BACKGROUND: Professional commitment is beyond a commitment for a particular organization and implies the individuals’ perspective towards their profession and the motivation that they have to stay in their job with willingness to strive and uphold the values and goals of the profession. InEthiopia, uptake of institutional delivery services is low. However, the level and factors associated with professional commitment is not known so far. Hence, our objective is to assess the level and factors associated with commitment of health professionals providing institutional delivery services in public health facilities of  Jimma Zone, Southwest Ethiopia.METHODS: A facility-based cross-sectional study design was conducted from March 01-20, 2016. A total of 442 eligible health professionals were included from randomly selected 7 districts and 47 respective health facilities. Health professionals were requested to fill self-administered questionnaire. After checking its completeness, the data was entered into EPI data version 3.1 and exported to SPSS version 20 for statistical analysis. Factor analysis was conducted. Simple and multiple linear regression were done using 95%CI and significance was declared at P<0.05. All assumptions of linear regression and principal component analysis were checked.RESULTS: The percentage mean score of professional commitment was 72.71% (SD21.88). The percentage mean score for perceived maternal health goal scale was 68.37% with the total variance explained being 69.68%. Perceived staff interaction, work-life balance, affective organizational commitment, normative organizational commitment, personal characteristics and perceived maternal health goal were independent predictors of professional commitment.CONCLUSION: The percentages mean score of professional commitment was medium. Hence, Health professionals should foster their level of professional commitment to increase uptake of institutional delivery services.KEYWORDS: Commitment, health professionals, Institutional delivery service uptake

Background: There is paucity of information on level of commitment among health professionals attending delivery service in public health facilities of low-income countries including Ethiopia. Hence, the aim of this study is to assess the level and factors associated with professional commitment among institutional delivery services providers at public health facilities in Shone District, Southern Ethiopia. Methods: A facility-based cross-sectional study design was conducted at primary level public health facilities in Shone District. All health facilities (one primary hospital and 7 health centers) were included in the study. Five hundred three study participants who fulfilled inclusion criteria in proportion to obstetric care providers in each public health facilities were selected by applying simple random sampling method. Self-administered Likert scale type of questionnaire was used. Data were analyzed using SPSS version 20. Bivariate and multivariable logistic regression analyses were done to see the association between dependent and explanatory variables. Results: The magnitude of professional commitment for obstetric care providers working in public health facilities of Shone district was 69.4%. In this study, those who worked at hospital, those who had positive attitude toward organizational commitment, and those who had positive attitude toward personal characteristics were 2.4, 2.3 and 1.76 times more likely committed to profession compared with their counterparts respectively. Conclusion: The professional commitment among institutional delivery service provision was medium as compared to other study finding. All health professional should manage their own personal characteristics to behave in good way to be committed for their profession. Organizational commitment had great influence on professional commitment.

Abstract Background: Organizational commitment is the relative strength of an individual’s identification with and involvement in a particular organization. It is an important predictor of absenteeism, turnover, organizational performance and success. Even though organizational commitment has a paramount importance for health care organizations, very few studies were done so far in Ethiopia particularly among health professionals. Therefore, the aim of this study was to measure level of organizational commitment and associated factors among health professionals working in public health facilities of Bench Sheko zone southwest Ethiopia. Methods: Facility based cross-sectional study was conducted in 14 Public health facilities found in randomly selected districts of Bench Sheko zone. Structured self-administered questioner was used to collect data from a total of 610 Health professionals from 10th of March – 30th of April. Data were entered into Epi-data manager Version 3.1 and exported to SPSS version 24 for further analysis. Factor analysis was done to create factor scores. Simple and multiple linear regression were done. Variables with p- value ≤ 0.25 in simple linear regression were candidate for multiple linear regression. Independent sample t-test and one-way ANOVA were done. Statistical significance was declared at p-value ≤ 0.05. Results: The response rate of the study was 96.8%. The percentages mean score of organizational commitment of health professionals’ was 74.6%. Perceived recognition of employees (B 0.806 [95% CI: 0.711 - 1.00, p=0.000], perceived conducive work climate (B: 0.421 [95%CI: .322 - 0.520], perceived transformational leadership style (B 0.749 [95%CI: .604 - .894, p=0.000], perceived transactional leadership styles (B: 0.294 [95%CI: .198 - .390 p=0.000] and not having managerial position(B:-.293 [95%CI:-.559 -0.028] were predictors of organizational commitment. Conclusion: Overall level of organization commitment of health professionals’ was higher than what is reported in many other studies. Organizational commitment was affected by job satisfaction, leadership styles and managerial position of health professionals. Hence, policy makers and human resource managers need to pay special attention to intervene on these factors.

