Purpose: The purpose of the article is to determine the effectiveness of the use of perfluorocarbon gas vehicles in acute, severe anemia in animals caused by external blood loss and autoimmune intravascular hemolysis. Methodology: The effectiveness of the use of perfluorocarbon gas transport agents in acute, severe anemia in animals caused by external blood loss and autoimmune intravascular hemolysis was carried out based on the assessment of lethality in groups, biochemical criteria that were determined using an automatic biochemical blood analyzer and an assessment of hematocrit, which was determined using an automatic an analyzer of a general clinical analysis of blood with an impedance counting system. Results: The high efficiency of the use of perfluorocarbon gas transport drug - Perftoran in acute and severe anemia caused by external blood loss and autoimmune intravascular hemolysis was revealed on the basis of an assessment of the lethality of animals in groups in comparison with a blood preparation - donor erythrocyte mass. The lethality in the groups of animals that used the perfluorocarbon gas transport agent was significantly lower than that in the animals that used the erythrocyte mass, especially in acute and severe anemia caused by autoimmune intravascular hemolysis.

The objective: to increase efficiency of diagnostics and treatment of proliferative processes an endometrium at women of post-menopausal age on the basis of improvement and introduction of algorithm of diagnostic, treatment-and-prophylactic and rehabilitational actions. Materials and methods. We surveyed 213 patients of the post-menopausal period with existence of proliferative processes an endometrium. To all patients carried out all-clinical (the clinical analysis of blood, the biochemical analysis of blood, hemostaziograma), laboratory (research of level of receptors to sexual hormones) and tool inspection. Results. The data received at performance of echography by all observed after ablyation an endometrium, confirmed high safety of such sparing method of low-invasive surgery. According to ultrasonic research of organs small to basin in the first two days after operation testified to lack of any complications, at control ultrasonic inspections of organs of genitalias in more remote terms taped existence at the kept centers surveyed existence an endometrium which had good-quality character. It confirms high efficiency, small injury and a good acceptability of ablyation and a hysterorezectoscopia. Dany methods of surgical interventions are alternative to traditional methods of treatment of patients of the senior age group with proliferative processes an endometrium, especially in cases burdened by accompanying somatic pathology and in the presence to expeditious abdominal operative treatment and hormonetherapy. Сonclusion. Use of advanced algorithm of diagnostic, treatment-and-prophylactic and rehabilitational actions allows to increase efficiency of treatment and prophylaxis development of recurrence. Abstract In hemolytic paroxysmal nocturnal hemoglobinuria (PNH) patients, hemoglobin levels are typically maintained by transfusions, without which severe anemia can occur. The terminal complement inhibitor eculizumab was previously shown to effectively control intravascular hemolysis and the need for transfusions in PNH patients. We now report on compassionate use of eculizumab in a severely anemic Jehovah’s Witness PNH patient. RF is a 61 year old Hispanic woman with a 10 year history of PNH and s/p nephrectomy. In Jan 2004 she was hospitalized for a severe hemolytic paroxysm with jaundice, weakness, lower abdominal pain, vomiting, hemoglobinuria, LDH of 15,000 IU/L, hemoglobin (Hgb) of 5–6 g/dL, and 40–50,000/mm3 platelets. Flow cytometry showed 41% RBC and 70% granulocyte PNH clone sizes. To her prednisone (80 mg qd), Procrit 40,000 U thrice per week and coumadin 2.5 mg qd were added. In Feb, she improved somewhat after high dose IVIG but remained icteric and had a bed-chair existence. In mid-March, her Hgb was 5.1 g/dL, LDH 5000 IU/L, and haptoglobin <26 mg/dL. Walking 5–10 feet resulted in shortness of breath and tachycardia. From Apr through Oct she generally stabilized with chronic fatigue, a bed-chair existence, LDH in the 3–4000’s IU/L, and 80–100,000 platelets/mm3; prednisone was now 20 mg qd. Her Hgb decreased from 5–7 g/dL through mid Jul, to 4–6 g/dL through Oct. She then worsened during a febrile illness: Hgb dropped to 3.6 g/dL. An investigator sponsored IND was granted by the FDA and the local Institutional Review Board approved the single patient open label trial; the patient was consented for use of eculizumab. Eculizumab therapy (600 mg/week for four weeks, then 900 mg on week 5 and every other week thereafter) was initiated on Nov 2 at which time she had severe hemoglobinuria, Hgb of 4.1 g/dL, LDH of 4326 IU/L, haptoglobin of <26 mg/dL and reticulocyte count of 250,000; she was bed-wheel chair bound, using supplemental O2. One week after the first dose, she noted clear urine, her Hgb rose almost 2 g to 5.4 g/dL, LDH dropped to 1019 IU/L, haptoglobin increased to 46 mg/dL, and she was no longer icteric. She was now able to walk short distances. At two weeks her LDH was 473 IU/L. Within a month was able to walk several blocks without difficulty. After 6 weeks of eculizumab therapy, her Hgb had risen to 9.3 g/dL and her LDH was in the normal range. Flow cytometry in Jan 2005 showed a 95% PNH RBC clone size. Her Procrit was converted to Aranesp. From Jan to early Mar her Hgb ranged from 8.1 to 9.1 g/dL with LDH remaining in the normal range; in Mar her Hgb dropped to 6.5 g/dL but without hemoglobinuria or intravascular hemolysis (LDH remained < 250 IU/L), and PNH RBC clone size was 90%. There was no evidence of GI blood loss. Though fatigued, she was still able to perform her normal daily routines. In Apr she was hospitalized with a Klebsiella UTI with a Hgb of 3.7 g/dL, but recovered to 6.6–8.0 g/dL after the resolution of the infection and initiation of IV iron supplementation. Currently, she is on Aranesp 300 mcg q2wk, prednisone 15 mg qd, oral and IV iron, folate, and 900 mg eculizumab q2wk. Her LDH is normal and Hgb is 8 g/dL. She has no hemoglobinuria, feels well, performs her daily routines and is “leading a normal life”. This case report demonstrates that intravascular hemolysis can cause severe anemia in PNH, and that eculizumab can rapidly improve Hgb through resolution of hemolysis and protection of the PNH RBC clone.

