Abstract Background Smoking cessation is a major public health issue. In France, primary care physicians (PCP) are the first contact points for tobacco management. The objective of this study was to understand how PCPs are involved in the management of smoking cessation: ownership, commitment, barriers. Methods A qualitative study was conducted using group and individual semi-structured techniques with PCPs. A thematic analysis of verbatim transcripts was performed to identify concepts and sub-concepts of interest. Saturation was evaluated retrospectively to ensure adequate sample size. Results A sample of 35 PCPs were interviewed, 31 in four focus groups and four in individual interviews. PCPs discussed their roles in the management of tobacco smoking cessation, including the different strategies they are using (e.g., Minimal Intervention Strategy, Motivational Interviewing), the multiple barriers encountered (e.g., lack of time, patients’ resistance to medical advice), the support resources and the treatment and intervention they prescribed (e.g. nicotine replacement therapy, supporting therapist). Conclusions This study provides a better understanding of the beliefs, attitudes, and behaviors of PCPs in managing smoking cessation. Guiding and encouraging patients toward smoking cessation remains a major objective of PCPs. While PCPs reported that progress has been made in recent years in terms of tools, technology and general awareness, they still face major barriers, some of which could be overcome by appropriate training.

Abstract The paper presents an analytical approach for designing grooved cam mechanisms with a modified arrangement of the common translating follower. That is, an intermediate link having three rollers is added between the cam and the common follower. On the basis of an existing cam mechanism with a common roller follower, an intermediate link that has three rollers is added between the cam and the common follower. Such a cam mechanism has two set of profile and can create multiple contact points between the cam and the follower at any instant. The two sets of profiles of such a cam mechanism can serve as the grooved types. Since the follower has three rollers that can simultaneously contact the cam at any instant, it can be positive driven along the guided groove of cam contour. The contact forces and contact stresses of such cam mechanisms are analyzed to illustrate the advantage of spreading force transmission and reducing contact stress of this uncommon follower. The obtained results indicate that the contact stress at the surface of the cam and the follower for such a cam mechanism can be reduced by 30% to 47% in comparison to those of cam mechanism with a common translating roller follower. In conclusion, the cam mechanism with a translating follower having an added ternary-roller intermediate link can be a preferable choice for the applications that follower is against heavy loads or move at high speed.

Fuel elements in a high-temperature gas-cooled reactor (HTGR) core may be stacked with a hexagonal close-packed (HCP) structure; therefore, analyzing the temperature distribution and heat transfer efficiency in the HCP pebble bed is of great significance to the design and safety of HTGR cores. In this study, the heat transfer characteristics of an HCP pebble bed are studied using CFD. The thermal fields and convective heat transfer coefficients under different coolant inlet velocities are obtained, and the velocity fields in the gap areas are also analyzed in different planes. It is found that the strongest heat transfer is shown near the right vertices of the top and bottom spheres, while the weakest heat transfer takes place in areas near the contact points where no fluid flows over; in addition, the correlation of the overall heat transfer coefficient with the Reynolds number is proposed as havg = 0.1545(k/L)Re0.8 (Pr = 0.712, 1.6 × 104 ≤ Re ≤ 4 × 104). It is also found that the heat transfer intensity of the HCP structure is weaker than that of the face-centered-cubic structure. These findings provide a reference for reactor designers and will contribute to the development of safer pebble-bed cores.

This chapter deals with the formation of biofilms, their resistance to antibacterial agents, the importance and risk of biofilms, and nanotechnology methods for biofilm control in the food industry. Biofilm is a multi-layer cell cluster embedded in an organic polymer matrix, which protects microbial cells from environmental stress, antibiotics, and disinfectants. Microorganisms that live in contact points and the environment in food processing are mostly harmful because the microbial community in the wrong location can lead to contamination of the surfaces and products produced during the processing. When new nanomaterials (for example, silver or copper are incorporated) are used, the growth of surface biofilms can also be reduced. In recent years, new nanotechnology-based antimicrobials have been designed to kill planktonic, antibiotic-resistant bacteria, but additional requirements rather than the mere killing of suspended bacteria must be met to combat biofilm-infections.

Substrate binding to the cytochrome P450 OleP is coupled to a large open-to-closed transition that remodels the active site, minimizing its exposure to the external solvent. When the aglycone substrate binds, a small empty cavity is formed between the I and G helices, the BC loop, and the substrate itself, where solvent molecules accumulate mediating substrate-enzyme interactions. Herein, we analyzed the role of this cavity in substrate binding to OleP by producing three mutants (E89Y, G92W, and S240Y) to decrease its volume. The crystal structures of the OleP mutants in the closed state bound to the aglycone 6DEB showed that G92W and S240Y occupied the cavity, providing additional contact points with the substrate. Conversely, mutation E89Y induces a flipped-out conformation of this amino acid side chain, that points towards the bulk, increasing the empty volume. Equilibrium titrations and molecular dynamic simulations indicate that the presence of a bulky residue within the cavity impacts the binding properties of the enzyme, perturbing the conformational space explored by the complexes. Our data highlight the relevance of this region in OleP substrate binding and suggest that it represents a key substrate-protein contact site to consider in the perspective of redirecting its activity towards alternative compounds.

