Abstract Background: Organizational commitment is the relative strength of an individual’s identification with and involvement in a particular organization. It is an important predictor of absenteeism, turnover, organizational performance and success. Even though organizational commitment has a paramount importance for health care organizations, very few studies were done so far in Ethiopia particularly among health professionals. Therefore, the aim of this study was to measure level of organizational commitment and associated factors among health professionals working in public health facilities of Bench Sheko zone southwest Ethiopia. Methods: Facility based cross-sectional study was conducted in 14 Public health facilities found in randomly selected districts of Bench Sheko zone. Structured self-administered questioner was used to collect data from a total of 610 Health professionals from 10th of March – 30th of April. Data were entered into Epi-data manager Version 3.1 and exported to SPSS version 24 for further analysis. Factor analysis was done to create factor scores. Simple and multiple linear regression were done. Variables with p- value ≤ 0.25 in simple linear regression were candidate for multiple linear regression. Independent sample t-test and one-way ANOVA were done. Statistical significance was declared at p-value ≤ 0.05. Results: The response rate of the study was 96.8%. The percentages mean score of organizational commitment of health professionals’ was 74.6%. Perceived recognition of employees (B 0.806 [95% CI: 0.711 - 1.00, p=0.000], perceived conducive work climate (B: 0.421 [95%CI: .322 - 0.520], perceived transformational leadership style (B 0.749 [95%CI: .604 - .894, p=0.000], perceived transactional leadership styles (B: 0.294 [95%CI: .198 - .390 p=0.000] and not having managerial position(B:-.293 [95%CI:-.559 -0.028] were predictors of organizational commitment. Conclusion: Overall level of organization commitment of health professionals’ was higher than what is reported in many other studies. Organizational commitment was affected by job satisfaction, leadership styles and managerial position of health professionals. Hence, policy makers and human resource managers need to pay special attention to intervene on these factors.

Abstract Background Antenatal depression is the most common psychiatric disorder during pregnancy and it’s associated with psychosocial and obstetric factors. Antenatal depression has serious consequences for the mother and fetus, such as recurrent spontaneous abortions, pregnancy induced hypertension, pre-eclampsia, postpartum bleeding, pre-term delivery, postnatal depressive disorders and low birth weight. However it doesn’t get appropriate attention.Methods Institutional based cross-sectional study design was conducted on 446 pregnant women coming for antenatal care service at Jinka public health facilities, from June 01 to June 30, 2018. Data was collected through semi-structured and pretested questionnaire by face to face interview technique and Beck Depression Inventory was used to assess women’s depression condition. Collected data was entered in to Epi data version 3.3.1 after checking their completeness and exported to statistical package for social science version 20.0 for analysis. Logistic regression was used to find out association between explanatory and response variables. Explanatory variables which fulfill the assumption of logistic regression and had P-value less than 0.25 from bi-variable logistic regression were considered for the multivariable logistic regression model. Strength of association was evaluated using odds ratio at 95% confidence interval and P-value < 0.05 was considered to declare significant associations.Result The magnitude of antenatal depression in this study was 24.4%(20.2-28.5 at 95% CI) and it had statistically significant association with unmarried marital status AOR =13.39 [(95%CI); (3.11-57.7)], chronic medical illness AOR=3.97 [(95%CI); (1.07-14.7)], unplanned pregnancy AOR =6.76 [ (95%CI); (2.13-21.4)], history of abortion AOR =2.8 [(95%CI); (1.14-7.02)], history of previous pregnancy complication AOR =4.8 [(95%CI); (2.12-17.35)] and fear of pregnancy related complications AOR=5.4 [(95%CI); (2.32-12.4)].Conclusion and recommendation The magnitude of antenatal depression was high. So integrating mental health service in antenatal care assessment, provide comprehensive family planning service and improve obstetric service are recommended.

Abstract Background: Ante natal care is a key entry point for a pregnant woman to receive abroad range of promotion and preventive health services. Objective: was to assess the quality of antenatal care services at public health facilities of western Ethiopia. Methods: Facility-based cross-sectional study was conducted from May 30th to June 30th, 2016. All public health facilities in the zone were audited, 316 medical records were reviewed, and 316 pregnant women were interviewed. The data was entered using EPI Data version 3.1 and analyzed using SPSS version 20.0. Descriptive statistics, binary, and multiple logistic regressions were conducted. Variables with a P-value of <0.05 were considered as statistically significant predictors of the outcome variable.Results: A total of 316 pregnant women were enrolled in the study. All facilities were categorized as “good” by the possession of necessary equipment, 3/4 by basic amenities and 87.34% by general and gynecologic examination. The information was provided for 222(86.21%), which is categorized as poor.252 (79.7%) of the women were satisfied with ANC. A urine sample taken during ANC visit AOR 3.36(1.70, 6.61) and counseling on food eaten during pregnancy AOR 2.27(1.16, 4.45) were predictors of client satisfaction on ANC. Conclusions: The majority of pregnant women were satisfied with the ANC they received. A urine sample taken during the ANC visits and being counseled on the types of food eaten during pregnancy were predictors for client satisfaction on ANC. Concerned bodies need to improve laboratory tests and information provision.

