei province, China. A total of 583 respondents were surveyed by the Outpatient Experience Questionnaire with good validity and reliability in July 2015. T-tests were conducted to compare the experience scores among different outpatient groups. Multiple linear regression was performed to determine the significant factors that influenced the outpatient experience. Outpatients between 18 and 44 years old had the lowest experience scores (65.89 ± 0.79), whereas outpatients completely paying out-of-pocket had the lowest experience scores (64.68 ± 0.81) among all participants. Outpatients with poor self-rated health status had the lowest experience scores (66.14 ± 1.61) among different self-rated health status groups. While age, type of payment, and self-rated health status were significantly risk factors that influenced outpatient experience in the multiple linear regression. Thus, health-care providers should pay more attention to outpatients who are young (age <45), completely out-of-pocket and poor health status, and provide precision health care to improve outpatient experience.

Objetivou-se analisar o conhecimento das gestantes primíparas acerca de métodos de alivio da dor no primeiro estágiodo parto. Trata-se de estudo exploratório e transversal. Foram entrevistadas 49 gestantes cadastradas na rede municipal dePalmas, Tocantins. Os dados foram processados no programa estatístico Epi Info, versão 3.3 de 2004. A análise foi atravésde estatística descritiva simples. Os resultados mostram gestantes com 25,04 ±5,71 anos de idade, com parceiro fixo, ensino médio completo, renda média de 1473,14 ±936,18 e 7,02 ±2,03 consultas pré-natal, encontravam-se com 34,8±3,42semanas de gestação. 100% afirmaram que o tema não foi abordado no acompanhamento pré-natal. 75,5% das gestantesforam classificadas com nenhum conhecimento. Concluiu-se que as gestantes apresentam necessidades de ter conhecimento sobre os métodos de alivio a dor, para o exercício do processo de parir de forma humanizada.Palavras-chave: Primeira fase do Trabalho de Parto; Dor do Parto; Conhecimento. The results show pregnant women who were 25.04 ± 5.71years old, with a fixed partner, complete high school, average income of 1473.14 ± 936.18 and 7.02 ± 2.03 prenatal consultations, were with 34.8 ± 3.42 weeks of gestation. 110% stated that the topic was not addressed in prenatal care. 75.5%of the pregnant women were classified with having no knowledge. It was concluded that the pregnant women present theneed to be aware of the methods of pain relief, to exercise the process of giving birth in a humanized way.Keywords: Labor Stage, First; Labor Pain; Knowledge.