This study examined the differences of knee joint forces between lowering to, or rising from squat, and typical final postures of squatting and kneeling. A biomechanical model of the lower limb was configured considering large knee flexion angles, multiple floor contact points, and the soft tissue contact between the thigh and calf. Inverse dynamics were used to determine muscle and compressive joint forces in the tibiofemoral and patellofemoral joints. Data were obtained from a group of 13 male subjects by means of 3D motion capturing, two force plates, a pressure-sensitive pad, and electromyography. During lowering into the kneeling/squatting positions and rising from them, the model exhibited the anticipated high maximum forces of 2.6 ± 0.39 body weight (BW) and 3.4 ± 0.56 BW in the tibiofemoral and patellofemoral joints. Upon attainment of the static terminal squatting and kneeling positions, the forces fell considerably, remaining within a range of between 0.5 and 0.7 BW for the tibiofemoral joint and 0.9 to 1.1 BW for the patellofemoral joint. The differences of the knee joint forces between the final postures of squatting and kneeling remained on average below 0.25 BW and were significant only for the tibiofemoral joint force.

Abstract Objective The aim of this study was to assess the evolution of chewing force in edentulous geriatric patients rehabilitated with complete dentures. Chewing function is compromised in individuals who lose all their teeth, as well as in those with complete dentures. The maintenance and recovery of the chewing function in these patients require replacement of the lost natural teeth. Materials and Methods In this study, the chewing force was evaluated by electromyographic analysis of the temporalis and masseter muscle endings in 120 patients with old and new bimaxillary complete dentures. Results Replacement of complete dentures was shown to improve stability, retention, occlusion, and support of the prosthesis. All jaw movements were evaluated using electromyography to assess the evolution of chewing force with and without occlusal adjustment, identify premature contact points, identify occlusal interference, respect the balanced occlusal scheme, and assess the increase in muscle strength. This study found that muscle strength in patients with prosthesis without occlusal adjustment was 527.1 N but was higher for prostheses with occlusal adjustment at 614.7 N, and this strength of 614.7 N was maintained after 1 month of control. Conclusion The chewing force evolved in patients with complete dentures, and an increase was observed when occlusal adjustment was performed.

A new type of compound transmission gear pair was put forward, called eccentric curve-face gear pair with curvilinear-shaped teeth. It could realize reciprocating motion of the gear shaft when the intersecting shafts achieve transferring motion and power through its unique tooth profile. The compound transmission principle of this gear pair was fully established based on the profile-closure process of axial direction and meshing process of the end face. The tooth surfaces of the eccentric curve-face gear and non-circular gear were generated. The contact paths of different teeth were obtained, and the compound transmission principle of eccentric curve-face gear pair with curvilinear-shaped teeth was verified by tooth contact analysis. By analyzing the mechanical characteristics of time-varying contact points, the changing rule of contact force was studied, and the compound transmission principle of the gear pair was further revealed from mechanics. Moreover, the experimental platform for transmission of eccentric curve-face gear pair with curvilinear-shaped teeth was set up to measure the motion law and contact area, and the correctness of the analysis results was verified.

Accra is one of the largest and most important cities in sub-Saharan Africa. The aim of this article is to assess the evolution of urban studies in Accra and its main historical and contemporary foci. Early knowledge on urban Accra is fragmentary and orientated toward European contact points and urban plans, ostensibly from the gaze of Europeans. Writings from Euro-Africans such as Carl Reindorf provide a different prism into the precolonial, indigenous, urban society, whereas most indigenous urban knowledge was situated in the oral tradition at this time. Around independence, officially appointed social anthropologists wrote about an indigenous community in Tema and surveyed the multiethnic Accra environment. From independence in 1957 until the early 1980s, social scientists viewed the urban settlement as an alien, Western intervention. Local scholarship on Accra was sidelined as the academy in a poor, emergent nation became preoccupied with the genesis of nation-state building and the establishment of viable academic departments in national universities, and growing proportions of migrants regarded “home” as somewhere else, that is, ancestral villages. In the 1970s Accra was inserted into world history and social history, and social scientists began to study residential geographies, but scholarship at the city-scale remained sparse. Engagement with world and social histories and the social sciences demonstrated that history matters, but not in linear and teleological ways. The liberalization era ushered in by structural adjustment policies (SAPs) in 1983 invigorated studies of Accra’s urban impacts and effects. Much of this research was disseminated by international scholars, as Ghanaian scholars had to contend with the negative impacts of SAPs on their own universities and households. Since the turn of the 21st century, scholarship on Accra, and African cities in general, has been increasing. Diverse research questions and a multiplicity of methodologies and frameworks seek to engage Western urban theories and other variants, undertake policy-relevant work, assess ethnic and residential dynamics, contribute to international urban debates, and advance postcolonial and revisionist accounts of urbanism. Viewed at the third decade of the 21st century, scholarship on Accra is of diverse origins, encompassing scholarship from locals, members of the diaspora, and international urbanists, and a promising tilt is local–international collaborations co-producing knowledge.

The emotional potential of sound is an excellent resource for companies and institutions seeking to test new ways of communicating with their stakeholders through the senses. However, there are still few organizations that include sound as a conveyer of their corporate identity. Audio branding aims to expand the contact points with audiences by incorporating sound elements that facilitate the recognition of the brand's values. This research attempts to understand the use that corporations make of sound as a communication tool and to evaluate its presence, but above all it wants to find out if this use is due to a strategic approach or if it is a specific decision marked by the temporality of advertising campaigns. For this reason, in this paper we have taken the radio as an advertising media. On the one hand, because it is the sound media par excellence. On the other hand, because it is where we can find the most well-known modes of audio branding such as the brand song, the jingle or the sonotype. The sample, composed of 239 inserts from the 3 generalist radio channels with the highest audiences in Spain, reveals that only 21% of items contain an element of audio branding, indicating that this is a field yet to be explored by organizations.