Введение. Расширенные комбинированные вмешательства являются основным методом лечения пациентов с местнораспространенными солидными злокачественными опухолями. Основным следствием масштабной онкохирургии является риск развития острой массивной кровопотери (ОМОК). Цель исследования: оценка переносимости ОМОК у онкологических больных. Материалы и методы. В одноцентровое ретроспективное исследование с января 1999 г. по декабрь 2018 г. были включены 4236 больных, оперированных в радикальном объёме по поводу злокачественных опухолей различных локализаций, течение операции у которых осложнилось ОМОК. Критерием ОМОК являлась потеря ≥ 50% от расчётного объёма циркулирующей крови (ОЦКр) в течение 3 ч операции. Пациенты, вошедшие в исследование, были разделены на 4 группы в зависимости от объёма кровопотери: от 51 до 100%, от 101 до 200%, от 201 до 300% и свыше 300% ОЦКр. Результаты. Пациенты исследованных групп не различались ни по общим показателям, ни по исходному соматическому статусу. Про анализированы качественный и количественный состав инфузионно-трансфузионной терапии (ИТТ), показатели баланса жидкости интраоперационно, продолжительность пребывания пациентов в отделении реанимации и интенсивной терапии, летальность. Заключение. Соблюдение определенного протокола ИТТ при онкологических операциях, осложнённых ОМОК, является залогом успешного лечения и позволяет снизить интраоперационную летальность до 0,8% и госпитальную летальность до 6,45%. Метод аппаратной реинфузии аутоэритроцитов продемонстрировал высокую эффективность. Background. Cancer surgery remains the backbone of treatment approaches in patients with locally advanced solid malignancies. Risk of acute massive blood loss (AMBL) remains the main complication of such large-scale cancer surgery. Objectives: assessment of AMBL tolerance in cancer patients. Patients/Methods. A single- center retrospective study from January 1999 to December 2018 included 4,236 patients who underwent radical surgery for malignant tumors of various localizations, whose course of surgery was complicated with AMBL. The AMBL criterion was the loss of ≥ 50% of the calculated circulating blood volume (СBVс) within 3 hours of the operation. The patients included in the study were divided into 4 groups depending on the volume of blood loss: from 51 to 100%, from 101 to 200%, from 201 to 300% and over 300% of СBVс. Results. Patients of the studied groups did not differ either in general parameters or in baseline somatic status. The qualitative and quantitative composition of infusion-transfusion therapy (ITT), intraoperative fluid balance, patients stay in the intensive care unit, and mortality were analyzed. Conclusions. Compliance with a specific ITT protocol during cancer surgery complicated by AMBL is the key to successful treatment and allows to reduce intraoperative mortality to 0.8% and hospital mortality to 6.45%. The method of intraoperative red cell salvage and autologus transfusion has demonstrated high efficiency.