Abstract Background: Health Operational Assistance (BOK) is a central government fund allocation to support the operation of Health Center (Puskesmas) to increase promotive and preventive activities related to public health, including maternal health. Objective: This study aims to determine the relationship of BOK utilization on maternal health service coverage. Method: The study used secondary data from the Ministry of Health e-Renggar Planning and Budget Bureau, Statistics Indonesia, the Ministry of Finance and the 2018 Public Health Development Index (IPKM) report. The unit of analysis is districts. Bivariate analysis was performed by Pearson correlation test, while multivariate analysis was performed by linear regression using SPSS software version 15. Results: The result of Pearson correlation test showed that BOK utilization was positively correlated with an increase in the antenatal care coverage (r = 0.294; p-value = 0,000) and coverage of deliveries by health workers in health facilities (r = 0.227; p-value = 0,000). The results of linear regression analysis showed BOK utilization, the percentage of poverty and the adequacy of posyandu have a significant relationship with antenatal care coverage and coverage of delivery assisted by health workers in health facilities. Conclusion: The health operational fund utilization increase antenatal care coverage and coverage of delivery assisted by health workers in health facilities. Therefore, it is necessary to optimize the use of BOK for the achievement of priority public health programs, especially for innovative activities that have a high degree of leverage towards the achievement of SDGs so that the target in reducing Maternal Mortality Rate (MMR) can be achieved. Keywords: health operational fund, maternal health, coverage, services   Abstrak Latar Belakang: Bantuan Operasional Kesehatan (BOK) merupakan salah satu sumber pembiayaan dari pemerintah pusat untuk menunjang operasional Puskesmas dalam meningkatkan upaya promotif dan preventif terkait kesehatan masyarakat, salah satunya kesehatan ibu. Tujuan: Studi ini bertujuan untuk mengetahui hubungan pemanfaatan BOK terhadap cakupan pelayanan kesehatan ibu. Metode: Studi ini menggunakan data sekunder dari laporan e-renggar Biro Perencanaan dan Anggaran Kementerian Kesehatan, Badan Pusat Statistik, Kementerian Keuangan dan laporan Indeks Pembangunan Kesehatan Masyarakat (IPKM) tahun 2018. Unit analisis adalah Kabupaten/Kota. Analisis bivariate dilakukan dengan uji korelasi pearson, sedangkan analisis multivariate dilakukan dengan regresi linier menggunakan software SPSS versi 15. Hasil: Hasil analisis uji korelasi pearson menunjukkan bahwa persentase realisasi BOK berkorelasi positif dengan peningkatan cakupan pemeriksaan kehamilan K4 (r=0,294; p-value=0,000) dan cakupan persalinan oleh tenaga kesehatan di fasilitas kesehatan (r=0,227; p-value=0,000). Hasil analisis regresi linier menunjukkan pemanfaatan BOK, persentase penduduk miskin dan kecukupan posyandu mempunyai hubungan bermakna dengan cakupan K4 dan cakupan persalinan ditolong oleh tenaga kesehatan di fasilitas kesehatan. Kesimpulan: Pemanfaatan BOK berhubungan dengan peningkatan cakupan K4 dan cakupan persalinan ditolong oleh tenaga kesehatan di fasilitas kesehatan. Optimalisasi pemanfaatan BOK untuk pencapaian program prioritas kesehatan masyarakat diperlukan, terutama untuk kegiatan-kegiatan inovatif yang memiliki daya ungkit tinggi terhadap pencapaian SDGs sehingga target dalam menurunkan Angka Kematian Ibu (AKI) dapat tercapai. BACKGROUND Background: The improved telecommunication infrastructure and reduced device costs in developing countries, including in Ethiopia increases the mobile phone penetration significantly. Nowadays, digitization of information to improve the access, quality and equity of health services becomes increasing important. However, resistance to use mobile technology for improving the health service among health professionals remains the problem in the health care system. OBJECTIVE Objective: This study aimed to determine the attitude and willingness to use mobile health technology and its associated factors among health professional in Dessie town public health facilities northeast, Ethiopia. METHODS Methods: A facility based cross-sectional study was conducted from March to April, 2016 in Dassie town of public health facilities, Northeast Ethiopia. A total of 422 health professionals who were working at public health facilities were selected using the proportionate probability sampling technique. Data collected using a standard structured and self-administered questionnaire, and an observational checklist were cleaned, coded, and entered into Epi-info version 7.2.1, and transferred into SPSS version 20 for further statistical analysis. Variables with a p-value of less than 0.05 at the multiple logistic regression analysis were considered as statistically significant. RESULTS Result: In this study, attitude and willingness to use mHealth among health professionals were found to be76.5% and 80.1%, respectively. In multivariable logistic regression analysis, health professionals’ age ranged from 20-29 years [AOR= 2.50, 95% CI: 1.22, 5.12], being male [AOR = 2.73, 95% CI: 1.46, 5.09], had access to internet [AOR=3. 51, 95% CI: 1.55, 7.96], and prior use of mobile phone for expected date of delivery (EDD) and for pulse rate monitoring were significantly associated with the attitude to use mHealth. While, willingness to use the technology was significantly associated with access to the internet [AOR=3. 92, 95% CI: 1.67, 9.16], prior mobile phone use of for diagnosis [AOR= 2.84, 95%CI: 1.35, 5.95], EDD calculation [AOR =2. 72, 95% CI: 1.23, 6.03] and pulse rate monitoring [AOR=2. 68, 95% CI: 1.28, 5.59]. CONCLUSIONS Conclusions: Health professional’s attitude and willingness to use mHealth were low when compared to previous studies. Access to internet, previous use of mobile phone for health services such as for pulse rate monitoring and for calculating date of delivery, younger age (20-29 years) and being male were among the reported significant variables. Therefore, motivating health professional to use the mobile phone to improve health services and working to access free internet at the health facilities are highly recommended.

