EVALUATION OF THE EFFICACY OF DADRU VIDRAVANA MALAHARA IN DA-DRU KUSHTA- A RANDOMISED CONTROLLED CLINICAL STUDY

Skin disorders are one among the common manifestations. Normally, 10-15% of the general practitioner’s encounter patients with skin disorders. Ayurveda covers a number of skin manifestations under Kushta Roga. Dadru is one among Kshudra Kushta. It is identified by symptoms such as Kandu, DeerghaPratana, Utsanna, Mandala, Raaga, Pidaka with predominance of Pitta Kapha Dosha. It can be correlated with Tinea infection. Being a Twak Vikara, Bahirparimarjana Chikitsa like Lepadhi treatments plays an im-portant role in the treatment of Kushta including Dadru Kushta. Present study was conducted on 60 diag-nosed patients of Dadru Kushta who were randomly allocated with 30 each in two groups. Dadru Vidra-vana Malahara was given for local application in one group and Gandhaka Malahara, a widely used for-mulation in clinical practice was given for local application in another group. These local applications were advised for a duration of 7 days for under the treatment concept of Lepa. The ingredients of which are the drugs processing Shodana and Kushtahara property. The data was collected from patients at baseline,4th day and 8th day (on completion). Assessment was based on KASI method of grading, number of lesions, perimeter of each lesion, pidaka (present or absent). The results of the study showed that, a statistically significant difference was not seen between the effect of Dadru Vidravana Malahara and Gandhaka Malahara in Dadru Kushta.

Purpose To compare 4 different bearings in total hip arthroplasty (THA) in a randomised controlled clinical study on clinical performance. Methods 393 patients with osteoarthritis of the hip or avascular necrosis were included and allocated to 1 of the head-and-cup couples zirconia-on-polyethylene (group A), metal-on-metal (group B), zirconia-on-polyethylene with the liner moulded into the shell (group C), or alumina-on-alumina (group D). In the individual case the surgeon could choose other implants if indicated. Results 299 patients were operated with the allocated prosthesis. The estimated cumulated prosthesis survival percentages and 95% confidence interval after 10 years were: group A 84.6 (75.8-93.4); group B 95.0 (89.5-100); group C 93.2 (86.7-99.7); group D 66.1 (54.5-77.7). The patients' physical function was significantly improved and remained equally good in all 4 groups, however slightly declining with ageing. The luxation rate was initially high, and equal between the groups, but was reduced by improving the surgical procedure introducing capsule repair by reinsertion of the short external hip rotators. Conclusions The metal-on-metal or zirconia-on-polyethylene prostheses had high 10-year survival percentages. The longevity of these bearing couples by themselves was not related to the combination of materials. The zirconia-on-polyethylene prosthesis with the liner mounted peroperatively possibly exhibited cases of backside wear which the corresponding bearing couple with moulded polyethylene (Asian) did not. The alumina-on-alumina bearing performed poorly - it was redesigned after our study initiation and later withdrawn from the market.

Pandu is a Pitta Pradhana Tridoshaja Vikara affecting the Rasavaha srotas causing Sapta Dhatu Kshaya and Ojokshaya1. The disease is characterized by Panduvamata and Arohana Ayasa. Karnakshweda, Agnimandya, Daurbalya, Annadwesha, Srama, Bhrama, Gathrashoola, Arohanaayasa, Aruchi, Gaurava etc. are the other symptoms commonly found in the patients of Pandu. In the Present era, Anaemia is a burning issue around the world and W.H.O Global data base 2011 reveals that, in a 120 million population, 83% people are suffering from anaemia2.Present study was conducted on 60 diagnosed patients of Panduwho were randomly allocated with 30 each in two groups. Darvyadhi Lehya was given for one group and Drakshadhi Lehya a widely used formulation in clinical practice was given for another group. The study was conducted in 60 subjects for a period of 30 days. Clinical features and haematological parameters were documented before and after the treatment. The results of the study showed that, a statistically significant difference was not seen between the effect of Darvyadi and Drakshadilehya in Pandu roga.

ObjectiveTo compare the performance and safety in cataract surgery of two ophthalmic viscoelastic devices (OVDs), each having separate dispersive and cohesive characteristics and different concentrations.Methods and analysisIn this prospective, randomised, controlled clinical study, one eye of each patient was injected with OVD1 (Viscopack14) or OVD2 (DuoVisc) during phacoemulsification and intraocular lens implantation. Endothelial cell count, intraocular pressure (IOP), central corneal thickness (CCT), intraocular inflammation and corrected distance visual acuity (CDVA) were compared 3 months postoperatively.ResultsThe study enrolled 127 patients. Randomisation assigned 50 eyes of as many patients to each arm of the study. The postoperative mean endothelial cell loss was 14.4% and 7.1% from baseline in the OVD1 and OVD2 groups, respectively (p=0.08). The incidence of IOP spikes at 2 hours was 0% and 8%, respectively (p=0.02). There were intergroup differences in postoperative IOP values, the OVD2 group showed significantly higher values at all of the follow-up visits starting from the 24 hours examination (p<0.05). There was no statistically significant difference in the CCT, intraocular inflammation and CDVA at the end of follow-up.ConclusionBoth OVDs showed similar clinical performances and were equally effective during cataract surgery. Viscopack14 showed more corneal endothelial cell reduction, while DuoVisc had more occurrences of IOP values and spikes. Future studies are mandatory to support these preliminary results.