Abstract Background: Health Operational Assistance (BOK) is a central government fund allocation to support the operation of Health Center (Puskesmas) to increase promotive and preventive activities related to public health, including maternal health. Objective: This study aims to determine the relationship of BOK utilization on maternal health service coverage. Method: The study used secondary data from the Ministry of Health e-Renggar Planning and Budget Bureau, Statistics Indonesia, the Ministry of Finance and the 2018 Public Health Development Index (IPKM) report. The unit of analysis is districts. Bivariate analysis was performed by Pearson correlation test, while multivariate analysis was performed by linear regression using SPSS software version 15. Results: The result of Pearson correlation test showed that BOK utilization was positively correlated with an increase in the antenatal care coverage (r = 0.294; p-value = 0,000) and coverage of deliveries by health workers in health facilities (r = 0.227; p-value = 0,000). The results of linear regression analysis showed BOK utilization, the percentage of poverty and the adequacy of posyandu have a significant relationship with antenatal care coverage and coverage of delivery assisted by health workers in health facilities. Conclusion: The health operational fund utilization increase antenatal care coverage and coverage of delivery assisted by health workers in health facilities. Therefore, it is necessary to optimize the use of BOK for the achievement of priority public health programs, especially for innovative activities that have a high degree of leverage towards the achievement of SDGs so that the target in reducing Maternal Mortality Rate (MMR) can be achieved. Keywords: health operational fund, maternal health, coverage, services   Abstrak Latar Belakang: Bantuan Operasional Kesehatan (BOK) merupakan salah satu sumber pembiayaan dari pemerintah pusat untuk menunjang operasional Puskesmas dalam meningkatkan upaya promotif dan preventif terkait kesehatan masyarakat, salah satunya kesehatan ibu. Tujuan: Studi ini bertujuan untuk mengetahui hubungan pemanfaatan BOK terhadap cakupan pelayanan kesehatan ibu. Metode: Studi ini menggunakan data sekunder dari laporan e-renggar Biro Perencanaan dan Anggaran Kementerian Kesehatan, Badan Pusat Statistik, Kementerian Keuangan dan laporan Indeks Pembangunan Kesehatan Masyarakat (IPKM) tahun 2018. Unit analisis adalah Kabupaten/Kota. Analisis bivariate dilakukan dengan uji korelasi pearson, sedangkan analisis multivariate dilakukan dengan regresi linier menggunakan software SPSS versi 15. Hasil: Hasil analisis uji korelasi pearson menunjukkan bahwa persentase realisasi BOK berkorelasi positif dengan peningkatan cakupan pemeriksaan kehamilan K4 (r=0,294; p-value=0,000) dan cakupan persalinan oleh tenaga kesehatan di fasilitas kesehatan (r=0,227; p-value=0,000). Hasil analisis regresi linier menunjukkan pemanfaatan BOK, persentase penduduk miskin dan kecukupan posyandu mempunyai hubungan bermakna dengan cakupan K4 dan cakupan persalinan ditolong oleh tenaga kesehatan di fasilitas kesehatan. Kesimpulan: Pemanfaatan BOK berhubungan dengan peningkatan cakupan K4 dan cakupan persalinan ditolong oleh tenaga kesehatan di fasilitas kesehatan. Optimalisasi pemanfaatan BOK untuk pencapaian program prioritas kesehatan masyarakat diperlukan, terutama untuk kegiatan-kegiatan inovatif yang memiliki daya ungkit tinggi terhadap pencapaian SDGs sehingga target dalam menurunkan Angka Kematian Ibu (AKI) dapat tercapai. Abstract Background Institutional delivery is one of the important strategies to reduce the maternal related risk at the time of delivery. Satisfaction of women on labor and delivery care services has good influence on their health and results in subsequent utilization of health services. This study was focused to assess women’s satisfaction and its’ associated factors with institution delivery services in public health institutions.Methods Cross- sectional study was conducted among 169 participants from June 2018 to November 2018 among women (having under one year children) residing in Tanahun district. Simple random sampling, face to face interview and semi structured interview schedule was used for the collection of data. Data were entered in Epi-Data and analyzed by SPSS. Ethical approval was obtained from Institutional Review Committee at Pokhara University and maintained during the process of research.Results The age of the participants were between 16 to 40 years with median age 25 years (IQR = 7 Years). Most (93.5%) of the participants were Hindu. Majority of the participants (57.4%) and their husbands (60.4%) had secondary level education. Almost all pregnancies (99.4%) are planned and normal delivery was most common (77.5%). The study shows that 55% of the women performed delivery in public health facilities. The study states high satisfaction score in health status of women after delivery (4.8) and lowest in availability of visitor's bed (2.99). Women's satisfaction and religion was seen to be significantly associated.Conclusion Majority of the participants were satisfied with the services provided by public health facilities. Although, number of birthing centers were increasing, facilities in the health institution is not sufficient as required. Different factors such as cleanliness of delivery room, availability of staffs, medicine, visitor's bed and behavior of staff are some issues to be improved.

Improving access to the formal health care sector is a primary public health goal in many low-income countries. But the returns to this access are unclear, given that the quality of care at public health facilities is often considered inadequate. We exploit temporal and geographic variation in the cost of traveling to formal sector health facilities to show that treatment at these facilities improves short-term health outcomes for acutely ill children in Tanzania. Our results suggest that these improvements are driven in part by more timely receipt of and better adherence to antimalarial treatment. (JEL I11, I12, I15, I18, J13, O15)