Abstract Abstract Background: Implanon discontinuation is unacceptably high in developing countries, including Ethiopia; furthermore there is an observed problem of high unintended pregnancy rate after method discontinuation this might stride to program failure. Therefore, the purpose of this study was to assess the level and determinants of Implanon discontinuation among women who used Implanon in Adabet district, public health facilities, North-West Ethiopia, 2017. Methods: Facility-based cross-sectional study was conducted among 537 women, from Feb.03 to April 28, 2017, by face to face interview. Systematic random sampling technique was used to select the study subjects. The collected data were entered into Epi Info- version 7 then exported to SPSS version 20 for analysis. Both descriptive and analytical statistical analysis was computed. On multi-variable binary logistics regression, p-value and odds ratio (AOR) with 95%Ci was used to showing statistical association with the outcome variable. Results: In this study, 36.9% of Implanon users were discontinued the method before the intended time period. Among those women who discontinue the method 85.9% of them were discontinued before two years of Implanon insertion. Women who had no live child at the time of Implanon insertion[AOR=2.17,95%CI:1.25-3.77], didn’t received pre-insertion counseling on potential side effects [AOR=1.85,95%CI: 1.15-2.97], developed side effect secondary to Implanon insertion [AOR=5.17,95%CI:3.18-8.40], received appointment follow-up [AOR=0.23,95%CI:0.13-0.41], and not satisfied by the service provided [AOR=5.40,95%CI:3.04-9.57] were statistically associated with Implanon discontinuation. Conclusions: level of Implanon discontinuation before its intended period was high. Hence, to increase Implanon continuation rate; provide pre-insertion counseling including its possible side effects, improve client’s service satisfaction and strength appointment follow-up for Implanon users should be made.

Abstract Background: Many interventions have been implemented to increase institutional delivery in Ethiopia. However, only 26% of women delivered in health facilities. This increases the curiosity of the scientific community to know more about the quality of maternity care in the country. Objective: This study aimed to assess the proportion of respectful delivery care and associated factors among women delivering in Debre Berhan town public health facilities, Ethiopia, 2019.Methods: Institution-based cross-sectional study was conducted among 412 women who delivered in Debre Berhan town public health facilities from November 15 to December 30, 2019. A consecutive sampling method was used to identify study participants. A pretested structured questionnaire was administered by the interviewer. Bivariable and multivariable logistic analyses were conducted. Adjusted Odds ratio along with a 95% confidence interval was used to assess the magnitude and direction of the association. Statistically significant association was declared at a P-value of less than 0.05.Results: A total of 412 postnatal women were participated in this study, yielding a 99.8% response rate. About 35.7% (95% CI: 31%, 40.3%) women reported that they have received respectful delivery care. Daytime delivery [AOR=2.48; 95% CI (1.55, 3.99)], secondary school educational status and plus [AOR= 3.59; 95% CI (1.53, 8.42)], having a companion during delivery [AOR=2.45; 95% CI (1.47, 4.07)], and having four or more antenatal care visits [AOR= 2.54; 95% CI (1.60, 4.01)] were positively associated with respectful delivery care. Conclusions: The proportion of women reporting respectful delivery care was low. Women who had four or more ANC visits, who were secondary-level or higher educated, delivered during the daytime, and had a companion during delivery were more likely to receive respectful delivery care. Thus, allowing the mother to have a companion, improve ANC service utilization, and improving education accomplishment should be the focus area of intervention.
