Dental ceramics have an excellent ability to reproduce natural teeth appearance, but once the substrate has an altered coloration, it is necessary to mask this intercurrence, considering the characteristics of the ceramic and cement selected. This article aims to present a clinical case of rehabilitation with ceramic laminates to mask a darkened central incisor by planning a more opaque porcelain structure under a translucent contact lens. A female patient sought dental care complaining of dissatisfaction with smile aesthetics. It was clinically identified the presence of a Class IV darkened restoration in composite resin in element 11. In treatment planning harmony in the shape and color of elements 11 and 21 was aimed at through lithium disilicate lenses. An opaque intermediate structure was used to cover the darkening in tooth 11. The double molding technique was performed to obtain the working model. Prior to cementing, the surfaces were treated. In ceramics, conditioning with 10% hydrofluoric acid and 37% phosphoric acid, silane application and universal adhesive; in dental substrate, conditioning with 37% phosphoric acid and application of universal adhesive were performed. In laminates definitive cementation Allcem Venner cement was chosen. Patient reported satisfaction with treatment final aesthetics. Thus, cementation technique of a translucent laminate over an opaque facet is a good option for masking substrates with darkened coloration.

Abstract Background The present study aimed to quantitate the wear of the highly transparent Yttria-stabilized tetragonal zirconia polycrystals (Y-TZP) ceramic monolithic zirconia crown on the enamel in vivo and discuss the prone position of the wear and the underlying mechanism. Methods A total of 43 patients with 43 posterior teeth were selected for full zirconia crown restoration and examined immediately, at 6 months, and at 1 year after restoration. During the follow-up visit, the fine impression of the patients’ monolithic zirconia crowns, the antagonist teeth, the corresponding contralateral natural teeth, the super plaster cast, and epoxy resin model was ontained. The model of epoxy resin was observed under a stereo microscope, and the microstructure parts were observed under a scanning electron microscope. Results After 1 year, the mean depth and volume of wearing of the monolithic zirconia crown were the smallest (all P < 0.01), while those of the antagonist teeth were significantly larger than those of the natural teeth (P < 0.0001), and no significant difference was found among the natural teeth (P = 0.3473, P = 0.6996). The amount of wear after one year was remarkably higher than that at 6 months (P < 0.0001). The microscopic observation revealed the tendency of wearing of the monolithic zirconia crown on the antagonist teeth at the protruding early contact points. Electron micrographs of tooth scars showed that the wearing mechanism of the monolithic zirconia crown on natural teeth was mainly abrasive and fatigue wear. Conclusions Although the self-wearing is insignificant, the monolithic zirconia crown can cause wear of the antagonist teeth via occlusal or early contact significantly; the amount of wearing is higher than that of natural teeth and increases over time. The wearing mechanism is mainly abrasive and fatigue wear.

Single tooth implant restorations in the aesthetic area are a demanding challenge. If a complete osseointegration is mandatory, the final result has to result in a higher standard of biomimetic and soft tissue health among natural teeth. This outcome is traditionally pursued by cementing crowns over individualized abutments. However, in recent years, the need for controlling peri-implant health and the preference towards a retrievable solution has led to an increase in screw-retained crowns, which is not always applicable when the implant axis is not ideal. In the aesthetic area, the use of a novel technical solution represented by the angled screw channel (ASC) of the abutment has been proposed in order to match the advantages of the screwed solution with the aesthetic demands. The aim of this study was to compare ASC crowns to cemented crowns (CC) in single implant restorations using the white esthetic score (WES) and pink esthetic score (PES) at the crown delivery and at a follow-up of a minimum of 2 years. Peri-implant health and marginal bone loss (MBL) were also evaluated. The mean follow-up was 44.3 months, with a mean MBL of 0.22 mm in the ASC group and 0.29 mm in the CC group. The total WES/PES score was 16.6 for ASC, compared with 17.3 for CC at baseline, and 16.2 and 17.1, respectively, at follow-up. Both of the groups reached a high WES/PES, and this was maintained over time, without signs of peri-implant diseases or bone loss, regardless of the choice of connection. In conclusion, ASC can be adopted in cases where the implant axis is not ideal, with aesthetic and functional results that are comparable to implants restored by cemented crowns.

Immediate complete denture (ICD) is a mucosa-supported prosthesis manufactured before the removal of natural teeth and installed soon after their extraction. This paper aims to describe a clinical case of oral rehabilitation with upper ICD and lower removable partial denture (RPD). A 57-year-old female patient sought treatment with dissatisfaction with her aesthetics and mobility of the upper teeth. After anamnesis, intra and extra-oral examinations, radiographic examination and analysis of study models mounted on a semi-adjustable articulator, periodontal pockets were found in the upper teeth and extensive bone loss. So, upper tooth extraction, superior ICD and lower RPD were indicated as treatment. For ICD preparation, anatomical and functional moldings were made, made up of orientation and intermaxillary relationship plans for the assembly of the semi-djustable articulator models. The remaining upper teeth were removed from the model for assembly of the artificial teeth and the ICD was polymerized and polished. The lower RPD was made following the standard steps. After dentures manufacturing, the surgery for upper tooth extraction was performed and prostheses installed immediately after surgery. The results demonstrated that the performed treatment provided aesthetic, phonetic, and functional restoration to the patient avoiding her to undergo a period of edentulism. It was concluded that ICD is a good indication for prosthetic rehabilitation of patients with advanced periodontitis, whose planning is the exodontia of all the teeth of an arch.