A Radium Delayed Coincidence Counter (RaDeCC) includes 3 channels (223Ra channel,224Ra channel, and total channel). It has been newly designed and assembled at Nuclear Research Institute. To determine 223Ra and 224Ra in seawater samples, the system efficiency at all 3 channels were investigated and calibrated. The research results showed that the RaDeCC operates stably and reliably with high efficiency of 26%. In this project, a procedure for measuring short half-life radium isotopes was established with a low detection limit (LOD (223Ra) = 0.002 Bq; LOD (224Ra) = 0.01 Bq), good reproducibility, and high precision. The technique is suitable for qualitative analysis of 223Ra, 224Ra in seawater samples at low concentration. The 11 coastal water samples were collected in a coastal of Ninh Thuan province. The analytical data of short-lived radium isotopes concentration in seawater at Ninh Thuan coastal area are 11.2 × 10-3 ÷ 45.5 × 10-3 mBq/L for 223Ra, and 34.7 × 10-2 ÷ 21.9 × 10-1 mBq/L for 224Ra.

Major blood loss following trauma is common, but severe anemia is generally not life-threatening when managed with the administration of blood and blood products. Severe anemia becomes particularly challenging and potentially lethal when the patient is a Jehovah's Witness, for whom receiving a transfusion is contrary to religious principles. This case report describes the management and hospital course of a Jehovah's Witness who was seriously injured in an airplane crash.

Abstract Red cell stroma has not been prepared free of iron and/or hemoglobin. It is not certain whether the iron is a contaminant or an essential constituent of the stroma. Extensive washing does not eliminate iron from the stroma but does cause loss of the stroma constituents, protein and lipid. Red cell stroma protein is increased in anemia due to blood loss in the dog, on the average in severe anemia, almost twice the figures recorded in the pooled normal samples of red cell stroma. Lipid fractions under the same conditions show minor fluctuations: approximately 90 per cent of total lipids in the red cells are recovered in the stroma by these methods. The technic for isolation and fractionation of dog erythrocyte stroma is described. Analyses made from the same blood sample by 3 different workers give comparable results. Red cell stroma deserves careful study in controlled experimental conditions.

In this paper, the application of various non-ozone-depleting low-boiling working fluids for the Rankine cycle is analyzed based on exergy efficiency. The Rankine cycle is used for high-efficiency waste heat recovery from flue gases generated in gas transport systems at natural gas compressor stations. The system was modelled based on calculations using the CHEMCAD software in conjunction with the “Energy unit” module compatible with the main program interface, which was used for assessing system energy efficiency. It was concluded that the neopentane Rankine cycle with a regenerator is more efficient. Preliminary economic analysis has shown that the proposed system has an acceptable payback period (up to 6 years).
